The relationship between coronary blood flow reserve and decreased insulin sensitivity in patients with hypertensive left ventricular hypertrophy

Abstract:Aim: To study the relationship between coronary blood flow reserve (CFR) and decreased insulin sensitivity (DIS) in patients with hypertensive left ventricular hypertrophy (LVH). Methods: CFR of normal subjects (NS, n = 20), hypertensive non-LVH patients (HT+non-LVH, n = 32) and hypertensive patients with LVH (HT+LVH, n = 33) were studied with transesophageal Doppler echocardiography (TEE) technique (Iliceto's method). The ratio of dipyridamole to rest peak diastolic velocity (D/R PDV) and dipyridamole to rest peak systolic velocity (D/R PSV) were considered as indices of CFR. Insulin sensitivity (IS) was quantified using euglycemic insulin clamp technique (EICT). Results: It was found that (1) D/R PDV and D/R PSV were significantly less in HT+LVH group versus group NS and group HT+non-LVH (1.74 �� 0.37, 1.67 �� 0.41 versus 2.66 �� 0.58, .30 �� 0.37 and 2.56 �� 0.46, 2.45 �� 0.48 respectively, P<0.01), with no difference of D/R PDV and D/R PSV between group NS and HT+non-LVH (2.66 �� 0.58, 2.30 �� 0.37 versus 2.56 �� 0.46, 2.45 �� 0.48, P>0.05). (2) The incidence of impaired CFR was higher in hypertensive patients with DIS (77.8%) compared to hypertensive patients with normal insulin sensitivity (NIS) (20.7%). (3) The most significant reduction in CFR was found in patients with hypertensive LVH accompanied with DIS (D/R PDV: 1.60 �� 0.27 versus 2.19 �� 0.35, 2.29 �� 0.43; D/R PSV: 1.54 �� 0.29 versus 2.18 �� 0.37, 2.19 �� 0.45, respectively, P<0.05), with no significant differences of impaired CFR between patients with hypertensive LVH with NIS and hypertensive patients with non-LVH and DIS (P>0.05). Conclusion: The above data showed that LVH and DIS are the two additive risk factors for impaired CFR in patients with hypertension.
