Lateralization Reliability of Scalp/sphenoid Recordings in Patients with Medial Temporal Epilepsy

Abstract:Objective To assess the lateralization value of scalp interictal and ictal recordings with a sphenoidal derivation.Methods 39 patients were retrospectively analysed who were postoperatively class I for at least two years according to Engel’s classification of postoperative outcome among all the 110 patients who underwent selective amygdalo-hippocampectomy in our hospital. In this study,our classification was used to assess the lateralization value of both interictal epileptic discharge (IED) and ictal discharge (ID) in scalp EEG with sphenoidal derivation.Results Results showed that all IED arose from either sphenoidal area or anterior temporal lobe. 51% of patients had independent bitemporal IED,among them the IED predominant side was concordant to the side of resection only except one patient. Our results also prove that the presence of independent bitemporal IED with low ratio of ipsilateral IED does not exclude the possibility of successful epilepsy surgery. 64% of the patients had more than one grade of ictal patterns. 92% of these patients demonstrated that the lateralization of all their various grade were ipsilateral to the side of seizure onset except two patients,who had unilateral seizure onset on the side with larger number of ictal scalp/sphenoid EEGs regardless of different kinds of ictal EEG pattern. Extracranial EEG findings must be interpreted with caution in suspected MTLE patients and be analyzed with other presurgical clinical data.Conclusions Preoperative scalp EEG with sphenoidal electrodes (scalp/sp) is valuable in the diagnosis of medial temporal lobe epilepsy (MTLE). IED predominant side was more likely to be concordant with the side of seizure onset. However,the presence of independent bitemporal IED with low ratio of ipsilateral IED does not exclude the possibility of successful epilepsy surgery. For ID,we prove that it is more reliable than IDE for detecting the side of seizure onset.
