Abstract:Urolithiasis has become more prevalent throughout China.Knowing stone composition helps identify their possible underlying etiology of urolithiasis, provide appropriate individualized treatment and prevent stone recurrence.To explore characteristics of urinary stone composition in China, and determine the effects of gender, age, body mass index (BMI), stone localization and geographical region on stone composition.We used Fourier transform infrared spectroscopy to analyse urinary stones from consecutive patients at 46 hospitals in seven geographic areas of China between June 1, 2010 and May 31, 2015.Chi-squared tests and logistic regression analyses were performed to determine associations between stone composition and sex, age, body mass index (BMI), stone location, and geographic region.The most common stone components were calcium oxalate (CaOx; 65.9%), carbapatite (15.6%), urate (12.4%), struvite (2.7%), brushite (1.7%), and cystine (0.9%). CaOx and urate occurred more frequently in males, whereas carbapatite and struvite were more common in females (p<0.01). CaOx and carbapatite stones were more common in those 30-50 and 20-40 years old, respectively than in other groups. Brushite and struvite stones were most common among those younger than 20 and older than 70 years old. Urate stone incidence increased with age. Cystine stone incidence decreased with age. Obese patients were more likely to have urate stones than carbapatite or brushite stones (p<0.01). CaOx, carbapatite, brushite, and cystine stones were more frequently observed in the kidney than other types, whereas urate and struvite stones were more frequent in the bladder (p<0.01). Stone composition varied by geographic region.Stone composition differed significantly in patients grouped by gender, age, BMI, stone localization, and geographic region.

Abstract:Background Interracial disparities in nephrolithiasis prevalence have been reported, but the interplay between genetics and the environment for urinary stone disease risk factors is poorly understood. To examine how environment may alter genetic predisposition for stone formation, we established the International Chinese Consortium on Nephrolithiasis (ICCON) as a multi-institutional collaboration to examine patterns of nephrolithiasis presentation between Chinese patients living in different countries. Methods Chinese patients undergoing percutaneous nephrolithotomy (PCNL) at six participating institutions in China and North America over 4 years were reviewed retrospectively. Patient demographics and clinical data were compared between Chinese patients living in China and North America. Results A total of 806 patients were included, encompassing 721 Chinese patients living in China and 85 living in North America. Nephrolithiasis patients living in China were more likely to be male (67% vs. 56%, P=0.02), present at a younger age (48.6±15.0 vs. 55.0±13.0 years, P<0.01), and have a lower BMI (24.6±4.0 vs. 25.9±5.7, P=0.04) but were less likely to form struvite stones (5.5% vs. 14.1%, P<0.01). No cystine stone patients were seen in North American Chinese patients, whereas 1.8% of nephrolithiasis patients living in China presented with cystine stones. Similar rates of calcium-based and uric acid calculi as well as urinary pH were seen among both groups. Conclusions Significant differences exist between Chinese nephrolithiasis patients living in China compared to those living in North America, highlighting the importance of environmental factors in addition to genetics in modulating risk for urinary stone disease.

Abstract:Background: Accumulated evidences indicate that dysbiosis of the urinary microbiota is associated with kidney stone formation. In the present study, we aimed to investigate the urinary microbiota composition and functionality of patients with calcium oxalate stones and compare it with those of healthy individuals. Method: We collected bladder urine samples from 68 adult patients with calcium oxalate stones and 54 age-matched healthy controls by transurethral catheterization. 16S rRNA gene and shotgun sequencing were utilized to characterize the urinary microbiota and functionality associated with calcium oxalate stones. Results: After further exclusion, a total of 100 subjects was finally included and analyzed. The diversity of the urinary microbiota in calcium oxalate stone patients was not significantly different from that of healthy controls. However, the urinary microbiota structure of calcium oxalate stone formers significantly differed from that of healthy controls (PERMANOVA, r = 0.026, P = 0.019). Differential representation of bacteria (e.g., Bifidobacterium ) and several enriched functional pathways (e.g., threonine biosynthesis) were identified in the urine of calcium oxalate stone patients. Conclusion: Our results showed significantly different urinary microbiota structure and several enriched functional pathways in calcium oxalate stone patients, which provide new insight into the pathogenesis of calcium oxalate stones.

Abstract:Urolithiasis is a common urinary tract disease with several etiologic factors and is a public health burden worldwide. Studies have revealed that geographic and gender differences characterize its incidence. In addition, urolithiasis is characterized by high morbidity and recurrence rates. According to the stone formation mechanism, stones can be categorized as infectious or metabolic. Identifying stone components can benefit patients by depicting potential metabolic risk factors while helping physicians determine appropriate stone treatment and prevention strategies. The current study aimed to analyze the stone composition characteristics in 3637 cases with upper urinary tract stones from a single center in northern China while clarifying the relationship with gender, age, and time.

Abstract:Background: The purpose of the study was to compare the incidence of urinary stones of She minority and Han nationality and analyze the composition of urinary stones. Methods: The study was performed in 381 cases with 181 She minority and 200 Han nationality. The composition of stones was mainly analyzed by infrared absorption spectrum. The incidence of urinary stones at different ages, different gender and different parts was compared between She minority and Han nationality. Results: The urinary stone incidence of males was about twice as high as in women in She minority and Han nationality, and the incidence reached its maximum between the ages of 41 and 60, but the incidence decreased after 60 years of age. The distribution characteristics of urethra stones between She minority and Han nationality showed a significant difference (p < 0.05). The differences of carbonate apatite and struvite in male and female were statistically significant between She minority and Han nationality (p < 0.05). The level of Ca2+ and HPO42- in serum showed significant difference between She minority and Han nationality (p < 0.05). Conclusions: According to these results, we put forward corresponding preventive measures of urinary stones in She minority.

Abstract:To better understand the basis of urinary calculi formation, we studied the composition, microstructure, and element analysis of different types of urinary calculi. Sixty people with urinary stones in Shanxi province were selected randomly. The composition of urinary stones was analyzed using Fourier transform infrared spectroscopy. The microstructure of material components was observed by a scanning electron microscopy and the elemental distribution and composition were analyzed by an X-ray energy spectrometer. Furthermore, general information, BMI, history of medicine, chronic medical history, family history, and recurrence rates were collected. Female-to-male ratio was 1:2.5; median age was 43.2 years old. Of the patients, 13.3% were found definite family history and 46.6% of patients for recurrence history. It was found that mixed stones account for the largest proportion (65%), followed by calcium oxalate monohydrate calculi (26.67%). In mixed stones, the mixture of calcium oxalate monohydrate and hydroxyapatite had the largest proportion, accounting for 71.79%. Stones showed different microcosmic characteristics and element distribution. Stones varied widely in distribution, infrared spectrum, microstructure, and element composition, which provided an important basis for urinary calculi research regarding urinary stone formation.

Abstract:Urinary stones pose a significant health concern globally. The incidence rates vary and are influenced by diverse factors. Despite advancements in the treatment modalities, increasing rate of urinary stones necessitates the development of stone prevention strategies and understanding of metabolic risk factors. Objective: To determine the relationship between chemical composition of urinary calculi and evaluation on metabolic disturbance in 1080 Uyghur, Kazakh and Han patients of Xinjiang. Methods: The stone composition of 1080 patients and analysis of 24-h urine and serum biochemistry results of 775 cases was evaluated by infrared spectroscopic analysis and automatic biochemical machine respectively. Results: Among 644 cases of urinary stones, calcium oxalate stones (409 cases) and mixed type stones (436 cases) were predominant. Metabolic disturbances were found in 643 cases (82.97%) out of 775 patients, showing various urinary abnormalities like hyperuricosuria, hyperoxaluria, hyper-phosphaturia, hyper-cystinuria, hypercalciuria, hypomagnesuria, hypocitraturia, and reduced urine volume (<2000ml in 407 cases). Conclusions: Urinary calculi have intimate connection with metabolic evaluation. Metabolic evaluation and composition of urinary calculi have great significance in treatment and prevention of urinary calculi.

Abstract:Objective:To compare the efficacy and safety of percutaneous nephrolithotomy(PCNL) for stones in upper urinary tract with different composition.Method:The clinical data of 190 patients underwent PCNL between April 2009 and November 2010 were retrospectively reviewed.According to the postoperative stone composition analysis result,the patients were divided into 3 groups(oxalate salt group,uric acid salt group and phosphate salt group).Stone burden,surgical time,stone free rate,change of hemoglobin level in peripheral blood and fever were compared with the three groups.Result:The stone volume was10.07±25.60 cm3(mean±standard deviation) in oxalate group,10.91±11.38cm3 in uric acid salt group and 19.19±56.16cm3 in phosphate salt group,P=0.584.The stone free rate was 75.0% in oxalate group,68.3% in uric acid salt group and 63.3% in phosphate salt group,P=0.316.The operative time was(76.2±38.9) minutes in oxalate group,(90.4±46.8) minutes in uric acid salt group and(76.6±37.0) minutes in phosphate salt group,P=0.155.Thirty patients got fever after operation in oxalate group,15 patients in uric acid salt group,17 patients in phosphate salt group,P=0.740.The perioperative hemoglobin level change was(12.6±11.1) g/L in oxalate group,(12.2±11.2) g/L in uric acid salt group,and(10.2±12.1) g/L in phosphate salt group,P=0.469.There were 4,1 and 5 patients received blood transfusion respectively.Conclusion:The efficacy and safety of precutaneous nephrolithotomy for oxalate salt stone,uric acid salt stone and phosphate salt stone were similar.
Study of Risk Factor of Urinary Calculi According to the Association Between Stone Composition with Urine Component

Abstract:Urolithiasis is a common urinary disease with high recurrence. The risk factor for the recurrence of calculi is not very clear. The object of the present study was to evaluate the association between calculi composition and urine component and analyse the risk factor for the recurrence of urolithiasis. In this study, a total of 223 patients with calculi and healthy control were enrolled, and the components of the calculi and urina sanguinis collected before surgery were analysed. Of the 223 patients, 157 were males and 66 were females. According to the stone composition, the case group was subdivided into three groups. 129 patients had single calcium oxalate stones, 72 had calcium oxalate stones mixed with other stones and 22 had other type of stones excluding calcium oxalate stones. Urine biochemicals were analysed and the associations were found between the chemicals in each group. Multivariate logistic analysis demonstrated that reduced urinary magnesium and uric oxalic acid were independent risk factors when comparing all cases with normal controls. Only decreased urinary magnesium was found to be a risk factor comparing the single calcium oxalate group with normal control group. Low level of urinary magnesium and uric oxalic acid were found to be risk factors comparing the mixed calcium oxalate group with normal control group. No risk factor was found comparing the other stone group with normal control group. In conclusion, there were clear relationships between stone components and urine chemicals. Urine chemicals might be risk factors to predicate the occurrence of urolithiasis.
Metabolic Differences Between Unilateral and Bilateral Renal Stones and Their Association with Markers of Kidney Injury

Abstract:Urinary stone disease (USD) has been associated with an increased risk for chronic kidney disease (CKD) and end-stage renal disease (ESRD) in Western populations. However, the metabolic disorders associated with unilateral and bilateral renal stones and the association of these types of stones with CKD and kidney tubular injury markers, such as urine N-acetyl-ß-D-glucosaminidase (NAG) and alpha-1-microglobulin (α1-MG), have not been fully examined. We performed a cross-sectional study of 10,281 participants in rural China in 2014. All the subjects underwent renal ultrasound to detect USD; stone formers were divided into groups with unilateral or bilateral renal stones by ultrasound exams. CKD was defined as a decreased estimated glomerular filtration rate (eGFR, <60 mL/min/1.73 m 2 ) and/or albuminuria (albumin-to-creatinine ratio ≥30 mg/g). Increased urine NAG and α1-MG levels were defined as their values above the 75th percentile of the sample distribution. Among all the participants, 4.9% (n=507) had unilateral renal stones, and 0.7% (n=75) had bilateral renal stones. The proportion of CKD in the non-stone, unilateral and bilateral renal stone formers was 11.0%, 19.2%, and 29.7%, respectively (p for trend <0.001). Individuals with bilateral renal stones had the highest proportion of metabolic components, such as elevated blood pressure and serum glucose. In multivariate analyses after adjustment for multiple confounders, bilateral renal stones were significantly associated with an increased risk of decreased eGFR (odds ratio (OR) 3.38; 95% CI 1.05–10.90), albuminuria (OR 3.01; 95% CI 1.76–5.13), CKD (OR 3.18; 95% CI 1.88–5.36), increased urine NAG/Cr (OR 1.95; 95% CI 1.21–3.16) and α1-MG/Cr levels (OR 2.54; 95% CI 1.56–4.12) compared with the lack of stones. Bilateral renal stones were associated with a higher risk of CKD and higher levels of kidney tubular injury markers. Clinicians should pay attention to metabolic disorders in bilateral renal stone formers.

Urinary stone composition analysis of 3684 patients in the eastern Shandong region of China

Abstract:Objective: There has not been a study that analyzed the composition of urinary stones from patients in the eastern Shandong region of China. Thus, we explored the composition of urinary stones in the eastern Shandong region of China and discuss the clinical significance of the findings. Methods: A total of 3684 urinary calculi from the eastern Shandong region were collected in our study. Compositions of the stones were analyzed using an Automatic Infrared Spectrum Analysis System (type LIIR-20). The results were verified through manual analysis of the spectrograms, which was accompanied by polarizing microscopy and chemical analysis if necessary. Results: Among the 3684 specimens, there were 1767 single-component stones and 1917 mixed-component stones. According to the main components of the stones, the stones were divided into the following types: calcium oxalate monohydrate stones (1779, 48.3%), anhydrous uric acid stones (1105, 30.0%), carbonate apatite stones (590, 16.0%), ammonium magnesium phosphate hexahydrate stones (143, 3.9%), calcium oxalate dihydrate stones (36, 1.0%), and cystine stones (31, 0.8%). Conclusions: There was a relatively high rate of uric acid stones in patients from the eastern Shandong region of China.

Abstract:目的研究结石成分与代谢异常的关系.方法分析284例泌尿系结石患者的结石成分及其中191例完整血生化及24 h尿分析结果.结果草酸钙结石195例(68.7%),感染性结石41例(14.4%),尿酸结石38例(13.4%),磷酸盐结石6例(2.1%),胱氨酸结石4例(1.4%).191例有完整血生化及24h尿分析结果患者中代谢异常176例(92.1%),其中高钙尿症27例(14.1%),高尿酸尿症54例(28.3%),高草酸尿症41例(21.5%),高尿磷55例(28.8%),低枸橼酸尿症128例(67.0%),低尿镁症80例(41.9%),24 h尿量＜2000 ml者65例(34.0%),高胱氨酸尿症4例(2.1%).94例随访10～58个月,平均22个月.无泌尿系结石复发52例,复发42例,无复发组代谢无异常12例,复发组2例(P＜0.01),无复发组和复发组4种异常者分别为2例和7例(P＜0.01).结论结石复发与多种代谢异常关系密切;确定结石成分及患者的代谢评价对泌尿系结石的成因、治疗和预防有重要的临床意义。

Abstract:The aim of this study is to perform proteomic and metabolomic analyses in bilateral renal pelvis urine of patients with unilateral uric acid kidney stones to identify the specific urinary environment associated with uric acid stone formation. Using cystoscopy-guided insertion of ureteral catheters, bilateral renal pelvis urine samples are collected. Liquid chromatography-tandem mass spectrometry (LC-MS/MS) is employed to identify differentially expressed proteins and metabolites in the urine environment. Differentially expressed proteins and metabolites are further analyzed for their biological functions and potential metabolic pathways through Gene Ontology (GO) analysis and Kyoto Encyclopedia of Genes and Genomes (KEGG) enrichment analysis. In the urine from the stone-affected side, eight differential proteins were significantly upregulated, and six metabolites were dysregulated. The uric acid stone urinary environment showed an excess of α-ketoisovaleric acid and 3-methyl-2-oxovaleric acid, which may contribute to the acidification of the urine. Functional and pathway analyses indicate that the dysregulated metabolites are mainly associated with insulin resistance and branched chain amino acid metabolism.

Urinary metabolic evaluation of stone formers-a Malaysian perspective

Abstract:Objective: To investigate the urinary metabolic excretion pattern among local stone formers given the great differences in the intrinsic and extrinsic risk factors as well as the urinary metabolic excretions compared with other populations. Methods: Thirty urinary stone formers out of an initial 62 recruited provided a complete 24-hour urine sample for metabolic evaluation. Student's t-test and Pearson correlation test were used for the statistical analysis. Results: Urinary volume (1719 ± 712 vs 1215 ± 575, P < .05) and oxalate excretion (0.386 ± 0.111 vs 0.306 ± 0.104, P < .05) were significantly higher among stone formers than controls. Other commonly studied urinary parameters and urinary melamine did not differ significantly between the 2 groups. Similarly, the calcium/citrate ratio was unable to discriminate the stone formers from their controls. Hypocitraturia was the most prevalent urinary abnormality found in stone formers and low urinary citrate excretion was a general phenomenon in both stone formers and controls. Comparing within the stone formers cohort, the recurrent stone formers had a significantly higher urinary saturation and calcium excretion than their first-time stone former counterparts. Conclusion: Elevated urinary oxalate level was the most important urinary risk factor among the local stone formers. A low urinary citrate excretion appeared to be a general phenomenon among the studied cohorts.

Abstract:The urinary concretion is a kind of urinary system disease which does harm to body health.The calcium oxalate stone is the most common type.This article summarized research progress in recent years of different simulation system to this kind of urinary concretion formation and the influencing factors in an unified riew of chemistry and medicine in the hope of providing some enlightenment for the understanding of form ation mechanism and the clinical treatments.

Abstract:A series of 5,248 urinary stones was analyzed by Fourier transform infrared spectroscopy between 1999 and 2008. This study evaluated the percentage of each stone type and the association with sex and age in Chinese stone formers presenting with the first stone episode. The overall sex ratio (male:female) was 2.34:1. Results showed that the preponderant type of stone was calcium oxalate, followed by carbapatite, anhydrous uric acid, struvite and cystine. Struvite stones in this study accounted for a relatively low rate compared to that reported by others. Of 5,248 stones, only 38.1% had one component, 42.5% consisted of two components, and 20.4% consisted of three components. Our results also showed the higher percentage of carbapatite stones in females than in males and the increment of anhydrous uric acid stones with age. In addition, the percentage of calcium oxalate stones decreased with increase in the percentage of carbapatite stones over the period.

Abstract:The formation and physicochemical properties of kidney stones (KSs) are closely associated with diet. In view of the differences in ethnicity and dietary composition between Chinese and Western populations, the present study aimed to investigate the association between intestinal dysbacteriosis and KSs in China. The current study examined the differences in intestinal microbes between the KS disease (KSD) and the healthy control (HLT) groups, and statistically significant differences based on 16s rRNA gene amplicons were identified using a Student's t‑test or one‑way ANOVA. In addition, the calcium oxalate KS (COKS), uric acid KS (UAKS) and carbonate apatite KS(CCKS) groups were compared with a non‑parametric statistical test. Determination of bacterial abundance was performed via the analysis of 16s rRNA marker gene sequences using next‑generation sequencing. Firmicutes (F) and Bacteroides (B) levels were significantly higher in the KSD group compared with the HLT group (B/F=0.67 vs. 0.08; P<0.001), as were the overall levels of B (6.19‑fold higher compared with the HLT group; 22.2 vs. 3.6%; P<0.001). The Prevotella‑9 abundance levels in the KSD group were 4.65‑fold higher compared with those in the HLT group (8.8 vs. 1.9%; P<0.001). The levels of Blautia and Lachnoclostridium were significantly decreased in the KSD group (13.3 vs. 6.0%; and 5.0 vs. 7.9%; both P<0.05). Moreover, Prevotella‑9 levels were higher in non‑calciferous KSs (UAKS) compared with calciferous KSs (COKS and CCKS). Therefore, the findings of the present study indicated a key association between specific KS components and intestinal flora, providing a theoretical basis for new treatment methods for KSs. Moreover, differences and interactions between these bacteria could initially predict specific types of urolithiasis.
