Abstract:Objective To investigate the resorption of the bone grafts in the patients who experienced mandibular reconstruction with free autogenous iliac bone.Methods In this article,26 out of 84 patients were followed up.The heights of the bone grafts on the panoramic radiographs were measured for study.Results The bone grafts were gradually absorbed after operation.The grafts were most quickly resorpted during the first 6 months,and then become slowly resorpted until one year after operation.The distal area of the graft was resorpted much more than the proximal area.In some of the cases,the grafts for reconstruction of ramus and condyle could reduce in height up to 50%.While in the patients whose condyles were reserved during mandibulectomy,the reconstructed ramuses heights reduced only 8.2%.Conclusion It is very beneficial for reconstruction of the mandible to preserve the condyle if possible.
Precise placement and rigid fixation of free iliac bone graft in reconstruction of mandibular defects

Abstract:Objective　To investigate the precise placement and rigid fixation of free iliac bone graft in reconstruction of mandibular defects.Methods　The mandibular defects of 46 patients were devided into 4 types according to the extent.Referring to the templates,iliac bone grafts were cut into segments and shaped.Concerning the location and feature of every type of defects,the method of rigid inernal fixation and the number of screws were designed.Results　All patients were satisfied with their final esthetic results.Infection rate was 21.7% in total.Recurrence rate of tumors was 2%.Callus formation was shown by X-ray films in 3～6 months after surgery.Conventional rehabiliation rate was 13%,implant-supported denture rate 8.7%.Conclusion　Application of template and prepared-fixed reconstruction plate make the grafted bone placed precisely.The most stable internal fixation could be chosen based on the stress distribution of every type of defects.A satisfied final functional and esthetic result of free avascular iliac bone graft in mandibular reconstruction with a low infection rate could be obtained.
Changes of Bone Height after Vascularized Fibular Bone Graft in Mandibular Reconstruction

Abstract:OBJECTIVE:To investigate the changes of vascularized fibular bone height after transplantation for mandibular reconstruction.METHODS:Thirty-two patients with mandibular defects were reconstructed with vascularized fibular bone. Postoperative follow-up pantomography was used to evaluate the changes of bone height.RESULTS:At different distance from the vessel pedicles, the changes of bone height showed no significant difference. The height of fibular bone decreased gradually with time and reduced by 6.64% at 1 year postoperatively.CONCLUSIONS:Multiple segmental osteotomies had no effect on blood flow and the different pieces of graft after modeling by osteotomy experienced the similar reconstruction. The volume of grafted fibular bone almost remained stable.
Reconstruction of Mandibular Defects Using Non-Vascularized Iliac Bone Grafts with Placement of Screw-Type Dental Implants

Abstract:Objective To investigate the method and asses the outcomes of reconstruction of mandibular defects using non-vascularized iliac bone garfts with placement dental implants.Methods Fifteen patients with 38 dental implants were researched.The outcomes were observed and X-ray was taken,then the chewing rate was measured. Results A good stability for dental implants placed in non-vascularized iliac bone grafts was achieved.But 6 months after openition,2 cases of greater defect showed an irregular absorption in interface.After mandibles were reconstructed,the chewing rate could reach as much as normal.The result showed implants and non-vascularized iliac bone grafts could be used for mandibular reconstruction to recover the chewing functionally.Conclusion It can be used to reconstruct functionally mandibular defects using non-vascularized iliac bone grafts with placementdental implants,but a wide range of mandibular defect is not fit for reconstruction by the way.

Abstract:Abstract Background Implant-supported dentures have become an essential means of restoring occlusal function after jaw reconstruction. Bone mineral density (BMD) may influence the success rate of implant denture restorations. This study aimed to explore whether the Hounsfield unit (HU) value can be used to monitor the changing trend of fibular BMD after jaw reconstruction. Results A total of 54 patients who underwent maxillar/mandibular reconstruction with a fibula flap were included in this study. There was a significant correlation between the HU value and BMD at 1 week, 3 months, and 6 months after surgery, and both were significantly correlated with follow-up time. The difference between each pair of absorption rates (DAR) was less than 10% in 66.7% and 75.9% of patients at 3 and 6 months; however, the DAR was more than 20% in 12% and 13.8% of patients at 3 and 6 months, respectively. Conclusions There is a significant correlation between HU value and BMD. The HU value can be used to roughly reflect the fibular BMD changing trend in a group of patients as opposed to an individual, and the HU value is not equivalent to BMD. Abstract:The biomechanical effects of the reconstructed mandible with autogenous bone grafts were analyzed and evaluated, under simulated mechanical environment of occlusion. Based on anatomical feature of the human skeleton, muscle system and the clinical B, BS"S" and RB"S"defect patterns, the modularized model of mandible and the three finite element models of the fibula grafts and iliac crest grafts reconstruction mandible were made. The stress state calculations under bite force were also processed. The findings are as follows: Along the right connection area to left connection area (corresponding to far-end of the molar teeth alveolar ridge and the chin section respectively), the stress state on the various grafted bone all changes from compression stress to tensile stress. There is obvious change on the stress distribution between the fibula type reconstruction mandible and normal one. The stress on the grafted bone which is mainly made up of cortical bone is approximately 5 times higher than that of normal, which indicates the obvious stress shielding effect; the stress distribution of the iliac crest type reconstruction mandible is close to the normal mandible, the iliac crest used is mainly made up of cancellous. Therefore, the mechanical properties of the reconstructed mandible with iliac crest are more similar to normal mandible, which is beneficial to wound healing and further functional reconstruction.
Three-dimensional Finite-Element Analysis Investigating the Biomechanical Effects of Human Mandibular Reconstruction with Autogenous Bone Grafts

Abstract:Purpose: To investigate the biomechanics of the mandible following reconstruction with autogenous bone grafts. Material: Computerized tomography scan images of a human mandible, fibula and iliac crest were collected and used to build models on a PC. Methods: Four finite-element analysis (FEA) models of mandibles reconstructed with autogenous bone were created. The principal stresses of marked points, the Von Mises stresses at anatomical index regions, and the force values of temporo-mandibular joints and masticatory muscles were calculated. Results: Compared with the normal mandible, the one repaired with a fibula had greater Von Mises stresses on the grafted bone; the one repaired with iliac crest bone had the similar distribution of the Von Mises stresses as in the normal mandible. The principal stresses in the autograft varied between tensile and compressive stresses from the right graft/bone binding interface to the left in all reconstructed mandibles. On the whole, the maximum Von Mises stress was greater on the mandible reconstructed with fibula than that reconstructed with iliac crest. Conclusion: Mandibles repaired with iliac crest grafts have more mechanical properties similar to normal than those repaired with fibula grafts. Abstract:PURPOSE Vascularized iliac bone graft combined implantation has become a routine procedure in the functional reconstruction of mandibular defects. The aim of this study was to assess the clinical outcomes of the implants immediately placed into the vascularized iliac bone grafts. METHODS In this study, 12 consecutive patients with 36 ITI dental implants who had reconstruction of mandibular defect between 2000 and 2004 were presented. All implants were inserted immediately after bone grafting and loaded after 3 to 5 months of submerged healing. RESULTS In general, a primary stability for implants placed in vascularized iliac bone grafts were achieved. After 1 year of observation the mean vertical bone loss was less than 1mm. There was no implant failure in the observation period. CONCLUSION The vascularized iliac bone grafts are safe implant-bearing areas and allows a reliable and predictable restoration with dental implants. Implant-supported bridges and overdentures can be used in this procedure.
Preventing Early-Stage Graft Bone Resorption by Simultaneous Innervation: Innervated Iliac Bone Flap for Mandibular Reconstruction

Abstract:Background: Postoperative resorption of vascularized bone grafts jeopardizes the success of dental implant(s) and functional rehabilitation of the jaw. Recent evidence supports the crucial role of innervation in bone regeneration and turnover. Methods: This study reports a new technique for simultaneous innervation of vascularized iliac flaps in mandibular reconstruction, through neurorrhaphy between ilioinguinal nerves, which innervate iliac bone, and inferior alveolar nerves or great auricular nerves. Twenty-two patients (aged 50 to 69 years) with postoncologic continuity defects of the mandible underwent mandibular reconstruction (10 innervated flaps and 12 control flaps). Graft bone resorption was analyzed by computed tomographic scans at 6 and 12 months postoperatively, and bone quality was evaluated for dental implantation, with histologic and histomorphometric analyses for graft samples. Results: At 12-month follow-up, graft bone density loss in the control group was significantly higher than in the innervated group (p < 0.05). Bone quality evaluation indicated a suitable condition for dental implantation in all patients in the innervated group but in 41.7 percent of patients in the control group. Histologic and histomorphometric analyses showed successful innervation in the innervated group but not in the control group. Osteoclast activity was significantly higher in the control group than in the innervated group (p < 0.05). Conclusions: Innervated iliac flaps may effectively prevent bone resorption of grafts in mandible reconstruction that otherwise jeopardize the success of dental implants. This new strategy of innervation of bone flaps appears clinically valuable and provides insights into the homeostasis of grafts for functional reconstruction. Abstract:Objective:To observe the bone growth and osseointegration during healing process after autogenous nonvascular iliac bone graft and simultaneous implantation and supply proof of this type surgery in clinic by investigating the different results in animal experiment. Methods: Sixteen healthy and white adult male Japanese rabbits, with iliac bone loss imposed and bilateral iliac bone isolated, and then self-made column hydroxide phosphorite implanted simultaneously in the bilateral area of iliac bone with and without autogenous nonvascular bone graft, were randomly divided into 4 groups and four rabbits in each group. the rabbits in every groups was sacrificed at 2,4,8,12 weeks postoperation respectively and the dissected samples were used for general and histological observation, electron microscope scanning and reverse pushing out tests. Results: Implants were connected tightly with iliac bone and the implants weren't flexible in every group from general observation. Osseointegration between the implants and the iliac bone can be found and a thin fibrous tissue was observed only in some sample areas of two-week and four-week autogenous nonvascular iliac bone graft group by the naked eyes. Radial examination shows: There is no obvious x-ray transmission area around the implant but in two-week group . The density of grafting bone area is a bit lower than that of non- grafting area at two-week and four-week groups while at eight-week? twelve week group there is basically no difference in both areas. Histological and scanning electron microscope examination shows: Implants in grafting bone and non-grafting bone group are both osseointegrated with bone while osseointegration in non-graftting bone group is much better. Along with time, osseointegration in every group is much better in turn. Mechanics test (reverse pushing out test) shows that the maximum load of grafting bone group in four, eight and twelve weeks is 66.3 N/cm2?143.9N/cm2?194.6 N/cm2?248.3 N/cm2?respectively while it is 108.6 N/cm2?229.2 N/cm2?307.1 N/cm2?377.6 N/cm2 in non-graftting bone group. Conclusions: The method of autogenous nonvascular iliac bone graft and simultaneous implantation is an effective way to repair the bone difficiency.

Abstract:Objective: To evaluate the surgical methods and treatment effects of reconstruction of bone defects for dental implants with onlay bone grafts from intro-oral donor. Methods: 18 consecutive patients underwent onlay bone grafting from autogenous mandible. 22 implants were inserted 4-6 months later or simultaneously. The implants were exposed and final porcelain restorations were made 4-6 months later. Results: There were no evident complications happened. 2 cases showed evident bone resorption. The average bone resorption rate was 20 percent when inserting implant. The final restorations were made for all the cases. There was no implant defeated after 6-28 months' observation. Conclusion: The onlay bone grafting technique is simple and reliable for the restoration of severely atrophic alveolar crest.
Volumetric stability of autogenous bone graft with mandibular body bone: cone-beam computed tomography and three-dimensional reconstruction analysis

Abstract:OBJECTIVES: The purpose of this study was to estimate the volumetric change of augmented autobone harvested from mandibular body cortical bone, using cone-beam computed tomography (CBCT) and three-dimensional reconstruction. In addition, the clinical success of dental implants placed 4 to 6 months after bone grafting was also evaluated.MATERIALS AND METHODS: Ninety-five patients (48 men and 47 women) aged 19 to 72 years were included in this study. A total of 128 graft sites were evaluated. The graft sites were divided into three parts: anterior and both posterior regions of one jaw. All patients included in the study were scheduled for an onlay graft and implantation using a two-stage procedure. The dental implants were inserted 4 to 6 months after the bone graft. Volumetric stability was evaluated by serial CBCT images.RESULTS: No major complications were observed for the donor sites. A total of 128 block bones were used to augment severely resorbed alveolar bone. Only 1 of the 128 bone grafts was resorbed by more than half, and that was due to infection. In total, the average amount of residual grafted bone after resorption at the recipient sites was 74.6%±8.4%.CONCLUSION: Volumetric stability of mandibular body autogenous block grafts is predictable. The procedure is satisfactory for patients who want dental implants regardless of atrophic alveolar bone.

Abstract:The aim of this study was to evaluate the resorption of the iliac bone after maxillary reconstruction with a vascularized free iliac flap. Twenty-seven patients with maxillary defects who underwent maxillary reconstruction with the vascularized free iliac flap between January 2017 and January 2021 were included. Computed tomography (CT) images taken at 1 week, approximately 6 months, and 1 year after the surgery were used for evaluation. The total iliac bone thickness and height, cortical bone thickness, and cancellous bone density were measured in the CT images. Compared with 1 week after the surgery, the total thickness and height of the iliac bone were reduced significantly 1 year after the surgery, and the cortical bone thickness and cancellous bone density were reduced significantly at 6 months and 1 year after the surgery. Compared with 6 months after the surgery, cancellous bone density was reduced significantly 1 year after the surgery. In conclusion, during the first year after maxillary reconstruction with a vascularized free iliac flap, there was significant resorption of iliac bone, including the total iliac bone thickness and height, the cortical bone thickness, and the cancellous bone density.
Long-Term Stability of Autogenous Bone Grafts Following Combined Application with Guided Bone Regeneration

Abstract:The aim of the study was to compare the long-term stability of membranous and endochondral autogenous bone grafts with or without combined application of guided bone regeneration (GBR). Twenty-five, male, 6-month old, albino rats were used in the study. The animals were divided into four groups (A5, A11, B5 and B11). Group A5 (control): The inferior border of the mandible was exposed in both sides. At one side of the jaw, a calvarial bone graft (baseline -3 x 4 x 0.64 mm) was placed at the inferior border of the mandible and was fixed with a standardized screw-type titanium microimplant. At the contralateral side, an ischiac bone graft (baseline -3 x 4 x 0.87) was transplanted. The healing period was 5 months. Group A11 (control): The animals were treated in the same manner as in Group A5 with the difference that the healing period was 11 months. Group B5 (test): The animals were treated in the same manner as in Group A5 with the difference that an e-PTFE membrane was adapted over the bone graft on each side of the jaw. Group B11 (test): The animals were treated in the same manner as in Group B5 with the difference that 5 months following transplantation the animals were subjected to a second operation and the membranes were removed. The healing period was 11 months. The animals were killed at 5 (Groups A5 and B5) or at 11 months (Groups A11 and B11) following mandibular augmentation and the jaws were defleshed. The width, the length and the thickness/height of the bone graft were evaluated by means of a stereomicroscope. At 5 months, both types of the membrane-treated bone grafts presented increase in all dimensions compared with baseline. However at 11 months, both types of the membrane-treated bone grafts exhibited a decrease in their dimensions which were similar to the baseline measurements. In the control groups, both types of bone graft presented significant resorption both at 5 and at 11 months with the ischiac bone grafts presenting more resorption in width and length than the calvarial bone grafts. It can be concluded that the long-term volume stability of autogenous endochondral and membranous onlay bone grafts combined with GBR is superior to that of autogenous endochondral and membranous onlay bone grafts alone.
Dental Implant Performance in Vertically Distracted Fibular Grafts after Mandibular Reconstruction: A Pilot Series of 12 Patients

Abstract:PURPOSE:To clinically and radiographically evaluate the performance of loaded dental implants placed in vertically distracted fibular grafts after mandibular reconstruction, and to evaluate patient satisfaction with implant function and esthetics after oral rehabilitation.MATERIALS AND METHODS:This retrospective study involved patients who received implants in vertically distracted fibular bone after mandibular reconstruction between February 2006 and January 2008. Clinical assessments were performed 6 months after loading and annually thereafter. Patient satisfaction was evaluated with respect to function, phonetics, facial contour esthetics, and oral hygiene practices. The data were evaluated statistically.RESULTS:Thirty-seven dental implants were inserted in 12 patients (mean age, 42.9 ± 8.0 years). Three patients were completely edentulous and nine were partially edentulous. The low mean plaque score (< 20%) indicated a good level of oral hygiene during the mean follow-up period of 42.1 ± 4.1 months. Probing depths and modified Sulcus Bleeding Index increased slightly at follow-up, but there was no significant difference in either parameter at years 1 and 3. After 1 year, the average marginal bone loss was 0.8 ± 0.3 mm (range, 0.2 to 2.1 mm); this increased to 0.9 ± 0.4 mm (range, 0.5 to 2.4 mm) in year 3, which was not a significant difference. The cumulative survival rate was 100% and the cumulative success rate was 84.8%. Seven of the 12 patients were fully satisfied with the function and esthetics of the restoration.CONCLUSIONS:On the basis of the present study, implant placement in vertically distracted fibulae for mandibular reconstruction may be considered a predictable option in terms of mid- to long-term implant survival and success rates.

Abstract:Objective:To experimentally and clinically study the effectiveness of autogenous monvascular iliac bone graft and simultaneous implantation without guiding membrane.Methods:In animal experiments,osteointegration strength was measured between autogenous nonvascular iliac bone graft and simultaneous implantation without guiding menbrane.Furthermore,its effectiveness of clinical application was observed.Results:In every animal groups,the osteointegration strength increased with time,and the clinical application is satisfied.Conclusions:The method of autogenous nonvascular iliac bone graft and simultaneous implantation without guiding membrane is clinically practicable.
Nonvascularized bone grafts: how successful are they in reconstruction of segmental mandibular defects?

Abstract:Objective: Segmental mandibular defects can occur due to various etiologies, including trauma and tumor resection. Reconstruction should provide adequate support for subsequent dental rehabilitation and allow for proper occlusion. Nonvascularized bone grafts have been used for reconstructing mandibular defects in cases where vascularized grafts were not feasible. The objective of this study was to assess the success rate of these grafts in reconstruction of segmental defects of various sizes in the mandible. Study design: Fifty patients were included in this retrospective chart review. Length of the grafts varied from 3 to 20 cm and patients were followed up from 4 to 80 months. Fifteen grafts were harvested from anterior iliac crest, 23 from posterior iliac crest, 9 grafts were a combination of either with costochondral graft, and 3 were solely allografts. Bone morphogenetic protein was utilized in 41 cases as an adjunct. Results: Success was defined as continuity of bone clinically and radiographically at a 4-month follow-up. Nonvascularized bone grafting was successful in 90% of cases. Complications were observed in 34% of cases, of which the most common were infection followed by wound dehiscence. Conclusions: Our study demonstrated substantial success rate with nonvascularized bone grafts in reconstruction of segmental mandibular defects.
A Retrospective Analysis of the Factors Affecting Implant Retention Rate after Mandibular Reconstruction

Abstract:Objective To explore the factors affecting implant retention rate after mandibular reconstruction with fibula bone flap and provide theoretical direction for clinical therapy. Methods From January 2015 to December 2017, a total of 20 cases (male 12cases, female 8 cases,20 to 45 years old, average 27.2 years old) who received dental implantation after mandibular reconstruction were retrospectively analyzed. The data such as the status of keratinized gingiva, the thickness of peri-implant soft tissue and the height of bone graft were collected, and were all compared with the marginal bone loss (MBL). Results Of all the 20 patients, keratinized gingival loss was observed in 12 cases bilaterally, 5 cases unilaterally, and preserved in 3 cases. Three months, 6 months and 12 months after the reconstruction, the thickness of soft tissue were (5.88±0.54)mm,(3.46±0.30)mm,(3.17±0.31)mm,separately;and the height of bone graft were(2.36±0.23)mm,(2.17±0.19)mm, (2.05±0.22)mm,separately.Three months,6 months and 12 months after the implantation,the mesial MBL were(1.09±0.32)mm, (1.22±0.37) mm, (1.36±0.46) mm, separately;and the distal MBL were (1.10±0.29) mm, (1.25±0.31) mm, (1.35±0.39) mm, separately.Statistical analysis revealed that the keratinized gingiva and thickness of soft tissue were both positively correlated with the MBL,and the height of bone graft was inversely correlated with the MBL.Conclusion For the oral implant patients after mandibular reconstruction,the height of the bone graft should be increased and the thickness of soft tissue should be reduced.And keratinized gingiva should be preserved as much as possible if oncological safety is allowed during mandibular resection.
Simultaneous Innervation Revitalizes Bone Marrow Mesenchymal Stem Cells from Postoperative Iliac Grafts

Abstract:PURPOSE：To compare the osteogenic potential of bone marrow mesenchymal stem cells (BMMSCs) from innervated iliac graft bone flap and traditional one for reconstructing mandibular defects. METHODS：Graft bone marrow samples were harvested 1 year after free vascularized iliac reconstruction of mandibular defects, with or without innervation through simultaneous nerve anastomosis. BMMSCs were isolated and cultured in vitro. Colony forming units-fibrosis observation, Brdu incorporation assay, population doubling, Alizarin red staining for in vitro calcified nodule formation and in vivo assay of subcutaneous osteogenesis in nude mice were used to detect BMMSCs proliferation, self-renewal and osteogenic differentiation capabilities, respectively. SPSS 24.0 software package was used for statistical analysis. RESULTS：Colony formation, proliferation, population doubling and osteogenic differentiation abilities of BMMSCs from innervated group were significantly higher than those from non-innervated group（P<0.05）. CONCLUSIONS：Simultaneous innervation of free vascularized iliac during reconstruction of mandibular defects may maintain self-renewal and osteogenic differentiation potentials of BMMSCs in graft bones, thereby maintaining bone homeostasis and reducing postoperative graft bone resorption.
