A clinical study on the experiment of auto-skeletal muscle satellite cells transplantation to treat coronary heart diseases

Abstract:Aim: To investigate effect of auto-skeletal muscle satellite cells (SMSCs) transplantation on cardiac function in the infarcted myocardium of both porcines and patients with coronary heart disease. Methods: 1 In the animal experiment: ten young Suzhong porcines were divided randomly into treatment group and control group(n = 5). In the treatment group, SCs were isolated from skeletal muscle, and then the SCs were cultured in vitro and labeled with DAPI. The proximal potion of left anterior descending (LAD) coronery atery was ligated in young porcines to produce acute myocardial infarction (AMI) model. Young porcines were as donors and recipients of autologous SC transplantation. The SCs were implanted into AMI area through intramyocardium. In the control group, the same volume of cells culture medium was implanted into AMI area through intramyocardium. Gated cardiac blood pool was performed to detect cardiac function at the end of 1 and 4 weeks after AMI respectively. The cardiac specimens were harvested and examined by pathology at the end of 4 weeks. 2 Following the successful animal study, four patients with coronary heart disease and myocardial infarction were undergone implantation of auto-SMSCs alone or with coronary artery bypass grafting (CABG) simultaneously. Peri-operative changes of left ventricular ejection fraction (LVEF) and left ventricular end-diastolic diameter (LVDd) were observed. Results: SCs were spindle-shaped and adherented on the bottom of the flask after 48 hours culture. If calf bovine serum concentration was decreased to 20 g/L, SC commenced the fusion process, which resulted in formation of multinucleated myotubes. New island and striated muscles were found in the treatment group by HE staining in the infarction area and intramyocardium at the site of implantation. The new muscle cells did not show histologic evidence of intercalated discs and centrally located nuclei. Electron microscope observation: striated muscle fiber was observed, while no histologic evidence of intercalated discs was found in muscle cells. LVEF was increased both in the treatment group and control group at 1 week. There was no significant difference between them[ (23 ± 2) % vs (26 ± 3) %, t = 0.074, P > 0.05]. But LVEF in the treatment group was higher than that in control the group [ (33 ± 2) % vs (40 ± 2) %, t = 6.085, P < 0.001] at 4 week. Clinical trial: four patients recovered from CABG and/or auto-SMSC transplantation. No arrhythmia appeared through peri-operation and 8-12 months follow-up. Tc99m-MIBI and 18F-FDG indicated great improvement of perfusion and survival cells in implanted area. 2D-UCG showed that LVEF increased from 1% to 11%, and LVDd decreased 2-9 mm. Infarcted wall got thickened. Conclusion: Differentiation into striated muscles of SMSCs after they are implanted into infarcted myocardial area proves improvement of heart function. It also shows in clinical trial that auto-SSC implantation alone or with CABG simultaneously can both improve LVEF and decrease the enlarged left ventricular volume.
