Abstract:Posterior tibial slope (PTS) is an important parameter in total knee arthroplasty (TKA). Clinicians can use X-ray, CT and MRI to measure PTS. When using X-ray, clinicians need to select a suitable reference axis. The available reference axes include: tibial proximal anatomic axis, tibial shaft anatomic axis, fibular shaft axis, anterior tibial cortex, posterior tibial cortex. Different reference axes have their own advantages and disadvantages. The X-ray measurement of PTS is affected by the projection angle, and cannot measure the lateral posterior tibial slope and the medial posterior tibial slopeseparately. CT and MRI can measure PTS by using 3D reconstruction techniques, which is not influenced by the projection angle and can measure the lateral and medial tibial slope separately. MRI can also measure PTS containing cartilage and meniscus. In TKA, PTS can affect the range of motion, stability and ligaments’ tension of the knee through a variety of mechanisms. Increasing of PTS can effectively increase postoperative flexion through the roll-back mechanism and relaxation of the ligament. However, increasing of PTS will also cause relaxation of the ligament and anterior impingement of the tibial prosthesis, which results in instability of the knee and wear of the prosthesis. Furthermore, increasing of PTS can significantly reduce the tension of the posterior cruciate ligament, but increase the risk of posterior cruciate ligament injury due to osteotomy. In addition, increasing of PTS can reduce the tension of the quadriceps tendon in extension by increasing the torque. It is worth noting that clinical affects of PTS is influenced by different osteotomy references and the femoral condyle. At present, the researches on PTS are mostly based on European population. The measurement and clinical significance of PTS in Asian population need further exploration.

Abstract:Posterior tibial slope that is created during proximal tibial resection in total knee arthroplasty has emerged as an important factor in the mechanics of the knee joint and the surgical outcome. But the ideal degree of posterior tibial slope for recovery of the knee joint function and preventions of complications remains controversial and should vary in different racial groups. The objective of this paper is to investigate the effects of posterior tibial slope on contact stresses in the tibial polyethylene component of total knee prostheses.
Posterior tibial slope impacts intraoperatively measured mid-flexion anteroposterior kinematics during cruciate-retaining total knee arthroplasty

Abstract:Purpose: Posterior tibial slope (PTS) for cruciate-retaining (CR) total knee arthroplasty (TKA) is usually pre-determined by the surgeon. Limited information is available comparing different choices of PTS on the kinematics of the CR TKA, independent of the balancing of the extension gap. This study hypothesized that with the same balanced extension gap, the choice of PTS significantly impacts the intraoperatively measured kinematics of CR TKA. Methods: Navigated CR TKAs were performed on seven fresh-frozen cadavers with healthy knees and intact posterior cruciate ligament (PCL). A custom designed tibial baseplate was implanted to allow in situ modification of the PTS, which altered the flexion gap but maintained the extension gap. Knee kinematics were measured by performing passive range of motion (ROM) tests from full extension to 120° of flexion on the intact knee and CR TKAs with four different PTSs (1°, 4°, 7°, and 10°). The measured kinematics were compared across test conditions to assess the impact of PTS. Results: With a consistent extension gap, the change of PTS had significant impact on the anteroposterior (AP) kinematics of the CR TKA knees in mid-flexion range (45°-90°), but not so much for the high-flexion range (90°-120°). No considerable impacts were found on internal/external (I/E) rotation and hip-knee-ankle (HKA) angle. However, the findings on the individual basis suggested the impact of PTS on I/E rotation and HKA angle may be patient-specific. Conclusions: The data suggested that the choice of PTS had the greatest impact on the mid-flexion AP translation among the intraoperatively measured kinematics. This impact may be considered while making surgical decisions in the context of AP kinematics. When using a tibial component designed with "center" pivoting PTS, a surgeon may be able to fine tune the PTS to achieve proper mid-flexion AP stability.
Measurement of femoral posterior condylar offset and posterior tibial slope in normal knees based on 3D reconstruction

Abstract:Abstract Background Femoral posterior condylar offset (PCO) and posterior tibial slope (PTS) are important for postoperative range of motion after total knee arthroplasty (TKA). However, normative data of PCO and PTS and the correlation between them among healthy populations remain to be elucidated. The purpose of this study was to determine PCO and PTS in normal knees, and to identify the correlation between them. Methods Eighty healthy volunteers were recruited. CT scans were performed followed by three-dimensional reconstruction. PCO and PTS were measured and analyzed, as well as the correlation between them. Results PTS averaged 6.78° and 6.11°, on the medial and lateral side respectively ( P = 0.002). Medial PCO was greater than lateral (29.2 vs. 23.8 mm, P < 0.001). Both medial and lateral PCO of male were larger than female. On the contrary, male medial PTS was smaller than female, while there was no significant difference of lateral PTS between genders. There was an inverse correlation between medial PCO and PTS, but not lateral. Conclusions Significant differences exhibited between medial and lateral compartments, genders, and among individuals. An inverse correlation exists between PCO and PTS in the medial compartment. These results improve our understanding of the morphology and biomechanics of normal knees, and subsequently for optimising prosthetic design and surgical techniques.

Influence of posterior tibial slope on postoperative outcomes after postero-stabilized and condylar-stabilized total knee arthroplasty

Abstract:Purpose: To compare clinical outcomes and surgical times of two different types of total knee arthroplasty(TKA), postero-stabilized(PS) and condylar-stabilized(CS), in relation to posterior tibial slope(PTS) values and the deviation of these values from the surgical technique. Methods: 168 patients undergoing TKA surgery between 2016 and 2020 met our inclusion criteria. For each case, gender, age at surgery, operating time, type of implant and preoperative PTS(preop-PTS) and postoperative PTS(postop-PTS) measurements were collected; difference(∆PTS) between preop-PTS and postop-PTS was also calculated. Short Form 12 Mental and Physical scores(SF-12 M and P) and functional Knee Society Score(fKSS) were collected preoperatively and at a minimum of 12 months postoperatively. Four subgroups were thus created in relation to PS or CS system and postop-PTS value(≤ 5°and > 5°). Results: Of the 168 patients, 96 had a PS system and 72 CS system. Performing a CS-TKA took less time than a PS-TKA(p < 0.05). SF-12P showed better results(p < 0.05) in CS-TKA group than PS-TKA, probably because of the younger age of CS-TKA patients(p < 0.05). In the PS > 5° all examined postoperative scores were better(p < 0.05) than PS ≤ 5°, while only fKSS was better(p < 0.05) in CS > 5° than CS ≤ 5°. No significant difference(p > 0.05) in terms of postoperative outcomes between the PS > 5° and CS > 5° was noted, whereas only SF-12P was better in the CS ≤ 5° compared with PS ≤ 5°. Highest values of SF-12 M and fKSS were obtained in the PS ≤ 5°, in which postop-PTS was closer to technique. Conclusion: When performing a PS-TKA, the best result was obtained with a postop-PTS > 5°, but comparable outcomes between the two systems were evident with postop-PTS > 5°. It is crucial to come as close as possible to the indications reported in surgical technique regardless ∆PTS.

Abstract:OBJECTIVE:To retrospectively explore the correlation between anterior cruciate ligament (ACL)-ruptured knees, stability of ACL-rupture knee and posterior tibial slope (PTS).METHODS:From January 2008 to October 2012, 150 knees with ACL rupture underwent arthroscopic surgery for ACL reconstruction. A control group was established for subjects undergoing arthroscopic surgery without ACL rupture during the same period. PTS was measured on a digitalized lateral radiograph. Lachman and mechanized pivot shift tests were performed for assessing the stability of knee.RESULTS:There was significant difference (P = 0.007) in PTS angle between the patients with ACL rupture (9.5 ± 2.2 degrees) and the control group (6.6 ± 1.8 degrees). Only among females, increased slope of tibial plateau had effect on the Lachman test. There was a higher positive rate of pivot shift test in patients of increased posterior slope in the ACL rupture group.CONCLUSION:Increased posterior tibial slope (>6.6) appears to contribute to non-contact ACL injuries in females. And the changes of tibial slope have no effect upon the Lachman test. However, large changes in tibial slope affect pivot shift.
Effects of Posterior Tibial Slope on the Mid-Term Results of Medial Unicompartmental Knee Arthroplasty

Abstract:To evaluate the effect of medial posterior tibial slope (PTS) on mid-term postoperative range of motion (ROM) and functional improvement of the knee after medial unicompartmental knee arthroplasty (UKA). Medical records of 113 patients who had undergone 124 medial UKAs between April 2009 through April 2014 were reviewed retrospectively. The mean follow-up lasted 7.6 years (range, 6.2–11.2 years). Collected were demographic data, including gender, age, height, weight of the patients. Anteroposterior (AP) and lateral knee radiographs of the operated knees were available in all patients. The knee function was evaluated during office follow-up or hospital stay. Meanwhile, postoperative PTS, ROM, maximal knee flexion and Hospital for Special Surgery (HSS) knee score (pre−/postoperative) of the operated side were measured and assessed. According to the size of the PTS, patients were divided into 3 groups: group 1 (<4°), group 2 (4° ~ 7°) and group 3 (>7°). The association between PTS and the knee function was investigated. In our cohort, the average PTS was 2.7° ± 0.6° in group 1, 5.6° ± 0.9° in group 2 and 8.7° ± 1.2° in group 3. Pairwise comparisons showed significant differences among them (p < 0.01). The average maximal flexion range of postoperative knees in each group was 112.4° ± 5.6°, 116.4° ± 7.2°, and 117.5° ± 6.1°, respectively, with significant difference found between group 1 and group 2 (p < 0.05), and between group 1 and group 3 (p < 0.05). However, the gender, age, and body mass index (BMI) did not differ between three groups and there was no significant difference between groups in terms of pre−/postoperative HSS scores or postoperative knee ROM. A mid-term follow-up showed that an appropriate PTS (4° ~ 7°) can help improve the postoperative flexion of knee. On the other hand, too small a PTS could lead to limited postoperative knee flexion. Therefore, the PTS less than 4° should be avoided during medial UKA.
Medial-pivot Total Knee Arthroplasty Enhances Tibiofemoral Axial Rotation Stability in Weight-Bearing Mid-Range Flexion Compared to Posterior-Stabilised System

Abstract:Purpose: Total knee arthroplasty (TKA) stands as a primary intervention for severe knee ailments, yet concerns remain regarding postoperative patient satisfaction and flexion instability. This study aims to evaluate the in-vivo kinematics of medial-pivot (MP) and posterior-stabilised (PS) designs during step-up activity, in comparison to the kinematics of the nonoperated contralateral knee. Methods: Sixteen patients with PS-TKA and 14 with MP-TKA were retrospectively examined. Clinical outcomes were assessed using patient-completed questionnaires. Motion during step-up was captured using a dual fluoroscopic system. Statistical analysis was applied to evaluate the in-vivo tibiofemoral six-degree-of-freedom kinematics and articular contact positions between the two groups. Results: Despite being older, patients in the MP group reported higher postoperative subjective scores for weight-bearing functional activities. The axial rotation centres of MP-TKA located on the medial tibial plateau exhibited less variance compared to PS-TKA and contralateral knees. Compared to the contralateral knee (contralateral to medial-pivot [C-MP] or contralateral to posterior-stabilised [C-PS]), the MP group exhibited limited range of motion in terms of anteroposterior translation (MP: 3.6 +/- 1.3 mm vs. C-MP: 7.4 +/- 2.5 mm, p < 0.01) and axial rotation (MP: 6.6 +/- 1.9 degrees vs. C-MP: 10.3 +/- 4.9 degrees, p = 0.02), as well as in the PS group for anteroposterior translation (PS: 3.9 +/- 1.7 mm vs. C-PS: 7.2 +/- 3.7 mm, p < 0.01). Conclusion: The MP group with better postoperative ratings demonstrated a more stable MP axial rotation pattern during step-up activity compared to the PS group, underscoring the pivotal role of prosthetic design in optimising postoperative rehabilitation and functional recovery.
Effect of Joint Line Elevation after Posterior-stabilized and Cruciate-retaining Total Knee Arthroplasty on Clinical Function and Kinematics

Abstract:BACKGROUND:Joint line (JL) is a very important factor for total knee arthroplasty (TKA) to restore. The objective of this study was to evaluate the early clinical and kinematic results of TKAs with posterior-stabilized (PS) or cruciate retaining (CR) implants in which the JL was elevated postoperatively.METHODS:Data were collected from patients who underwent TKA in our department between April 2011 and April 2014. The patients were divided into two groups based on the prosthesis they received (PS or CR). At 1-year postoperatively, clinical outcomes were evaluated by the American Knee Society (AKS) knee score, AKS function score, and patella score. In vivo kinematic analysis after TKA was performed on all patients and a previously validated three-dimensional to two-dimensional image registration technique was used to obtain the kinematic data. Anteroposterior (AP) translation of the medial and lateral femoral condyles, and axial rotation relative to the tibial plateau, were analyzed. The data were assessed using the Mann-Whitney test.RESULTS:At time of follow-up, there were differences in the AKS knee scores (P = 0.005), AKS function scores (P = 0.025), patella scores (P = 0.015), and postoperative range of motions (P = 0.004) between the PS group and the CR group. In the PS group, the magnitude of AP translation for the medial and lateral condyle was 4.9 ± 3.0 mm and 12.8 ± 3.3 mm, respectively. Axial rotation of the tibial component relative to the femoral component was 12.9 ± 4.5°. In the CR group, the magnitude of AP translation for the medial and lateral condyle was 4.3 ± 3.5 mm and 7.9 ± 4.2 mm, respectively. The axial rotation was 6.7 ± 5.9°. There were statistically different between PS group and CR group in kinematics postoperatively.CONCLUSION:Our results demonstrate that postoperative JL elevation had more adverse effects on the clinical and kinematic outcomes of CR TKAs than PS TKAs.
Posterior cruciate-retaining versus posterior stabilized total knee arthroplasty: a meta-analysis of randomized controlled trials

Abstract:Purpose To compare the outcomes between posterior cruciate-retaining and posterior stabilized total knee arthroplasty (TKA) in order to evaluate which approach is superior. Methods Randomized controlled trials (RCTs) comparing posterior cruciate-retaining with posterior stabilized TKA were reviewed which were published up to August 2011. Methodological quality of each included RCT was assessed using the Physiotherapy Evidence Database (PEDro) scale. The relevant data were analysed using Review Manager 5.1. The Grading of Recommendations Assessment, Development and Evaluation (GRADE) approach was used to determine the quality of the evidence. Results Eight RCTs involving 888 patients with 963 knee joints met predetermined inclusion criteria. The postoperative range of motion (ROM) and flexion angle were 11.07° and 2.88° higher for patients with a posterior stabilized TKA than those with a posterior cruciate-retaining TKA, respectively [weighted mean difference (WMD), −11.07; 95 % confidence interval (CI), −18.06 to −4.08; p < 0.01 and WMD, −2.88; 95 % CI, −5.63 to −0.12; p = 0.04]. No statistical differences were observed between the two designs for knee society pain score, extension angle, 2- and 5-year knee society score, 2- and 5-year knee society function score and complications after primary TKA. Conclusion Posterior cruciate-retaining and posterior stabilized TKA have similar clinical outcomes with regard to knee function, postoperative knee pain and the other complications. Prosthesis survivorship for both posterior cruciate-retaining and posterior stabilized TKA is satisfactory, and there are no differences between them at short- and middle-term follow-up. Effect of Tibial Component Alignment and Posterior Slope on Tibial Coverage in a Chinese Population: A Three-dimensional Anthropometric Study

Abstract:The standard for rotational alignment and posterior slope of the tibial component in total knee arthroplasty remains controversial. This study aimed to evaluate the effect of tibial component alignment and posterior slope on tibial coverage. Computer tomographic scans of 101 tibial specimens were used to reconstruct three-dimensional tibia models. A virtual surgery was performed to generate a resection plane with different posterior slopes on the proximal tibia. Symmetrical and anatomical tibial components were placed aligning to the medial one-third of tibial tubercle (Insall) and the medial edge of patella tendon (Akagi), respectively. Differences in coverage and mismatch were evaluated and statistically compared across alignments, slope angles, and genders. The tibial coverage increased from 83.21 to 85.96% for Akagi's alignments and from 85.19 to 87.22% for Insall alignments along with the increasing of posterior slope from 0 to 7 degrees. Regardless of the prosthesis design, there was a significant difference between two rotational alignments as the Insall alignment was significantly higher in tibial coverage. With a slope of 7 degrees, more overhang along with less underhang anteromedially were found in males compared with females. The current anatomical tibial design has a tendency of overhang in the anteromedial zone which does not exist in the symmetrical design. The current tibial baseplate design has better tibial coverage when aligning to the medial third of tibial tuberosity with slopes of 5 and 7 degrees. Gender difference should be taken into account and given priority for prosthesis design.
Similar Midterm Outcomes of Total Knee Arthroplasties with Anterior and Posterior Tibial Slopes Performed on Paired Knees at a Minimum Follow-up of 5 Years

Abstract:A small posterior tibial slope (PTS) is generally recommended in posterior stabilized (PS) total knee arthroplasty (TKA). An unwanted anterior tibial slope (ATS), which can affect postoperative results, may be created in PS TKA because of the inaccuracy of surgical instruments and techniques, as well as high interpatient variability. We compared midterm clinical and radiographic results of PS TKAs with ATS and PTS performed on paired knees using the same prosthesis. One-hundred-twenty-four patients who underwent TKAs with ATS and PTS on paired knees using ATTUNE posterior-stabilized prostheses were retrospectively reviewed after a minimum follow-up period of 5 years. The mean follow-up period was 5.4 years. The Knee Society Knee and Function scores, Western Ontario and McMaster Universities Osteoarthritis Index, Feller and Kujalar scores, and range of motion (ROM) were evaluated. The preferred TKA out of ATS and PTS was also investigated. The hip-knee-ankle angle, component positions, tibial slope, posterior femoral offset, Insall-Salvati ratio, and knee sagittal angle were measured by radiography. There were no significant differences in the clinical results, including ROM, between TKAs with ATS and PTS preoperatively and at the last follow-up. Regarding patient preference, 58 patients (46.8%) were satisfied with bilateral knees, 30 (24.2%) preferred knees with ATS, and 36 (29%) preferred knees with PTS. There was no significant difference in the rate of preference between TKAs with ATS and PTS (p = 0.539). Except for the postoperative tibial slope (-1.8 vs. 2.5 degrees, p < 0.001), there were also no significant differences in the radiographic results, including the knee sagittal angle, preoperatively and at the last follow-up. The midterm outcomes were similar between PS TKAs with ATS and PTS performed on paired knees at a minimum of 5 years of follow-up. Nonsevere ATS did not affect midterm outcomes in PS TKA with proper soft tissue balancing and the current prosthesis of improved design. However, a long-term follow-up study is required to confirm the safety of nonsevere ATS in PS TKA. LEVEL OF EVIDENCE:: III.
The Effect of Sagittal Prosthesis Position on Joint Mobility after Surgery of PS-type TKA

Abstract:Objective To evaluate the effect of femoral prosthesis flexion angle and tibial plateau posterior inclination on the postoperative joint mobility in patients with bilateral total knee arthroplasty.Their own left and right limbs were compared.Methods The patients who had bilateral total knee arthroplasty first time and significant difference in flexion and extension function were divided into two groups:relatively good function and relatively poor.The t-test was used to analyze the difference of femoral prosthesis flexion angle(γ)and tibial plateau posterior inclination(δ)between the two groups.The Pearson test was used to analyze whetherthere is correlation between knee range of motion and γandδ.Results 31 cases of 62 knees were included in the study.The groupwith a relatively better postoperative extension function had an average extension angle of(-0.3 ± 0.9)°,γ angle(2.8 ± 2.2)°.The other grouphad an average extension angle of(-4.0 ± 2.3)°,γ angle(6.1± 3.4)°.The groupwith a relatively better postoperative flexion function had an average flexion angle of(106.5±9.3)°,δangle(5.2±2)°.The other grouphad an average flexion angle of(99.4±8.2)°,δangle(4.2±2)°The difference was statistically significant(P<0.001).The Pearson test showed correlation between the ROM range of the knee and theδangle(P<0.001).The larger theδangle,the greater the ROM,but there was no correlation with theγangle(P=0.400).Conclusion Ensuring propertibial plateau posterior inclination(δ)and avoiding femoral prosthesis flexion can help to obtain satisfactory postoperative activity.The greater the rearward posterior,the better the degree of joint mobility in a 2°-9° backward range.5° rear angle is recommended.

After excluding patients with incomplete data and lost to follow up data for 231 patients were analyzed. Information was collected from pre-operative as well as post-operative skyline views and three functional outcome scores including SF-36, KSS, and WOMAC at pre-op, post-op 1 year, and post-op 2 years. Patella tilt (PT), patella displacement was calculated from radiology software in PACS. Patients were divided according to PT into 3 subgroups (PT < = 5 degrees, 5.1–10, > 10 degrees). Statistical analysis was done to identify the relationship of patella tilt and 3 functional. Results There was a significant improvement in post-operative patella tilt as compared to pre-operative patella tilt (3.35+-3.91vs5.65+-4.41, p-value = 0.0000). There was no significant difference in patella displacement in the postoperative period as compared to the pre-operative period. KSS functional score was significantly poor at 1 year and KSS objective score at 2 years in patients with PT more than 10 degrees. SF-36 and WOMAC were not significantly different in three subgroups based on PT. Conclusion Poor functional after TKA can be associated with an increase in patella tilt at 2 years follow up. Abstract:Although knee biomechanics has been examined, hip and ankle biomechanics in incline ramp walking has not been explored for patients with total knee arthroplasty (TKA). The purpose of this study was to investigate the hip and ankle joint kinematic and kinetic biomechanics of different incline slopes for replaced limbs and non-replaced limbs in individuals with TKA compared to healthy controls. Twenty-five patients with TKR and ten healthy controls performed walking trials on four slope conditions of level (0°), 5°, 10° and 15° on a customized instrumented ramp system. A 3x4 (limb x slope) repeated analysis of variance was used to evaluate selected variables. The results showed a greater peak ankle dorsiflexion angle in the replaced limbs compared to healthy limbs. No significant interactions or limb main effect for other ankle and hip variables. The peak dorsiflexion angle, eversion angle and dorsiflexion moment were progressively higher in each comparison from level to 15°. The peak plantarflexion moment was also increased with each increase of slopes. Both the replaced and non-replaced limbs of patients with TKA had lower hip flexion moments than the healthy control limbs. Hip angle at contact and hip extension range of motion increased with each increase of slopes. Peak hip loading-response internal extension moment increased with each increase in slope and peak hip push-off internal flexion moment decreased with each increase of slope. Our results showed increased dorsiflexion in replaced limbs but no other compensations of hip and ankle joints of replaced limbs compared to non-replaced limbs and their healthy controls during incline walking, providing further support of using incline walking in rehabilitation for patients with TKA

Abstract:Aseptic loosening is the primary cause of failure following posterior-stabilized total knee arthroplasty. It is unclear whether tibial post loading of posterior-stabilized prosthesis increases the risk of aseptic loosening of the tibial prosthesis. The purpose of this study is to investigate the biomechanical effects of tibial post loading on the tibial prosthesis fixation interface during level walking, squatting, stair descent, and standing up-sitting down activities. In this paper, finite element models with and without post were established to compare the effects of tibial post loading on the von Mises stress of the proximal tibia, shear stress of the cement, and the bone-prosthesis interface micromotion during four physiological activities. The tibial post loading had an insignificant influence on tibial biomechanics and bone-prosthesis interface micromotion during leveling walking activity. However, compared to the insert without post condition, tibial post loading significantly increased the maximum tibial von Mises stress, the maximum shear stress in the medial of cement, and the bone-prosthesis interface peak micromotion by 912.84%, 612.77%, and 921.09%, respectively, at the moment of the maximum flexion angle for the stair descent activity, and 637.92%, 351.43%, and 519.13%, respectively, at the moment of the maximum flexion angle for the standing up-sitting down activity. Tibial post loading increased the risk of postoperative aseptic loosening of tibial prosthesis in patients with posterior-stabilized total knee arthroplasty, and it was recommended that the post-cam contact mechanism of posterior-stabilized prosthesis should be optimized to reduce the biomechanical impact of tibial post loading on tibial prosthesis fixation.

Abstract:Orthopaedic Journal of Sports Medicine, Volume 12, Issue 11_suppl4, November 2024. Objectives:The posterior tibial slope assessment is crucial in preoperative planning for various surgeries. Different measurement methods exist, but no study has compared them. This study aims to verify the equivalence of four techniques for measuring the posterior tibial slope in lateral knee radiographs and assess inter-observer reliability among evaluators with different academic training levels.Methods:Forty-three lateral knee radiographs from patients with osteoarthritis and ligament injuries who had indications for tibial osteotomy were evaluated. Three evaluators with varying levels of academic training performed measurements using four parameters: anterior cortex, posterior cortex, anatomical axis of the proximal tibia, and anatomical axis of the proximal fibula. Results were expressed as mean values ± standard deviation, and a p-value < 0.05 was considered statistically significant.Results:The average degree values were 12.6±4.4 for the anterior cortex, 10.1±3.8 for the tibial axis, 7.8±4.1 for the posterior cortex, and 11.6±4.6 for the fibular axis. There was a significant difference between all these measurements, except for the comparison between the anterior cortex and the fibular axis. In terms of the tibial axis, the evaluator with the lowest level of training showed significant variation compared to the other two evaluators.Conclusion:The four methods for measuring the posterior tibial slope are not equivalent, except for the anterior cortex and fibular axis measurements. Lower-level trained professionals had difficulty performing tibial axis measurements. Further studies are needed to determine the most accurate technique for assessing posterior tibial slope angulation

AimTo explore the effects of tibial osteotomy varus angle combined with posterior tibial slope (PTS) on the stress of polyethylene liner in total knee arthroplasty (TKA) by building finite element model (FEM).MethodsEstablished the FEM of standard TKA with tibial osteotomy varus angle 0° to 9° were established and divided into 10 groups. Next, each group was created 10 FEMs with 0° to 9° PTS separately. Calculated the stress on polyethylene liner in each group in Abaqus. Finally, the relevancy between tibial osteotomy angle and polyethylene liner stress was statistically analyzed using multiple regression analysis.ResultsAs the varus angle increased, the area of maximum stress gradually shifted medially on the polyethylene liner. As the PTS increases, the percentage of surface contact forces on the medial and lateral compartmental of the polyethylene liner gradually converge to the same. When the varus angle is between 0° and 3°, the maximum stress of the medial compartmental surfaces of polyethylene liner rises smoothly with the increase of the PTS. When the varus angle is between 4° and 9°, as the increase of the PTS, the maximum stress of polyethylene liner rises first and then falls, forming a trough at PTS 5° and then rises again. Compared to the PTS, the varus angle has a large effect on the maximum stress of the polyethylene liner (p < .001).ConclusionWhen the varus angle is 0° to 3°, PTS 0° is recommended, which will result in a more equalized stress distribution of the polyethylene liner in TKA.

Abstract:Objective Application of preliminary experiment research,this paper discusses the relationship between the femoral postrior condyle and the maximum flexion of the knee after the total knee arthroplasty. Methods To take the total knee arthroplasty on the six lower limb corpse knees(six knees),adjusting the drill of the guide to decide posterior condylar osteotomy plane,depanding on the amount of postrior condylar osteotomy change and then adjust the value of the tibial osteotomy plane,keeping the knee flexion gap and posterior tibial slope constantly,then install the appropriate type of the femoral prosthetic tryout in turn,get the Maximum flexion knee pure lateral 1 ∶ 1 X-ray images,measure and record the value of the femoral condyle and knee maximum flexion degree for statistical analysis. Results The data were assessed by the Pearson and Spearman correlation coefficients,the same sample of different femoral condylar offset and knee maximum flexion degree after total knee arthroplasty has no relevance. (P>0. 05,had no significant difference). Conclusion Different femur posterior condyle and knee maximum flexion degree has no relevance after total knee arthroplasty by using the posterior stability prothesis.
