Prevalence survey of the mental and behavioral disturbances in elderly people with normal cognitive function in community

Abstract:Aim: Neuropsychiatric disturbances are the important clinical manifestations of dementia, so study on it is good for early differentiations of dementia. However, its distribution in normal crowd of communities is still not clear, so the aim of this paper is to determine the prevalence of mental and behavioral disturbances in elderly people with normal cognitive function. Methods: A population survey was carried out in a total of 3 108 participants with normal cognitive function, aged no less than 40 years old who were living in Beijing city and rural areas. The subjects with dementia, mental disease, Parkinson disease and sever cerebrovascular diseases were excluded. All the individuals were evaluated with the neuropsychiatric inventory (NPI). Results: Of the 3 108 normal individuals, 17.2% of them exhibited at least one NPI abnormal symptom within one month, and 5.76% had significant clinical symptoms. The prevalence rate in the population over 60 years old was 17.72% and 6.25%, separately. Depression was the most common symptom before 60 years old, and sleep disorder became the first after 60 years old, which was followed by anxiety, irritability and apathy. Euphoria and disinhibition were rare in all the three groups. Sleep disturbance was the only symptom that increased significantly with age (P = 0.004). The multifactor analysis showed that effects of high education (OR = 1.13) and female (OR = 1.25) for NPI were significant as a whole, and that of aging was near to significant (OR = 1.12, P = 0.052). Aging (OR = 1.36), high education (OR = 1.13) and female (OR = 1.30) had significant influence on sleep disturbance. Only female had effect on depression (OR = 1.54). Conclusion: The neuropsychiatric disturbances in elderly people with normal cognitive function also should not be neglected. The mental and behavior abnormalities should be emphasized particularly during the clinical diagnosis of dementia, but the certain prevalence of neuropsychiatric symptoms in cognitive normal elder population and its related factors need further research.
