Detection and application of circulating tumor cells in patients with small cell lung cancer

Abstract:OBJECTIVE: To summerize the recent advances in research on circulating tumor cell (CTC) and its application in small cell lung cancer (SCLC) patients. METHODS: Application of Medline, PubMed and CNKI periodical full-text database retrieval system were searched with "circulating tumor cell, small cell lung cancer, detection" as keywords, from 2007 to 2012.Inclusion criteria: 1)Detection and application of CTC; 2)CTC detection in primary lung cancer patients. 3)Application of CTC in SCLC patients. By extraction according to the inclusion criteria, 23 literatures were finaly analyzed. RESULTS: CTC as a form of non-invasive "liquid biopsy" for the primary tumour, provide the possibility to perform real-time and dynamic testing for lung cancer patients. Methods to detect circulating tumor cells include Flow Cytometry Assay, CTC-chip, CellSearch System, and the Reverse Transcription Polymerase Chain Reaction. Cell search system can provide accurate and reproducible result, but this test is expensive. Despite the higher sensitivity of CTC-chip to detect CTC as compared to other methods, the test is limited to detect EpCAM-positive CTC only. CTC were present in 67% to 86% of patients with SCLC, and were related to the clinical stages, metastasis and recurrence of tumor. Monitoring the changes of CTC number during treatment could predict therapeutic effieacy and survival outcome, and higher numbers of CTCs were associated with worse survival compared with patients with lower CTCs. But a large prospective clinical study is required. CONCLUSIONS: CTC detection and its clinical application in lung cancer is still lack of unified standard and many research is still in processing. With the improvement of the detection method, more clinical research support, CTC detection in diagnosis, treatment and prognosis will be widely applied in SCLC in the future.
