Use of tissue inhibitors of metalloproteinase-1 (TIMP-1) as non-invasive diagnostic marker in hepatic fibrosis of chronic viral hepatitis

Abstract:Objective To evaluate the value of TIMP 1 in the diagnosis of liver cirrhosis. Methods To detect the serum TIMP 1 concentration in 142 chronic hepatitis patients and compare with the results of serum biochemical, immunological, imaging diagnostic markers and histologic features of liver biopsy, in order to determine TIMP 1 as an useful index in the diagnosis of liver fibrosis.Results TIMP 1 was positively correlated with serum ALT,AST,GGT,ALP,SB,HA and ApoAl and negatively correlated with serum albumin, but not correlated with PGA, PGAA scores. As regards to viral markers (HBsAg, anti HBs, anti HBc, HBcAg, anti HBe HBVDNA, anti HCV) and serum lipids (TG,TC,HDL,LDL), TIMP 1 was not correlated with either of them. Serum TIMP 1 was correlated with CD3, CD8, NK and lgG ( P 0.05). On relative analysis to B ultrasound, we found it was correlated with thickness of gallbladder wall, shape of gallbladder and thickness of spleen ( P 0.05). As with CT/MRI, it was correlated with thickness of splenn ( P 0.01). In relative analysis of serum TIMP 1 concentration and liver histologic features. TIMP 1 was significantly correlated with intrahepatic inflammation and fibrosis. The accuracy of serum TIMP 1 in the production of hepatic inflammation, fibrosis, absence or presence of liver cirrhosis was 72.54%, 40.85% and 81.25% respectively.Conclusion Serum TIMP 1 level can reflect its improtant changes of intrahepatic inflammation and fibrosis and is an useful index for diagnosis and follow up study of hepatic fibrosis.
