Abstract:A variety of drugs have been known to induce disulfiram-like reactions in individuals exposed to ethanol, including certain cephalosporin antibiotics with methylthiotetrazole (MTT) substituents or methylthiodioxotriazine (MTDT) rings. Among cephalosporins, cefixime is known to cause fewer disulfiram-like reactions. This case report, the first involving a pediatric patient, presents the scenario of a 14-year-old female who exhibited drowsiness, loss of consciousness, and cold extremities within an hour after ingesting 9 cefixime capsules. Upon admission, drug intoxication was considered, prompting immediate gastric lavage and toxicology tests, which revealed the presence of both cefixime and alcohol. Subsequent monitoring of vital signs, rehydration, and symptomatic treatments aimed at facilitating toxic excretion were administered during hospitalization. Following initial assessment by a clinical pharmacist, drug intoxication was deemed improbable, though an atypical disulfiram-like reaction or alcohol intoxication could not be ruled out. Further evaluation, coupled with the child's cefixime overdose, suggested an atypical disulfiram-like reaction. This case underscores the potential for disulfiram reactions even with cephalosporins lacking MTT substituents or MTDT rings. Notably, it is the first report of an atypical disulfiram-like reaction triggered by alcohol consumption following cefixime overdose, emphasizing the importance of caution in cefixime usage and avoidance of alcohol or alcohol-containing substances.

Abstract:A 70-year-old man received cefoperazone sodium/sulbactam sodium,vancomycin,and omeprazole for worsened cough and expectoration accompanied by a fever and short of breath.On day 8,he was given citalopram 10 mg once daily for depression.On day 9,his ALT and AST levels increased from 47 U/L and 52 U/L to 139 U/L and 145 U/L,respectively.The dosage of citalopram was increased to 20 mg once daily due to poor control of depression.Subsequently,his hepatic enzyme levels increased markedly and,on day 19,the laboratory tests revealed a ALT level of 529 U/L and a AST level of 256 U/L.Abnormal hepatic function was considered to be induced by citalopram,so citalopram was stopped and other drugs were continued.Then his hepatic function gradually returned to normal.
Mortality Associated with Third Generation Cephalosporin-Resistance in Enterobacteriaceae Infections: a Multicenter Cohort Study in Southern China.

Abstract:ABSTRACTBackgroundEmerging third generation cephalosporin-resistant Enterobacteriaceae (3GCR-EB) pose a global healthcare concern. We assessed excess mortality in patients infected with 3GCR-EB compared to patients infected with third-generation cephalosporin-susceptible Enterobacteriaceae (3GCS-EB).MethodsThe study cohort comprised all inpatients with a community-onset or healthcare-associated infection caused by Enterobacteriaceae in three tertiary-care public hospitals in 2017. Excess in-hospital mortality was assessed using competing risk survival models, adjusting for baseline patient characteristics.ResultsOf 2,343 study patients (median age 60 years; 45.2% male), 1,481 (63.2%) had 3GCS-EB and 862 (36.8%) 3GCR-EB infection. 494 (57.0%) 3GCR-EB isolates were co-resistant to fluoroquinolones and 15 (1.7%) co-resistant to carbapenems. In-hospital mortality was similar in the 3GCS-EB and 3GCR-EB groups (2.4% vs. 2.8%; p=0.601). No increase in the hazard of in-hospital mortality was detected for 3GCR-EB infections compared to 3GCS-EB infections (sub-distribution hazard ratio [HR] 0.80; 95%CI, 0.41 - 1.55) in multivariable analysis adjusting for patient age, sex, intensive care admission, origin of infection and site of infection. Analysis of cause-specific hazards showed that 3GCR-EB infections significantly decreased the daily rate of hospital discharge (cause-specific HR=0.84; 95%CI, 0.76 - 0.92) thereby leading to lengthier hospitalizations.ConclusionThird-generation cephalosporin resistance in Enterobacteriaceae infection per se was not associated with increased in-hospital mortality in this study. However, 3GCR-EB infections were seen to place significant healthcare burden by increasing the length of hospitalization compared to 3GCS-EB infections.
Case report: Decreased hemoglobin and multiple organ failure caused by ceftizoxime-induced immune hemolytic anemia in a Chinese patient with malignant rectal cancer

Abstract:Background: Drug-induced immune hemolytic anemia (DIIHA) is a rare but serious condition, with an estimated incidence of one in 100,000 cases, associated with various antibiotics. This study reports on a case of ceftizoxime-induced hemolysis observed in a patient in China. Case description: A Chinese patient diagnosed with malignant rectal cancer underwent antimicrobial therapy after laparoscopic partial recto-sigmoid resection (L-Dixon). After receiving four doses of ceftizoxime, the patient developed symptoms including rash, itchy skin, and chest distress, followed by a rapid decline in hemoglobin levels, the presence of hemoglobin in the urine (hemoglobinuria), renal failure, and disseminated intravascular coagulation. Laboratory analysis revealed high-titer antibodies against ceftizoxime and red blood cells (RBCs) in the patient's serum, including immunoglobulin M (IgM) (1:128) antibodies and immunoglobulin G (IgG) (1:8) antibodies, with noted crossreactivity to ceftriaxone. Significant improvement in the patient's hemolytic symptoms was observed following immediate discontinuation of the drug, two plasma exchanges, and extensive RBC transfusion. Conclusion: This case, together with previous reports, underscores the importance of considering DIIHA in patients who exhibit unexplained decreases in hemoglobin levels following antibiotic therapy. A thorough examination of the patient's medical history can provide crucial insights for diagnosing DIIHA. The effective management of DIIHA includes immediate cessation of the implicated drug, plasma exchange, and transfusion support based on the identification of specific drug-dependent antibodies through serological testing.
A Case-Report of Drug-Induced Mixed Liver Injury Resulting From Cefepime Exposure

Abstract:A 99-year-old African-American male presented to the hospital with severe sepsis secondary to a urinary tract infection. Upon initial presentation he was tachycardic, hypotensive and had leukocytosis. While he had signs of acute kidney injury, no signs of acute liver injury were present with his alanine transferase (ALT) and amino transferase (AST) levels measuring at 22 and 44 U/L, respectively. During the treatment course the patient began to show signs of clinical improvement. Despite this, his ALT and AST began to increase on day 2 of treatment and reached their peak of 210 and 239 U/L on day 4. Cefepime-induced liver injury was suspected and cefepime was discontinued. Upon cefepime discontinuation, liver enzymes downtrended and gradually returned to normal. No other likely medication causes of liver injury could be identified and alternative medical causes were ruled out. The lack of an alternative cause and the temporal relationship of cefepime use to hepatic dysfunction support the diagnosis of cefepime-induced liver injury. The patient’s Roussel Uclaf Causality Assessment Methods score was 7, indicating this was a possible case of cefepime-induced liver injury, and the Naranjo Nomogram score was 5 indicating this was a probable case of cefepime-induced liver injury. While cefepime-induced liver injury is rare, clinicians should be cognizant of the potential for this adverse effect if liver enzyme elevation is detected during cefepime therapy and other common causes have been ruled out.

Abstract:There has previously been a report of a patient developing haemolytic anaemia following exposure to cefoperazone. Another case has been reported involving the detection of cefoperazone-dependent antibodies in the absence of immune haemolytic anaemia. To date, no serological evidence has been reported to suggest that cefoperazone can lead to drug-induced immune haemolytic anaemia (DIIHA). This report aims to fill these gaps in knowledge by describing a case of DIIHA caused by cefoperazone-dependent antibodies. A 59-year-old man developed fatal haemolytic anaemia while receiving cefoperazone-tazobactam or cefoperazone-sulbactam for the treatment of a lung infection that occurred after craniocerebral surgery. This eventually led to renal function impairment. Prior to the discontinuation of cefoperazone treatment, the patient showed strong positive (4+) results for both anti-IgG and anti-C3d direct antiglobulin test (DAT), while cefoperazone-dependent IgM and IgG antibodies were detected. The patient's plasma and O-type RBCs were incubated with tazobactam or sulbactam solution at 37°C for 3 h, the results of DAT for anti-IgG and anti-C3d were both positive. Forty-three days after the discontinuation of cefoperazone, the results of DAT for anti-IgG and anti-C3d were negative. Meanwhile incubation of the patient's fresh serum and his own RBCs with cefoperazone at 37°C, gave rise to mild haemolysis, and the results of DAT for both anti-IgG and anti-C3d were positive. It is suggested that cefoperazone-dependent antibodies can activate complement, and the non-immunologic protein adsorption effect of tazobactam or sulbactam can enhance IgG and complement binding to RBCs. This may promote the formation of immunocomplexes and complement activation, thereby aggravating haemolysis.

Cefepime-Induced Neurotoxicity in a 74-Year-Old Woman

Abstract:Cefepime is a fourth-generation cephalosporin with anti-pseudomonal coverage. It has been known to cause neurotoxicity, especially in critically ill patients and those with renal impairment. This neurotoxicity is poorly characterized and under-recognized. We present a case of cefepime-induced neurotoxicity in a 74-year-old woman being treated for cellulitis and osteomyelitis. Symptoms were gradual in onset and included confusion, verbal perseveration, and myoclonus. EEG findings included generalized periodic discharges (GPD) and generalized rhythmic delta activity with admixed sharps (GRDA + S). Symptoms resolved one to two days after the cessation of cefepime and anti-epileptic therapy with lorazepam, topiramate, and levetiracetam. We follow this with a discussion of available literature and recommend regular therapeutic drug monitoring in the future.
Ceftriaxone-Induced Encephalopathy in a Patient With Chronic Kidney Disease

Abstract:Neurotoxicity is an acknowledged side effect of third and fourth-generation cephalosporins, but its occurrence with ceftriaxone is not widely recognized. This article presents a case involving a 56-year-old woman with multiple comorbidities who sought medical attention after experiencing lipothymia. The initial diagnosis suggested a urinary tract infection with acute kidney failure, leading to the initiation of ceftriaxone and hemodialysis. Subsequently, the patient exhibited a progressive deterioration of her neurological state, characterized by agitation and chorea. Metabolic encephalopathy, seizure/nonconvulsive status epilepticus, and acute central nervous system lesions were considered primary differential diagnoses, all of which were subsequently ruled out through thorough investigations. Days later, a remarkable recovery of the patient's neurological state was observed. A retrospective analysis revealed a correlation between the improvement and the fourth day of antimicrobial suspension. Consequently, a presumptive diagnosis of ceftriaxone-induced encephalopathy was made. This unusual case underscores the importance of recognizing the potential for pharmacological encephalopathy, particularly with ceftriaxone, and emphasizes its reversibility upon discontinuation of the implicated drug. Clinicians should remain vigilant to this uncommon adverse effect, promoting timely intervention and improved patient outcomes.
Cefadroxil Tablets Induced-Drug Hypersensitivity Syndrome Complicated with Acute Myocarditis and Death

Abstract:A 61-year-old female patient took cefadroxil tablets 0. 5 g twice daily herself due to fever after blepharorrhaphy in her right eye. Her temperature returned to normal,but the medication was not discontinued and 2 weeks later,the patient developed skin rash on her limbs with severe itching,and fever (39 ℃). Then cefadroxil tablets were stopped but her symptoms were gradually aggravated,and then facial swelling appeared 5 days later. On the seventh day of onset,she was admitted to the hospital,with white blood cell count 25 × 109 / L,percentage of eosinophils 0. 29,eosinophil count 7. 4 × 109 / L,and normal liver and kidney function,myocardial enzymes,and electrocardiogram. Antiallergic and symptomatic treatments were given after the admission. On day 4 of admission,the patient developed dizziness and general fatigue. Her blood pressure was 96 / 61 mmHg and her heart rate was 110 beats per minute. The electrocardiogram showed multilead ST segment elevations and the echocardiography revealed a small amount of pericardial effusion. Laboratory tests showed that the cardiac troponin T (cTnT)was 1. 52 μg/ L,creatine kinase (CK)was 236 U/ L,CK-MB was 63 U/ L,and N-terminal pro-brain natriuretic peptide (NT-proBNP)was 9708 ng/ L. The patient was diagnosed with drug hypersensitivity syndrome complicated with acute myocarditis and pericarditis. Intravenous infusion of high dose methylprednisolone (500 mg/ d), human immunoglobulin (40 g/ d),and myocardial nutrition treatments were given. On day 5 of admission, the patient was transferred to the cardiovascular care unit (CCU),when her blood pressure was 80 / 50 mmHg,CK was 498 U/ L,CK-MB was 98 U/ L,cTnT was 5. 33μg/ L,and left ventricular ejection fraction was 55％ . However,the patient′ myocarditis and pericarditis progressed rapidly. On day 3 in the CCU,the laboratory tests showed CK 1252 U/ L,CK-MB 231 U/ L,cTnT 4. 57 μg/ L,NT-proBNP 17979 ng/ L,and left ventricular ejection fraction 10％,which then decreased to 5％ on day 6. Finally,the patient died on day 9 in the CCU.
Severe drug-induced immune hemolytic anemia due to cefmetazole: A case report

Abstract:Objective: To report a case of drug-induced immune hemolytic anemia (DIIHA) that was suspected to have been caused by cefmetazole. Case summary: A 93-year-old woman with no previous history of liver complications underwent a contrast-enhanced computed tomography scan, which resulted in a diagnosis of acute cholecystitis. The patient experienced intravascular hemolysis and rapid progression of anemia after being exposed to 2 g/day of cefmetazole. After 48 hours of cefmetazole administration, the patient was transferred to the intensive care unit (ICU) of our facility. In view of the severe autoimmune hemolytic anemia, the patient was started on steroid immunosuppression. The patient's condition further deteriorated for 13 hours after treatment and showed increased lactic acidosis and decreased consciousness, thus, the patient was intubated and managed on a ventilator. Lactic acidosis was not easily controlled, and the patient required continuous renal replacement therapy within 15 hours of ICU admission. Blood pressure was unable to be maintained even with the use of catecholamine, and the patient subsequently died 28 hours after ICU admission. Blood taken immediately after death was used to perform a drug-dependent antibody test where DIIHA due to cefmetazole was diagnosed. Conclusion: If there is rapid progression of anemia following drug administration, the possibility of DIIHA needs to be considered. If DIIHA is suspected, identification and immediate discontinuation of the causal drug are essential, and a drug-dependent antibody test should be considered.

Abstract:We report a fatal case of New Delhi metallo-β-lactamase (NDM)–producing Escherichia coli in a bacteremic patient with sequential failure of aztreonam plus ceftazidime-avibactam followed by cefiderocol. Acquired resistance was documented phenotypically and mediated through preexisting and acquired mutations. This case highlights the need to rethink optimal treatment for NDM-producing organisms.

Abstract:Therapeutic Advances in Drug Safety, Volume 15, Issue , January-December 2024. Background:Cefuroxime has played a crucial role in the prevention and treatment of bacterial infections. However, the differences in adverse events across formulations and routes remain unclear.Objectives:This study aimed to investigate the post-marketing safety of cefuroxime, particularly concerning formulations and routes.Design:A retrospective pharmacovigilance study of cefuroxime was conducted using the data from Food and Drug Administration Adverse Event Reporting System database.Methods:The clinical characteristics and concomitant drugs reported with cefuroxime were investigated. Adverse event signals of cefuroxime were identified based on four disproportionality algorithms. The signal differences of cefuroxime across formulations and routes were further examined.Results:A total of 1810 adverse event reports associated with cefuroxime were identified, and 181 cefuroxime-associated signals were detected. Compared with tablets, injections were more likely to cause preferred terms 'blood pressure decreased' and 'anaphylactic shock'. In addition, system organ class 'eye disorders' significantly increased when cefuroxime was administered intraocularly, underscoring the importance of exercising caution regarding ocular toxicity.Conclusion:The adverse events associated with cefuroxime were significantly different across formulations and routes, which deserve special attention in clinical use.Plain language summaryPost-marketing safety concerns with cefuroximeBackground:Cefuroxime is a commonly used antibiotic. Neutropenia Induced by Ceftriaxone and Meropenem

Abstract:Neutropenia by non-chemotherapy drugs is an extremely rare idiosyncratic life-threatening drug reaction. Ceftriaxone and meropenem are widely used broad-spectrum antibiotics and are generally safe and well tolerated. The authors present a case of neutropenia induced by ceftriaxone and meropenem in an adult patient. The resolution of neutropenia occurred within 48 hours of ceftriaxone and meropenem being discontinued. Although antibiotic-induced neutropenia is uncommon, clinicians should be mindful of this adverse drug effect because of its potential development of severe neutropenia, septicaemia, septic shock, deep-seated infections and even death. Therefore, neutropenic sepsis treatment should be initiated without delay, particularly if the patient becomes septic and febrile. Granulocyte-colony stimulation factor (G-CSF) may be administered to facilitate the recovery process with daily monitoring of neutrophil count. Mortalities from antibiotic-induced neutropenia remain rare, with a range of 2.5-5%. Learning points: Beta-lactam antibiotics and cabapenem are widely prescribed antibiotics for the treatment of various infections, but they can uncommonly cause neutropenia as adverse effects.Severe neutropenia may lead to severe life-threatening sepsis, shock and even death.Drug-induced neutropenia typically improves with the cessation of offending agents, supportive treatment and granulocyte-colony stimulating factor (G-CSF) which may shorten the recovery time.

Abstract:Objective:To probe the regularity and clinical characters of adverse reactions caused by Cefepime injection. Methods: Adverse reactions induced by Cefepime injection in 56 patients who signed the monitoring register from January, 2006 to January, 2008 were analyzed. Result: With higher incidence among senior patients, adverse reactions of Cefepime injection most commonly manifested as neurological disorders followed by dermatological and digestive dysfunctions. Conclusion: To decrease the incidence of adverse reaction, Cefepime injection should be properly used. To senior patients Cefepime injection should be applied with special awareness.
Cefepime-induced Neurotoxicity:Literature Analysis of 46 Cases

Abstract:OBJECTIVE:To analyze the clinical characteristics of cefepime-induced neurotoxicity and to provide reference for rational use of cefepime.METHODS:Medical records of cefepime-induced neurotoxicity were collected from medical literatures from 1997 to 2009.Age and gender of 46 patients,primary diseases,drug usage,onset time of ADR,clinical symptom and prognosis were analyzed.RESULTS:Of 46 patients,25 cases were male and 21 female.Their average age was (59.56±18.84) years old (unknown age of 1 case).38 cases received cefepime alone and 8 cases cefepime combined with other drugs.All patients were administered by intravenous infusion.Of 46 patients,32 patients suffered from chronic renal failure,among which 10 patients received overdosage of medicine (unknown dosage of 1 case).Clinical symptoms of cefepime-induced neurotoxicity were encephalopathy,myoclonus and epilepsy.The occurrence time of neurotoxicity was (4.39±2.18)d (unknown occurrence time of 2 cases).The symptoms of 36 cases were relieved and cured after drug withdrawal and treatment with mean cure duration of (2.67±2.64)d.CONCLUSION:Great importance should be attached to the antibacterial effects of cefepime and cefepime-induced neurotoxicity.
[Effects of cephem antibiotics on ethanol metabolism]

Abstract:Disulfiram-like reactions have been reported in patients receiving cephem antibiotics which possess a mercaptomethyltetrazole (Me-TZ) side chain in their molecular structure. The present study focused on the elucidation of the relationship between the formation of Me-TZ and its inhibitory action on alcohol metabolism after treatment with the cephem antibiotics. The cumulative urinary excretion of Me-TZ was determined in healthy volunteers and laboratory animals after i.v. administration of the cephem antibiotics: cefmetazole (CMZ), cefoperazone (CPZ) and latamoxef (LMOX). In humans, monkeys and rodents, the extent of urinary excretion of Me-TZ was found in the order of CPZ greater than LMOX greater than CMZ-treatment. To assess their influences on dehydrogenation of ethanol (EtOH) and acetaldehyde (AcH), rats and monkeys were treated with a single or multiple i.v. dose of Me-TZ and cephem antibiotics prior to the EtOH treatment. The blood EtOH levels were not affected with either the Me-TZ or the cephem pretreatments. On the contrary, the AcH levels were significantly elevated with each of the pretreatments. The dose response curves for AcH levels showed parallel lines corresponding to the urinary excretion of Me-TZ, i.e., CPZ greater than LMOX greater than CMZ-treatment. These results suggest that there are some differences in the disulfiram-like reactions among animal species and antibiotics, which will be attributed to the intrinsic distribution and stability of each of the antibiotics in the body.

Abstract:Objective Investigate the drug-induced hyperthermia due to cefotiam and other antibacterials in order to promote safe and rational medical treatment.Methods Medical records of 258 in-patients,including 125 cases treated with cefotiam were analyzed,drug-induced hyperthermia due to cefotiam and other medication ws summarized.Results Cefotiam is widely used in China.The patiensts of drug-induced hyperthermia due to cefotiam were 6 cases possible,8 cases affirmed,totally 14 cased,and the rate among patients administrated cefotiam was 11.2%.The total rate of drug-induced hyperthermia among all the patients was 7.8%.Conclusion The package insert should suggest the possible hyperthermia induced by cefotiam in order to keep the safe and rational medical treatment.Considering the poor PK and PD,and its adverse drug reactions of cefotiam,cefotiam must be used as less as possible.

Abstract:Objective: Disulfiram, an FDA-approved medication for AUD, has shown significant potential as a repurposed drug in therapeutic areas including oncology and infectious diseases. The purpose of study is to analyze adverse events (AEs) associated with disulfiram by examining the FAERS database, with a focus on understanding its safety profile in both traditional and emerging applications. Methods: AE reports concerning disulfiram in the FAERS database from the fourth quarter of 2002 to the third quarter of 2023 were extracted. Various signal detection methods, including ROR, PRR, BCPNN, and MGPS, were used to detect and categorize adverse events. Results: The study collected 52,159,321 AE reports, with 508 reports primarily suspecting disulfiram, identifying 104 Preferred Terms (PTs) across 25 System Organ Classes (SOCs). Major categories of AEs included off label use, psychiatric symptom, liver transplant, and polyneuropathy, with off label use being notably the most reported issue. Strong and new potential AEs were identified, including neurological and psychiatric issues like hypomania, delirium, and vocal cord paralysis; cardiac issues such as electrocardiogram st segment depression; and off label use-related issues like Jarisch-Herxheimer reaction. Conclusion: Disulfiram poses risks of various adverse reactions while having promise as a "repurposed" agent. In clinical applications, practitioners should closely monitor occurrences of hepatobiliary disorders, psychiatric disorders, and nervous system disorders.

Abstract:BackgroundThe incidence of cefepime-induced neurotoxicity (CIN) has been previously underestimated, and there have only been sporadic reports from critical neurological settings. The present study aimed to investigate the potential factors associated with disease development, electroencephalography (EEG) sub-classification, and outcome measures.MethodsThe 10-year medical records of patients who underwent EEG between 2007 and 2016 at a tertiary medical center in Taiwan, and developed encephalopathy after cefepime therapy were retrospectively reviewed. Age- and sex-matched controls were included for further analysis. Demographic data, the occurrence of clinical seizures, non-convulsive status epilepticus (NCSE), use of antiepileptic drugs (AEDs), receiving maintenance or urgent hemodialysis, EEG findings, and functional outcomes were analyzed. The Chi-square test and a logistic regression model were applied to survey significant prognostic factors relating to mortality.ResultsA total of 42 CIN patients were identified, including 25 patients from wards and 17 from intensive care units; their mean age was 75.8 ± 11.8 years. Twenty-one patients (50%) had chronic kidney disease, and 18 (43%) had acute kidney injury. Among these patients, 32 (76%) received appropriate cefepime dose adjustment. Three patients had a normal renal function at the time of CIN onset. The logistic regression model suggested that maintenance hemodialysis and longer duration of cefepime use were independently associated with the development of CIN, with odds ratios of 3.8 and 1.2, respectively. NCSE was frequently noted in the CIN patients (64%). Generalized periodic discharge with or without triphasic morphology was the most common EEG pattern (38%), followed by generalized rhythmic delta activity and generalized spike-and-waves. AEDs were administered to 86% of the patients. A total of 17 patients (40%) did not survive to hospital discharge. Adequate cefepime dose adjustment and early cefepime discontinuation led to a better prognosis.ConclusionsCIN was associated with high mortality and morbidity rates. Neurotoxic symptoms could still occur when the cefepime dose was adjusted, or in patients with normal renal function. Patients with maintenance hemodialysis or a longer duration of cefepime therapy tended to develop CIN. Early recognition of abnormal EEG findings allowed for the withdrawal of the offending agent, resulting in clinical improvements and a better prognosis at discharge.
