Abstract:Objective To provide an efficient and accurate method for the detection of the lateral femoral cutaneous nerve (LFCN) by ultrasound and to describe the anatomical variations of the LFCN.Methods A total of 300 LFCNs in 150 volunteers were evaluated with 18 MHz ultrasound;the intermuscular space between the tensor fasciae latae muscle and the sartorius was used as an initial sonographic landmark.The number of nerve branches at the level of the inguinal ligament and the relationship between the LFCN and the inguinal ligament was assessed.The time taken to identify the nerve was recorded.Results Each nerve was identified using ultrasound in all participants.The mean distance between the LFCN and the anterior superior iliac spine was 15.4 ± 4.1 mm.The nerve passed under the inguinal ligament in 252 cases.The LFCN consisted of 1-4 branches just after its passage under or through the inguinal ligament.The mean time for identifying the nerve was 7 s for unilateral LFCN.Conclusions It is easier to identify the LFCN if the intermuscular space between the tensor fasciae latae muscle and the sartorius is used as an initial sonographic landmark.The anatomical variation of the LFCN can be viewed by ultrasound.

Abstract:INTRODUCTION: During decompressive procedures, localizing the lateral femoral cutaneous nerve (LFCN) using only the anterior superior iliac spine (ASIS) can be difficult due to variations in nerve course and specific patient anatomy, e.g., body habitus. Therefore, the present anatomical study was performed to provide additional landmarks for localizing this nerve. METHODS: The LFCN was dissected in 30 adult cadavers (60 sides). The nerve was exposed distal to the ASIS in the proximal thigh and along an imaginary line connecting the ASIS to the midpoint of the patella with the lower limb in a neutral position. RESULTS: The main trunk of the LFCN was identified deep to the fascia lata on all sides, 5-8 cm distal to the ASIS along the ASIS-patella line. The nerve was bordered by the sartorius muscle superficially and medially, the tensor fasciae latae muscle laterally, and the proximal rectus femoris tendon on its deep surface. Proximal to this triangular region, the LFCN was found to cross superficially to the proximal sartorius muscle on 58 sides and through the muscle on two sides. CONCLUSIONS: The study findings suggest an alternative method for identifying the LFCN, which could help locate the nerve during challenging cases and reduce the incidence of iatrogenic LFCN injuries resulting from inadequate anatomical landmarks.

Diagnosis and treatment for entrapement syndrome of lateral femoral cutaneous nerve

Abstract:Objective　To explore the clinical peculiarity, the diagnosis and treatment on entrapement syndrome of lateral femora1 cutaneous nerve(LFCN).Methods　15 patients were diagnozed as the entrapment syndrome of LFCN with application of cortical sensoly evoked potential (CSEP) and treated with conservative and surgical therapy.Results　Out of 15 cases who were treated by local block,11 cases healed and the other 4 cases also healed by surgical treatment.Conclusion　CSEP can benefit the diagnosis of LFCN entrapement syndrome and both conservative and surgical treatment show some effects.

Abstract:The lateral femoral cutaneous nerve originates from the anterior branch of the second and third lumbar spinal nerves and innervates the skin sensation of the lateral thigh, and is prone to anatomic variation. Compared with the traditional blind method, nerve block methods assisted by electrical nerve stimulator and guided by ultrasound have higher success rate. There are two approaches to block the femoral cutaneous nerve. The first approach is medial to the anterior superior iliac spine, the second one is below the anterior superior iliac spine which is now recommended. The nerve can be found between the sartorius and tensor fascia latae, where the lateral femoral cutaneous nerve is relatively fixed.
Value of High-Resolution Sonography Mapping and Locating the Banchial Plexus

Abstract:Objective To demonstrate the images of the brachial plexus and locate by means of high-resolution sonography.Methods Thirty volunteers underwent brachial plexus examination using an L10-14 MHz probe in the interscalene,supraclavicular,infraclavicular,and axillary regions.Thereafter,an block needle was advanced under direct ultrasound guidance to target nerves before confirmation by electrical nerve stimulation in eleven volunteers in each region.The quality of brachial plexus images,anatomic variations,and the needle advancement for nerve localization were dynamic recorded.Results The brachial plexus were successfully identified in the transverse view as round to oval hypoechoic structures with small internal punctuate echoes in all regions examined without one case in infraclavicular region. Hypoechoic structures were stimulated electrically and confirmed to be nerves by ultrasonic monitoring.Conclusions The high-resolution sonography can provide good quality brachial plexus ultrasound images.The needle technique for ultrasound-assisted nerve localization provides real-time guidance and is potentially valuable for brachial plexus blocks.

Abstract:BACKGROUND:The diagnosis of meralgia paresthetica (MP) is usually based on clinical symptoms and physical examination. Therefore, the present study aimed to investigate the lateral femoral cutaneous nerve (LFCN) sonographic features in MP patients.METHODS:A total of 86 clinically suspected MP patients and 40 asymptomatic volunteers were prospectively recruited in the study. The sonographic features of the LFCN were observed by 18 MHz high-frequency ultrasound. At the level of the anterior superior iliac spine, the cross-sectional area of the LFCN was measured.RESULTS:Of the 86 clinically suspected MP patients, 82 (95.3%) had sonographic findings positive for MP. There were 54, 63, and 44 cases of abrupt caliber change, indistinct perineurium, and abnormal intraneural vascularity. The average value of the cross-sectional area of the LFCN at the level of the anterior superior iliac spine was 4.47±2.64 mm2, and the cut-off value was 2.65 mm2.CONCLUSIONS:The sonographic diagnosis of MP may be achieved based on the following signs: nerve abrupt caliber change, indistinct perineurium of the nerve, intraneural vascularity, or increased cross-sectional area.
Cutaneous nerve fields of the anteromedial lower limb-Determination with selective ultrasound-guided nerve blockade

Abstract:Background: This study aimed to determine the peripheral cutaneous nerve fields (CNF), their variability, and potential overlap by selectively blocking the intermediate (IFCN) and medial (MFCN) femoral cutaneous nerves and the infrapatellar branch of the saphenous nerve (IPBSN) in healthy volunteers. Methods: In this prospective study, ultrasound-guided nerve blockades of the IFCN, MFCN, and IPBSN in 14 healthy volunteers were administered. High-frequency probes (15-22 MHz) and 1 ml of 1% lidocaine per nerve were used. The area of sensory loss was determined using a pinprick, and all fields were drawn on volunteers' skin. A three-dimensional (3D) scan of all lower limbs was obtained and the three CNF and their potential overlap were measured. Results: The mean size of innervation areas showed a high variability of peripheral CNF, with 258.58 ± 148.26 mm2 (95% CI, 169-348.18 mm2 ) for the IFCN, 193.26 ± 72.08 mm2 (95% CI, 124.45-262.08 mm2 ) for the MFCN, and 166.78 ± 121.30 mm2 (95% CI, 94.1-239.46 mm2 ) for the IPBSN. In 11 volunteers, we could evaluate an overlap between the IFCN and MFCN (range, 4.11-139.68 ± 42.70 mm2 ), and, in 10 volunteers, between the MFCN and IPBSN (range, 11.12-224.95 ± 79.61 mm2 ). In only three volunteers was an overlap area found between the IFCN and IPBSN (range, 7.46-224.95 ± 88.88 mm2 ). The 3D-scans confirmed the high variability of the peripheral CNF. Conclusions: Our study successfully determined CNF, their variability, and the overlap of the MFCN, IFCN, and IPBSN in healthy volunteers. Therefore, we encourage physicians to use selective nerve blockades to correctly determine peripheral CNF at the anteromedial lower limb.

Abstract:Objective To explore the etiology clinical findings and treatments of entrapment syndrome of lateral femoral cutanous nerve(LFCN) Methods The patients were diagnosed as the entrapment syndrome of LFCN with application of somato-senory evoked potential(SEP) and treated by local block and surgery for those who failed to response to local block treatment. Results The majority of the 19 cases treated by local block fully recovered. Only 3 cases(5.7% )had surgery. Conclusion The diagnosis of LFCN mainly depends on symptoms,signs;SEP can benefit the diagnosis of LFCN entrapment. Both conservative and surgical treatment are helpful for the treatment.

Abstract:BACKGROUND:Identifying the arterial variation of the lateral circumflex femoral artery (LCFA) is a vital step in planning surgical and radiological approach. The aim of the study was to evaluate the variations and discuss the clinical correlates of the LCFA.MATERIALS AND METHODS:Fifty eight adult cadavers (male 45, female 13) with 115 usable sides were used to assess and classify the origin and branches of the LCFA. Also its external diameter, distance from mid-inguinal ligament to sites of origin from the profunda femoris artery or femoral arteries.RESULTS:There were seven types of LCFA variations in this sample. We classified them as types A to G, of which type A was normal, that is, the one showing a single LCFA arising from the profunda femoris artery. Nearly 50.43% of the sample had type B-G variations, each having 13, 10, 23, 4, 4, and 3 cases, accounting for 11.30%, 8.70%, 20.00%, 3.48%, 3.48%, and 2.61%, respectively.CONCLUSIONS:There are many variant types in the LCFA. To avoid iatrogenic injuries, clinicians must have a sound understanding of the variation types of this important blood vessel.

Abstract:Cutaneous nerve entrapment plays an important role in neuropathic pain syndrome. Due to the advancement of ultrasound technology, the cutaneous nerves can be visualized by high-resolution ultrasound. As the cutaneous nerves course superficially in the subcutaneous layer, they are vulnerable to entrapment or collateral damage in traumatic insults. Scanning of the cutaneous nerves is challenging due to fewer anatomic landmarks for referencing. Abstract:BACKGROUND AND OBJECTIVE: Lateral femoral cutaneous nerve (LFCN) decompression and transposition are surgical treatment options for meralgia paresthetica. Identifying the LFCN during surgery may be challenging, and preoperative localization is a valuable adjunct in this case. The objective of this study was to explore a new technique using preoperative ultrasound-guided clip localization (USCL) of the LFCN. METHODS: After Institutional Review Board approval, data were collected on patients who underwent both preoperative ultrasound-guided wire localization (USWL) and USCL over the past 13 years. Skin-to-nerve time was calculated prospectively. RESULTS: Fifty-six patients were identified, 51 had USWL and 5 had USCL; the skin-to-nerve median time was 7.5 and 6 minutes, respectively. Six wires were misplaced, and this was at the beginning of utilization of the USWL technique. There were no nerve injury, infection, or bleeding complications related to either wire or clip placement. CONCLUSION: USWL or USCL is safe and time-efficient in LFCN surgeries.

Examination of sural nerve with high-frequency ultrasonography

Abstract:Objective To explore the examining method and high-frequency ultrasonographic imaging features of normal sural nerve,and to establish the normal reference range.Methods Totally 180 subjects underwent high-frequency ultrasound examination of the sural nerve.Ultrasonic appearance and the position relation between sural nerve and small saphenous vein were observed.The width and thickness of sural nerve and the thickness of subcutaneous fat at cruris posterior were measured.The correlations between the sural nerve and age,body height,body weight,body mass index(BMI),the thickness of subcutaneous fat at cruris posterior were analyzed.Results Normal sural nerve appeared as hyperechoic,oval reticular structure on transverse sections and hyperechoic,narrow fascicular structure on longitudinal sections under high-frequency ultrasound.The differences in width and thickness of sural nerve between men and women were not statistically significant(all P>0.05).Neither was there significant difference between left and right sural nerves.Nerve size was not correlated with age or body height(all P>0.05),but was positively correlated with body weight and BMI(P<0.05).The thickness of subcutaneous fat at cruris posterior was positively correlated with nerve thickness(P=0.001),and not correlated with nerve width(all P>0.05).Conclusion High-frequency ultrasonography can clearly display the sural nerve and will be a valuable method to locate the sural nerve and diagnose related diseases.

Abstract:Objective To evaluate the morphological changes of sural nerve in patients with type 2 diabetes mellitus by hyperfrequency ultrasound.Methods Fifty-six sural nerves of symptomatic group,64 sural nerves of asymptomatic group,and 60 sural nerves of control group were identified by 22 MHz Ultrasound.The thickness/width (T/W) ratio,cross-sectional areas (CSA) and the maximum thickness of neuronal fascicles (MT) of the sural nerve were calculated in transverse sonograms.Results ①Ultrasound can clearly show these structures of sural nerve such as epineurium,perineurium and nerve bundles and so on.②In symptomatic group,asymptomatic group and control group,T/W ratio and MT were gradually increased,and the differences among the three groups were statistically significant (P＜0.05).Whereas,there were no statistically significant differences in CSA among the three groups (P=0.257).Conclusions22 MHz ultrasound may be a valuable tool in evaluating the diabetic cutaneous nerve neuropathy

Abstract:Background The application of 22-MHz high-frequency ultrasound allows for visualization of the inner part of the sural nerve. The aim of this study was to evaluate the morphological changes of sural nerves in patients with type 2 diabetes mellitus using ultrasound. Materials and Methods The thickness/width (T/W) ratio, the cross-sectional area (CSA) of the sural nerves and the maximum thickness (MT) of the nerve fascicles were measured in 100 patients with type 2 diabetes mellitus and 50 healthy volunteers using 22-MHz ultrasound. Receiver operating characteristic (ROC) curves were plotted to determine the optimal cut-off values as well as the sensitivities and specificities. All parameters were significantly different between the subject and control groups. The ROC curves demonstrated that the MT was the most predictive of diabetic cutaneous neuropathy, with an optimal cut-off value of 0.365 mm that yielded a sensitivity of 90.3% and a specificity of 87.7%. Conclusions The results of this study suggest that 22-MHz ultrasound may be a valuable tool for evaluating diabetic cutaneous nerve neuropathy.
An Ultrasound Study of Anatomic Variants of the Sural Nerve.

Abstract:Introduction: There are anatomic variations of the sural nerve (SN). Knowledge of these morphological types and the length of the SN are important for planning surgical reconstruction of peripheral nerves. Our purpose was to classify the morphological types of the SN by using ultrasound. Methods: Two‐hundred SNs in 100 normal subjects were examined by ultrasound with 13‐MHz linear‐array transducers. Classification of the SN was evaluated by its formation. The distance between the union and the distal point of the lateral malleolus was measured. Results: SN variants could be classified into three types: type I (81%); type II (18%); and type III (1%). In type I, the average length of the SN was 15.8 ± 5.7 cm. Conclusions: The morphological types of the SN can be determined using ultrasound. Ultrasound classification of the SN is useful with regard to donor nerve surgical reconstruction of peripheral nerves. Muscle Nerve, 2011

Abstract:Introduction/Aims While ultrasound assessment of cross‐sectional area and echogenicity has gained popularity as a biomarker for various neuropathies, there is a scarcity of data regarding fascicle count and density in neuropathies or even healthy controls. The aim of this study was to determine whether fascicles within select lower limb nerves (common fibular, superficial fibular, and sural nerves) can be counted in healthy individuals using ultrahigh‐frequency ultrasound (UHFUS). Methods Twenty healthy volunteers underwent sonographic examination of the common fibular, superficial fibular, and sural nerves on each lower limb using UHFUS with a 48 MHz linear transducer. Fascicle counts and density in each examined nerve were determined by a single rater. Results The mean fascicle number for each of the measured nerves included the following: common fibular nerve 9.85 (SD 2.29), superficial fibular nerve 5.35 (SD 1.59), and sural nerve 6.73 (SD 1.91). Multivariate linear regression analysis revealed a significant association between cross‐sectional area and fascicle count for all three nerves. In addition, there was a significant association seen in the common fibular nerve between fascicle density and height, weight, and body mass index. Age and sex did not predict fascicle count or density (all p > .13). Discussion UHFUS enabled the identification and counting of fascicles and fascicle density in the common fibular, superficial fibular, and sural nerves. Knowledge about normal values and normal peripheral nerve architecture is needed in order to further understand and identify pathological changes that may occur within each nerve in different disease states.

Abstract:One hundred anterior branches of the lateral cutaneous nerve of the thigh were dissected and measured in 50 adult cadavers. The results show that this nerve was present in all sides, and the average length from the point of the nerve piercing the fascia lata to the point of its first branch was about 79 mm, the average width was 2.25 mm, and the average thickness was 0.79 mm. It was found that the nerve was approximately along the line between the anterior superior iliac spine and the midpoint of the upper margin of the patella. This study presents a simple method of locating this nerve for clinical reference. The results also reveal that this nerve is a possible donor for facial nerve repair.

Abstract:Aims Lateral femoral cutaneous nerve (LFCN) injury is a complication after periacetabular osteo-tomy (PAO) using an anterior approach, which might adversely affect the outcome. However, no prospective study has assessed the incidence and severity of this injury and its effect on the clinical outcomes over a period of time for longer than one year after PAO. The aim of this study was to assess the incidence and severity of the symptoms of LFCN injury for ≥ three years after PAO and report its effect on clinical outcomes. Methods A total of 40 hips in 40 consecutive patients who underwent PAO between May 2016 and July 2018 were included in the study, as further follow-up of the same patients from a previous study. We prospectively evaluated the incidence, severity, and area of symptoms following LFCN injury. We also recorded the clinical scores at one year and ≥ three years postoperatively using the 36-Item Short Form Health Survey (SF-36) and Japanese Orthopaedic Association Hip Disease Evaluation Questionnaire (JHEQ) scores. Results A total of 20 patients (50%) had symptoms of a LFCN injury at one year after PAO. At ≥ three years postoperatively, the symptoms had completely resolved in seven of these patients and 13 (33%) had persistent symptoms. The severity and area of symptoms did not significantly differ between one and ≥ three years postoperatively. The JHEQ showed significant differences in the patient satisfaction and mental scores between those with and those without sypmtoms of LFCN injury at ≥ three years postoperatively, while there was no significant difference in the mean SF-36 scores. Conclusion The incidence of LFCN injury after PAO using an anterior approach is high. The outcome of PAO, ≥ three years postoperatively, is poorer in patients with persistent symptoms from a perioperative LFCN injury, in that patient satisfaction and mental health scores are adversely affected. Ultrasound of Small Nerves

Abstract:Nerve ultrasound has become an integral part of the diagnostic workup of peripheral neuropathies. Especially in the examination of small nerves, ultrasound provides superior image quality by using high frequency transducers. For a selection of small nerves, this article summarizes the local anatomy and common pathologies and offers simple instructions for determining their location with ultrasound including some cases with pathologies. This selection of nerves comprises the great auricular nerve, the supraclavicular nerves, the suprascapular nerve, the medial antebrachial cutaneous nerve, the lateral antebrachial cutaneous nerve, the palmar cutaneous branch of the median nerve, the long thoracic nerve, the intercostobrachial nerve, the posterior cutaneous nerve, the infrapatellar branch of the saphenous nerve, the medial calcaneal nerve, and the deep peroneal nerve at the ankle. Following our recommendations, these nerves can be swiftly located and tracked along their course to the site of the pathology.

Abstract:Objective: To summarize the morphological characteristics of the transverse branch of lateral femoral circumflex artery (LFCA) using digital subtraction angiography (DSA) and explore its clinical significance. Methods: A retrospective observational study was conducted. From October 2020 to May 2021, 62 patients with soft tissue injuries in the extremities were hospitalized in Suzhou Ruihua Orthopedic Hospital, including 40 males and 22 females, aged from 20 to 72 years. DSA was performed in the lateral femoral region of patients before the anterolateral thigh flap transplantation, and in combination with imaging scale to observe and measure the general condition of the blood vessels and the occurrence (with the occurrence rate being calculated), source artery, location of the origin point, direction of course, and the location of the perforating point of the cutaneous perforator of the transverse branch of LFCA, and in addition to classify the morphological characteristics of the transverse branch. Results: DSA detection showed that the femoral artery, the deep femoral artery, and the branches of LFCA were clearly distinguishable in 62 patients. Transverse branches of LFCA were observed in 59 patients, including 52 cases with a single transverse branch, and 7 cases with double transverse branches. The occurrence rate of transverse branches was 95.2% (59/62). A total of 66 transverse branches of LFCA were observed, of which 3 originated from the deep femoral artery, and 63 originated from the LFCA. The origin point of the transverse branch was 6.5-12.7 cm away from the anterior superior iliac spine. The transverse branch which was approximately perpendicular to the long axis of the body, originated outwards, ran between the ascending branch of LFCA and the oblique branch of LFCA, and branched along the way, with the trunk running under the greater trochanter. The perforating point of the cutaneous perforator of the transverse branch was 8.0-18.0 cm away from the anterior superior iliac spine. In the classification of morphological characteristics of the transverse branch of LFCA, the most common type was the one that originated from the same trunk with other branches of LFCA, accounting for 50.0% (31/62), followed by the one that originated from the singular trunk of LFCA (12 cases) or deep femoral artery (3 cases), accounting for 24.2% (15/62); the special type accounted for 21.0% (13/62), including 7 cases of double transverse branches and 6 cases of the transverse branch originated from the same trunk with multiple other branches of LFCA; those with small/absent transverse branch only accounted for 4.8% (3/62). Among the above-mentioned common trunk relationship of two branches, those with shared trunk of ascending and transverse branches were most frequently observed, accounting for 77.4% (24/31); those with shared trunks of the transverse and oblique branches (5 cases) and the transverse and descending branches (2 cases) accounted for 22.6% (7/31) altogether. Conclusions: A high incidence rate of the transverse branch of LFCA is observed through DSA. The transverse branch originates from the lateral femoral artery approximately perpendicular to the long axis of the body, mainly from the same trunk with another main branch of LFCA, especially the ascending branch. This positioning analysis can provide an important reference for the design and resection of anterolateral femoral flaps.
