Helicobacter pylori antibodies in relation to precancerous gastric lesions in the high-risk Chinese population

Abstract:Helicobacter pylori (HP) serum IgG antibody was assayed among 2,940 adults aged 35-64 yrs. in a population-based gastroscopic screening survey in an area of China with one of the world's highest incidence rates of stomach cancer (SC). About half of the population was HP antibody positive. HP antibody positivity was lower among those who drink water from deep wells or running water sources, and the positivity prevalences varied notably with gastric pathology. The percentage of positivity increased from 19% to 40% to 63% in individuals with superficial gastritis (SG), mild chronic atrophic gastritis (CAG) and severe CAG respectively, and ranged 50%-54% among those with intestinal metaplasia (IM), dysplasia or SC. HP antibody positivity also was strongly correlated with serum pepsinogens (PG) II concentrations. but HP status did not markedly improve serologic identification of advanced gatric precancerous lesions than that provided by PG I/II ratios alone. The findings from this asymptomatic high-risk population suggest that HP infection probably contributes to gastric carcinogenesis in early stages, influencing transition from SG to mild and then severe CAG.
