The influence of topical all-trans retinoic acid on inhibiting sebum secretion in acne patients

Abstract:Objective: Topical all-trans retinoic acid cream is effective in treating acne. It is controversy whether the topical all-trans retinoic acid cream can inhibit sebum secretion. Since there is limited information available about this issue, here to determine whether topical all-trans retinoic acid cream can inhibit sebum secretion in acne patients and to analyze sebum lipid composition is attempted. The mechanisms how the topical all-trans retinoic acid cream can affect acne are studied. Methods: Thirty-three cases of acne patients were randomly divided into experimental group (n=20) and control group (n=19). The patients' foreheads were topically treated with either 0.025% all-trans retinoic acid cream or cream base alone for one week. Before and after treatment, the lipid composition including squalene, cholesterol esters, triglycerides, cholesterol in sebum from the foreheads was determined by thin layer chromatography. The AlphaImager IS-2200 imaging system was used to quantitatively and qualitatively analyze sebum lipid composition. Results: Sebum secretion rates, the contents of wax esters and free fatty acids decreased significantly following 0.025% all-trans retinoic cream treatment (t = 4.18, P = 0.000 5 for secretion rates; t = 4.03, P = 0.000 7, for wax esters; t = 4.443, P = 0.000 3 for free fatty acids). The percentage of triglyceride increased (t = 4.78, P = 0.000 1) whereas the percentage of fatty acids decreased significantly (t = 3.43, P = 0.003). As compared with the cream base control, experimental group had significantly lower sebum secretion rates (t = -2.086, P = 0.046), wax esters (t = -2.82, P = 0.008) and free fatty acids contents (t = -2.196, P = 0.034), and dramatically higher percentage of triglyceride (t = 3.335, P = 0.002). Conclusion: The topical 0.025% all-trans retinoic acid cream can inhibit sebum secretion, mainly free fatty acids and wax esters.
