Abstract:Primary extragenital choriocarcinoma is a highly malignant tumor and usually occurs in the midline of the body, such as the mediastinum, retroperitoneum, pineal gland and the middle part of the brain. In this report we present a case of primary extragenital choriocarcinoma located in the posterior mediastinum. A 40-year-old man was admitted to hospital with severe chest wall pain for 2 months. Enhanced chest computed tomography (CT) showed a mass of soft tissue measuring 5.2 cm × 4.5 cm located in the posterior mediastinum. CT-guided percutaneous fine needle aspiration (FNA) biopsy was performed and adenocarcinoma was diagnosed. After careful examination, complete tumor resection with combined resection of part left upper lobe of lung and lymph node dissection were performed. Combining hematoxylin/eosin staining and immunohistochemical a pathological diagnosis of choriocarcinoma was made.

Abstract:Background Non-gestational choriocarcinoma, also known as primary choriocarcinoma, is extremely rare in men, manifesting with specific signs such as breast feminization, testicular atrophy, and loss of libido. The presentation typically includes elevated serum β-hCG levels, widespread metastatic disease, and a rapid progression of the condition. Case report We present a rare case of a 41-year-old man diagnosed with choriocarcinoma, exhibiting a unique combination of multiple metastases, including lung, brain, bone, and retroperitoneal lymph node metastases, as confirmed by 18 F-FDG PET/CT imaging. The patient was treated with aggressive chemotherapy and pembrolizumab, and the prognosis remained poor. The patient’s overall survival was a mere 5 months following diagnosis. Conclusion Non-gestational choriocarcinoma represents a rare entity in clinical practice and should be considered in young men presenting with gynaecomastia and elevated β-hCG levels alongside normal gonads. Thus, we advocate for a more comprehensive inquiry into medical history and a systematic examination. The 18 F-FDG PET/CT examination not only visually delineates the lesion’s location and extent but also serves as a cornerstone for clinical tumor staging, providing valuable support for treatment monitoring and subsequent follow-up.

Abstract:We report the case of a 68-year-old male whose Computed Tomography (CT) scan presented a mass (68*62*54 mm) of the right anterior mediastinal and pathologically diagnosis was mucinous adenocarcinoma(MA). The peripheral vessels are surrounded by the big mass in the anterior mediastinum which was associated with multiple metastases, thus we performed palliative chemoradiotherapy and we tried Human Epidermal Growth Factor Receptor-2 (HER-2) inhibitors based on the Next Generation Sequencing. The patient passed away 16 months after the onset of the disease. In this report, we review the rare case of anterior mediastinum MA as well as perspectives for potential future treatments.
Primary retroperitoneal choriocarcinoma with lung and liver metastasis in a male patient: case report

Abstract:Abstract Objectives Non-gestational primary choriocarcinoma is an extremely rare malignant tumor with atypical clinical symptoms, especially in males. It usually occurs in the midline of the body, such as the mediastinum and retroperitoneum. Pathological diagnosis of primary retroperitoneal choriocarcinoma presents many challenges. More importantly, it is insensitive to therapy and has a poor prognosis. To date, there is still no standard treatment strategy for primary choriocarcinoma. Case presentation This case report presented a 27-year-old male with acute abdominal pain as the main symptom. And then, retroperitoneal choriocarcinoma with lung and liver metastasis was diagnosed. Palliative surgery was performed to alleviate the abdominal pain but complete tumor removal was not achieved. Subsequently, we gave the treatment with cytotoxic chemotherapy and immune checkpoint inhibitor blockade. The tumor was significantly reduced in size after six cycles of immunotherapy and chemotherapy, and also β-hCG level returned to normal. The tumor was not in complete remission, so penpulimab immuno-maintenance therapy was given. So far, the tumor control is stable, and the patient’s quality of life is also very well. Conclusions Pathological diagnosis of primary choriocarcinoma is very necessary, and the related molecular markers can assist. Immunotherapy combined with chemotherapy is effective in the treatment of primary retroperitoneal choriocarcinoma.

Primary Small Cell Cancer of Cervical Trachea: A Case Report and Literature Review

Abstract:Primary small cell carcinoma of trachea is even more uncommon and only a few cases have been reported. Our search revealed only 90 cases in the English-language literatures. Case report: we report a case of cervical tracheal small cell cancer. A 67-year-old male presented with over 2-month history of cough and dyspnea. CT and MRI revealed a 1.0 cm x 2.5 cm intraluminal, irregular soft tissue mass in the upper trachea, approximately 2.5 cm below the glottis. A bronchoscopic examination disclosed a large tumor in the cervical trachea and the lesion occupied more than 60% of the tracheal lumen. Cytological examination suggested some poorly differentiated carcinoma cells. The patient received concurrent chemoradiotherapyand did not perform surgery. One week after CCR, the patient occurred difficulty in breath and tracheal stent was implanted. The symptom was improved markedly. Four days after implant of tracheal stent, the patient presented irritable cough and hemoptysis. The amount of bleeding was about 300 ml. The hemorrhage stopped by treatment of vasoconstrictor and fresh plasma. However, two days later, hemoptysis was continuing even if treatment of vasoconstrictor and fresh plasma. The patient and relatives waived the further therapies. The patient died of massive hemoptysis one week out of hospital. Conclusions: The tracheal small cell cancer is rare. The optimal treatment is unclear. In general, the strategy is introduced concurrent chemoradiotherapy following as small cell lung cancer. In cervical trachea, we suggest that surgical resection should be performed followed by postoperative adjuvant therapy.

Abstract:Introduction: Primary squamous cell carcinoma of the pancreas (SCCP) is a rare malignant tumor that has been reported in individual case reports only. The clinical data on primary SCCP treatment are limited. Therefore, the appropriate management strategy for this disease should be standardized. Case Presentation: We present the case of a 63-year-old man admitted to our hospital for upper left abdominal pain for 2 months without weight loss or jaundice. Enhanced computed tomography (CT) and magnetic resonance imaging (MRI) of the abdomen revealed a mixed solid and cystic lesion in the pancreatic tail, measuring 35 × 37 mm in maximum diameter with enhancement. The patient was diagnosed with primary SCCP without metastasis, based on radiological and pathological findings. He did not receive neoadjuvant therapy postoperatively and was followed up by CT and MRI for 18 months without recurrence or metastasis. Result: Complete resection is the most effective treatment for early stage primary SCCP. Abdominal MRI is an effective imaging tool for preoperative evaluation and postoperative follow-up of primary SCCP. The need for neoadjuvant therapy depends on various factors. Conclusion: Primary SCCP is a tumor with poor prognosis. Risk factor control, early accurate radiologic evaluation, and individualized treatment strategies improve the quality of life and prolong the overall survival period of patients.
Primary Seminoma Arising in the Middle Mediastinum: A Case Report

Abstract:Primary mediastinal seminoma often occurs in the anterior mediastinum of young males. It is unusual for the tumor to originate in the middle or posterior mediastinum, and such cases have rarely been reported in the English literature. The present study reports the case of a 52-year-old man with a 3.0-cm primary seminoma arising in the middle mediastinum. The patient presented with the symptoms of cough and chest tightness. Fluorine-18 fluorodeoxyglucose-positron emission tomography (18F-FDG-PET) scans revealed unique abnormal FDG uptake in the middle mediastinum. A mediastinoscopy was performed and integral excision was found to be difficult. A biopsy was performed and the histological examination revealed a primary seminoma. Following 4 cycles of a standard bleomycin, etoposide and cisplatin chemotherapy regimen, and chest irradiation at a total dose of 40 Gy in 20 fractions, the tumor exhibited a partial response, decreasing in size, and FDG uptake was no longer observed on 18F-FDG-PET scan. The last follow-up date was April 2016 and the patient has remained disease-free for 20 months.

Primary chondroma of posterior mediastinum with Horner’s syndrome: a case report

Abstract:Background Chondroma is a slowly growing, benign cartilaginous tumor which predominantly occurs in long bones of the hands and feet. Primary mediastinal chondroma is rare, especially with Horner’s syndrome. Case presentation We reported the case of a 31-year-old woman with a posterior mediastinum mass associated with Horner’s syndrome. After complete dissection of the mass, a pathological diagnosis of the primary mediastinal chondroma was rendered. The patient has shown no local recurrence or distal disease in a 3.5-year follow-up period. Conclusions The preoperative diagnosis of chondroma should combine various examinations for comprehensive evaluation. Complete surgical resection should be the first choice of the treatment due to the risk of malignancy.
Primary Intracranial Choriocarcinoma: A Report of 8 Cases with Review of Literature

Abstract:Objective: Primary intracranial choriocarcinoma is a rare intracranial neoplasm. We discuss its clinical features and treatment in a pediatric population.Methods: We retrospectively studied choriocarcinoma cases in our center from May 2002 to January 2015. Eight patients were diagnosed with primary intracranial choriocarcinoma and included. Pre- and post-operative CT and MRI were examined and blood serum and cerebrospinal fluid (CSF) were sampled to measure alpha-fetoprotein (AFP), carcinoembryogenic antigen (CEA), and human chorionic gonadotrophin (HCG).Results: All patients were male, aged between 5-17 years, (mean 11 years). The major presenting symptoms were headache, and dizziness with vomiting. All patients presented with hydrocephalus. The duration of onset was 10 days to 3 months; five patients had eye movement disorder and diplopia. Two patients had precocious puberty; and one patient had diabetes insipidus. HCG was elevated in all patients (serum level ranged from 1190-12000 IU/L CSF level ranged from 990-4651.56 IU/L). Three patients received preoperative radiotherapy. Eight patients received ventriculo-peritoneal shunts immediately after admission. Seven patients underwent surgery the remaining patient received chemotherapy with radiotherapy. Seven patients received postoperative chemotherapy. Hemorrhage and tumor growth occurred in patients who underwent preoperative radiotherapy. Follow-up was from 1 to 60 months; three patients died during follow-up and four survived. The mean survival time was 23.6 months.Conclusions: Extremely elevated HCG levels in serum and CSF may help diagnosis of primary intracranial choriocarcinoma. Based on these cases, we are concerned that preoperative radiotherapy may increase the hemorrhage occurrence. Surgery combined with chemotherapy may be a more effective treatment.

Abstract:Primary undifferentiated high-grade pleomorphic sarcoma (UPS) in the mediastinum is exceptionally rare. This paper reports a unique case of anterior mediastinal UPS in an 84-year-old Asian male presenting with recurrent upper respiratory tract infections and upper abdominal discomfort. Imaging via CT and MRI suggested an invasive thymoma, but postoperative pathology confirmed UPS. Despite radical surgery, local recurrence occurred within three months, and palliative radiotherapy was ineffective. This case provides the first comprehensive imaging data of UPS in the anterior mediastinum, aiming to improve diagnostic accuracy for clinicians and radiologists by summarizing imaging features across various modalities.

Primary mediastinal seminoma with leiomyosarcoma: a rare case report

Abstract:Germ cell tumor is the most common malignant tumor of the gonads, sometimes they are found in locations other than the gonads, called Extra-gonadal Germ cell tumours (EGCTs). Primary mediastinal germ cell tumors (PMGCTs) are a kind of rare neoplasm in the anterior mediastinum, including seminoma and non-seminomatous, or appear as a mixture. Primary mediastinal seminoma mixed with sarcoma is an extremely rare clinicopathologic entity. Previous studies have revealed that primary pure mediastinal seminomas are commonly sensitive to chemoradiotherapy and possibly to palliative excision. The treatment options for mixed germ cell tumor composed of seminoma and sarcoma remain unknown. Only one case of primary mediastinal seminoma with rhabdosarcoma has been reported in the literature up to date and the patient benefited from chemotherapy as the neoadjuvant therapy. However, cases of primary mediastinal seminoma with leiomyosarcoma have not been documented. Herein, we report a case of an 18-year-old patient, who presented with dyspnea, orthopnea, and chest pain, the CECT scan of the chest showed a large mass in the anterior mediastinum, which turned out to be seminoma mixed with leiomyosarcoma after partial excision. We investigate the treatment strategy and potential molecular mechanism of this disease. Finally, our study demonstrated that the patient benefited from the treatment of chemotherapy alone, or combined with target therapy after the operation. Meanwhile, the BRAF p.G466V, TP53 mutations, MTOR p.T1977I and exons 2-5 deletion of FLCN may be potential molecular mechanisms and oncogenic drivers of this disease.
Primary mediastinal seminoma with azoospermia: case report and review of the literature

Abstract:Introduction: Since the first report, primary mediastinal seminoma has a low incidence in the population, and it mainly affects young and middle-aged men, is clinically rare, and accounts for a very small proportion of mediastinal tumors. In this study, we describe the first case of primary mediastinal seminoma with azoospermia and hypothesize that the coexistence of the two disorders may not be a coincidence. Case report: A 16-year-old man presented with chest tightness and chest pain, a mediastinal mass on chest CT, and abnormal 18F-fluoro-deoxyglucose uptake on a PET-CT scan. By biopsy of the mass, the pathological diagnosis was a primary mediastinal seminoma. Because chemotherapy is included in the treatment of the tumor, the patient underwent sperm freezing before treatment, considering that chemotherapy can affect fertility, but the patient was diagnosed with azoospermia. Finally, the patient underwent tumor resection and postoperative chemotherapy. No tumor recurrence was observed at the current follow-up. Conclusion: Primary mediastinal seminoma is mainly confirmed by histopathological examination, and surgery and chemoradiotherapy are the current treatments. In patients with mediastinal seminoma or azoospermia, doctors should be aware that the two disorders may coexist, especially in men who have fertility requirements or long-term infertility, and that examination of the mediastinum and semen may lead to unexpected findings in the diagnosis and treatment. For mediastinal germ cell tumors, genetic testing is of great value in the treatment of tumors and the prediction of associated diseases. Future studies exploring the potential correlation between mediastinal seminoma and azoospermia will be prospective.

Clinicopathological characteristics and survival outcomes for testicular choriocarcinoma and non-testicular choriocarcinoma: A SEER database analysis.

Abstract:e17026 Background: Primary Choriocarcinoma (CC) is a rare germ cell tumor that can occur in males. The most common primary site in men is the testis. It can occur in extragonadal sites such as the mediastinum, retroperitoneum, lungs or brain. The clinical and prognostic differences between testicular CC and non-testicular CC are unclear. We aim to study the differences in clinical features, prognostic factors, and outcomes between the two subtypes of CC. Methods: We analyzed data from the Surveillance, Epidemiology, and End Results (SEER) database for patients diagnosed with pure CC between 2000 and 2018. Patients with a previous history of cancer or with other concurrent malignancies were excluded. These patients were divided into two groups: testicular and non-testicular CC. The clinicopathological features of each group were compared using chi-square tests. Kaplan-Meier estimator, log-rank tests, and Cox proportional hazard regression were used to identify prognostic factors over overall survival (OS) and cancer-specific survival (CSS). Results: The study included 363 patients, with 270 (74.4%) patients classified as having testicular CC and 93 (25.6%) as having non-testicular CC. The results showed that testicular CC was more likely to occur in white patients, be bilateral in distribution, have a smaller size, and be in more advanced SEER stages compared to non-testicular CC. Additionally, testicular CC patients were more likely to undergo surgical treatment, but not radiation. Testicular CC patients had a better OS and CSS (HR = 0.52, 95% CI: 0.38-0.71 and HR = 0.52, 95% CI: 0.37-0.71, respectively) compared with non-testicular CC. Our Cox multivariate analyses for OS and CSS identified the following independent prognostic factors for testicular CC: localized stage, surgery, tumor size ≥ 4cm, radiotherapy, and chemotherapy. Interestingly, the use of radiotherapy was actually associated with poor OS and CSS (HR = 1.8, 95% CI: 1.17-2.85 and HR = 1.7, 95% CI: 1.05-2.73, respectively). For non-testicular CC, the independent prognostic factors for OS and CSS were age ≥ 30 years, non-paired site, regional stage, surgery, and chemotherapy. Conclusions: Our study provides important insights into the differences between testicular and non-testicular CC. The use of radiotherapy is generally not recommended in testicular CC which might explain its bad prognostic effect as these patients might be naturally at higher risk of dying. However, the bad poor prognosis of patients using radiotherapy persisted even after controlling for other important prognostic factors. Further studies with larger sample sizes are needed to validate and build upon these findings, but our results provide a starting point for a better understanding of such rare cancer.

Primary hepatic neuroendocrine carcinoma: report of two cases and literature review

Abstract:Abstract Background Primary hepatic neuroendocrine carcinoma (PHNEC) is extremely rare. The diagnosis of PHNEC remains challenging—partly due to its rarity, and partly due to its lack of unique clinical features. Available treatment options for PHNEC include surgical resection of the liver tumor(s), radiotherapy, liver transplant, transcatheter arterial chemoembolization (TACE), and administration of somatostatin analogues. Case presentation We report two male PHNEC cases and discuss the diagnosis and treatment options. Both cases presented with abdominal pain; case two also presented with symptoms of jaundice. The initial diagnosis for both cases was poorly differentiated grade 3 small-cell neuroendocrine carcinoma, based on imaging characteristics and the pathology of liver biopsies. Final diagnoses of PHNEC were arrived at by ruling out non-hepatic origins. Case one presented with a large tumor in the right liver lobe, and the patient was treated with TACE. Case two presented with tumors in both liver lobes, invasions into the left branch of hepatic portal vein, and metastasis in the hepatic hilar lymph node. This patient was ineligible for TACE and was allergic to the somatostatin analogue octreotide. This limited treatment options to supportive therapies such as albumin supplementation for liver protection. Patient one and two died at 61 and 109 days, respectively, following initial hospital admission. Conclusions We diagnosed both cases with poorly differentiated grade 3 small-cell PHNEC through imaging characteristics, immunohistochemical staining of liver biopsies, and examinations to eliminate non-hepatic origins. Neither TACE nor liver protection appeared to significantly extend survival time of the two patients, suggesting these treatments may be inadequate to improve survival of patients with poorly differentiated grade 3 small-cell PHNEC. The prognosis of poorly differentiated grade 3 small-cell PHNEC is poor due to limited and ineffective treatment options.
Neuroendocrine Lesions Arising From Mediastinal Teratoma-A Case Report and Literature Review

Abstract:Background. Neuroendocrine lesions arising from mediastinal teratomas are rare tumors with only small number of patients reported in literature. The behavior of these lesions appears to be different from traditional neuroendocrine neoplasms. A comprehensive review will be valuable for histologic assessment and treatment planning for similar cases. Case presentation. We present an example of a 57-year-old man who presented with cough. Subsequent work-up revealed an anterior mediastinal mass of 2.1 cm on computed tomography. The patient underwent robot-assisted thoracoscopic thymectomy. Histological examination revealed a mature cystic teratoma with a neuroendocrine component consisting of clusters of tumor cells with round to oval nuclei and a "salt-and-pepper" chromatin pattern. The tumor cells were immunoreactive to cytokeratin, synaptophysin, chromogranin, and INSM1, with a Ki-67 proliferative index of 4%. A histological diagnosis was mature teratoma with well-differentiated low-grade neuroendocrine tumor (carcinoid) was made. The patient was well and without disease after complete surgical excision at 10 months. Literature review. Literature reviewed yielded 13 examples of neuroendocrine lesions arising from mediastinal teratomas. No disease-related mortality was reported, even in lesions with high-grade neuroendocrine, carcinomatous, or immature teratomatous components. Conclusions. Surgical removal is the mainstay of treatment of these lesions, and the presence of a neuroendocrine component does not appear to negatively affect prognosis.
Retroperitoneal Primary Mucinous Adenocarcinoma: A Case Report

Abstract:Retroperitoneal primary mucinous adenocarcinoma (RPMA) is extremely rare and the histogenesis of this tumor remains unclear. There is no consensus on the appropriate treatment for RPMA. Surgical resection is standard for the treatment of RPMA, whereas the benefits of chemotherapy with regard to this tumor remain to be established. This case report concerns a 21-year-old female individual with RPMA. The patient initially presented with chronic lower back pain and weight loss. Additionally, carcinoembryonic antigen (CEA) levels were found to be elevated. A computed tomography scan revealed a mass in the abdominal cavity. Consequently, laparotomy was performed, which revealed a well-defined tumor in the right retroperitoneum. Chemotherapy as a monotherapy was not considered as a viable treatment option. Therefore, the patient was initially administered a combined treatement of oxaliplatin and 5-fluorouracil. This treatment was then changed to paclitaxel and 5-fluorouracil. Findings showed a decrease in the CEA serum levels, indicating that this combination treatment may be efficacious in the treatment of RPMA since local recurrence following surgical resection was well controlled with chemotherapy.
