ULBP5 is a Susceptibility Gene for Henoch-Schönlein Purpura in Chinese Population

Abstract:Background: Henoch-Schönlein Purpura (HSP) is one of the most common systemic small vasculitis in children. At present, it is believed that genetic factors play an important role in the etiology and pathogenesis of HSP.Methods: In order to find the susceptible genes associated with Henoch-Schönlein purpura (HSP), the whole exome sequencing (WES) was carried out in 4 Chinese families with HSP. Then, ULBP5 was sequenced by Polymerase Chain Reaction (PCR) in 123 unrelated patients with HSP group (UHSPG), in 115 patients with connective tissue disease group (CTDG, not including HSP) and in 105 health children group (HCG) respectively. Besides, analysis of ULBP5 protein sequence conservation and amino acid properties by software. And statistical analysis the frequency and clinical phenotype of HSP patients with ULBP5 mutations. In vitro, the plasmids of 10 variants and wild type (WT) of ULBP5 were synthesized, that were transfected into 293T cells and COS-7 cells, and were incubated with NK cells respectively to study the impact of ULBP5 mutations on the cytolysis mediated by NK cell.Findings: It revealed that ULBP5 with mutations, which included c.260G>A, c.8C>T, and c.143G>T respectively, were found simultaneously in 3 families and showed that there were 8 different heterozygous variants (c.355C>G, c.472T>G, c.203G>A, c.944T>C, c.421C>T, c.913G>A, c.143G>T, and c.85+13del) in UHSPG (9 patients with ULBP5 mutations, and the variants of 2 patients were same), and there was a variant (c.143G>T) between 2 patients in CTDG (c.143G>T also being identical with one found in UHSPG and family B), and there was a variant (c.493G>C) in HCG. So, a total of 11 variants were found in this study, that 10 variants were found in patients with HSP, and one was found in HCG. The frequency (9.5%, 12/126) of ULBP5 mutations in patients with HSP was far higher than that (1.7%, 2/115) in CTDG and (0.95%, 1/105) in HCG. In vitro, the results indicated that the cytolysis of NK cells incubated with 293T cells and COS-7 cells were significantly stronger from HSP than that from HCG and WT. Of 19 HSP patients with ULBP5 mutations, 18 patients (95%, 18/19) presented with abdominal HSP, and 10 patients (52.6%, 10/19) presented with gastrointestinal bleeding.Interpretation: These results suggest that ULBP5 is susceptible to HSP, that are more prone to abdominal HSP by enhancing the cytotoxicity of NK cells.Funding Information: Fund of Science and Technology Development and Innovation of Pudong New Area, Shanghai, China (No. PKJ2018-Y44).
