Abstract:Background: Chylous reflux (CR) in primary lymphedema (PL) of the lower extremities is rare and requires particular diagnostic procedures. This study aimed to determine clinical and lymphoscintigraphic traits for efficient screening. Methods and Results: Pediatric patients with PL of lower extremities treated in our institution between January 2020 and December 2023 were reviewed. The medical history, lymphoscintigraphy, non-contrast MR lymphography, and direct lymphangiography were analyzed. Lymphoscintigraphic visual patterns were classified to detect CR. Patients with CR showed significantly higher proportions of childhood symptom onset (65.63% vs. 25.19%, p < 0.01), genital lymphedema (62.50% vs. 19.26%, p < 0.01), and proximal-to-distal swelling (31.25% vs. 8.89%, p < 0.01). Eight patients without skin lesions had milk discharge after acupuncture or skin breakdown. Pattern 4 shows diffused dermal backflow of the leg and an absence of superficial lymphatic drainage, or normal initial cephalad flow with subsequent reflux of tracer into the affected limb. CR showed significantly higher percentages of pattern 4 (87.50% vs. 12.59%, p < 0.01), normal/widened iliac lymphatics (78.13% vs. 11.11%, p < 0.01), and thigh/calf outlining (87.50% vs. 29.63%, p < 0.01). Genital swelling preceding limb swelling and genital radioactivity were more common in the CR group. Conclusions: CR tends to occur in childhood initially, begin centrally and progress distally, and involve genitals prior to lower extremities. Milk discharge after acupuncture or skin breakdown is important for screening CR without chylous vesicles. The patterns with typical "profiling of the leg" and normal or widened iliac lymphatic trunks strongly correlate with CR. Genital lymphedema with increased radioactivity indicates CR.
Unipedal Diagnostic Lymphangiography Followed by Sequential CT Examinations in Patients with Idiopathic Chyluria: A Retrospective Study

Abstract:OBJECTIVE. The objective of our study was to investigate the clinical value of diagnostic lymphangiography followed by sequential CT examinations in patients with idiopathic chyluria. MATERIALS AND METHODS. Thirty-six patients with idiopathic chyluria underwent unipedal diagnostic lymphangiography and then underwent sequential CT examinations. The examinations were reviewed separately by two radiologists. Abnormal distribution of contrast medium, lymphourinary leakages, and retrograde flow were noted, and the range and distribution of lymphatic vessel lesions were recorded. The stage of idiopathic chyluria based on CT findings and the stage based on clinical findings were compared. Therapeutic management and follow-up were recorded. Statistical analyses were performed. RESULTS. Compared with CT studies performed after lymphangiography, diagnostic lymphangiography showed a unique capability to depict lymphourinary leakages in three patients. Lymphourinary fistulas and abnormal dilated lymphatic vessels were found in and around kidney in all patients. CT depicted retrograde flow of lymph fluid in 47.2% of patients. The consistency in staging chyluria based on CT findings and clinical findings was fair (kappa = 0.455). Twenty-nine patients underwent conservative therapy, and seven underwent surgery. Surgical therapy was superior to conservative management (no recurrence, 85.7% of patients who underwent surgery vs 62.1% of patients who underwent conservative therapy; p = 0.025). CONCLUSION. From assessing the drainage of contrast medium on unipedal diagnostic lymphangiography and the redistribution of contrast medium on sequential CT examinations, it is possible to detect the existence of lymphourinary fistulas, the precise location of lymphatic anomalies, the distribution of collateral lymphatic vessels, and hydrodynamic pressure abnormality in the lymph circulation in patients with idiopathic chyluria. CT staging of chyluria provides additional information that can be used to guide therapeutic management.
Post direct lymphangiography MSCT in diagnosis of chylothorax

Abstract:Objective: To explore the application value of MSCT after direct lymphangiography (DLG) in diagnosis of chylothorax. Methods: DLG and postprocedure MSCT imaging of 30 patients with chylothorax were retrospectively analyzed and compared. Results: DLG and postprocedure MSCT imaging showed dilated iliac and retroperitoneal lymphatic vessel circuitous in all 30 cases (100%). Reflux to contralateral iliac and retroperitoneal lymphatic vessel was demonstrated on postlymphatic MSCT in 13 cases (43.33%), while in 9 cases (30.00%) on DLG (P=0.13). Abdominal reflux was detected in 4 cases (13.33%) by postlymphatic MSCT and 3 (10.00%) by DLG (P=1.00). Postprocedure MSCT showed 20 patients (66.67%) with thoracic duct occlusion, while DLG showed 22 (73.33%, P=0.50). Nine patients (30.00%) with partial thoracic ducts were demonstrated on postprocedure MSCT, while 8 (26.67%) were demonstrated on DLG (P=1.00). Ten (33.33%) and 4 (13.33%) cases of contrast medium entering blood were demonstrated on postlymphatic MSCT and DLG, respectively (P=0.07). Postlymphatic MSCT showed 8 (26.67%) hydrothorax chyle leak, whereas DLG detected 1 (3.33%, P=0.02). One (3.33%) lymph vessel dilatation was detected simultaneously by MSCT and DLG (P=1.00). Conclusion: DLG and postlymphatic MSCT are complementary, therefore providing essential information of diagnosis and therapy for chylothorax.
The Clinical and Imaging Findings of Chyloptysis

Abstract:Objective To explore the clinical and imaging findings of chyloptysis.Methods The complete clinical,direct lymphangiography and post-procedural CT data of 6 cases of chyloptysis were collected.1 case had an additional to bronchoscopy.The CT imaging and abnormal lymphatic reflux was analyzed.Results The 6 patients with chyloptysis had milk white sputum from 5 months to 27 years,accompanied by various degrees of chylothorax and/or chyloperitonium.Chyloptysis showed acinar and/or patchy ground glass exudation,accompanied by interval thickening of the lung in 2 cases.In the 6 cases,5 had thoracic duct end obstruction,accompanied by abnormal distribution of contrast agent in right lymphatic duct in 3 cases,in mediastinum in 3 cases,in lung in 2 cases.All of 6 cases had tortuous and dilated iliac and lumbar lymphatic vessels.One case showed lymphatic vessels dilation of bronchial submucosa under bronchoscopy.Conclusion The chyloptysis has characteristic clinical manifestations.The direct lymphangiography and post-procedural CT can provide a certain amount of information for abnormal lymphatic circumfluence of chyloptysis.
Chronic Lower Extremity Lymphedema: A Comparative Study of High-Resolution Interstitial Mr Lymphangiography and Heavily T2-Weighted Mri

Abstract:Purpose: To assess the role of heavily T2-weighted image and interstitial MR lymphangiography (MRL) for the visualization of lymphatic vessels in patients with disorders of the lymphatic circulation.Methods: Forty lower extremities in 31 patients (9 bilateral and 22 unilateral) with primary lymphedema were examined by heavily T2-weighted image and indirect MRL Maximum-intensity projection (MIP) was used to reconstruct the images of the lymphatic system. Two experienced radiologists analyzed the images with regard to the differences in image quality, number of lymphatic vessels, its maximum diameter and two other findings: accumulated lymph fluid in the tissue and honeycombing pattern.Results: The beaded appearance of the affected vessels in 73 leg segments of 40 lower extremities were present on both modalities 3D MIP. Larger amount of the dilated lymphatic vessels were visualized on heavily T2-weighted image than that on MRL (p = 0.003) and the maximum diameter of it was 4.28 +/- 1.53 mm on heavily T2-weighted image, whereas 3.41 +/- 1.05 mm on MRL (p < 0.01). The dilated lymphatic vessels on MRL showed better image quality and greater SNR and CNR than that on heavily T2-weighted image (p < 0.01). The regions of accumulated lymph fluid and the honeycombing pattern extent were identified on heavily T2-weighted image scored statistically higher than that on MRL (p < 0.01).Conclusion: The heavily T2-weighted imaging has greater sensitivity and the MRL image has higher legibility for detecting the pathologically modified lymphatic vessels and accompanying complications non-invasively. Abstract:Chylous ascites is a rare form of ascites usually associated with cirrhosis, abdominal malignancies, surgeries or infections. We presented a case of chylous ascites after robotic laparoscopic prostatectomy (PLDN-RALP), in which the correct diagnosis was achieved by SPECT/CT lymphoscintigraphy. A 72-year-old male developed chylous ascites after surgery and underwent lymphoscintigraphy with radiolabeled albumin nanocolloids for the supplementary study of the lymph flow and to detect a possible site of leakage. The scintigraphic imaging demonstrated the abdominal effusion and lymph stasis in the left iliac region. The combination of planar imaging with SPECT/CT can resolve the assessment of chylous disorders.
Imaging lymphatics in human normal and lymphedema limbs-Usefulness of various modalities for evaluation of lymph and edema fluid flow pathways and dynamics

Abstract:The human lymphatic system morphology and function still remain largely unknown to clinicians and biologists. How does the lymphatic vascular system look like in comparison to the blood transport system, how does lymph flow, where does capillary filtrate accumulate in cases with lymphatic obstruction caused by inflammation, trauma, and cancer therapy, remain as basic questions. Visualization of the lymphatic pathways and dynamics of lymph flow, and in cases of obstruction, the localization of the capillary filtrate/edema fluid accumulation becomes indispensable. The contemporary methods only partly meet these requirements. Since the early 1950s of the 20th century only few specific clinical methods of imaging of limb lymphatics are being used in human clinic. Each of the applied modalities provides different images due to different physical chemistry and distribution of tracer, methods used for its detection in tissues, their sensitivity and specificity and clinical type of lymph vessel pathology. Here, the advantages and disadvantages of the most commonly used 3 methods of imaging: the iodinated oil X-ray, isotopic, and fluorescent lympangiographies are presented. The study is based on retrospective and recent collections of lymphangiograms from large cohorts of patients. Imaging of lymph nodes has not been included as it is requiring different interpretation compared with vessels. Composite evaluation of X-ray, isotopic, and fluorescent lymphographic images or, as it is practiced now the isotope and indocyanine green near infrared lymphographies, provide most clinically important information. Special attention was directed at methods enabling early diagnosis of imminent lymphedema especially in cases with cancer therapy-related lymphedema. Groups of typical images obtained with the 3 methods are presented.

Abstract:BACKGROUND This study presents an evaluation of the computed tomography lymphangiography (CTL) features of lymphatic plastic bronchitis (PB) and primary chylothorax to improve the diagnostic accuracy for these two diseases. AIM To improve the diagnosis of lymphatic PB or primary chylothorax, a retrospective analysis of the clinical features and CTL characteristics of 71 patients diagnosed with lymphatic PB or primary chylothorax was performed. METHODS The clinical and CTL data of 71 patients (20 with lymphatic PB, 41 with primary chylothorax, and 10 with lymphatic PB with primary chylothorax) were collected retrospectively. CTL was performed in all patients. The clinical manifestations, CTL findings, and conventional chest CT findings of the three groups of patients were compared. The chi-square test or Fisher's exact test was used to compare the differences among the three groups. A difference was considered to be statistically significant when P < 0.05. RESULTS (1) The percentages of abnormal contrast medium deposits on CTL in the three groups were as follows: Thoracic duct outlet in 14 (70.0%), 33 (80.5%) and 8 (80.0%) patients; peritracheal region in 18 (90.0%), 15 (36.6%) and 8 (80.0%) patients; pleura in 6 (30.0%), 33 (80.5%) and 9 (90.0%) patients; pericardium in 6 (30.0%), 6 (14.6%) and 4 (40.0%) patients; and hilum in 16 (80.0%), 11 (26.8%) and 7 (70.0%) patients; and (2) the abnormalities on conventional chest CT in the three groups were as follows: Ground-glass opacity in 19 (95.0%), 18 (43.9%) and 8 (80.0%) patients; atelectasis in 4 (20.0%), 26 (63.4%) and 7 (70.0%) patients; interlobular septal thickening in 12 (60.0%), 11 (26.8%) and 3 (30.0%) patients; bronchovascular bundle thickening in 14 (70.0%), 6 (14.6%) and 4 (40.0%) patients; localized mediastinal changes in 14 (70.0%), 14 (34.1%), and 7 (70.0%) patients; diffuse mediastinal changes in 6 (30.0%), 5 (12.2%), and 3 (30.0%) patients; cystic lesions in the axilla in 2 (10.0%), 6 (14.6%), and 2 (20.0%) patients; and cystic lesions in the chest wall in 0 (0%), 2 (4.9%), and 2 (4.9%) patients. CONCLUSION CTL is well suited to clarify the characteristics of lymphatic PB and primary chylothorax. This method is an excellent tool for diagnosing these two diseases.
Lymphography using microbubbles and contrast-enhanced ultrasound (CEUS) before lymphatic supermicrosurgery for severe lymphorrhea of lower extremity

Abstract:Lymphaticovenous anastomosis (LVA) is an effective surgical treatment for lymphorrhea. However, the traditional indocyanine green (ICG) fluorescent lymphography mapping for lymphatic vessels has limitations, it can only depict the initial capillary lymphatic network localized in the dermis of the skin, which cannot visualize lymphatics deeper than 1.5 cm. A new mapping technique, microbubbles and contrast-enhanced ultrasound (CEUS) can address the problem. In one case of lymphocutaneous fistula, for the first time, we used microbubbles and CEUS technique for preoperative localization for LVAs. Microbubbles and CEUS can identify deep lymphatic vessels and better evaluate the function of lymphatic vessels. The patient's symptoms of edema and lymphorrhea improved clinically. Microbubbles and CEUS is an effective methods for identifying lymphatic vessels in lower limbs.

Abstract:Objective: To explore the value of CT lymphangiography in patients with chyluria after direct lymphangiography (DLG). Methods: Data of 11 patients with chyluria who underwent lymphoscintigraphy, DLG and CT lymphangiography were analyzed retrospectively. All the patients were confirmed by pathology after surgeries. Results: Among 11 patients, lymphoscintigraphy detected ectasia of thoracic duct in 8 cases, lumbar trunk dilation in 5 and renal pelvis backflow in 7 cases. DLG detected ectasia of thoracic duct in 10 cases, lumbar trunk dilation in 11, contralateral lumbar trunk and retroperitoneal lymph vessels backflow in 4 and renal pelvis backflow in 10 cases. CT lymphangiography detected ectasia of thoracic duct in 9 cases, dilation of right lymph vessels in 1 case, lumbar trunk, retroperitoneal, iliac and pelvic lymph vessels dilation in 11 cases, backflow to contralateral lumbar trunk and retroperitoneal lymphatic vessel in 10 and renal pelvis and sinus renalis backflow in all cases. Conclusion: CT lymphangiography can detect more abnormal lymph vessels than lymphoscintigraphy and DLG, demonstrate surrounding anatomical configuration, therefore complementing low spatial resolution of the latters.
Measuring Diagnostic Accuracy of Imaging Parameters in Pelvic Lipomatosis

Abstract:Objectives: To study whether the individual radiological findings can help predict diagnosis of pelvic lipomatosis (PL) or, specifically appreciate its progression. Methods: Data from 32 clinically proven cases of PL and 25 controls were collected. Two reviewers were recruited for a blinded evaluation, image features were recorded in terms of: (1) bladder shape; (2) bladder-rectosigmoid morphological indexes including ratio of superior-inferior to anterior-posterior length of bladder (SI/AP), angle between anterior and posterior wall (AAP), relative length of posterior urethra (rLPU), angle between bladder and seminal vesicle (ABS) and rectosigmoid morphological index (RMI); (3) secondary complications. Results were evaluated by an unpaired t test and ROC analysis. Results: The sensitivity and specificity were 40.6% and 100% for pear and banana-shaped bladder, 62.5% and 100% for SI/AP, 40.6% and 100% for AAP, 62.5% and 100% for ABS, 78.1% and 72% for rLPU, 59.4% and 96% for RMI, respectively. These radiological findings partially correlated with the severity of disease weighted by hydronephrosis and treatment grade. Image analysis demonstrated high prevalence of glandular cystitis (100%) and hydronephrosis (73.4%). Conclusion: We conclude that PL is a progressive disease involving multiple pelvic organs with high prevalence of intractable cystitis and hydronephrosis. Abstract:To investigate MRI features of chronic lymphedema of extremity 13 patients with lower extremity lymphedema were studied. MRI showed deformity of lymphatics at different levels. In subcutis MRI characteristically displayed diffuse edema or a honeycomb pattern consistent with reticular lymphangiectasis and lymph "lakes" with a marked increase in signal intensity at T2-weighted imaging. In lymphedema hyperplasia and chylous reflux MR imaging clearly displayed dilated lymphatic collectors and lumbar branches. The findings of MRI are valuable not only for the differential diagnosis but also for choosing treatment method. Lymphedema is an common lymphatic disorder of extremity. Clinical differential diagnosis is not always easy. An easy and repeatable examination method of accurate display of peripheral lymphatic system is essential not only for the differentiation but also for choosing proper treatment. Conventional lymphography using oil contrast has been the gold standard for depicting lymphatics and nodes (1). However it is an invasive, time-consuming procedure and may exacerbate lymphedema. The commonly used lymphangioscintigraphy can not readily depict lymphatics and lymph nodes in anatomic areas especially when the lymphatic pathway is blocked. MRI is a safe and noninvasive imaging modality, provides more detailed information of soft tissue changes. However, its value for visualizing and depicting the lymphatic system is not extensively studied. The aim of this study was to evaluate MR imaging characteristics of lymphatic disorders in limb - chronic lymphedema, both primary and secondary.
The Role of Radionuclide Lymphoscintigraphy in Extremity Lymphedema

Abstract:The characteristics of lymphedema on radionuclide lymphoscintigraphy were studied, and the diagnostic value of radionuclide lymphoscintigraphy in lymphedema was evaluated. In this report radionuclide lymphoscintigraphy was performed in 110 cases of clinically suspected lymphedema. A retrospective study method was used to analyze the imaging results. The typical pattern of lymphedema on radionuclide lymphoscintigraphy was summarized. It was found that the characteristics of lymphedema on radionuclide lymphoscintigraphy were diverse. The most common pattern was increased radiotracer accumulation in the soft tissue and lymphatic webs. Surgery and infection dominated as the causes of lymphedema in this study. It was concluded that radionuclide lymphoscintigraphy is a useful noninvasive method for diagnosing lymphedema. It is easy to operate and provides reliable results.

Abstract:BACKGROUND:The value of MRI presentation of primary lower extremity lymphedema in assessing the severity of lower extremity lymphedema is uncertain. The purpose of this study was to assess the role of MRI presentation in staging primary lower extremity lymphedema. METHODS:228 patients with clinically diagnosed primary lower limb lymphoedema from January 2018 to December 2019 in our hospital were enrolled retrospectively. Patients were divided into stages I, II, and III based on the 2020 International Society of Lymphology (ISL) clinical staging standards. Two radiologists assessed the following characteristics of the Short Term Inversion Recovery (STIR) sequence: the extent of edema (longitudinally and transversely), the frequency of MRI manifestations, including the presence of dermal thickening, and the morphology of edema (grid, honeycomb, parallel-lines, banded, crescent, and lymphatic lake). The Kappa test was used to assess interobserver agreement. The chi-square test was used to compare the frequency differences of MRI manifestations between different clinical stages. The Spearman test evaluated the correlation between edema extent and clinical stage. RESULTS:The extent of edema was positively correlated with clinical stage, both longitudinally and transversely. When comparing stages, the incidence of dermal thickening in stages II and III was significantly higher than in stage I. The incidence of parallel-lines in stage I was significantly higher than that in stages II and III. The grid and banded sign incidence in stages I and II were significantly higher than in stage III. The incidence of honeycomb in stages II and III was significantly higher than in stage I. The incidence of lymphatic lake and crescent in stage III was significantly higher than in stages I and II (P<0.001). CONCLUSION:STIR can sensitively diagnose lymphedema and assist in clinical staging. MRI manifestations of primary lower extremity lymphedema in different stages have specific MRI features.
Lymphoscintigraphy - Beyond Lymphedema

Abstract:Lymphoscintigraphy is an established modality for imaging the lymphatic system using radiocolloids and is routinely indicated to find the cause of limb lymphedema. However, in this case series, we are highlighting other less-known indications of lymphoscintigraphy like chylothorax and chyluria which present as lymphatic leaks in the thorax and abdomen, respectively. Once the site of the lymphatic leak is established by lymphoscintigraphy, definitive management like thoracic duct ligation or sclerotherapy can be done. The other indication discussed is postrenal transplant perinephric fluid collection which can be challenging to confirm whether it is urinoma, lymphocele, or any other collection. And finally, sentinel lymph node localization is another, now, well-established indication of lymphoscintigraphy.

Abstract:Objective To discuss the value of 3.0T dynamic enhanced MR lymphangiography in evaluating the configuration and function of lymphatic vessels in low extremity lymphedema.Materials and Methods 25 patients with low extremity lymphedema(34 affected low extremity) received dynamic enhanced MRL,the morphologic changes of the lymphatic vessel were observed and recorded,and the relative signal intensity(RSI) curve with the time point of 5 min,10 min,15 min,20 min,25 min and 30 min after contrast injection,time signal intensity curve(TIC),T peak and wash in rate were then measured.Results The configuration changes,the obstructed position and the pathophysiological processes of lymphatic vessel could be demonstrated by DEC MRI.lymphatic vessels could be displayed at delayed stage with the continuing time over 30 mins.There were significant differences of T peak,wash in rate and RSI in different stage.Conclusion MR lymphangiography is a safe,feasible and new imaging method to provide more useful information of the configu ration and function of lymphatic vessels in patients with lymphedema.
Diagnosis of Lymphedema and Subjective Symptoms in the Lower Abdomen and Genital Region

Abstract:Background: Lymphedema in the lower abdomen and genitals is unnoticeable and has no established diagnostic methods. In this study, we evaluated it using four examinations. Methods: We evaluated 25 patients with lymphedema in the legs, dividing the abdomen and genitals in four areas (right and left, upper and lower). The mean age was 58.6 years. In lymphoscintigraphy and indocyanine green (ICG) lymphography, we diagnosed lymphedema when dermal backflow was observed. ICG lymphography was performed in 13 patients. In ultrasonography, we determined the presence of edema when cobblestone pattern was observed. Subcutaneous fat thickness was also measured. The patients' subjective symptoms were identified on an interview. We compared the results among the examinations. Results: The positivity rates for lymphedema based on lymphoscintigraphy, ICG lymphography, ultrasonography, and subjective symptoms were 45.0%, 42.3%, 8.0%, and 34.0%, respectively. Two of the 13 patients who underwent all examinations complained of subjective symptoms of edema in areas that showed no abnormalities in the examinations. In contrast, 14 of the 25 patients had areas where they had no subjective symptoms despite having abnormalities in at least one of the tests. Those with subjective symptoms of edema tended to have thinner abdominal fat in both the upper and lower abdomen, but no significant difference was observed. Conclusion: Large differences were observed in the positive rate of edema in subjective symptoms and examinations of lymphedema in the lower abdomen and genitals. It is not important to determine which examination is best but rather to combine multiple examinations.
