Abstract:With the development of medical health, regional health information sharing has become the inevitable trend of the development of medical information. However, due to the regional medical information construction, it hinders the exchange and sharing of clinical data for some disadvantages like low-level degree of system integration, low real-time of information exchange, poor reliability, etc. According to the existing problems of the regional health, this paper constructs a regional health information collaborative platform, utilizes information integration to form patient as core of clinical data, uses clinical document architecture (CDA) as data exchange standard, and implements the data exchange and sharing between medical institutions and the regional medical center.

Abstract:To meet the demand of big data sharing and communication in regional medical systems in a secure manner, we designed and developed a security system within HIE-oriented EMR, including identity authentication, privilege management, access log, and data encryption modules. The establish and deployment of the system could prevent access from the illegal users, keep EMR system accessibility while controllability, record the modification traces and make it undeniability, protect the medical data from intercepted by intruders, keep the data confidentiality from none stakeholders, and finally construct a complete security system in EMR.

Abstract:The health information systems of most medical institutions in China are isolated. Communications across these systems are generally realized through point-to-point interfaces at the database level, which tend to lack interoperability, extensibility, and security. In the resent study, we developed localized, document-oriented and data-focused clinical document architecture (CDA) templates based on health level 7 (HL7) CDA. Then, by combining these templates with the Service-oriented architecture for HL7 middleware, we accomplished interoperability across multiple heterogeneous systems. Modules of our system have been put into trial use in six medical institutions, including the Second University Hospital (main campus and regional campuses) of the School of Medicine, Zhejiang University and other collaborating hospitals.

Abstract:Legacy Electronic Health Records (EHRs) systems were not developed with the level of connectivity expected from them nowadays. Therefore, interoperability weakness inherent in the legacy systems can result in poor patient care and waste of financial resources. Large hospitals are less likely to share their data with external hospitals due to economic and political reasons. Motivated by these facts, we aim to provide a set of software implementation guidelines, i.e., MedShare to deal with interoperability issues among disconnected healthcare systems. The proposed integrated architecture includes: 1) a data extractor to fetch legacy medical data from a hemodialysis center, 2) converting it to a common data model, 3) indexing patient information using the HashMap technique, and 4) a set of services and tools that can be installed as a coherent environment on top of stand-alone EHRs systems. Our work enabled three cooperating but autonomous hospitals to mutually exchange medical data and helped them develop a common reference architecture. It lets stakeholders retain control over their patient data, winning the trust and confidence much needed towards a successful deployment of MedShare. Security concerns were effectively addressed that also included patient consent in the data exchange process. Thereby, the implemented toolset offered a collaborative environment to share EHRs by the healthcare providers.

Abstract:The existing independent hospital information system hinders the cooperation and integration of healthcare service processes. In order to solve this problem, hospital information architecture based on cloud computing was put forward through analyzing the characteristics of medical information. The platform, system architecture and key technologies of the hospital information architecture were discussed. Software prototype was developed by using Hadoop, Tashi and MapReduce and implementation of this model proved to be feasible and effective.

Abstract:As network technology advances and more people use devices, data storage has become a significant challenge due to the explosive growth of information and the threat of data leaks. In traditional medical institutions, most medical data is stored centrally through cloud computing technology in the institution’s data center. This centralized storage method has many security risks, and once the central server is attacked, it will lead to the loss of medical data, which will lead to the leakage of patients’ private data. At the same time, electronic medical records are the most critical data in the current medical field. In the traditional centralized healthcare service system (HSS), there are data leakage problems and tampering with electronic medical records due to human factors. At the same time, each hospital is built independently, resulting in the current centralized healthcare service system having a data silo problem, making it difficult to share medical data between institutions securely. With the increase in the number of users in the system, the electronic medical record data in the system also increases gradually, resulting in the increasing overhead of decryption calculation. Therefore, this paper proposes a blockchain-based access control scheme with multiparty authorization to ensure the security of electronic medical records. The scheme uses an SM encryption algorithm to encrypt the medical data in the system. It adds the patient’s signature to ensure the confidentiality and security of the data, and the encrypted electronic medical records (EMRs) are stored in the InterPlanetary File System (IPFS) to realize the distributed storage of EMR. In addition, role-based multiauthorization access control is implemented through smart contract-based to ensure the security of EMR. We have analyzed the security of this paper’s solution and compared its performance with the existing schemes based on other cryptographic algorithms. The experimental results show that the proposed solution significantly improves the secure sharing of EMR and provides system performance.

Abstract:In recent years, Electronic health records (EHR) has gradually become the mainstream in the healthcare field. However, due to the fact that EHR systems are provided by different vendors, data is dispersed and stored, which leads to the phenomenon of data silos, making medical information too fragmented and bringing some challenges to current medical services. Therefore, in view of the difficulties in sharing EHR between medical institutions, the risk of privacy leakage, and the lack of EHR usage control by patients, an EHR sharing model based on consortium blockchain is proposed in this paper. Firstly, the Interplanetary File System is combined with consortium blockchain, which forms a hybrid storage scheme of EHR, this technology effectively improves data security, privacy protection, and operational efficiency. Secondly, the model combines unidirectional multi-hop conditional proxy re-encryption based on type and identity with distributed key generation technology to achieve secure EHR sharing with fine grained control. At the same time, users are required to link the operation records of EHR, so as to realize the traceability of EHR usage. A dynamic Byzantine fault-tolerant algorithm based on reputation and clustering is then proposed to solve the problems of arbitrary master node selection, high latency and low throughput of PBFT, enabling the nodes to reach consensus more efficiently. Finally, the model is analyzed in terms of security and user control, showing that the model is less energy intensive in terms of communication overhead and time consumption, and can effectively achieve secure sharing between medical data.

Abstract:The standardization degree of traditional Chinese medicine clinical data in the real world is low and heterogeneous data aggregation among institutions is difficult, which leads to the difficulty of sharing clinical data and scientific research data of traditional Chinese medicine. This paper designs and implements a real world traditional Chinese medicine clinical scientific research information electronic medical record sharing system. The system consists mainly of two subsystems, namely electronic medical record collection system and electronic medical record integration system. The collection system can collect, normalize and structured storage inpatient electronic medical records, outpatient electronic medical records and cloud platform electronic medical records. The integration system can integrate heterogeneous data from different traditional Chinese medicine diagnostic and treatment institutions to realize the sharing of traditional Chinese medicine clinical and research data.

Abstract:The EHR is a secure, real-time, point-of-care, patient-centric information resource for healthcare providers. Many countries and regional districts have set long-term goals to build EHRs, and most of EHRs are usually built based on the integration of different information systems with different information models and platforms. A number of hospitals in Shanghai are also piloting the development of an EHR solution based on IHE XDS/XDS-1 profiles with a service-oriented architecture (SOA). The first phase of the project targets the Diagnostic Imaging domain and allows seamless sharing of images and reports across the multiple hospitals. To develop EHRs for regional coordinated healthcare, some factors should be considered in designing architecture, one of which is security issue. In this paper, we present some approaches and policies to improve and strengthen the security among the different hospitals' nodes, which are compliant with the security requirements defined by IHE IT Infrastructure (ITI) Technical Framework. Our security solution includes four components: Time Sync System (TSS), Digital Signature Manage System (DSMS), Data Exchange Control Component (DECC) and Single Sign-On (SSO) System. We give a design method and implementation strategy of these security components, and then evaluate the performance and overheads of the security services or features by integrating the security components into an image-based EHR system.

Abstract:Electronic Health Record (EHR) and its privacy have attracted widespread attention with the development of the healthcare industry in recent years. As locking medical data in a single healthcare center causes information isolation, healthcare centers are motivated to build medical data sharing systems. However, existing systems highly rely on the trusted centralized servers, which are vulnerable to distributed denial of service (DDoS) attacks and the single point of failure. Moreover, it is a non-trivial matter to authorize multiple users to search and access EHR in a privacy-preserving manner. In this article, we propose MedShare, a decentralized framework for secure EHR sharing. Our design utilizes the smart contract technique of blockchain to establish a trusted platform for healthcare centers to share their encrypted EHR. Considering that fine-grained access control is essential in practical EHR sharing service, we devise a constant-size attribute-based encryption (ABE) scheme, where the access policy is embedded in search result on the blockchain. Besides, we propose an efficient scheme that enables authorized MedShare users to perform multi-keyword boolean search operations over encrypted EHR. We formally analyze the security strengths and implement the system prototype on Ethereum. Evaluation results demonstrate that MedShare is efficient for EHR sharing.

Abstract:In the medical system, the verification, preservation and synchronization of electronic medical records has always been a difficult problem, and the random dissemination of patient records will bring various risks to patient privacy. Therefore, how to achieve secure data sharing on the basis of ensuring users' personal privacy becomes the key. In recent years, blockchain has been proposed to be a promising solution to achieve data sharing with security and privacy preservation due to its advantages of immutability. So, a distributed electronic medical records searchable scheme was proposed by leveraging blockchain and smart contract technology. Firstly, we perform a hash calculation on the electronic medical data and store the corresponding value on the blockchain to ensure its integrity and authenticity. Then, we encrypt the electronic medical data and store it in the interplanetary file system which is a distributed storage protocol. These operations not only can solve centralized data store of servers of several medical institutions, but also be good at lowering stress from data store and high-frequency access to blockchain. Secondly, the encrypted keyword index information of electronic medical records was stored on the Ethereum blockchain, meanwhile a smart contract deployed in the Ethereum blockchain is used to realize keyword search instead of depending on a centralized third party. Furthermore, we use attribute-based encryption scheme to ensure that only the attributes meeting the access policy can decrypt the encrypted electronic medical records. Finally, our performance analysis and security analysis show that the scheme is secure and efficient.

Abstract:Shanghai is piloting to develop an EHR system to solve the problems of medical document sharing for collaborative healthcare, the solution of which is considering to following IHE XDS (cross-enterprise document sharing) and XCA (cross-community access) technical profiles as well as combined with grid storage for images. The first phase of the project targets text and image documents sharing cross four local domains or communities, each of which consists of multiple hospitals. The prototype system was designed and developed with service-oriented architecture (SOA) and Event-Driven Architecture (EDA), basing on IHE XDS.b and XCA profiles, and consists of four level components: one central city registry; the multiple domain registries, each of which is for one local domain or community; the multiple repositories corresponding to multiple local domain registries; and multiple document source agents, each of which is located in each hospital to provide the patient healthcare information. The system was developed and tested for performance evaluation including data publication, user query and image retrieval. The results are extremely positive and demonstrate that the designed EHR solution based on SOA with grid concept can scale effectively to serve medical document sharing cross-domain or community in a large city.
A Controllable Secure Blockchain-Based Electronic Healthcare Records Sharing Scheme

Abstract:The sharing of electronic healthcare records (EHRs) is important to healthcare and medical research. However, institutions are faced with difficulties in privacy protection and efficiently secure data exchange. The main objective of this study is to propose a controllable secure blockchain-based EHRs sharing scheme. For this purpose, blockchain technologies are combined with interplanetary file systems (IPFS) to provide efficient secure EHRs sharing. Firstly, the IPFS-based EHR file system (IEFS) is designed to save and share large-size EHR files among medical institutions. With the high-throughput content-addressed block storage model and appropriate redundant backup of IPFS, IEFS is tamper-resistant and free of a single point of failure. Secondly, the blockchain is used to implement the blockchain-based EHR abstract system (BEAS) to manipulate EHR abstracts access. In BEAS, the EHR file addresses generated by IEFS are encrypted and saved in EHR abstracts for privacy protection. Since EHR abstracts are encrypted by patients' public keys, the sharing of EHR files is under the control of patients. In our experiment, a prototype system is developed to validate the proposed scheme. The experimental results showed that (1) EHRs are securely shared under the control of patients and (2) EHR files are retrieved at an acceptable speed supported by IPFS technology. In this paper, solutions to some important practical issues such as incapacitated patients, encryption key forgetting/missing, and efficient interaction of doctors with EHRs sharing scheme are also seriously discussed.
