Abstract:BACKGROUND:Retinal detachment (RD) secondary to macular hole (MH) is a common complication in highly myopic eyes, usually leading to a poor visual prognosis. The purpose of this study was to evaluate the surgical outcome of silicone oil (SO) tamponade and internal limiting membrane (ILM) peeling in the treatment of RD caused by MH (MHRD) in highly myopic eyes with chorioretinal atrophy, and to identify clinical factors associated with the anatomical outcomes.METHODS:We retrospectively reviewed 21 eyes of 21 highly myopic patients affected by RD secondary to MH and chorioretinal atrophy. All eyes were treated with pars plana vitrectomy (PPV) with ILM peeling and SO tamponade. Anatomical success was defined as reattachment of the retina with the closure of the MH, as assessed by optical coherence tomography (OCT), after SO removal. Logistic regression was performed to determine the clinical factors influencing anatomical success.RESULTS:The mean patient age was 59.95 years [standard deviation (SD), 10.39; range, 34-77 years] and the mean axial length was 30.58 mm (SD, 1.52; range, 27.99-34.51 mm). After the first surgical procedure, the anatomical success rate was 61.9% (13 eyes in 21 eyes), with initial retinal attachment of 16 eyes (76.2%). A second surgical approach was performed for the five eyes with persistent or recurrent RD, and the final retinal reattachment rate was 100% (21/21). Logistic regression analysis showed that no specific factors were significantly associated with anatomical success.CONCLUSIONS:Primary silicone oil tamponade and ILM peeling can be a practical treatment for repairing MHRD in highly myopic eyes with chorioretinal atrophy.
Clinical Outcomes of Treatment of Macular Hole by Vitrectomy with Internal-Limiting Membrane Peeling

Abstract:Objective To evaluate the treatment results of macular hole using pars plana vitrectomy and internal limiting membrane peeling in a series of consecutive patients.Methods Sixteen eyes of 16 patients with full-thickness macular hole underwent pars plana vitrectomy,internal limiting membrane (ILM) peeling using technique for staining the ILM with indocyanine green (ICG).Specimens were submitted for electron microscopy.Results ILM was stained ultramaring green while epiretinal membrane was not,this stain greatly facilitated ILM peeling by improving direct visualization of the membrane.Electron microscopic studies confirmed removal of the ILM and ERM.13 eyes of 16 macular holes were closed and 3 eyes were unclosed with 1 operation.Conclusion Vitrectomy with IIM peeling can successfully colse full-thickness macular holes with significant benefit in visual acuity and unserious complication.ICG staining technique leads to the better visualization and performation of successful removal of an ILM with less damage to the retina.

Abstract:Idiopathic macular hole (IMH) refers to full thickness defects of retinal neuroepithelial layer in macular area without clear reasons,and par plana vitrectomy with internal limiting membrane peeling (ILMP) has been routine operation to treat IMH.There are many possible factors influencing the prognosis of surgery,including age,sex,duration of disease,visual acuity before operation,stage and diameter of IMH,various MH index,surgical techniques,dyeing agent,vitreous tamponade,head position after operation,recoverment of ellipsoid zone,and so on.A comprehensive analysis of these prognostic factors could contribute to predict the patients' recoverment and communicate better with patients.It could also help to exclude confounding factors when the new clinical trials were conducted.Furthermore,more prospective clinical trials are needed to obtain high quality evidence.
Prognosis factors analysis of vitrectomy for idiopathic macular hole

Abstract:Objective: Many surgical techniques have been used to improve the post-operative visual acuity for idiopathic macular hole during recent decade. This study evaluated the relationship between vitrectomy for idiopathic macular hole and the factors associated with post-operative visual acuity. Methods: A retrospective review of a consecutive series of 109 patients (109 eyes) who underwent vitrectomy for macular hole surgery was performed. The multiple factors related with prognosis including preoperative visual acuity, duration of disease, stage and size of macular hole, internal limit membrane peeling technique, autologous concentrated platelet technique and postoperativve anatomic closure statues of the hole were analyzed. All the patients were assigned to three groups according to the difference of postoperative visual acuity from initial. The data was statistically analyzed using one way ANOVA, chi-square test and logistic regression. Results: There were significant difference of pre-op visual acuity (P = 0.016), macular hole size (P = 0.028) and autologous concentrated platelet technique (P < 0.005) among the three groups. Patients who were performed autologous concentrated platelet technique gained better visual acuity after surgery (r = 3.106, P = 0.014). Conclusion: Using autologous concentrated platelet in vitrectomy is helpful for the improvement of visual acuity in idiopathic macular hole patients.
Analysis of choroidal features to predict surgical prognosis of idiopathic macular hole

Abstract:Objective: To investigate choroidal features of idiopathic macular hole (IMH) and determine their relationship with surgical outcomes. Materials and methods: Patients above stage II unilateral IMH who received pars plana vitrectomy (PPV) with the internal limiting membrane (ILM) peeling were enrolled for the retrospective observational study. Preoperative choriocapillaris perfusion (CCP), central choroidal thickness (CCT), base/minimum diameters (BD/MD) and height (HH) of MH were analyzed by optical coherence tomography angiography (OCTA). At 1, 3 and 6 months after PPV, CCT, central foveal thickness (CFT) and maximum parafoveal thickness (MPT) of closed MH were measured. Best-corrected visual acuity (BCVA) was assessed at every visit. The correlations between preoperative characteristics and surgical outcomes were assessed. Results: Twenty-seven patients were evaluated. All eyes (100%) showed successful MH closure after the primary surgery. Until postoperative 6 months, BCVA continued to improve significantly (p < 0.001), while CFT and CCT progressively thinned (p < 0.001, p < 0.001). On correlation tests, final postoperative BCVA was associated with preoperative BCVA (R = 0.506, p = 0.007) and CCP (R = -0.475, p = 0.012), while final CFT was related with preoperative CCT (R = 0.392, p = 0.043). Multiple regression analysis revealed that preoperative CCP was significantly related with final postoperative BCVA (β = -0.403, p = 0.049). Conclusion: Preoperative CCP and CCT were respectively associated with functional and anatomical prognosis of IMH after PPV.
Optical Coherence Tomography Predictive Factors for Idiopathic Macular Hole Surgery Outcome

Abstract:OBJECTIVE:To evaluate the use of preoperative optical coherence tomography (OCT) image and some related indexes as predictive factors for the visual outcomes of the idiopathic macular hole (IMH) after pars plana vitrectomy and the internal limiting membrane (ILM) peeling surgery.METHODS:This was a case-series study. Twenty-five eyes of 23 IMH patients undergoing a successful vitrectomy and ILM peeling surgery and following up more than 6 months were included in this retrospective study. Of the 23 patients, 6 were male and 17 were female. The median patient age was 67-year old (range 63 to 71 years). The median duration of IMH was 6 months (range 3.5 to 12.0 months).In the patients, the macular hole was found in 9 eyes at stage II, 11 eyes at stage III, and 5 eyes at stage IV. Best corrected visual acuity (BCVA) were examined before and after the surgery. The minimum diameter (a) , the height (b) and the base diameter (c) of macular holes were measured by OCT. According to the indexes, macular hole index (MHI, b/c) , tractional hole index (THI, b/a) , diameter hole index (DHI, a/c) were calculated, respectively. Spearman rank correlation analysis was performed to understand the correlations between postoperative BCVA and the related indexes. Cut-off values were obtained for MHI and THI, respectively, from receiver operating characteristic (ROC) curve analysis.RESULTS:The median preoperative BCVA was 0.100(range 0.050 to 0.180). The median MHI, THI and DHI was 0.476(range 0.416 to 0.684), 1.048(range 0.761 to 1.346) and 0.536(range 0.431 to 0.645), respectively. The median 3-month postoperative BCVA was 0.150 (range 0.120 to 0.275), markedly improved after surgery (Mann-Whitney U test:P = 0.002) . The median latest postoperative BCVA was 0.250 (range 0.135 to 0.350), also markedly improved (Mann-Whitney U test, P = 0.002) . MHI, THI and preoperative BCVA correlated significantly with postoperative BCVA by Spearman rank correlation analysis (rMHI = 0.481, P = 0.015; rTHI= 0.516, P = 0.008; preoperative BCVA = 0.560, P = 0.004, respectively). Areas under the curve of 0.740 and 0.760 for MHI and THI were obtained respectively. Cut-off values of 0.475 and 0.973 were obtained for MHI and THI, respectively, from ROC curve analysis. Postoperative BCVAs in the MHI ≥ 0.475 group and in the THI ≥ 0.973 group was better than that in the MHI < 0.475 and in the THI < 0.973 group, respectively (Mann-Whitney U test:P MHI = 0.013, P THI = 0.008) .CONCLUSION:An MHI ≥ 0.475 or a THI ≥ 0.973 may be predictive factors for a good visual prognosis after IMH surgery.
Postoperative anatomical and functional outcomes of different stages of high myopia macular hole

Abstract:Background: Recently it was suggested that high myopia macular holes (HMMH) and macular holes accompanied by retinal detachment occur in the advanced stages of myopia traction maculopathy (MTM), while macular retinoschisis, shallow retinal detachment without holes, and lamellar macular holes occur in the early stages of MTM. Complete vitreous cortex removal associated with internal limiting membrane peeling is now widely used to treat HMMH. However, it remains uncertain at what HMMH stage patients would benefit most from surgical intervention. Our study was aimed to evaluate the postoperative anatomical changes and functional outcomes of high myopia macular holes (HMMH). Methods: Patients were retrospectively collected between March 2009 and August 2011. Before and 1st, 3rd, and 9th month after 23G pars plana vitrectomy, all patients underwent a complete ophthalmologic examination, spectral domain optical coherence tomography (SD-OCT) and MP-1. At each follow-up, best-corrected visual acuity (BCVA), photoreceptor inner and outer segments (IS/OS) defects, and retinal sensitivity (RS) were investigated. According to different preoperative macular hole morphologies, patients were divided into three groups: Group 1, macular hole with epiretinal membrane (ERM) traction and macular retinoschisis; Group 2, full-thickness macular hole (FTMH); Group 3, FTMH with subretinal fluid. Results: 43 eyes from 43 patients met the inclusion criteria. The mean age was 60 years. BCVA and RS were significantly improved after vitrectomy; the mean IS/OS defect was significantly reduced. At 9 postoperative months, 11 of 43 (25.6 %) eyes achieved IS/OS junction integrity; 9 of these 11 (81.8 %) eyes belonged to Group 1, 2 (18.2 %) belonged to Group 2. Conclusions: Pars plana vitrectomy combined with ILM peeling and gas tamponade results in limited functional outcomes in patients with HMMH. The appearance of subretinal fluid indicates a worse prognosis for surgical intervention.
Stage or size? The identity of anatomical and visual outcomes in stage 3 and stage 4 idiopathic macular holes after vitrectomy

Abstract:Background: Several previous reports suggested that stage 4 idiopathic macular holes (IMHs) may exhibit lower rate of anatomical success and poorer functional results comparing with stage 3 IMHs, while some others showed no differences. Actually, few studies focused on comparison of prognosis between stage 3 and stage 4 IMHs. Our previous study found that IMHs of these two stages demonstrate similar preoperative characteristics, and this study aims to compare anatomical and visual outcomes of IMHs between stage 3 and stage 4, and tries to figure out the outcome-associated factors. Methods: This retrospective consecutive case series reviewed 317 eyes with IMHs of stage 3 and stage 4 from 296 patients who underwent vitrectomy with internal limiting membrane peeling. Preoperative characteristics like age, gender, and hole size, and intraoperative interventions such as combined cataract surgery were evaluated. Outcome measures included the primary closure rate (type 1), best-corrected visual acuity (BCVA), foveal retinal thickness (FRT) and prevalence of outer retinal defect (ORD) at the last visit. The pre-, intra-, and post-operative information were respectively compared between stage 3 and stage 4. Results: The preoperative characteristics and intraoperative interventions exhibited no significant differences between stages. With comparable follow-up durations (6.6 vs. 6.7 months, P = 0.79), IMHs of the two stages exhibited similar primary closure rate (91.2% vs. 91.8%, P = 0.85), BCVA (0.51 ± 0.12 vs. 0.53 ± 0.11, P = 0.78), FRT (134.8 ± 55.5 μm vs. 138.8 ± 60.7 μm, P = 0.58), and prevalence of ORD (55.1% vs. 52.6%, P = 0.39). IMHs, either < 650 μm or larger, exhibited no significant difference in outcomes between the two stages. However, smaller IMHs (< 650 μm) demonstrated higher rate of primary closure (97.6% vs. 80.8%, P < 0.001), better postoperative BCVA (0.58 ± 0.26 vs. 0.37 ± 0.24, P < 0.001), and thicker postoperative FRT (150.2 ± 54.0 vs. 104.3 ± 52.0, P < 0.001) comparing with larger ones regardless of stage. Conclusion: IMHs of stage 3 and stage 4 exhibited considerable identity of anatomical and visual outcomes. In large IMHs, the hole size, instead of stage, may be more important for prediction of surgical outcomes and choice of surgical techniques.
An Analysis of the Prognosis and Factors of Vitrectomy for a Traumatic Macular Hole

Abstract:Objective To evaluate the prognosis and factors of vitrectomy for a traumatic macular hole.Methods A retrospective case study was performed on a consecutive series of 54 patients (54 eyes) who underwent vitrectomy for macular hole surgery.The postoperative anatomic closure status of the hole,visual acuity prognosis,and multiple factors related to prognosis,including preoperative visual acuity,duration of disease,size of the macular hole,and preoperative anterior or posterior segment complications,were analyzed.Data were analyzed with a paired t test and chi-square test.Results After surgery,48 (89％) traumatic macular hole eyes had closure; 6 eyes with traumatic macular holes eyes had joint results.The closure time was 20.6±10.1 days.Visual acuity (VA) increased in 28 eyes (52％).There was a significant difference between preoperative VA and postoperative VA (t=4.496,P＜0.01).There were no significant differences in preoperative VA,duration of disease,or preoperative anterior segment complications between the two different VA prognosis groups.There were significant differences in macular hole size and preoperative posterior segment complications between the two different VA prognosis groups (x2=6.006,10.650,P＜0.05).Conclusion Using vitrectomy is helpful for the prognosis of anatomy and function in traumatic macular hole patients,but the patients with preoperative posterior segment complications had a poor VA prognosis.
The impact of preoperative parameters on postoperative foveal displacement in idiopathic macular hole

Abstract:This study examined the effect of vitrectomy combined with internal limiting membrane (ILM) peeling on foveal displacement in 42 eyes with idiopathic macular hole (IMH). A retrospective analysis was conducted to measure various macular hole parameters before surgery, including basal diameter, minimum diameter, hole height, and areas affected by traction such as macular hole area (MHA), macular hole cystoid space area (MHCSA), macular hole retinal area (MHRA), and total area (TA). The results showed a postoperative shift of the fovea towards the optic disc in all cases. Notably, the extent of foveal displacement was significantly linked to the preoperative basal diameter ( r s = 0.405, P = 0.008) but not to other preoperative parameters or postoperative visual acuity. Furthermore, the study found that the temporal side of the macular hole was more affected by traction than the nasal side preoperatively, leading to greater postoperative displacement (All P < 0.05).

Prediction of foveal microstructure and visual outcomes following surgery for idiopathic macular hole: An assessment using spectral-domain optical coherence tomography

Abstract:Purpose: To evaluate the efficacy of two novel indices, the hole closure index (HCI) and the hole healing index (HHI), in predicting both the anatomic outcome and postoperative visual acuity following surgical intervention for idiopathic macular holes. Methods: A total of 38 patients diagnosed with idiopathic macular hole (IMH) were included. All patients underwent standard surgical treatment, including vitrectomy, internal limiting membrane (ILM) peeling, and gas tamponade. Preoperative and postoperative spectral-domain optical coherence tomography (SD-OCT) was utilized to assess the anatomical status of the macular hole (MH). The maximum distance between the tips of the external limiting membrane (ELM) was designated as the hole size (HS). HHI, calculated as the ratio of hole height to HS, and HCI, calculated as the ratio of hole height to the average of minimum diameter and base diameter, were determined. Postoperative reconstruction of ELM and ellipsoid zone (EZ) was evaluated, along with analysis of best-corrected visual acuity (BCVA) on a logarithm of the minimum angle of resolution (logMAR) scale. Regression analysis was performed to evaluate the relationship between anatomical outcomes, postoperative visual acuity, and optical coherence tomography (OCT) parameters. Receiver operating characteristic (ROC) curves were generated for both HHI and HCI. Results: Regression analyses revealed significant correlations between HCI and the restoration of ELM and EZ at 6 months after surgery ( P = 0.002 and P = 0.014, respectively). In addition, a significant correlation was found between HHI and postoperative BCVA better than logMAR 0.52 also at 6 months after surgery ( P = 0.033). The area under the ROC curve (AUC) for HCI based on ELM and EZ reconstruction was high, with values of 0.942 and 0.842, respectively. AUC for HHI, determined by ROC curve analysis of postoperative BCVA, was 0.704. Conclusions: In conclusion, our findings indicate that HCI may be the most accurate predictor of type 1 closure, while HHI could be considered a potential predictor of postoperative visual acuity.

Abstract:Purpose.To evaluate the effect of various factors on the prognosis of functional outcome after surgical treatment of large macular holes. Materials and methods. The results of surgical treatment of idiopathic macular holes (over 400 μm) in 47 patients (47 eyes) were analyzed. Endovitreal intervention of 25 or 27 G was performed with removal of epiretinal membrane and air tamponade. Results. Mean values of the best corrected visual acuity (BCVA) and retinal photosensitivity have been significant improved postoperatively: BCVA – from 0,12 ± 0,1 to 0,24 ± 0,18 (p<0,01), photosensitivity – from 20,07 ± 3,85 dB to 21,43 ± 3,44 dB (p<0,05). The mean value of the predicted BCVA according to the maximum hole diameter was 0.21 ± 0.0898. There was no dependence of BCVA on patients' age, duration of macular hole (more than 6 months) and anterior-posterior eyeball size (p<0.05). Conclusion. Prediction of visual acuity after surgery is dependent by the initial retinal photosensitivity in the macular zone as well as the maximal diameter of the macular hole. Abstract:Objective To evaluate the associated factors for predicting anatomical outcomes of idiopathic macular hole (IMH) after vitrectomy.Methods This is a retrospective study.A total of 165 eyes in 164 IMH patients underwent a successful vitrectomy and ILM peeling surgery were included in this study.The patients included 43 males and 121 females,with the mean age of (64.0±6.7) years.The corrected vision acuity of logarithm of the minimum angle of resolution (logMAR),indirect ophthalmoscope and spectral domain optical coherence tomography (SD-OCT) were measured for all patients.The mean logMAR corrected vision acuity was 1.0.The duration of disease was (8.7±14.9) months.The minimum diameter (MIN),base diameter (BASE) and height (H) were 521,1010,406 μm respectively.The macular hole index (MHI),tractional hole index (THI),diameter hole index (DHI) and macular hole closure index (MHCI) were 0.43,0.82,0.57,0.92 respectively.Anatomical outcomes were divided into 3 levels.A:bridge-shaped healing; B:good healing; C:poor healing.The mean follow-up was (3.6±3.2) months.The multiple factors related with prognosis including age,sex,duration of disease,preoperative logMAR corrected vision acuity,MIN,BASE and H,MHI,THI,DHI,MHCI were analyzed.Results Duration of disease (r=0.141),preoperative logMAR corrected vision acuity (r=0.082),age (r=0.044),sex (r=0.109) was independent of anatomical prognosis (P＞0.05).MIN (r=0.397),BASE (r=0.276),H (r=-0.240),MHI (r=-0.363),THI (r=-0.432),DHI (r=0.272) was weak correlation to anatomical prognosis (P＜0.05).MHCI correlated significantly with anatomical outcomes (r=-0.543,P=0.000).The median MHCI of A,B and C were 1.07,0.91,0.56 respectively.There were significant difference of MHCI among the three levels (H =52.857,P＜0.05).Conclusions MHCI has the best correlation with anatomical outcomes.It can be considered a key factor for predicting anatomical outcomes of IMH after vitrectomy.
Long-term Visual Outcomes in Patients With Idiopathic Macular Hole Surgery

Abstract:Background and objective: This study assesses long-term outcomes following surgical repair of idiopathic full-thickness macular holes (FTMHs) in patients with at least 5 years of postoperative follow-up. Patients and methods: A retrospective study evaluated patients diagnosed with idiopathic FTMH who received surgical repair at a single tertiary center with at least 5 years of postoperative follow-up. Data collection included demographic and preoperative characteristics along with macular hole structural integrity as determined by spectral-domain optical coherence tomography (OCT). Functional and structural improvement were assessed by collection of visual acuity and findings on OCT at determined time points until 9 years of follow-up. Results: The study comprised 90 eyes of 80 patients with a mean age of 67.2 ± 6.8 years, with an average postoperative follow-up of 80.8 ± 17.4 months (range 54 to 130 months). The mean macular hole diameter was 239.7 µm ± 92.2. Macular hole reoperation occurred in four eyes (4%) at a mean duration of 5.5 ± 6 months (range 0.3 to 13 months). Over the study duration, ellipsoid zone (EZ) integrity was maintained in 67.8% of eyes, with an absence of intraretinal fluid (IRF) in 96% on final OCT. The preoperative mean Early Treatment Diabetic Retinopathy Study (ETDRS) best visual acuity (BVA) of 51 improved to a mean BVA of 76 at 5 years postoperatively, with an average gain of 24 letters at one year that remained stable over 5 years (P < 0.05). Eight years after surgical repair, more than 80% of patients achieved a BVA > 65. Conclusions: Vitreoretinal surgery for idiopathic FTMH resulted in successful hole closure and sustained visual acuity improvement over long-term follow-up. The observation of curative effect of different mode of surgery for the treatment of idiopathic senile macular hole

Abstract:Objective To investigate the difference of curative effect of various surgical methods for the treatment of idiopathic senile macular hole. Methods A retrospective analysis was made for 86 eyes with stage Ⅱ Ⅳ idiopathic full thickness macular hole treated with various modes of operation,ie, single vitrectomy (7 eyes),vitrectomy combined with autologous platelet concentrate (APC) as an adjuvant (40 eyes), vitrectomy with internal limiting membrane (ILM) peeling (14 eyes), vitrectomy with both ILM peeling and APC treatment (25 eyes). The main outcome measures included anatomic reattachment rate,change of visual acuity,findings of optic coherence tomography (OCT), Amsler grid and intra or postoperative complication evaluations. Results (1) In visual acuity improvement, the APC group (80.0%) was significantly better than anyone of the other three groups ( P 0.05). (2) In anatomic success rate, the single vitrectomy group was significantly lower than the vitrectomy with APC treatment group (87.5%) or vitrectomy with both ILM peeling and APC as an adjuvant group(92.0%)( P 0.05). (3) There was no significant difference in operative complication and improvement of distortion of vision. Conclusion Vitrectomy combined with APC as an adjuvant for the treatment of idiopathic macular hole is helpful to improve both the anatomic success rate and postoperative visual acuity. The usage of ILM peeling technique could improve the anatomic reattachment rate, but the vision prognosis of ILM peeling patients is not as good as the patients of APC as an adjuvant.
Functional Evaluation with Microperimetry in Large Idiopathic Macular Holes Treated by a Free Internal Limiting Membrane Flap Tamponade Technique

Abstract:Background Free internal limiting membrane (ILM) flap tamponade technique is an alternative choice for treating large idiopathic macular holes (IMHs). However, the functional recovery related to this surgical approach is not well-characterized. This study aimed to evaluate morphological and microperimetric outcomes 6 months after free ILM flap tamponade technique for large IMHs. Methods Twenty-two patients (22 eyes) with large IMHs (minimal diameter > 400 μm) were retrospectively enrolled in this study. All patients underwent 23-gauge pars plana vitrectomy with ILM peeling and free ILM flap tamponade procedures. Snellen best-corrected visual acuity (BCVA), optical coherence tomography (OCT), and MP-1 microperimetry were measured at baseline and 6 months after surgery. Associations of postoperative BCVA with retinal sensitivity were detected. Results Macular hole closure was achieved in 21 eyes (95.5%). Dislodgement of free ILM flap was found in non-closed eye. Mean logMAR BCVA improved from 1.10 ± 0.33 at baseline to 0.67 ± 0.32 at 6 months postoperatively ( P < 0.001). The mean overall macular sensitivity and foveal fixation stability increased respectively from 8.58 ± 3.05 dB and 65.64 ± 17.28% before surgery to 11.55 ± 2.72 dB and 78.59 ± 13.00% at 6 months after surgery ( P < 0.001). The mean change in foveal sensitivity (within 2°) was significantly greater than the change achieved for peri-foveal sensitivity (2° to 10°) by 1.50 ± 2.62 dB ( P = 0.014). Linear regression analysis showed that postoperative logMAR BCVA was significantly associated with duration of symptom ( B = 0.063, P = 0.001), preoperative logMAR BCVA ( B = 0.770, P = 0.000), preoperative peri-foveal ( B = − 0.065, P = 0.000) and foveal sensitivity ( B = − 0.129, P = 0.000). Moreover, multiple regression model revealed that preoperative foveal sensitivity was independently associated with postoperative logMAR BCVA ( B = − 0.430, P = 0.040). Conclusions Vitrectomy combined with ILM peeling and free ILM flap tamponade technique results in effective morphological and functional recovery for large IMHs. Preoperative foveal sensitivity might be a prognostic indicator for postoperative BCVA.

Abstract:Objective: To determine the optimum way to treat macular holes. Methods: Operative methods studied were gas-injection operation in vitreous, vitrectomy, and autologous concentrated platelets combined with internal tamponade sealing the macular hole. Postoperatively, the effect on the reattachment rate and visual acuity were evaluated. Data were analysed statistically by χ2-test. Results: (1)Rate of the reattachment: the success rate in the group of gas-injection in vitreous was 88.2%, the group of vitrectomy combined internal tamponade was 100%; (2) The group of vitrectomy combined with internal tamponade acquired better postoperative visual acuity than the group of gas-injection in vitreous (P < 0.01); (3) The group of macular holes sealed with autologous concentrated platelets acquired better postoperative visual acuity than the group without it (P < 0.05). Conclusions: Vitrectomy and autologous concentrated platelet combined with tamponade to seal macular holes is an ideal way to obtain best visual improvement.

Abstract:Objective To access the factors associated with the development of a macular hole after vitrectomy.Methods 7 eyes that developed macular holes after vitrectomy between March 2007 and Feb 2011 in our hospital were reviewed.Results The mean interval between the vitrectomy and onset of a macular hole was 7.4 months with a range from 2 to 15 months.The initial vitrectomy was performed on 3 eyes with rhegmatogenous retinal detachment and 4 eyes with proliferative diabetic retinopathy.All cases underwent vitrectomy with removal of the internal limiting membrane,and the mean follow-up time after surgery was 10.8 months with a range from 3 to 14 months.Single operation macular hole closure rate was 71.4%(5 eyes).Preoperative best corrected visual acuity ranged from 0.02 to 0.2,with a mean of 0.13.Postoperative best corrected visual acuity ranged from 0.08 to 0.4,with a mean of 0.18.Visual acuity had improved in 4 eyes(57.1%),unchanged in 2 eyes(28.6%),and declined in 1 eye.Conclusion The incidence of macular hole after vitrectomy is very rare,and these cases can be repaired by vitrectomy with removal of the internal limiting membrane.The pathogenesis of the formation of these holes remains unclear.

Abstract:Purpose: To investigate the anatomical and visual outcomes of inverted internal limiting membrane (i-ILM) flap insertion versus single-layered i-ILM flap covering in highly myopic macular holes (HMMHs) associated with macular retinoschisis (MRS). Methods: A retrospective study compared 23G vitrectomy with i-ILM flap insertion (30 eyes) or covering (31 eyes) in HMMH patients. Pre- and postoperative optical coherence tomography images and best-corrected visual acuity (BCVA) were evaluated. Macular hole schisis (MHS) was classified into three types based on the extent of outer layer schisis. Regression analysis identified predictors of closure rate and postoperative BCVA. Results: The baseline data of the two groups were matched, including BCVA, axial length, and minimum linear diameter, except for a higher hole height in insertion group (P=0.038). After a mean follow-up of 11.7 months, type I closure rates were 83.3% (25/30) in the insertion group and 90.3% (28/31) in the covering group (P=0.335), respectively. The intact external limiting membrane in the covering group (7/28) was higher compared to the insertion group (1/28) finally (P=0.026). Final BCVA improved significantly in both groups (P<0.001), the BCVA was better in closed HMMHs in the covering group (P=0.011). Multivariate linear regression analysis revealed that preoperative BCVA (β=0.386, P=0.001) and MHS stage (β=0.309, P=0.004) were independent predictive factors for the final BCVA. Conclusion: The single-layered i-ILM flap covering favored foveal structure recovery and provided a better visual prognosis in closed HMMHs compared with insertion. The preoperative BCVA and MHS stage were independent predictors of visual outcomes in patients with HMMHs.

Abstract:Purpose: To evaluate photoreceptor inner/outer segment (IS/OS) defects, best-corrected visual acuity (BCVA), macular sensitivity, and fixation stability to correlate morphologic changes with visual functional outcomes at different stages after macular hole surgery using spectral-domain optical coherence tomography combined with microperimetry. Methods: This study was an interventional, retrospective case series. Sixteen eyes of 16 patients with successfully operated idiopathic full-thickness macular holes were included in this study. The IS/OS defect maximal diameter and area, BCVA, central macular sensitivity, mean macular sensitivity, and fixation stability were measured using spectral-domain optical coherence tomography combined with microperimetry, preoperatively, and with a follow-up of 3 months postoperatively. Results: Both the IS/OS defect diameter and area improved after successful macular hole surgery (P < 0.001; P < 0.001). The BCVA, central macular sensitivity, mean macular sensitivity, and fixation stability of the central 2° also improved (P = 0.001; P = 0.004; P = 0.036; P = 0.031). Stable BCVA improvement was achieved as early as 1-month postoperation despite continuous repair of the IS/OS junction defect diameter and area and improvement in fixation and macular sensitivity within the first 3 months after surgery. The postoperative central macular sensitivity and mean macular sensitivity negatively correlated with preoperative linear IS/OS junction defect diameter (P = 0.033; P = 0.006) and the defect area (P < 0.001; P = 0.002). However, the postoperative BCVA and the improvement in BCVA, macular sensitivity, and fixation stability were not correlated with preoperative IS/OS defect diameter or area. Conclusion: Continuous anatomical and functional improvements can be observed after successful microinvasive macular hole surgery. The preoperative extent of the IS/OS junction defect is of good predictive value for postoperative macular sensitivity. However, the factors that influence BCVA are multiple. Prediction of BCVA based on a single anatomical parameter or assessment of macular function only based on BCVA should be avoided.
