Abstract:Aims To study the safety and effectiveness of a combination of both intravenous injection of lidocaine and intraocular pressure-lowering medications, in the intraocular pressure control and relief of symptoms of refractory acute primary angle-closure glaucoma (PACG). Methods Five consecutive patients with their first attack of acute PACG, with intraocular pressure ≥45 mmHg and a failure to release from the attack under antiglaucomatous medications for 4 hours, were recruited into the study. On presentation, each patient received topical pilocarpine and timolol, and systemic acetazolamide and mannitol as primary treatment. Then the patients accepted 2% lidocaine by intravenous injection at dose of 0.8 mg/kg in concert with antiglaucomatous medications. The intraocular pressures after intravenous injection at 30 minutes, and then at 1, 2, 4, 12, and 24 hours, were documented by applanation tonometry. Symptoms, visual acuity, intraocular pressure, corneal edema, angle status on gonioscopy, pupillary size, and reaction were also measured. Results Six eyes of five patients seen with acute PACG were recruited. The mean intraocular pressure was reduced from 50.83 ± 5.34 mmHg to 39.5 ± 3.45 mmHg at 30 minutes after intravenous injection, and then to 33.3 ± 3.56 mmHg at 1 hour, and 24.55 ± 5.09 mmHg at 2 hours after intravenous injection. The mean intraocular pressure was less than 21 mmHg at 4 hours and beyond. There was instant symptomatic relief for all patients. No complications were encountered. Conclusions From this preliminary study, intravenous injection of lidocaine seems to be safe and effective in controlling intraocular pressure and eliminating symptoms in acute PACG. But the exact efficacy and safety need further investigation in large case studies.
Clinical Observation of Intravitreal Injection of Triamcinolone Acetonide for Macular Edema Secondary to Retinal Vein Occlusion

Abstract:Objective To investigate the clinical effect and safety of intravitreal injection of triamcinolone ace-tonide for the treatment of macular edema secondary to retinal vein occlusion.Methods Twenty-three patients (23 eyes) with macular edema secondary to retinal vein occlusion were treated with intravitreal injection of triamcinolone acetonide 0.1 ml (4 mg) .Follow-up examination was done at 3 months after the surgery.Author observed macular with fundus fluoresce-in angiography ( FFA) and optical coherence tomography ( OCT) .Results Visual acuity improved 2 lines or more in 12 eyes (52.17%), changed less than one line in 8 eyes (30.43%), and remained same in 3 eyes (13.04%).Resolve of macular edema and fluorescence leakage was found in 8 eyes (34.78%).In 13 eyes (56.52%) there were obvious im-provement, and in 2 eyes (8.7%) there were no changes in FFA.The central macular thickness was 301.25 ±87.49μm. Complications included transient elevated intraocular pressure (1 eye) and pseudo-hypopyon (1 eye) that resolved within one week.Conclusion Intravitreal injection of triamcinolone acetonide is an effective and feasible way to treat macula e-dema secondary to retinal vein occlusion.

Abstract:Objective To evaluate the safety and effectiveness of intracameral different dosage triamcinolone acetonide(TA) for anterior uveitis after cataract combined with anti-glaucoma surgery.Methods Eighty-eight patients(88 eyes) with glaucoma and cataract were performed trabeculectomy combined with cataract surgery.All patients were randomly divided into normal control group(NC),TA1 group,TA2 group,TA3 group.After operation,NC group were received the subconjunctival injection of 2.5 mg dexamethasone sodium phosphate,and TA1 group,TA2 group,TA3 group were respectively performed anterior chamber injections of TA 0.5 mg,1.0 mg,2.0 mg.The best corrected visual acuity(BCVA),anterior chamber reaction,intraocular pressure and corneal endothelial count were observed and compared among all groups at1 day,3 days,5 days after operation.Results The recovery of postoperative BCVA in TA2 group was the fastest,which at postoperative 5 days was from 0.4 to 0.6 in 12 eyes(54.6%),more than 0.8 in 7 eyes(31.8%).Anterior chamber reaction of TA1 group,TA2 group and TA3 group at postoperative each time point was significantly lower than that of NC group,there were statistical differences at postoperative 3 days and 5 days(both P<0.05).The intraocular pressure of NC group,TA1 group,TA2 group and TA3 group at postoperative 5 days were(14.69±5.13) mmHg(1 kPa=7.5 mmHg),(12.78±4.89) mmHg,(13.21±4.63) mmHg and(14.16±5.33) mmHg respectively,there was no statistical difference among four groups(F=0.823,P=0.485).Postoperative corneal endothelial counts were all within normal range,there was no statistical difference among four groups.Conclusion Intracameral different dosage TA(0.5 mg,1.0 mg,2.0 mg) after cataract combined with anti-glaucoma surgery can be greatly suppressed postoperative anterior chamber reaction.No significant adverse reaction is found after operation,but the patients of intracameral 1.0 mg TA get better BCVA in the short term.

Abstract:Purpose To evaluate the efficacy and safety of compound gentamicin diclofenac sodium eyedrops for postoperative inflammation of phacoemulsification and fold lens implantation,as well as trabeculectomy.Methods A multicenter,randomized,double-blind and controlled study was adopted.Compound gentamicin diclofenac sodium eyedrops and Voltamicin eyedrops,four times each day,were administered in test and control group respectively.All patients were followed up for 14 days.Results The efficacy rates in test and control group were 71.43% and 65.52% respectively in intraocular surgery(include phacoemulsification and anti-glaucoma surgery).The incidence of adverse reactions in test group and control group is not different statistically.Conclusion Compound gentamicin diclofenac sodium eyedrops have an excellent efficacy and safety to control the inflammation after phacoemulsification and fold lens implantatian,and trabeculectomy.

Abstract:Objective To evaluate the efficacy and safety of compound diclofenac sodium eyedrops for postoperative inflammation of phacoemulsification and fold lens implantation, as well as trabeculaectomy. Methods A multicenter,randomized, and controlled study was adopted. Compound diclofenac sodium eyedrops and 0.1% diclofenac sodium eyedrops, four times each day, were administered in test and control group respectively. All patients have been observed for 14 days. Results The efficacy rates in test and control group were 97.06% and 95.31% respectively in cataract extraction section, and 100.00% and 98.36% in anti-glaucoma surgery. The incidence of adverse reactions in test group and control group is no different statistically. Conclusions Compound diclofenac sodium eyedrops have an excellent efficacy and safety to control the inflammmation after phacoemulsification and fold lens implantatin, and trabeculaectomy.
Long-Term Effects of Simultaneous Subconjunctival and Subscleral Mitomycin C Application in Repeat Trabeculectomy

Abstract:Purpose: To assess the efficacy and safety of simultaneous mitomycin C application under conjunctival and scleral flaps in patients with repeat trabeculectomy.Methods: A total of 44 patients (44 eyes) with previous failed filtering surgery were randomized to one of two groups. The both-flaps group comprised 22 patients (22 eyes) with trabeculectomy and intraoperative mitomycin C application under conjunctival and scleral flaps, whereas the subconjunctival group comprised 22 patients (22 eyes) with subconjunctival application of mitomycin C. Particular attention was paid to intraocular pressure, postoperative medications, visual acuity, filtering bleb appearance, and complications. The mean follow-up time was 38.18 +/- 12.48 months.Results: The mean preoperative intraocular pressure decreased from 39.1 +/- 7.3 mm Hg to the postoperative level of 15.6 +/- 4.8 mm Hg in the both-flaps group (P = 0.014), and from 39.4 +/- 8.4 to 18.7 +/- 5.8 mm Hg in the subconjunctival group (P = 0.018). There was a statistically significant difference in intraocular pressure at all follow-up times, except at 1 week and 1 month postoperatively. Kaplan-Meier survival analysis showed there was no significant difference in total success rate (complete plus qualified success) between the two group, (P = 0.622, log-rank test). However, the two survival curves for the complete success subgroups (without additional medications) confirmed that mitomycin C applications under both flaps had a higher success rate than subconjunctival application (P = 0.043, log-rank test). No statistically significant difference in medications was present between the two groups, and no severe complications developed in either group.Conclusions: Trabeculectomy augmented with mitomycin C application at both sites could produce a greater lowering of intraocular pressure with low incidence of postoperative complications, and could provide an increased chance of long-term success. The procedure is effective and safe in patients with repeat trabeculectomy.

Abstract:Purpose: To evaluate the efficacy of posterior sub-tenon injection of triamcinolone in non-resolving vitreoretinits. Introduction: Vitreoretinits is the inflammation of the vitreous and retina. In a non-resolving case of vitreoretinitis steroids are the mainstay of treatment. Steroids can be administered via various routes into the eye. Posterior subtenon injection of Triamcinolone acetonide is an effective treatment option for anatomical and functional improvement in intermediate and posterior uveitis. Methods: A 46-year-old female presented to the outpatient department with decreased vision in her left eye for 3 months. On examination, she had a BCVA of 6/60 in her left eye. Fundoscopy showed vitreous haze grade 3 due to vitritis, multiple vitreoretinal tractional bands extending along both the superior and inferior arcade, macular edema, and a retinitis patch of less than half disc diameter size just above the superior arcade. The patient was positive for IgG antibodies to Cytomegalovirus and Herpes Simplex virus. The patient was started on oral prednisolone 1mg/kgbw once a day and topical prednisolone acetate 1.0%, nepafenac 0.3%, and timolol 0.5%. The patient was given posterior subtenon injection of 0.5ml of triamcinolone acetonide 40mg/ml. Results: She had 6/12 BCVA at the end of 1 month with resolving vitritis and macular edema. By the end of 3 months, she had fully resolved vitreoretinitis with 6/6 vision. Discussion: Most steroids used in ophthalmology are glucocorticoids, which have anti-inflammatory and immunosuppressive activity. Triamcinolone Acetonide is a minimally water-soluble suspension. Triamcinolone crystals slowly dissolve into the vitreous. This creates a diffusional gradient from the vitreous to the macula with minimal systemic exposure. Our study also showed significant improvement in visual acuity and reduction in macular edema with a single posterior subtenon injection of triamcinolone acetonide. Conclusion: A subtenon injection of triamcinolone acetonide appears to be an effective treatment for decreased vision associated with intermediate and posterior uveitis without any significant side effects.
Intravitreal Triamcinolone Versus Bevacizumab for the Treatment of Macular Edema in Retinal Vein Occlusion

Abstract:Objective To compare the efficiency ofintravitreal injection oftriamcinolone acetonide or bevacizumab for the treatment ofmaeular edema in retinal vein occlusion. Methods One hundred and sixteen eyes of 116 patients with macular edema of retinal vein occlusion were diagnosed by examination of regular inspection, fundus fluorescein angiography (FFA) and optic coherence tomography (OCT). There were 75 patients in TA group and 41 patients in bevacizumab group. One hundred and sixteen patients received intravitreal injection of either 4mg/0. 1ml of triamcinolone acetonide or 1.25mg/0.05ml of bevacizumab. There were no significant differences between the two groups with regards to patients' age, duration of disease, best-corrected visual acuity (BCVA), central macular thickness (CMT) at baseline. Comprehensive ophthalmic evaluation was performed at baseline and at weeks 4, 8 and 12 after treatment. Main outcome measures included CMT measured with OCT and BCVA. Results Separate within-group analysis of showed significant improvement in BCVA from baseline in TA group at weeks 4 (P=0.000)、8 (P=0.000)、12 (P=0.000), and at weeks 4 (P=0.000), 8 (P=0.000) in bevacizumab group. The results showed significant reduction in CMT in TA group at weeks 4(P=0.000), 8(P=0.000), 12(P=0.004), and at weeks 4(P=0.003), 8(P=0.000) in bevacizumab group. But no significant interaction between groups were observed for BCVA at weeks 4(P=0.985), 8(P=0.989), 12(P=0.306) and for CMT at weeks 4(P=0.075), 8(P=0.453), 12(P=0.583). There was a significant increase in IOP in the TA group when compared with the bevacizumab group. Conclusion The result in our observation showed that intravitreai injection of either triamcinolone acetonide or bevacizumab was well tolerated with a significant improvement in BCVA and decrease in macular edema for patients with retinal vein occlusion. A randomly controlled multi-center clinical triai is necessary.
Intravitreal Bevacizumab Vs Triamcinolone Acetonide for Macular Oedema Due to Central Retinal Vein Occlusion

Abstract:Purpose To compare the effect of intravitreal bevacizumab vs intravitreal triamcinolone for the treatment of non-ischaemic central retinal vein occlusion (CRVO). Methods The comparative nonrandomized retrospective clinical interventional study included 72 patients with non-ischaemic CRVO, divided into a bevacizumab group of 30 patients (1.25 mg bevacizumab) and a triamcinolone group of 42 patients (4.0 mg triamcinolone). All patients were consecutively included. At baseline, both study groups did not vary significantly in visual acuity, macular thickness, and duration of symptoms (191±300 days vs 222±256 days). The minimal follow-up was 3 months (mean: 7.8±4.3 months; range: 3–12 months). During follow-up, 1.3±0.4 re-injections of the triamcinolone group (range:1–2 injections) and 2.7±1.8 re-injections of bevacizumab (range:1–6 injections) were administered. Results In both study groups, the mean visual acuity increased significantly ( P <0.001) from baseline during follow-up. The differences in gain in visual acuity were not statistically significant ( P >0.40) between both study groups at any time during follow-up. The mean macular thickness decreased significantly ( P <0.001) in both study groups, with the reduction being significantly ( P =0.006) more pronounced in the triamcinolone group. Intraocular pressure increased significantly ( P <0.001) in the triamcinolone group. Conclusions In long-standing non-ischaemic CRVO, intravitreal bevacizumab and intravitreal triamcinolone are both associated with a comparable gain in visual acuity. The reduction in macular oedema was more marked in the triamcinolone group. In view of the potential complications of intravitreal triamcinolone in terms of intraocular pressure rise and cataractogenesis, bevacizumab may be preferred compared with triamcinolone for intravitreal use in non-ischaemic CRVO.

Abstract:Cataract is a type of lens abnormality characterized by decreased transparency and increased turbidity, and it is the main cause of reversible visual impairment and blindness worldwide especially in lower socioeconomic status and developing countries. [ 1 ] Cataracts can be confirmed based on clinical evaluation of the eyes, assessment of patients' visual impairments and other symptoms, as well as accompanying eye diseases that may affect visual outcomes were performed. [ 2 ] The main treatment modality for cataracts is surgery. [ 3 , 4 ] Phacoemulsification and ocular lens implantation are the most popular therapeutic strategies for cataract extraction. Although phacoemulsification technology has made great progress in recent years, it still involves surgical trauma, making patients prone to postoperative inflammation. [ 5 ] Postoperative inflammation control is important, and serious complications may occur with poor control, such as including cystoid macular edema, increased IOP, adhesion formation, posterior capsule clouding and secondary glaucoma. [ 5 , 6 ]

Abstract:AIM: To assess the efficacy of intravitreal triamcinolone (IVTA) as an adjunct to the combination of anti-vascular endothelial growth factor ( VEGF) for the management of diabetic macular edema (DME).METHODS: A total of 51 patients with visual disabilities causing by DME from two sites were retrospectively collected and assigned to two groups according to the therapeutic method: intravitreal conbercept (IVC) combined with focal laser (24 eyes) and IVC combined with focal laser and IVTA (27 eyes). Best-corrected visual acuity (BCVA), the required number of IVCs, central retinal thickness (CRT), the mean costs of treatment burden and safety were compared over 12mo.RESULTS: From baseline to month 1 through month 12, IVC combined with focal laser and IVTA improved the mean average change in BCVA superior to IVC combined with focal laser (+ 5.20 vs + 2.71 letters). At month 12, 20.83% of the IVC combined with focal laser and 37.04% of IVC combined with focal laser and IVTA arms gained more than 10 BCVA letters. During the period, the mean CRT decreased significantly in the IVC combined with focal laser and IVTA arm (-245.9 mu m) compared to the IVC combined with focal laser arm (-98.45 mu m). The average of 6.45 and 1.25 conbercept injections performed in the IVC combined with focal laser and IVC combined with focal laser and IVTA arms, respectively. The mean cost of treatment burden for 12mo was $6247.44 +/- 4069.18 in the IVC combined with focal laser arm and $1679.19 +/- 542.73 in the IVC combined with focal laser and IVTA arm, with a statistically significant difference. Apart from occasional minor subconjunctival hemorrhage, no other significant ocular adverse events (AEs) were observed in either group during the12-month period.CONCLUSION: It is effective and cost-effective to treat DME by utilizing triamcinolone as an adjunct to the combination of anti-VEGF.

Because the macula is extensively surrounded by blood vessels, any resulting leakage can lead to macular edema and subsequent visual loss. Such leakage can be secondary to retinitis pigmentosa, and other retinal diseases including diabetic retinopathy, retinal vein occlusion, inflammatory processes such as uveitis, or can be a result of ocular surgery, referred to as Irvine-Gass Syndrome.

Abstract:Purpose: This study aimed to compare anterior segment parameters pre-and postoperatively in acute primary angle closure (APAC) and fellow primary angle closure suspect (PACS) eyes using anterior segment optical coherence tomography (AS-OCT) and evaluate the clinical effectiveness of cataract extraction in the treatment of APAC and fellow PACS eyes. Methods: Quantitative measurements of various parameters, including anterior chamber depth (ACD), anterior chamber volume (ACV), lens vault (LV), iridocorneal angle contact index (ITCI), iris thickness (IT), iris volume (IV), and iris curvature (IC), were obtained using Tomey CASIA2 AS-OCT on 60 eyes from 30 patients (APAC eyes and their fellow PACS eyes) before and after surgery. Simultaneous analysis of the differences between the APAC eyes and fellow PACS eyes in these parameters, visual acuity (VA), and intraocular pressure (IOP) were performed. Results: After surgery, both the APAC eyes and fellow PACS eyes (a total of 60 eyes) showed a significant increase in ACD and ACV, compared to preoperative measurements. Furthermore, LV and ITCI significantly decreased postoperatively. In the PACS group, IC significantly decreased postoperatively, while there was no statistically significant difference in the APAC group. In the APAC group, there was a significant decrease in IOP and improvement in VA at 1 day, 1 week, and the final follow-up compared to preoperative levels. The IOP values in the PACS group were within the normal range across various time points. VA in the PACS group showed significant improvement at 1 week postoperatively and at the final follow-up compared to preoperative levels. Significant differences of VA were observed in the initial, preoperative, first postoperative day, first postoperative week, and final follow-up, with better outcomes observed in the PACS group compared to the APAC group. Conclusion: Lens extraction surgery can significantly improve anterior segment crowding in APAC and PACS eyes. For APAC eyes, combined cataract extraction with intraocular lens implantation and gonioscopy-assisted goniosynechialysis under direct visualization is feasible and safe. Further, in the fellow PACS eye of APAC patients with either significant or mild cataracts, phacoemulsification can maintain or improve preoperative visual acuity to varying degrees and stabilize IOP.

Abstract:Objective To study the effect and safety of posterior Tenon subtenon triamcinolone acetonide (TA) injection as treatment for diffuse diabetic macular edema.Methods Forty-two patients (54 eyes) with diffuse diabetic macular edema were diagnosed by fundus fluorescence angiography and optical coherence tomography,posterior Tenon subtenon injection of TA 20 mg for treatment.All eyes were examined before treatment and the different time points after treatment by testing visual acuity,intraocular pressure,thickness of macular fovea.Results Preoperative visual acuity was 0.26 ± 0.15,postoperative was 0.29 ± 0.13,0.34 ± 0.14,0.43 ± 0.16,0.41 ± 0.13,0.38 ± 0.17 at 2 weeks,1 month,2 months,3 months and 6 months,respectively,visual acuity improved significantly,there was no significant difference between 2 weeks postoperative and preoperative (P ＞ 0.05),the remaining time points compared with preoperative differences were statistically significant (P ＜ 0.01).Preoperative thickness of macular fovea was (473.9 ±121.8) μ m,postoperative was (462.6 ± 93.3) μ m,(342.8 ± 87.1) μ m,(302.6 ± 67.8) μ m,(321.1 ±79.1) μ m,(355.2 ± 47.6) μ m at 2 weeks,1 month,2 months,3 months and 6 months,respectively,there was no significant difference between 2 weeks postoperative and preoperative (P ＞ 0.05),the remaining time points compared with preoperative differences were statistically significant (P ＜ 0.01).The intraocular pressure at 2 months were significantly higher than preoperative (P ＜ 0.05).No other complications were observed.Conclusion Posterior Tenon subtenon injection of TA as treatment for diffuse diabetic macular edema is safe and effective,which can inject repeatly.
A controlled clinical study on efficacy and safety of periocular triamcinolone acetonide injection for treating ocular myasthenia gravis

Abstract:Abstract Objective To evaluate the efficacy and safety of peribulbar triamcinolone acetonide injection for treating ocular myasthenia gravis (OMG), with a comparison of traditional oral drug therapy. Methods A total of 22 patients with OMG who received periocular triamcinolone acetonide injection (initially 20 mg weekly, then once per month later if symptoms were improved) from July 2019 to July 2022 were evaluated by a comparison of symptom degree before and after treatment. Adverse reactions were also monitored during the period of treatment. The period of follow-up was more than 6 months. Additionally, a comparison of the treatment efficacy between this periocular injection and traditional oral administration was performed in OMG patients. Results After 4 weeks of treatment, the degree of ptosis in OMG patients decreased to -3.00 ± 0.69, compared to the value (-0.86 ± 1.32) before treatment. The degree of ophthalmoplegia also decreased from 3.12 ± 0.72 to 0.86 ± 0.88 ( P < 0.001) after treatment. The achievement rates of minimal manifestations status (MMS)for ptosis and ophthalmoplegia after 4 week-treatment were 86.3% and 75%, respectively, while they were 50% and 30% in patients with traditional oral administration. There was statistically significant difference only in MMS (rather than symptom relief rate and generalization conversion rate) between two groups. No serious complications (except for intraorbital hematoma) were found in OMG patients during the treatment period. Conclusion Repeated peribulbar injection of triamcinolone acetonide can effectively alleviate the initial symptoms of OMG patients. However, the evaluation of its long-term efficacy is still needed. Abstract:Background: Post-surgical macular edema (ME) is a common cause of prolonged visual impairment. Here we report on the feasibility and clinical outcomes from the use of a novel suprachoroidal microcatheter to treat post-surgical chronic ME by the posterior suprachoroidal placement of a triamcinolone acetonide (TA) suspension. Methods: Two patients were catheterized with the Oxulumis suprachoroidal delivery system on two separate occasions starting 5 and 10 mm posterior to the limbus. The catheter only remains in the suprachoroidal space for the time of the drug administration. Visual acuity and spectral domain optical coherence tomography (SD-OCT) changes were followed over several weeks to months to determine the duration of ME resolution. Results: Suprachoroidal microcatheterization for posterior delivery of triamcinolone was possible in all attempts using the illuminated Oxulumis catheter. No reflux, scleral or choroidal trauma was observed. There was no intraocular pressure rise during the follow-up period. The triamcinolone deposit was visible on infrared imaging and on SD-OCT a choroidal elevation was visible. Both progressively disappeared over time. A rapid resolution of ME associated with improved vision was observed following each injection for 3 to 7 months with a TA dose of 2.4 mg or 4 mg. Conclusions: In these patients with poorly responsive ME, posterior suprachoroidal TA led to a visible suprachoroidal drug deposit and prolonged visual improvement. The Oxulumis microcatheterization device performed as expected and was not associated with any complications.
Effects of triamcinolone acetonide injected through vitreous cavity on intraocular pressure

Abstract:Objective To evaluate the effects of intravitreal triamcinolone acetonide(IVTA) on intraocular pressure(IOP) after the treatment of macular edema. Methods Forty-seven eyes of 47 patients with macular edema due to diabetic retinopathy (DR) or central retinal vein occlusion(CRVO) underwent IVTA with 4mg/0.1ml. Best-corrected visualacuity (BCVA) and IOP were observed before treatment and one month, three months, six months after treatment. Results Of all eyes the average BCVA was (0.15±0.12) before treatment, while one month, three months, and six months after treatment, the average BCVA was (0.36±0.25), (0.39±0.28) and (0.29±0.25) respectively, which had statistical significances compared with that before treatment (t=4.72,4.85,3.21,P<0.05).Meanwhile, Of all eyes IOP was normal ranging 11~18mmHg (13.76±2.18)mmHg before treatment, while one month, three months, and six months after treatment, the average IOP was (18.46±3.87)mmHg,(17.24±4.21)mmHg and (16.34±5.25)mmHg respectively, which had statistical significances compared with that before treatment(t=3.25,3.48,2.51, P<0.05). Some eyes had high IOP temporarily, but IOP could be controlled with topical medicines in all patients during observational period. Conclusion IOP elevation after treatment with IVTA 4mg/0.1ml is existing in a short time. It can be controlled and the treatment was safe and effective.

Abstract:Objective To compare the effect of intravitreal triamcinolone vs intravitreal bevacizumab for the treatment of macular edema in non-ischaemic central retinal vein occlusion.Methods The comparative single-center nonrandomized retrospective clinical interventionalstudy included 47 eyes of 47 patients with non-ischaemic CRVO, macular edema was diagnosed by examination of regular inspection, fundus fluorescein angiography(FFA) and optic coherence tomography(OCT ).This cases were divided into a a triamcinolone group of 28 patients (4.0 mg triamcinolone) and bevacizumab group of 19 patients (1.25 mg bevacizumab). During follow-up, 1.1±0.8 re-injections of the triamcinolone group (range: 1-2 injections) and 1.5± 1 re-injections ofbevacizumab (range: 1-3 injections) were administered.The mean follow-up were 5.98±4.35 months in TA group and 3.20±2.92 in bevacizumab group.At baseline, both study groups did not vary significantly in age, duration of symptoms ,visual acuity (BCVA),center macular thickness (CMT).Comprehensive ophthalmic evaluation was performed at baseline and at weeks 4,8,12 after treatment.Main outcome measures included CMT measured with OCT and BCVA. Results No significant differences between groups were observed for BCVA at week4(t=-0.141, P=0.889), 8(t=-1.637, P=0.127), 12(t=-0.479, P=0.650 )and for CMT at weeks4 (f=-0.479, P=0.650), 8 ((=0.743, P=0.478), 12 (t=1.979, P=0.083).There was a significant increase in 10P in the TA group. Conclusion The result in our observation showed that intravitreal injection of either triamcinolone acetonide or bevacizumab was well tolerated with a significant improvement in BCVA and decrease in macular edema for patients with non-ischaemic retinal vein occlusion .A randomly controlled multicenter clinical trial is necessary.
Microcoaxial Phacoemulsification Combined with Viscogoniosynechialysis for Patients with Refractory Acute Angle-Closure Glaucoma

Abstract:Purpose:To evaluate the safety and efficacy of microcoaxial phacoemulsification combined with viscogoniosynechialysis in managing refractory acute angle-closure glaucoma (AACG) unresponsive to conventional therapy.Methods:Seventeen consecutive eyes of patients with AACG>270 degrees peripheral anterior synechiae were treated with microcoaxial phacoemulsification combined with viscogoniosynechialysis. After intraocular lens implantation, a heavy viscoelastic agent was used to deepen the anterior chamber, and then injected near the angle for 360 degrees without touching any ocular structure, to release the peripheral anterior synechiae under gonioscopy. The viscoelastic agent was then removed.Results:Intraocular pressure was reduced from a median of 45.0 mm Hg initially to 15.0 mm Hg after the combined procedures at the final follow-up visit (P<0.001). All angles showed exposure of the trabecular meshwork over 360 degrees postoperatively without residual synechiae. The mean best corrected visual acuity improved from 0.7 to 0.18 logMAR at 6-month follow-up (P<0.001). Surgically induced astigmatism was 0.29 at 6-month follow-up. There was a 6.4% reduction in the mean corneal endothelial cell count. No severe complications were observed in any patient.Conclusions:Microcoaxial phacoemulsification combined with viscogoniosynechialysis is an effective and safe treatment for managing refractory AACG. Using a 2.2-mm incision caused less surgically induced astigmatism and also improved visual quality.

Abstract:Purpose: To compare the outcomes of congenital cataract surgery using intraoperative intracameral triamcinolone versus postoperative oral prednisolone to modulate ocular inflammation. Setting: Department of Congenital Cataract, Altino Ventura Foundation, Recife, Brazil. Design: Randomized clinical trial. Methods: Children younger than 2 years were randomly divided into 2 groups. The study group received an intraoperative intracameral injection of 1.2 mg/0.03 mL of triamcinolone acetonide. The control group (29 eyes) received 1 mg/kg per day of prednisolone syrup for 15 days postoperatively, which was then tapered over the following 2 weeks. Intraocular pressure (IOP), central corneal thickness (CCT), cell deposits on the intraocular lens (IOL), posterior synechiae, visual axis obscuration, additional surgical procedures, and IOL centration were assessed 12 months postoperatively. Results: The mean patient age at surgery was 10.45 months±6.22 (SD) in the study group (31 eyes) and 10.0±6.15 months in the control group (29 eyes) (P=.779). In both groups, the mean IOP and CCT did not change significantly postoperatively (study group P=.922 and P=.149, respectively; control group P=.483 and P=.416, respectively). The groups had similar incidences of cell deposits (P=.517) and posterior synechiae (P=.247). No eye developed visual axis obscuration or had additional surgical procedures. All eyes had a clinically centered IOL. Conclusion: One year postoperatively, the outcomes were similar with intraoperative intracameral triamcinolone injection and postoperative oral prednisolone for modulating inflammation after congenital cataract surgery.
