The study on the distribution of interleukin-1 alpha in herniated intervertebral disc, and the possible link to sciatic pain

Abstract:Objective: To study the distribution of IL-1α in different parts of the herniated intervertebral disc, and to determine the role of IL-1α in the mechanism of sciatic pain in lumbar disc herniation. Methods: All of samples of herniated lumbar discs were obtained from 30 adult patients operated on for primary transluminar microendscopic discectomy (MED) because of persistent radicular symptoms and subdivided into three samples: (1)The nearest to the nerve root in close contact with it (A group), (2)The sample obtained from the intervertebral space (B group), (3) The sample obtained from the vertebral plates (C group). In the present study, the specimens were analyzed immunohistochemically using monoclonal murine antibody against human IL-1α for the presence of IL-1α. As a control group, 6 lumbar disc specimens were harvested from 4 patients suffering from acute bursting fracture of lumbar spine were excised and analyzed by the same procedure. Results: Of 3o herniated lumbar discs, IL-1α positively expressing cells were presented in 25 samples in the A group. In the C group, IL-1α positively, expressing cells were presented in 21 samples. These staining cells were mainly chondrocyte and fibroblast. No positively expressing cells were detected in the normal lumbar discs. IL-1α positively expressing cells of the A group were most in 3 group (P < 0.05). Conclusion: Herniated lumbar disc cells can produce IL-1α. The distribution of IL-1α was not consistent in different parts of the herniated intervertebral disc. IL-1α may play an important role in the mechanism of sciatic pain.
