Abstract:Pulmonary alveolar microlithiasis (PAM) is a rare autosomal recessive disorder characterized by the deposition of calcium phosphate microliths throughout the lungs. Currently the mutation of SLC34A2 gene was considered responsible for PAM. Here we reported the studies on mutation analysis of the SLC34A2 gene in three familial members and one unrelated subject of PAM by DNA direct sequencing. Meanwhile, we also reviewed and analyzed the published studies of the SLC34A2 gene mutation in PAM patients. The three familial patients were siblings of an inbred family whose parents were cousins. All four patients presented recurrent cough and exertional dyspnea. Diagnosis of PAM was made according to the typical manifestation of radiology. One homozygous mutation of the SLC34A2 gene, c.910A > T (p.K304X) was identified. The review of the SLC34A2 gene mutation showed multiple mutation symbols in PAM patients from China, Turkey, and Japan respectively. The present study supports that the clinical features, pathological and radiological characteristics of Chinese PAM patients are similar to those reported in other countries. Our investigation revealed that the c.910A > T mutation in the SCL34A2 gene was responsible for PAM patients in China. The review of literatures suggests that exon7 and exon8 seemed liable to be affected typical Mongoloid of PAM, and exon8 might be the screen target for Chinese patients.

Abstract:Background and objective: Pulmonary alveolar proteinosis (PAP) is a rare pulmonary disease. It was first described in China in 1965, and more cases have been reported subsequently. A systematic review was performed on 241 cases of PAP in China and progress in the diagnosis and treatment of this disease is discussed. Methods: The Chinese biological and medical databases from 1965 to 2006 were searched and 241 cases with complete clinical and pathological data were identified. The clinical characteristics of the disease were summarized and longitudinal comparisons were made of diagnostic and treatment methods over time. Results: The morbidity associated with PAP has increased in recent years. The clinical manifestations were non-specific. Progressive dyspnoea, cough and sputum were the most common symptoms. The percentage of patients undergoing CT examination has increased over the years. The combination of bronchoscopic biopsy and bronchoalveolar lavage (BAL) was usually sufficient to establish the diagnosis. Treatment was reported for a total of 142 cases. BAL and whole lung lavage were both effective and were only required once by most patients. Conclusions: The demographic characteristics and clinical manifestations of PAP patients in China are largely consistent with previous reports. Morbidity has increased dramatically in recent years, mainly due to the broad application of bronchoscopy since 1995. CT is very important for diagnosis of the disease. The long-term effects of treatment by whole lung lavage and BAL are similar.

Abstract:Objective To describe the clinical features and diagnosis of pulmonary alveolar microlithiasis ( PAM ) in adults and children .Methods We reviewed 143 cases from the literature from year 1994 to 2014 and report 8 cases in Shanghai Pulmonary Hospital which were tissue-confirmed or diagnosed by chest computed tomography ( CT ) .Results There were 13 children and 130 adults .The disease presented a high incidence of familial occurrence ( 64 .60％ ,73/113) .In most patients ,the disease was diagnosed in routine examinations ( 37 .06％ ) .In symptomatic patients ,typical complaints were dyspnoea ( 30 .77％ ) , nonproductive cough ( 27 .97％ ) . One hundred and thirty patients had chest tomography scans . The diffuse small parenchymal nodules and interlobular septal thickening or calcification were the same findings in children and adults .Ground-glass opacities were more common in children than in adults (child:53 .85％ ;adult :26 .50％ ) .But the dense consolidations of calcification were usually found in adults ( adult :61 .54％ ;child:23 .08％ ;χ2 = 7 .091 , P < 0 .05) .Sixty-eight cases were diagnosed from the specimens and tissues under bronchoscopy and by lung puncture in 80 tissue-confirmed cases .There were no statistical significance between pathological diagnosed and CT diagnosed in children and adults ( χ2 = 0 .923 , P = 0 .337) .So CT had helped to characterize the imaging of the disease and could be very useful in the diagnosis of PAM .Seven children ( 70％ ) and 29 adults ( 46 .77％ ) had normal lung functions .The lung function disorder were restrictive ventilation dysfunction and the decline of diffuse lung function .The extent of this disorder was concerned with pulmonary fibrosis or emphysema .It might remain static ( from 1 month to 20 years in the literature) as regards to both symptoms and radiographic findings ,only 1 patients died .While in others it might worsen over time during the follow-up period ,complicated with pulmonary fibrosis ,infection ,pneumothorax ,emphysema ,leading to pulmonary hypertension ,respiratory failure ,and chronic pulmonary heart disease .Currently ,there was no specific medical therapy for PAM .Conclusions PAM is a rare diffuse lung disease .The absent clinical features is inconsistent with the CT findings .The preliminary diagnosis can be given by the characteristic CT and the family history ,which can be confirmed by the transbronchial lung biopsy and bronchoalveolar lavage .

Abstract:Pulmonary alveolar mierolithiasis (PAM) is known very early to be a rare slowly progressive lung disease,characterized by widespread deposition of calcium phosphate microliths within the alveolar airspaces. Etiology of PAM used to be unknown,mutation of the SLC34A2 gene in PAM patients is recently identified. This review summarized the progress study of SLC34A2 and PAM,in order to provide references for further researches in this field.

Abstract:The pulmonary alveolar proteinosis (PAP) is a kind of causally unknown and infrequent pulmonary diseases. The pathologic feature is that the phospholipoprotein like surfactant, positive while dyed with periodic acid schiff has excessively accumulated in the alveoli cavity and the terminus of the bronchus. In this article, we selected five patients who have probably caught PAP. Vitally examed with the micro bronchoscope,their pulmonary tissue were taken,the ultra thin sections were observed by TEM. Two of these five patients could be excluded from catching PAP and the cellular ultrastructural alteration of the rest was fundamentally the same and corresponded with the pathologic alteration of the pulmonary tissue of PAP. The cellular ultrastructural feature of PAP is that of the typeⅡovergrown of alveolar epithelial cell and the abundence of that cellular chips, pulmonary surfactant, protein like structures and a number of spumous alveolar macrophages in the alveoli cavity and the terminus of the bronchus. The aboundance of these substances lead to the depression of the alveolar function and render the patients dyspnoea. The alteration of these ultrastructures sometimes is difficult to find with the LM. The use of the EM can help greatly in the clinical diagnosis.

Abstract:This review focuses on Pulmonary Alveolar Microlithiasis (PAM), an autosomal recessive genetic disorder characterized by calcium crystal deposits (microliths) resulting from loss of function of the SLC34A2 gene. PAM is a rare disease with approximately 1100 reported cases globally. The historical context of its discovery and the genetic, epidemiological, and pathophysiological aspects are discussed. PAM falls under interstitial lung diseases and is associated with pulmonary hypertension (PH), primarily categorized as Group 3 PH. The clinical manifestations, diagnostic approaches, and challenging aspects of treatment are explored. A clinical case of PAM with severe pulmonary hypertension is presented, emphasizing the importance of comprehensive evaluation and the potential benefits of phosphodiesterase-5 inhibitors (PDE5i) therapy. Despite limited therapeutic options and challenging diagnosis, this review sheds light on recent developments and emerging treatments for PAM and associated pulmonary hypertension.

Clinical features of pulmonary alveolar proteinosis: experience with 12 cases and literature review

Abstract:Objective To analyze and evaluate clinical features, pathological characteristics, diagnosis and treatment of the identified pulmonary alveolar proteinosis (PAP) patients.Methods Retrospective study of 12 patients identified by biopsy through BALF was conducted. Focused analysis with imaging studies (chest x-ray or CT/HRCT) and clinical presentations were aimed to determine the diagnostic methods of PAP. Results Nonspecific clinical representation of PAP was composed of progressive dyspnea (100%) and nonproductive cough (91.67%). Physical examination showed signs of inner lungsawas seldom detected, such as inspiratory crackles or Velcro's rales (41.67%) and cyanosis (25%). Although, absence of specific chest X-ray findings, some typical radiological changes contributed to diagnose PAP correctly, such as patchy opacification(91.67%)and crazy paving pattern(50%), especially in HRCT. Conclusion Separability of clinical symptoms and signs, radiological results and clinical features exist as characteristic representation of PAP. Diagnostic bronchoalveolar lavage is a practical and useful technology to determine PAP.
[A Novel Mutation of the SLC34A2 Gene in a Chinese Pedigree with Pulmonary Alveolar Microlithiasis].

Abstract:OBJECTIVE:To identify the mutation of solute carrier family 34 member 2 (SLC34A2) gene in a Chinese family with pulmonary alveolar microlithiasis (PAM).METHODS:Genomic DNA was extracted from the family members. DNA sequencing was carried out to confirm the mutation detected by polymerase chain reaction-single strand conformation polymorphisms (PCR-SSCP). The fragments with variation were screened in 100 healthy controls by PCR-SSCP.RESULTS:In both patients of the family, a homozygous mutation of the SLC34A2 gene was identified in exon 8 (c.A910T), resulting in a premature stop codon. In addition, a homozygous single nucleotide polymorphism (SNP) was found in intron 2 in both patients and the daughter of proband.CONCLUSION:A novel homozygous mutation in SLC34A2 gene, leading to a premature stop codon therefore a truncated protein, was probably responsible for the PAM in this family. The SNP in intron 2 needs further study.

Abstract:BACKGROUND Pulmonary alveolar microlithiasis (PAM) is a rare idiopathic lung disease characterized by the accumulation of innumerable microliths. Currently, effective therapeutics for PAM are not available, and the only treatment for end-stage lung disease is lung transplantation (LuTx). Further, there are few reports that focus on LuTx for the treatment of PAM, and the follow-up reports of postoperative imaging are even rarer. CASE SUMMARY A 52-year-old man presented to Shanghai Pulmonary Hospital in 2017 after experiencing shortness of breath and exacerbation. The patient was diagnosed with PAM and referred for single-LuTx (SLuTx) on March 14, 2018. Preoperative imaging results from a chest X-ray demonstrated bilateral, diffuse, symmetrical, sandstorm-like radiopaque micronodules, and pneumothorax and a computed tomography scan revealed minute, calcified military nodules in both lungs. We performed a left SLuTx, and intraoperative pathology was consistent with PAM. One week after surgery, a chest X-ray revealed slight exudation of the left lung, and one month later, the left transplanted lung exhibited good dilation, mild pulmonary perfusion injury with local infection, and left pleural effusion. Fiberoptic bronchoscopy revealed left hyperplastic granulation at the left bronchial anastomosis. Multiple sputum cultures suggested the presence of Klebsiella pneumoniae and Acinetobacter baumannii. The last follow-up was conducted in April 2019; the patient recovered well. CONCLUSION This case presents the imaging findings of a patient with PAM before and after LuTx and confirms the effectiveness of LuTx for the treatment of this disease.

Abstract:Pulmonary alveolar microlithiasis (PAM) is a rare genetic disorder with no definitive therapy. Bilateral lung transplantation (BLx) offers hope for severe cases. We report the first successful BLx for PAM in Iran, supported by a review of similar cases worldwide. The procedure significantly improved the patient's condition during the 2‐year follow‐up, highlighting BLx as a promising treatment option for PAM patients with limited alternatives. Background Pulmonary alveolar microlithiasis (PAM) is a rare autosomal recessive genetic disorder with approximately 1000 known cases worldwide, in which calcium phosphate microliths deposit in the alveolar air spaces. As of writing this report, no definitive conventional therapy exists, and many PAM cases may progress to severe respiratory failure and potential death. Bilateral lung transplantation (BLx) seems to be the most optimal solution; however, this procedure is challenging along with limited reports regarding the outcome in PAM. We report a case of PAM successfully treated with BLx for the first time in Iran. Method We present the case of a 42‐year‐old female with a longstanding history of cough, not responding to conventional antitussive medication, who was diagnosed as a case of PAM following a hospitalization due to coughing, dyspnea on exertion, and hemoptysis. Despite treatment with corticosteroid and medical treatment, no improvement was achieved and she subsequently developed respiratory and right ventricular failure, with oxygen ventilation dependence. Eventually, she was scheduled for BLx. The operation was successful and during her 2‐year follow‐up, no recurrence or significant postoperative complications has been reported. Conclusion This case presentation and literature review confirm the effectiveness of BLx as a promising treatment for PAM‐diagnosed patients, improving both life expectancy and quality of life.

Familial interstitial lung disease in four members of one family: a case series

Abstract:IntroductionFamilial interstitial lung disease has been reported worldwide, mostly in Europe and North America; limited information is available on the disease among Arab patients.Case presentationA 45-year-old woman presented to our outpatient clinic with a 1-year history of progressive dyspnea. At the age of 37 years, based on clinical and radiological features, our patient was diagnosed with idiopathic pulmonary fibrosis. A family history showed that of five deceased siblings, four had died of disease of undetermined etiology. In addition, we screened other family members, and three were shown to have clinical, radiological, and pathological features consistent with interstitial pneumonia.ConclusionOur report illustrates that younger age at presentation appears to be a common feature in patients with familial interstitial pneumonia. Otherwise, clinical, radiological, and histological features are indistinguishable from those of sporadic cases. Furthermore, our work highlights the importance of compiling a thorough family history in individuals presenting with cough and dyspnea, particularly in younger patients identified with idiopathic pulmonary fibrosis.
Pulmonary Lymphangioleiomyomatosis in a 46-Year-old Female: A Case Report and Review of the Literature

Abstract:Pulmonary lymphangioleiomyomatosis (PLAM) is a rare disease occurring frequently in women. The present study reported a case of PLAM in a 46-year-old female and reviewed the literature regarding PLAM. The female experienced dyspnea on exertion for 1 month. The chest computed tomography (CT) revealed diffuse thin-walled cystic lesions, consistent with radiological features of PLAM. CT-guided lung biopsy showed that characteristic smooth muscle cells were positive for smooth muscle actin, human melanoma black 45 and D2-40 by histology and immunohistochemistry techniques, resulting in a diagnosis of PLAM. The patient had a hysterectomy for treating uterine myoma 12 years previously. Additionally, lung diffusion function was impaired slightly, abdominal CT showed a right renal cyst and pelvic ultrasound exhibited a right oophoritic cyst. Sirolimus was administered, which improved the dyspnea and pulmonary function, and the patient required long-term follow-up. Therefore, this is a rare case of PLAM with renal cyst, oophoritic cyst and uterine myoma in a female. In conclusion, PLAM is a rare proliferative disease in the lung. Chest CT characteristically showed that the cysts were bilateral, diffuse and thin-walled. Lung biopsy may provide evidence for the diagnosis of PLAM, which was treated with sirolimus.

Abstract:OBJECTIVE:To summarize and analyze the clinical manifestations of pulmonary alveolar proteinosis (PAP) and its treatment.METHODS:Retrospective analysis of 2 cases with literature was reviewed.RESULTS:PAP presented a protracted chronic clinical course with overt symptoms but few physical signs, distinct chest radiography, and was a benign process. It was manifested as primary or secondary pathological changes. Typical chest CT scan showed "map" or "cobble stone" changes. Clinically, PAP was characterized by its incompatibility between symptoms and signs, and also radiographic picture and physical signs. The examination of bronchoalveolar lavage fluid could be used to confirm the diagnosis of PAP with periodic acid-schiff staining and electro microscopic examination. Whole lung lavage was an efficient therapy for PAP.CONCLUSION:It is very important for physicians to pay much more attention on the early diagnosis and treatment of pulmonary alveolar proteinosis.
Clinical Features And Genetic Characterization Of Familial Pulmonary Fibrosis In A Chinese Family

Abstract:Familial pulmonary fibrosis (FPF) is defined as idiopathic pulmonary fibrosis occurring in at least two members of a family. Studies showed that genetic factors and environmental common play an important role in the pathogenesis of FPF. We present the clinical data of a Chinese family with FPF. Based on the clinical and radiological features, the proband was diagnosed with idiopathic pulmonary fibrosis at the age of 44 years. Her family history showed that two deceased relatives died of idiopathic pulmonary fibrosis, and four members may die from this disease. Furthermore, the genomic DNA of the proband was sequenced and showed mutations in telomerase reverse transcriptase gene, surfactant pulmonary-associated protein B, and surfactant pulmonary-associated protein A.

Abstract:Objective To summarize and analyze the clinical manifestation,pathogenesis,and treatment of pulmonary alveolar proteinosis (PAP).Methods The clinical data of two patients were retrospectively analyzed and the related literatures were reviewed.Results The duration of disease of two patients was more than two years,the clinical manifestation of PAP was characterized by the incompatibility between symptoms signs and radiographs.They were diagnosed by alveolar lavage and transbronchoscopic lung biopsy.After whole lung lavage therapy,blood gas and chest CT scan improved significantly.Conclusions PAP is a rare lung disease,clinical manifestation is not typical.But with the progress of transbronchoscopic lung biopsy technique and alveolar lavage fluid analysis,the found cases had a trend of increasing.Typical chest CT scan showed map,pave stone,and ground glass change.Pathological characteristics is alveolar cavity filling with small granular PAS positive substance.Its pathogenesis is complex,whole lung lavage is the most efficient therapy for PAP at present.\r\n\r\nKey words: \r\nPulmonary alveolar proteinosis ; Whole lung lavage

Abstract:We reported a case of pulmonary capillary hemangiomatosis (PCH) and introduced its diagnosis, differential diagnosis, pathogenesis and development of treatment based on the review of Dana Point 2008 Classification of Pulmonary Hypertensiona and current literatures. A 43-year-old female presented progressive dyspnea, elevated pulmonary arterial pressures and CT pulmonary angiography (CTPA) imaging of main pulmonary arterial enlargement and wide spread ill-defined centrilobular nodules of ground-glass opacity. Her histologic features were proliferation of capillary channels within alveolar walls as well as muscularization of arterioles and medial hypertrophy of muscular pulmonary arteries. The treatment with diuretics and warfarin was used promptly, but unfortunately was ineffective. The patient died three months after diagnosis. PCH is a very rare vascular disease with poor prognosis. The diagnosis of PCH rests on the integration of clinical and radiographic information with pathologic features, however pathology is the most reliable means. Because clinical symptoms, imaging and histological features of pulmonary veno-occlusive disease (PVOD) and PCH broadly overlap, differential diagnosis should be made carefully. Among the various pathologic features proliferation of capillaries within alveolar walls is the key point for diagnosing PCH, which is also the most critical criteria for differentiating PCH from PVOD. So far the only definitive treatment for PCH is lung transplantation, without which the patient will die several months after diagnosis.
Pulmonary Lymphangioleiomyomatosis Accompanied with Renal Angiomyolipoma:Report of a Case and Literature Review

Abstract:Objective To investigate the clinicopathological features and prognosis of pulmonary lymphangioleiomyomatosis(PLAM) accompanying with renal angiomyolipoma(AML) to emphasize the diagnosis and differential diagnosis of the disease.Methods By routine histological and immunohistochemical(IHC) methods,the clinicopathological features and prognosis were analyzed in 1 case of PLAM and the related literatures were reviewed.Results PLAM occured almost exclusively in women of reproductive age.The main clinical manifestations of PLAM were nonspecific,including cough,suffocated chest,short breathing and dyspnea,pneumothorax and chylothorax.The pathological features of PLAM were abnormal hyperplasia of smooth muscle cells(PLAM cells) surrounding alveolar walls,bronchioles,lymphatics and blood vessels leading to airflow obstruction and replacement of the lung parenchyma by cysts.Immunohistochemical staining showed that the characteristic smooth muscle cells were positive for SMA,HMB-45,desmin,estrogen receptor,and negative for progesterone receptor.Renal AML was composed of different proportions of blood vessels,smooth muscle cells and adipose tissues.Immunohistochemical staining showed that smooth muscle cells were positive for SMA,HMB-45,and desmin.Conclusion PLAM accompanied renal AML is a rare disease of unknown etiology,which belongs to perivasular epithelioid cell tumor(PEComa).The coexisting of PLAM and renal ALM may be associated with tuberous sclerosis complex.

Abstract:To describe the clinical, radiological and pathological characteristics of idiopathic pulmonary alveolar proteinosis (I-PAP) and to evaluate the methods of diagnosis and treatment. Three patients were successfully diagnosed and treated in our hospital and the literature on the subject was reviewed. Three patients, two males and one female (mean age 46 years), were diagnosed averagely in 4 months. Two severe patients presented with progressive dyspnea and type I respiratory failure, and one mild patient only with dry cough and hypoxemia. Chest X-ray radiographs all showed perihilar "butterfly" shadow and chest CT scans showed diffused ground-glass opacities (GGO), typically with "map" changes and "crazy paving" patterns. All the patients underwent bronchoscope, branchoalveolar lavage fluid (BALF) had grossly opaque and/or milky appearance and its sediment was periodic acid-Schiff stain positive. Trans-bronchoscopic lung biopsy (TBLB) specimens were obtained and under light microscopy alveoli and some of the small bronchioles were filled with eosinophilic proteinaceous material with needle-like clefts. By electron microscopy numerous cellular debris and extracellular multilamellated bodies were found. Two severe patients were successfully treated with sequential whole-lung lavage and one required repeated lavages. I-PAP is rare and prone to be misdiagnosed. The radiological features may indicate the diagnosis and examinations of TBLB and BALF can make the accurate diagnosis. Whole-lung lavage is the most effective therapy by now and granulocyte-macrophage colony-stimulating factor (GM-CSF) may be beneficial in some patients.
