Abstract:BACKGROUND: VTE has emerged as a major public health problem. However, data on VTE burden in China are seldom reported. METHODS: This study collected data on patients with a principal diagnosis of VTE, pulmonary embolism (PE), or DVT by using the International Classification of Diseases, 10th Revision, from 90 hospitals across China. The trends in hospitalization rates, mortality, length of stay (LOS), and comorbidities from 2007 to 2016 were analyzed. RESULTS: In total, 105,723 patients with VTE were identified. For patients with VTE, the age-and sex-adjusted hospitalization rate increased from 3.2 to 17.5 per 100,000 population, and in-hospital mortality decreased from 4.7% to 2.1% (P < .001). The mean LOS declined from 14 to 11 days (P < .001). In addition, the data in 2016 showed that the hospitalization rate of VTE was higher in elderly male patients (male patients vs female patients, 155.3 vs 125.4 per 100,000 population in patients aged >= 85 years; P < .001) and in northern China (north vs south, 18.4 vs 13.4 per 100,000 population; P < .001). Higher mortality rates were found in patients with cancer and Charlson Comorbidity Index scores > 2. Similar trends were also observed in patients with PE and those with DVT. The hospitalization rate in China was much lower than that of the United States or selected sites in Canada and Europe, the LOS was much longer, and the in-hospital mortality rates were similar. CONCLUSIONS: The hospitalization rates of VTE increased steadily, and the mortality declined. This study provides important information on the disease burden of VTE in China.

Abstract:The incidence rate of venous thromboembolism (VTE) has been increasing in China. However, the real-world treatment patterns for VTE patients is not well understood. This study aimed to assess patient characteristics and treatment pattern among hospitalized patients with a diagnosis of deep vein thrombosis (DVT) or pulmonary embolism (PE) in China. A large electronic medical record database from three tertiary hospitals in three metropolitan cities was used in this study. Hospitalizations with any diagnosis of DVT or PE from 2009 to 2014 were included. Patient demographics, clinical characteristics, and treatment patterns with anticoagulants and antiplatelets in these hospitalizations were described. A total of 1,169 DVT and 324 PE related hospitalizations were included. The DVT sample had a mean age of 56, 40.8% women, 47.0% provoked VTE, and 15.1% critical/emergent admissions. The PE sample had a mean age of 63, 45.7% women, 41.0% provoked VTE and 42.6% critical/emergent admissions. The most common comorbidities included hypertension, cerebrovascular disease, diabetes mellitus and heart failure (26.3%, 17.1%, 10.0% and 5.0% for DVT and 38.9%, 17.3%, 11.1% and 17.9% for PE). 24.0% of DVT related hospitalizations received anticoagulant therapy only, 13.4% had antiplatelet only, 43.8% had both, while 18.8% had neither. The corresponding numbers for PE were 38.3%, 3.1%, 50.9% and 7.7%, respectively. The most commonly used anticoagulant agent was low molecular weight heparin or fondaparinux (LMWH/F), accounting for 73.8% and 81.7% of anticoagulant use for DVT and PE, followed by unfractionated heparin (55.7% and 70.9%) and warfarin (35.6% and 63.3%). Among anticoagulant users, 42.6% and 75.1% DVT and PE related hospitalizations, respectively, had more than one anticoagulant. Majority, but not all VTE patients received anticoagulants or antiplatelet revealing gaps in clinical practice among hospitalized patients with VTE in China. LMWH/F was the most commonly used anticoagulant in VTE-related hospitalizations.

Abstract:SESSION TITLE: Pulmonary Vascular Disease: VTE SESSION TYPE: Original Investigation Poster PRESENTED ON: Saturday, April 16, 2016 at 11:45 AM - 12:45 PM PURPOSE: To improve treatment and prognosis of pulmonary thromboembolism (PTE) through retrospective analysis of the medical treatment and clinical outcomes METHODS: Clinical data of 510 consecutive cases of PTE hospitalized at Shanghai Pulmonary Hospital and Tongji Hospital from January 1, 2004 to December 31, 2013 were retrospectively reviewed. The patients were divided into three groups based on different treatment, including anticoagulation alone group (n=418), systemic thrombolysis group (n=47) and catheter-directed therapy group (n=45). Then we analyzed and compared therapeutic effect, bleeding complication and recurrence of three groups respectively. RESULTS: The total efficacy rates of anticoagulation group, thrombolysis group and catheter-directed therapy group were 77.5%, 78.7% and 77.7%. Meanwhile the survival rates of them were 91.9%, 87.2% and 91.1%, respectively. There was no diference in overall efficacy and survival rates among these three methods (P>0.05). The hemorrhage rates of anticoagulation group, thrombolysis group and catheter-directed therapy group were 3.8%, 12.8%, and 11.1%. Furthermore, one patient died of cerebral hemorrhage in thrombolysis group, the hemorrhage rate of this group was higher than that of anticoagulation group, and the difference was statistically significant (P<0.05). For patients discharged, after 6-month follow-up and 12-month follow-up, the recurrence rate of anticoagulation group was higher than that of thrombolysis group and catheter-directed therapy group, respectively, but there was no statistically significant difference (P>0.05). CONCLUSIONS: Prompt treatment for PTE could improve clinical outcomes and reduce the risk of recurrence. For the best PTE prognosis, it is important to take the optimal treatment based on the patient’s clinical characteristics and risk stratification. CLINICAL IMPLICATIONS: The cliniacl research shows it is important to take the optimal treatment based on the patient’s clinical characteristics and risk stratification. Costs of illness for patients with venous thromboembolism in China

Abstract:Venous thromboembolism (VTE), a condition that includes deep vein thrombosis (DVT) and pulmonary embolism (PE) is associated with major morbidity and mortality and causes huge economic burden. The purpose of this study was to evaluate direct and indirect costs of patients with VTE in China, producing an average cost per patient per year. A cost-of-illness analyses was performed. The economic data was collected from an observational retrospective study. We recruited 154 patients diagnosed with VTE in Beijing, Shanghai and Guangzhou between October 2012 and December2012. Patients or their carers completed a questionnaire about resource utilization and absenteeism from work in the past year by an interview. Direct medical costs included outpatient visit, hospitalization,, ambulatory, drug, diagnostic tests, and physiotherapy costs. Indirect costs were estimated using a human capital approach. All costs referred to 2011. Among 154 patients, DVT accounted for 85.7%,51.95% were female and the mean age was 61.5±12.4. 96.8% patients have at least one kind of health insurance. From the societal perspective, total costs per patient over 1 year amounted to Chinese Yuan (CNY) 21484 (median: CNY14039, IQR: CNY6714-CNY32036). The direct medical costs were CNY15899 accounting for 74% of the total. The informal care costs were CNY3623 accounting for 16.9% of the total. And the indirect costs were CNY1678 accounting for 7.8%. The economic burden of VTE in China is considerable. The primary burden on patients was due to the direct medical costs.

Abstract:Background: In developed countries, hospitalized patients with acute medical conditions are at significant risk for venous thromboembolism (VTE). Little is known about VTE risk and prophylaxis practices in China. Objective: To determine the VTE risk and the frequency of recommended VTE prophylaxis in hospitalized Chinese patients with acute medical conditions. Methods: Multi-center, cross-sectional, observational study. Eligibility criteria: >= 30 years, admitted to an intensive care unit (ICU)/coronary care unit (CCU) for acute medical illness, had >= 1 VTE risk factor/1 disease that predisposes to VTE, and provided informed consent. We used 2004 American College of Chest Physicians (ACCP) evidence-based consensus guidelines to assess VTE risk and the frequency of recommended VTE prophylaxis. Results: 1247 patients from 19 hospitals in 11 cities across 11 provinces of China were enrolled from July 2007 to June 2008. 57.3% patients had >2 VTE risk factors. Only 20.2% received ACCP-recommended VTE prophylaxis (CCU patients: 22.7%, ICU patients: 16.9%, p=0.0117). Limitations: Excluding some patients with VTE risk factors did not allow assessment of the prevalence of VTE risk in the acute hospital-care setting. We could not determine whether the duration of prophylaxis complied with the ACCP recommendations. Our results may not be representative of hospitals in small cities/rural areas in China. Conclusions: The prevalence of VTE risk factors in Chinese patients was similar to that in developed countries; however, only a small proportion of eligible patients received the recommended VTE prophylaxis. Our findings highlight the need for dissemination and implementation of appropriate VTE prophylaxis guidelines in China. (C) 2010 Elsevier B.V. All rights reserved.

Abstract:Introduction Venous thromboembolism (VTE) is a frequent cause of hospitalization, mortality, and long-term adverse consequence among medical and surgical inpatients. The current study is performed to identify the prophylaxis of patients with VTE risk within the 6 weeks prior to the diagnosis of VTE, and the treatment pattern during 3 months after VTE diagnosis in Chinese patients hospitalized for surgeries or medical illness. Methods This multicenter, non-interventional, observational registry plans to enroll 1200 patients from 40 centers in China. The study will retrospectively collect data from patients’ hospitalization record within 6 weeks of VTE diagnosis and prospectively follow-up patients for 3 months (in four visits). The primary outcome is to determine the percentage of patients receiving adequate prophylaxis in patients hospitalized within 6 weeks before VTE diagnosis and treatment pattern within 3 months after VTE diagnosis. Important secondary endpoints include determining patients with risk of VTE in the hospital setting, risk factors for VTE, and cost analysis of VTE treatment. Expected Outcomes The findings will determine the characteristics of VTE, its treatment practices, cost of treatment, and quality of life in patients; this information may help in building diagnostic and prophylaxis strategies for VTE in China. The Real-World Treatment Status of Venous Thromboembolism in Underdeveloped Areas of Southwestern China

Abstract:Background: Little is known about the current management status of venous thromboembolism (VTE) in the underdeveloped areas of China, where it is the third most frequent cardiovascular disease. We aimed to investigate the status of anticoagulant use of VTE in southwestern China and assess the related factors of deep venous thrombosis (DVT)-complicated pulmonary embolism (PE).Methods: We extracted data from YiduCloud database from July 1999 to November 2018 and performed a cross-sectional survey including 36,812,353 medical records in seven medical centers. Demographics and laboratory tests were collected and analyzed.Findings: A total of 7353 patients with VTE (mean age 64·3±17·5 years) were included in the study and were divided into the DVT group (only DVT), PE group (only PE), and DVT-PE group (DVT complicated PE). A total of 4579 (62·3%) patients were given traditional anticoagulants, and only 6·5% reached the international normalized ratio (INR) therapeutic levels (2·0-3·0). Only 510 (6·9%) patients were treated with novel oral anticoagulants (NOACs), and 2264 (30·8%) patients were not administered any anticoagulants. Patients in the DVT-PE group (10·9%) used NOACs more than those in the DVT group (8·8%). Moreover, logistic analysis demonstrated that older patients who were male and had higher INR, lipoprotein a, alanine aminotransferase (ALT) and direct bilirubin (DBIL) had higher risk of complicated PE.Interpretation: Nearly one-third of the VTE patients did not receive any anticoagulant therapies, and NOACs were insufficiently used. Even in patients under traditional anticoagulants, INR was poorly controlled. Analysis of Hospitalization Costs for 278 Venous Thromboeboembolism Patients with Health Insurance in China

Abstract:OBJECTIVE:To determine the hospitalization costs and their predictors for venous thromboembolism (VTE) [includes deep vein thrombosis (DVT) and pulmonary embolism (PE)] patients with health insurance in China.METHODS:In the study, 278 patients with DVT or PE were randomly selected by stratified two-stage sampling from the China Basic Medical Insurance Databases between 2009 and 2010. All the information of the patients' demographic characters, length of hospital stay, clinical characters and costs were collected for the analysis. The descriptive statistics, univariate and multivariable analyses were used in the data analysis.RESULTS:All the 278 patients (mean age, 64.4 years; 58.3%, male) were evaluated, of whom, 61.9% were with DVT and 38.1% with PE. For all the VTE patients, the mean length-of-hospital stay was 15.8 days (15.0 days for DVT patients and 17.0 days for PE patients). The mean total cost was Chinese Yuan (CNY) 16 487.9 (median: 9 039.1), CNY 17 698.61 (median: 8 643.7) in patients with DVT and CNY 14 523.4 (median: 9 879.3) in patients with PE. The multiple linear regressions showed that the factors affecting hospitalization costs were length of hospital stay (P<0.001), region (P<0.05), hospital level (P<0.001), and type of anticoagulants (P<0.001).CONCLUSION:The costs per hospitalization associated with DVT or PE events are substantial, which implies that standard preventions for VTE patients can be cost-saving.
Risk factors of venous thromboembolism in Chinese hospitalized patients:a meta-analysis

Abstract:Objective The aim of this study was to analyze the main risk factors associated with venous thrombo-embolism (VTE) and provide a useful reference for the prophylaxis and treatment of VTE.Methods Seven databases were systematically reviewed from their inception to March 2017 for relevant case-control studies on risk factors of VTE.Six studies involving 3312 patients (VTE,1312 patients;control,2000 patients) were included in the meta-analysis.All analyses were performed using Reviewer Manager 5.3.Results The pooled odd ratio values and 95％ CI for the risk factors were:patient confined to bed＞72 h OR=2.69 (1.69-4.30);central venous access OR=4.88 (2.38-9.99);malignancy OR=3.09 (1.16-8.25);prior major surgery (＜1 month) OR=3.34 (1.27-8.79);heart failure OR=1.66 (1.25-2.19);severe lung disease OR=2.52 (1.82-3.50);age OR=1.84 (1.51-2.25).Conclusion Patient confined to bed＞72 h,central venous access,malignancy,prior major surgery (＜1month),heart failure,severe lung disease,age were the main risk factors of VTE in the hospitalized Chinese patients.

Abstract:AIM: This study aimed to investigate the trend of venous thromboembolism (VTE) in China over the past 10 years and assess the clinical application of inferior vena cava filters (IVCFs).METHODS: A survey designed to investigate the diagnosis and management of VTE, especially on the application of IVCFs, was distributed nationally from January 2009 to December 2019. Respondents were mainly designated medical professionals and were asked to fulfill 4 major and 61 minor items on the survey.RESULTS: A total of 53 medical centers, including 27 Radiological and 26 Vascular Surgery Centers, from 21 provinces in China participated in the study. These centers have diagnosed and treated 171,310 VTE patients, 49% (n = 83,969) were inpatients. Over a 10-year period, an increasing trend of VTE diagnosis and inpatient management was observed, namely by 3.8-fold and 4.8-fold respectively. Characteristics of the inpatients were as followed: 15% bilateral lower extremity deep vein thrombosis (LEDVT), 27% right LEDVT, and 58% left LEDVT. Anticoagulation therapies were unfractionated heparin (UFH) with vitamin K antagonists (VKAs) (8%), lower molecular weight heparin (LMWHs) with VKAs (21%), LMWHs with transition to rivaroxaban (34.2%), LMWHs with transition to dabigatran (2.4%), rivaroxaban alone (33.4%), and dabigatran alone (1.0%). Patients who remained on anticoagulation at 3, 6, 12, 24, and >24 months were 36%, 35%, 18%, 6.0%, and 5% respectively. In-hospital mortality for VTE patients was 3.2%, where DVT and pulmonary embolism (PE) was responsible for 5.2% mortality and DVT alone was 2.7%. Thrombolytic therapy was initiated in 39,046 (39,046/83,969, 46.5%) patients where 85% (n = 33,189/39,046) was catheter-directed thrombolysis (CDT) and evaluation of iliac vein using ultrasound and/or venography was performed in 76% (n = 63,816/83,969) patients. Urokinase was the main thrombolytic drug used (98%) and the remainder was recombinant tissue-type plasminogen activator (rt-PA). Complete and partial thrombolysis was achieved in 70% and 30% patients. Bleeding complications were observed in 3.5% patients, where 20% of the patients with bleeding complications required intervention. Between 2009 and 2019, a total of 40,478 IVCFs (76% retrievable) were implanted in hospitalized VTE patients. During the enrollment period, the total number of IVCFs implantation increased by 3.8-fold, with a 4.8-fold increase in retrievable IVCFs use and 7.5-fold decline in permanent IVCFs. The removal rate of retrievable IVCFs was 72%. After IVCF implantation, 94.8% patients received anticoagulation on an average period of 9.1 ± 8.6 months. Overall complication rate associated with IVCF placement was 15.5% (n = 6,274/40,478), including tilting (54%), vena cava thrombosis (26.1%), caval penetration (12.6%), and migration (7.3%). No IVCF placement-related mortality occurred.CONCLUSIONS: There has been a significant increase in VTE diagnosis in China over the past decade. Anticoagulation therapy was the mainstay treatment and CDT was widely used. Most IVCFs placed belonged to retrievable types while permanent ones had been largely discarded.

Practice and Discussion on Prevention and Treatment of Venous Thrombosis Based on Multidisciplinary Integration

Abstract:Objective To evaluate the effective e measures of the prevention and treatment of venous thrombosis in hospital and evaluate the effect of"antibolt work"in patients with multidisciplinary cooperation. Methods Compare the level of venous thromboembolism (VTE) incidence, average hospital day, patient satisfaction and the level of knowledge of medical staff in the high risk department of VTE. Results After intervention in high-risk departments staff VTE related knowledge level improved, 2013 high- risk departments medical staff about the basic knowledge, risk assessment, VTE prevention knowledge of three dimensional awareness were 34.7％ (26/75), 49.3％ (37/75), 32.0％ (24/75), respectively, 2016 were 56.0％(42/75), 69.3％(52/75), 90.7％(68/75), respectively, two groups compare the difference was statistically significant (χ2=6.887, 6.217, 54.423, P<0.01 or 0.05). Reduced incidence of VTE 2 groups, 2013 annual incidence of deep venous thrombosis (DVT) and pulmonary embolism (PE) were 31.7％ (38/120), 11.7％ (14/120), respectively, in 2016 the annual incidence of DVT and PE were 14.2％(17/120), 4.2％(5/120), respectively, two groups compare the difference was statistically significant (χ2 =10.402, 4.630, P<0.01 or 0.05). VTE in 2016 patients with high-risk departments the average hospitalization time significantly below 2013, 2013 was (18.3 ± 3.5) days while 2016 was (12.7 ± 2.9 )days, two groups compare the difference was statistically significant (t = 13.496, P < 0.01). In 2016, the satisfaction rate of patients with VTE was higher than that in 2013, 77.5％(93/120) in 2013, 90.8％(109/120) in 2016, and the difference between the two groups was statistically significant (χ2=8.004, P=0.005). Conclusion The prevention and control of venous thrombosis based on multi-disciplinary integration is effective in preventing the occurrence of thrombosis in hospitalized patients and is worthy of clinical promotion.

Abstract:Objective Venous thromboembolism(VTE) is a major complication of cancer and one of the leading causes of death in patients with cancer.We therefore assessed the incidence and therapeutic options of VTE in Chinese lung cancer patients.Methods We retrospectively evaluated the charts of 260 patients with lung cancer at our ward between September 2007 and October 2008 and found 15 patients with VTE.We analyzed the clinical manifestations,diagnostic methods,treatment options,and outcomes of these patients.Results Of 15 patients,the VTE of 12 patients were contributed by central venous catheters.One patient with PE was diagnosed by CT scan,and others with venous thrombosis were confirmed by ultrasound.The low-molecular-weight heparins(LMWHs) were administered to these patients,and satisfactory results were obtained.Conclusion VTE should be carefully detected in patients with lung cancer include early detection and treatment.LMWHs have demonstrated optimistic efficacy and safety profiles.

Abstract:Background Studies from the western population proved inflammatory bowel disease (IBD) a risk factor for venous thromboembolism (VTE) and thromboprophylaxis has become standard care for hospitalised IBD patients with disease flares. In Asia, lack of studies on VTE among IBD patients makes practice of thromboprophylaxis far from standardised. Our aim is to explore the incidence and risk factors for VTE among Chinese IBD patients. Methods Retrospective analysis of IBD patients from 17 Chinese tertiary referral hospitals between 2011 and 2016 was performed. Patients with IBD diagnosis were identified and searched for VTE diagnosis. Incidence density of VTE was calculated; a case-control study that included IBD patients with and without VTE was performed to analyse VTE related risk factors among Chinese IBD patients. Results A total of 8459 IBD patients and 46 patients with VTE were identified, yielding an incidence of 37.18/10000 person-years. Risk of VTE increased with advanced age (RR=1.05, 95%CI=1.02–1.08) and history of bowel resection (RR=3.52, 95%CI=1.45–8.52). Severe disease flare (Crohn’s disease activity index [CDAI]>450, mayo score >10), but not disease flares with less severity (CDAI >150, mayo >2), was an independent risk factor for VTE among inpatients (for Crohn’s disease: OR=11.68,95%CI=2.33–58.47, ulcerative colitis: OR=4.68, 95%CI=1.11–19.58). In addition, history of glucocorticoids use was a risk factor for patients with Crohn’s disease (OR=5.01, 95%CI=1.22–21.16), whereas history of 5-aminosalicylic acid use was a protective factor for patients with ulcerative colitis (OR=0.16, 95%CI=0.04–0.65). Conclusions The incidence of VTE among Chinese IBD patients is not lower than that from the western population. VTE related risk factors included age, severe disease flare, past history of surgery and glucocorticoids use, whereas past use of 5-aminosalicylic acid was protective against VTE for ulcerative colitis. Thromboprophylaxis should be considered for patients with these risk factors.
Prescribing Patterns of Antithrombotic Therapy for Lower Extremity Endovascular Procedures and Venous Thromboembolism Management in Mainland China

Abstract:Objective The use of antithrombotic agents in vascular disease was a common clinical practice despite a lack of clear convincing evidence. The goal of this study was to explore prescribing patterns of antithrombotic drugs among vascular surgeons in China. Methods Attendees at a national vascular surgery meeting (CEC2012 with an attendance of 1453 vascular physicians) were asked to complete a voluntary survey indicating their prescribing patterns of antithrombotic drugs in patients undergoing infrainguinal endovascular procedures or suffering venous thromboembolism (VTE). Results A total of 155 valid responses were gathered, of which 80.6% were submitted by vascular surgeons. 93.5% of the responses were from physicians practicing in tertiary hospitals covering 25 provinces in China. For VTE prophylaxis , 80.0% of respondents would prescribe anticoagulation to high-risk nonorthopedic surgical patients. For VTE therapy , 76.1% of responding physicians would utilize extended bed rest strategy,and 77.4% would prescribe anticoagulation, of whom 11.2% would treat the patients for 3 months. The hospitals where the respondents practice had median monthly infrainguinal endovascular procedures of 21 (interquartile 13~41)cases. The rates of initial postoperative use of single, dual or triple antithrombotic treatment were 8.4%,72.3% and 11.7%, respectively. Among the latter two treatments,39.2%of respondents would ultimately transfer to mono antiplatelet therapy. Finally, 40.3% of respondents would adopt a life-long antithrombotic strategy. Cilostazol (63.4%) and prostacyclin analogue (61.4%) were the most commonly used adjunctive drugs prescribed following endovascular procedures. Proactive interventions against smoking cessation , hyperlipidemia and diabetes were offered by 90.2%, 80.5% and 77.3% respondents, respectively. Conclusions For VTE management, antithrombotic agents are chosen properly by most vascular surgeons in China , but the treatment duration is highly variable. Following infrainguinal endovascular procedures, a relative high proportion of combined antithrombotic therapy and low proportion of life-long antithrombotic therapy are prescribed.

Abstract:e14686 Background: According to previous reports, 1% to 30% patients with malignant tumors developed venous thromboembolism (VTE) and an increased risk of VTE in hospitalized patients. However, real-world clinical analysis of VTE in Chinese hospitalized patients with malignant solid tumors is scarce. Methods: We utilized the collaborative Linkdoc database to retrospectively analyze hospitalized cancer patients in Department of Medical Oncology of the Fourth Hospital of Hebei Medical University from May 2014 to November 2018. Patients who were pathologically confirmed malignant solid tumor, ≥ 18 years of age, ECOG PS 0-4, median overall survival ≥ 1 months, and absence of serious diseases of vital organs such as heart, liver, kidney and blood system or the above chronic diseases in inactive period were included in this study. Results: A total of 3831 patients were enrolled, of whom 180 (4.70%) patients had VTE. The highest incidence of VTE in solid tumors was gynecological cancer (61/1288, 11.90%), followed by gastric cancer (42/701, 5.71%), other cancers (11/206, 5.33%), lung cancer (61/1288, 4.74%), colorectal cancer (18/399, 4.51%), breast cancer (34/903, 3.77%), and esophageal cancer (6/250, 2.40%). Of the enrolled patients, 21.11% (809/3831) underwent relevant imaging examinations to screen for VTE as required by the guidelines of cancer associated VTE. Based on the imaging examinations, the anticoagulant rates of patients with or without VTE were 100% (61/61) and 34.09% (255/748), respectively. Notably, only one patient received the anticoagulant therapy (both anticoagulant dose and time) in lines with the guidelines. Conclusions: This single-center real-world study showed the overall rates of VTE in hospitalized cancer patients was similar with the data previously reported. Referring to the guidelines of cancer associated VTE, the proportion of Chinese cancer patients receiving normative imaging examinations and anticoagulant therapy is seriously low currently, which should be paid more attention to in clinical practices by Chinese medical workers.

Abstract:Objective To investigate the prevention and treatment effect of Venous Thromboembolism（VTE） in Shanghai municipal tertiary general hospitals, and to provide reference for improving the prevention and management level of VTE in hospitals. Methods The data related to VTE prevention and treatment in 20 municipal tertiary general hospitals from the single disease database of Development Center of Shanghai Shenkang Hospital from 2019 to 2020 were retrospectively collected, and field research was performed on VTE prevention and control capacity. Results In 2020, the incidence of VTE in 20 municipal tertiary general hospitals in Shanghai was 1.00%, which was 14.94% higher than that in 2019（χ~2=115.26, P＜0.001）. The VTE risk assessment rate and bleeding risk assessment rate of inpatients were 71.06% and 25.96%, respectively, which increased by 16.13%（χ~2=28 754.00, P＜0.001） and 53.25%（χ~2=2 167.74, P＜0.001） compared with those in 2019. The implementation rate of all preventive measures among inpatients at risk of VTE was 34.12%, which was 33.02% higher than that in 2019（χ~2=22 790.00, P＜0.001）. The mortality rate of hospital-associated VTE was 5.35%, which was 21.55% lower than that in 2019（χ~2=3.91, P＜0.05）. More than 2/3 of hospitals had established a relatively complete VTE management system and carried out the construction of related disciplines. Conclusion The standard prevention and treatment ability of VTE among inpatients in Shanghai tertiary general hospitals had been improved to a certain extent, but there was still a large room for improvement. Extending the VTE prevention and control gateways and establishing the full-cycle risk assessment process; multi-disciplinary cooperation to build an integrated VTE prevention and treatment system; strengthen training and education to improve the standard prevention rate of VTE; improve the technical level of diagnosis and treatment and reduce the mortality of VTE will be the focus of the hospital to improve the prevention and management level of VTE.

Abstract:Pregnancy and the puerperium are wellestablished risk factors for venous thromboembolic disease (VTE).Imaging studies,such as compression ultrasonography,V/Q scan and CTPA,play a major role in confirming suspected DVT or PE in pregnant women.LMWHs are recommended as first-line anticoagulants for thromboprophylaxis and treatment of VTE during pregnancy.If VTE occurs at term,delivery should be attended by an experienced multidisciplinary team.In recent years,the number of pregnant women with VTE increases in China,but there is a lack of actual morbidity and studies about VTE prevention and treatment.There is an urgent need for setting up prevention and multidisciplinary management strategies for Chinese pregnant women.
Effect of Knowledge Translation on Suspected Venous Thromboembolism in Hospitalized Patients

Abstract:Background Venous thromboembolism (VTE) is a preventable disease, but the implementing effect of preventive measures is not optimistic, and few patients receive optimal prophylaxis, which result in significant mortality, long-term complications and financial burden. The purpose of the article was to explore the effect of knowledge translation on suspected venous thromboembolism in hospitalized patients on admission. Methods The data of suspected VTE patients on admission was retrospectively collected from the Second Xiangya Hospital from October 2016 to September 2020. The Mann-Whitney test and Fisher’s exact test (or χ 2 test) were used to analyze the continuous and categorical variables. A decision tree model was developed to assess the cost-effectiveness of post-prophylaxis education compared with pre-prophylaxis education. The ranges of each parameter were set to perform sensitivity analysis. Results The awareness of preventable VTE has raised which expressed in the increasing use of Caprini score, incidence of VTE, and department distribution of suspected VTE patients on admission. The incremental cost effectiveness ratio was a negative value which was significantly smaller than willing to pay (WTP). When the WTP value was ¥212676, the probability of patients' acceptance was 84.3% in the post-prophylaxis group and 15.7% in the pre-prophylaxis group. No matter how WTP changed, the cost-effectiveness acceptability of post-prophylaxis education in preventing VTE after hospitalization was better than pre-prophylaxis education did. Conclusion Knowledge translation on VTE could have cost effectiveness on suspected VTE in hospitalized patients on admission.
Retrospective Analysis of Clinical Characteristics of Hospitalized Patients with Deep Venous Thrombosis of Lower Extremity in Regional Central Hospital

Abstract:Objective To investigate the characteristics of the disease and the trend of diagnosis and treatment of patients with deep venous thrombosis (DVT) admitted to the First Affiliated Hospital,Xi'an Jiaotong University from October 2015 to October 2018. Methods The clinical data such as age, gender, clinical manifestations, location of thrombosis, and proportion conforming to DVT risk factors and treatment trends of patients with DVT were analyzed retrospectively. Results There was no difference in the prevalence between men and women in 1032 patients with DVT. Patients in the acute phase were more common, accounting for 65.02％ (671/1032). DVT was easy to accumulate left lower limbs, and the hole limb type was the most common type of thrombus, accounting for 76.48％(868/1135). The common risk factors for DVT were age≥40 years (91.28％), history of cardiovascular and cerebrovascular disease (36.72％), history of trauma/surgery (27.33％), history of sedentariness (22.38％), history of smoking in male (43.51％), etc. . The most common types of surgery were orthopedic surgery (47.73％), gynecologic surgery (14.77％), general surgery (13.07％), vascular surgery (7.95％), urology surgery (7.39％) and neurosurgery (3.98％). In the acute phase, 90.57％ of the D-dimer mers were ≥1, while in the chronic phase, only 63.19％ of the D-dimer mers were≥1. In terms of treatment, the use of preventive recyclable filters has increased year by year. Among the patients in the acute phase, 22.95％(154/671) received catheter thrombolysis. In the long-term treatment plan, rivaroxaban gradually replaced the traditional warfarin and became a conventional anticoagulant. Conclusion The DVT of the lower extremities is currently high in our region, the risk factors are complex, and needs to be taken seriously. The standardization of diagnosis and treatment of DVT still needs to be strengthened.

Abstract:Venous thromboembolism ( VTE) is an important etiology for unexpected deaths and perioperative deaths of inpatients. Early diagnose and early prevention are main measures to reduce the incidence and mortality of VTE. Herein, we have established a comprehensive management system of VTE prevention, treatment and research. On one hand, we implemented the in-hospital VTE prevention management and improved the implementation status of VTE prevention and treatment. As a result, the number of diagnosed VTE cases and related medical disputes decreased by years. On the other hand, we have built up the VTE monitoring platform, providing useful statistical data for the clinical use of anticoagulants and further reducing the incidence of VTE. The VTE monitoring platform also revealed some significant differences of the VTE incidence between the medical patients and the surgical patients, which could provide reference for further revision of VTE guidelines.
