Current Status of Clinical Research on Gastroenteropancreatic Neuroendocrine Tumors in China

Abstract:Background: Neuroendocrine tumors (NETs) are groups of heterogeneous tumors that arise from the diffuse neuroendocrine system. Epidemiological data from abroad indicated that the prevalence of gastroenteropancreatic NETs (GEP-NETs) is only exceeded by that of colorectal cancer and higher than that of other gastrointestinal cancers. Recognition and studies regarding GEP-NETs are relatively deficient in China. Aims: To collect GEP-NETs-related reports published in Chinese over the years and to analyze the clinical profile and research status of GEP-NETs in China. Methods: Reports of GEP-NETs were retrieved from four main Chinese databases (CBM, WANFANG DATA, CNKI and VIP) by using 'neuroendocrine tumor' or 'carcinoid' (both in Chinese) as the search term. Information on geographical distribution, clinical features, diagnosis, treatment and prognosis of GEP-NETs were analyzed retrospectively. Results: A total of 11671 cases of GEP-NETs were reported in 863 eligible articles and 54.9% of the cases were collected from the east and north part of China. Pancreatic NETs were the most commonly reported (5807 cases) and 89.6% of them were functioning tumors. Five thousand and one cases (48.2%) had characteristic symptoms of NETs. The misdiagnosis rate of GEP-NETs was 55.1% (1544/2802). Tumors were mainly detected by routine examinations such as gastroscopy, colonoscopy, ultrasound and CT. Surgical resection was the most commonly used treatment modality and the recovery rate was 83.0% (4052/4882). Conclusions: Currently, GEP-NETs are not sufficiently recognized by Chinese clinicians, the misdiagnosis rate is high, and the overall incidence and prognosis are unclarified. A national database of NETs should be established for studying the clinical and epidemiological features of GEP-NETs.
