Effect of bivalirudin injection on fibrinogen and thrombin time of healthy Chinese subjects

Abstract:Objective: To investigate the effect of bivalirudin injection on fibrinogen (Fib) and thrombin time (TT) of healthy Chinese subjects. Methods: Forty-eight healthy volunteers were randomly assigned to receive 0.5, 0.75 or 1.05 mg ·kg-1 intravenous bolus, or 0.75 mg ·kg-1 intravenous bolus followed by an infusion of 1.75 mg ·kg-1 ·h-1 per hour for 4 hours. Nine subjects were randomized to receive bivalirudin, and three were randomized to receive placebos at each dose level. For the 0.5, 0.75, and 1.05 mg ·kg-1 intravenous bolus injection dose groups, blood samples were collected before dosing (time 0) and at 20 min, 45 min, 1 h, 2 h, 24 h, and 72 h after dosing. For the group receiving a 0.75 mg ·kg-1 intravenous bolus followed by an infusion of 1.75 mg ·kg-1 ·h-1 per hour for 4 hours, blood samples were collected before dosing (time 0) and at 2, 4, 6, 8, 24 and 72 h after bolus injection. Results: Bivalirudin prolonged TT in a dose dependent manner and the TT value returned to normal about 2 h after drug injection. Fibrinogen concentration decreased dramatically in bivalirudin infusion group, however, this may be due to the van Clauss assay, which would give falsely low results in the presence of direct thrombin inhibitors. Conclusion: In the future phase II clinical trial of bivalirudin, much more attention should be paid to anticoagulation monitoring. For Fib concentration determination in the presence of bivalirudin, van Clauss assay becomes inaccurate and should be replaced by other methods.
