Abstract:Objective:Since originally establishing genetic linkage at chromosome 9p21.3-9p21.1 in families with concomitant frontotemporal dementia (FTD) and amyotrophic lateral sclerosis (ALS) we have been actively genotyping one of the largest North American ALS and ALS/FTD cohorts for the C9orf72 (G4C2) expansion mutation to determine whether the frequency of the mutation is similar to other reports.Background:A hexanucleotide (G4C2) repeat expansion mutation on Chromosome 9 open reading frame 72 (C9orf72; C9) is currently identified as the most common cause of ALS and FTD. C9(+) expansion carriers are defined as having ≥30 (G4C2) repeats that generate a typical saw-tooth electropherogram pattern following repeat primed-polymerase chain reaction (RP-PCR) analysis.Methods:A cohort of patients with FALS or SALS, with and without FTD, and neurologically normal controls were screened for the C9 expansion mutation using the RP-PCR method.Results:We identified ~50[percnt] FALS and ~5[percnt] SALS cases as C9(+) mutations. While we found no statistical difference in gender distribution, ethnicity or age at onset when compared with C9(-) carriers, we found that disease onset was more likely to be bulbar or cognitive, and disease duration was shorter in C9(+) carriers compared with C9(-) carriers. Interestingly, we also found that ~40[percnt] of our C9(+) cases generated a previously unreported atypical electropherogram pattern that is characterized by a series (27) of low SI that requires manipulation of the SI range for adequate detection. Moreover, affected subjects with atypical expansion patterns were predominately Caucasians and had a longer disease duration than individuals with typical saw-tooth expansion patterns in our SALS cohort.Conclusions:Variances in the expansion pattern may serve as markers for disease with unique clinical features reflecting the phenotypic variability observed in ALS and FTD patients.
