Analysis of rehabilitation in 101 schizophrenic patients post-discharge within one-year return visit by telephone

Abstract:AIM: To explore the existing problems and recovery of schizophrenic patients post-discharge within 1 year. METHODS: 101 schizophrenic patients, who had been treated at the Institute of Mental Health, Second Xiangya Hospital, Central South University between January and July 2003, were selected. All the patients were fully cured or improved completely. They were followed up after the understanding and agreement of patients and their family members according to the telephone number of contact person recorded at hospitalization. Instruction and evaluation were performed on compliance of taking drug, drug side effect, self-care ability in daily life, skill on social intercourse, function of studying/working, marriage and procreation, etc. with a self-made questionnaire through the telephone number at the end of post-discharge the first week, the first month, three months and one year. RESULTS: Two cases dropped out from the 101 ones after three months, and nine ones did so after one year. ① Compliance of taking drug in schizophrenic patients post-discharge: Compliance was worse, when discharge time was longer in the four phases. Compared in the four phases, some subjects became complete compliance, and others refused medicine fully. ② Comparison of drug side effect in every follow up in schizophrenic patients: Incidence of some side effects existed in every time phase, in which extrapyramidal symptoms (EPS), somnolence, dry mouth, constipation, and tachycardia had significantly high incidence rate during the first week and the first month post-discharge than at the end of three months and one year. EPS was more serious at the end of one year than the just stage of discharging. Inerease of body mass was the most significant at the end of one year. ③ Comparison of social functional disturbance in schizophrenic patients post-discharge at every follow-up time: The functional disturbance of social intercourse was the most obvious. The degree of self-care ability in daily life was lighter than former, but the incidence was high. Moreover, the impaired degree had not significantly improved(P < 0.05) when it was slight at the end of three months. There were 89.1% of subjects to be influent in the function of studying/working during the first week. Some one recovered schoolwork or occupational ability at the end of three months, but the impaired was still 69.7%. The problem of marriage and procreation was obviously influent during the year (around 80% at the three time phase), and there was insignificant improvement within 3 months.CONCLUSION: There are obvious problems for schizophrenic patient post-discharge, including worse compliance of taking drug, severe drug side effect, disturbance of self-care ability in daily life, damage of skill on social intercourse and occupational ability, etc. Return visit by telephone is a better method for the regular rehabilitation of the schizophrenic patients at home.
