Abstract:OBJECTIVES:To study the relationship of the number and quality of embryos transferred with clinical pregnancy rate and multiple pregnancy rate in frozen-thawed embryo transfer (FET) cycle. METHODS:Retrospective analysis of the clinical data of 995 FET cycles. Patients were stratified as age < 35 or > or = 35, Both groups' clinical pregnancy rate and multiple pregnancy rate were compared according to the number of embryos and high-quality embryos transferred. RESULTS:(1) The clinical pregnancy rates were 50.0%, 56.6% and 56.5%, and twin pregnancy rates were 6.3%, 43.8% and 30.6% respectively in < 35 group when one, two or three embryos were transferred. There is not any significantly statistic difference among these subgroups (P > 0.05). However, the twin pregnancy rate in one-embryo transferred subgroup was significantly lower than that of two-embryo transferred subgroup (P < 0.05). Moreover, the clinical pregnancy rate was not different among single high-quality embryo transferred subgroup and other high-quality embryo transferred subgroups, the multiple pregnancy rate was lower than the others. (2) The clinical pregnancy rates were 0, 47.3% and 53.8%, and twin pregnancy rates were 0, 25.7% and 25.7% respectively in > or = 35 group when one, two or three embryos were transferred. The clinical pregnancy rate and twin pregnancy rate between two-embryo and three-embryo transferred subgroups were not significantly different (P > 0.05). Moreover, the clinical pregnancy rate was not different among two-embryo transferred containing one high-quality embryo subgroup and other high-quality embryo transferred subgroups, the multiple pregnancy rate was lower than the others. CONCLUSIONS:There is a close relationship between the number and quality of embryos transferred and clinical pregnancy rate, multiple pregnancy rate in FET cycles. For the patient aged < 35, we recommend performing single high-quality embryo transfer in order to reduce the multiple pregnancy. For the patient aged > or = 35, two-embryo transfer containing one high-quality embryo is enough to obtain a satisfied clinical pregnancy rate and an acceptable multiple pregnancy rate.

Abstract:BACKGROUND:Elective single embryo transfer (eSET) has been increasingly advocated, but concerns about the lower pregnancy rate after reducing the number of embryos transferred have encouraged transfer of multiple embryos. Extended embryo culture combined with electively freezing all embryos and undertaking a deferred frozen embryo transfer might increase pregnancy rate after eSET. We aimed to establish whether elective frozen single blastocyst transfer improved singleton livebirth rate compared with fresh single blastocyst transfer.METHODS:This multicentre, non-blinded, randomised controlled trial was undertaken in 21 academic fertility centres in China. 1650 women with regular menstrual cycles undergoing their first cycle of in-vitro fertilisation were enrolled from Aug 1, 2016, to June 3, 2017. Eligible women were randomly assigned to either fresh or frozen single blastocyst transfer. The randomisation sequence was computer generated, with block sizes of two, four, or six, stratified by study site. For those assigned to frozen blastocyst transfer, all blastocysts were cryopreserved and a delayed frozen-thawed single blastocyst transfer was done. The primary outcome was singleton livebirth rate. Analysis was by intention to treat. This trial is registered at the Chinese Clinical Trial Registry, number ChiCTR-IOR-14005405.FINDINGS:825 women were assigned to each group and included in analyses. Frozen single blastocyst transfer resulted in higher rates of singleton livebirth than did fresh single blastocyst transfer (416 [50%] vs 329 [40%]; relative risk [RR] 1·26, 95% CI 1·14-1·41, p<0·0001). The risks of moderate or severe ovarian hyperstimulation syndrome (four of 825 [0·5%] in frozen single blastocyst transfer vs nine of 825 [1·1%] in fresh single blastocyst transfer; p=0·16), pregnancy loss (134 of 583 [23·0%] vs 124 of 481 [25·8%]; p=0·29), other obstetric complications, and neonatal morbidity were similar between the two groups. Frozen single blastocyst transfer was associated with a higher risk of pre-eclampsia (16 of 512 [3·1%] vs four of 401 [1·0%]; RR 3·13, 95% CI 1·06-9·30, p=0·029).INTERPRETATION:Frozen single blastocyst transfer resulted in a higher singleton livebirth rate than did fresh single blastocyst transfer in ovulatory women with good prognosis. The increased risk of pre-eclampsia after frozen blastocyst transfer warrants further studies.FUNDING:The National Key Research and Development Program of China.

Comparison of transfers of 2 high-quality embryos with 1 high-quality and 1 fair-quality embryo showed that the CPR and LBR were significantly lower for the latter in Groups A and C, but Group B had no salient changes. Higher IR and LBR and lower MPR may be achieved by selection of optimal embryo types for patients of different ages. Two high-quality embryos need to be transferred in women older than 38 years. For women aged 36 to 37 years, 1 high-quality embryo or 1 high-quality plus 1 fair-quality embryo should be singled out for transfer. For women younger than 35 years, a single high-quality embryo should be selected for transfer.

Abstract:(Abstracted from N Engl J Med 2018;378:126–136) A previous randomized controlled trial involving women with polycystic ovary syndrome (PCOS) compared fresh-embryo transfer with elective freezing of all embryos followed by frozen-embryo transfer. The data showed that frozen-embryo transfer had a higher live-birth rate in these women compared with fresh-embryo transfer. Whether frozen-embryo transfer also increases live-birth rates among ovulatory women with infertility without PCOS is uncertain.

Abstract:BACKGROUND:Reduced endometrial receptivity in hyperstimulated cycles may lead to a lower implantation rate and a lower clinical pregnancy rate, but it is unclear if it is also associated with an increase in pregnancy loss rate. The aim of this study was to compare the implantation, miscarriage, and pregnancy rates between fresh and frozen thawed transfer of one or two day-3 embryos, with a view to understanding whether or not reduced endometrial receptivity encountered in hyperstimulated cycles is associated with an increase in miscarriage rate.METHODS:This study involved a consecutive series of 1 551 single day-3 embryo transfer cycles and consecutive 5 919 double day-3 embryo transfer cycles in the Assisted Reproductive Unit of the Sir Run Run Shaw Hospital, Hangzhou, China, between January 2010 and December 2012.RESULTS:The implantation and clinical pregnancy rates (single embryo 30.7% and double embryos 33.4% and 51.4%) using fresh cycle were both significantly lower than that of frozen-thawed cycles (single embryo 35.8% and double embryos 38.1% and 57.8%). There was no difference in biochemical loss or clinical miscarriage rates between the two groups.CONCLUSIONS:Impairment of endometrial receptivity associated with ovarian hyperstimulation leads to implantation failure at a very early stage, resulting in an increased number of non-pregnancy. It does not lead to increase in biochemical or clinical losses. The significantly reduced ongoing pregnancy rates in both fresh single and double embryo transfer are therefore due to failure to achieve a pregnancy, rather than pregnancy loss after conception.

Abstract:Multiple pregnancies are one of the major safety concerns of in vitro fertilization (IVF) due to the increased risk of maternal and neonatal complications. Single embryo transfer is the most effective way to reduce the risk of multiple pregnancies. Selection of the embryo and optimization of the implantation environment are crucial to retain the success rate when the number of transferred embryos is diminished. Fresh embryo transfer with supra-physiological levels of hormones has been suggested to have an adverse effect on implantation. Elective frozen embryo transfer has been suggested to result in a higher rate of live birth than fresh embryo transfer. However, there is still a lack of evidence from randomized clinical trials comparing the efficacy and safety between frozen and fresh single blastocyst transfers.

Abstract:Objective:To investigate the influence of post-thaw survival rate or pre-freeze embryo qulity and clinical pregnancy rate in frozen-thawed embryo transfer program(FET).Methods: A total of 683 FET of the patients aged <35 years and who were only transferred 2 embryos in a cycle were analyzed retrospectively from Jan 2008 to Dec 2010.Cases were divided into 4 groups according to the pre-freeze embryo qulity and post-thaw survival rate(Group A: fully intact embryos and ≥1 pre-freeze high-qulity embryo,Group B: fully intact embryos and no pre-freeze high-qulity embryo,Group C: post-thaw damaged embryos and ≥1 pre-freeze high-qulity embryo,Group D: post-thaw damaged embryos and no pre-freeze high-qulity embryo).And clinical pregnancy rate and multiple pregnancy rate were compared between groups.Results: There was no significant difference in the clinical pregnancy rate and multiple pregnancy rate between Group A and Group C,or Group B and Group D,or Group A+B and Group C+D.But there was a significantly higher pregnancy rate in Group A and Group C(A vs.B,A vs.D,A+C vs.B+D,P<0.01;C vs.B,P<0.05).Conclusions: If there was any pre-freeze high-qulity embryo in the post-thaw embyos,they will have a higher pregnancy rate.

Abstract:Objective: To compare clinical outcome of fresh embryo transfer(ET) cycles and frozenthawed embryo transfer(FET) cycles.Methods: A total of 287 cycles were analyzed retrospectively to compare the pregnancy rates,implantation rates and abortion rates between fresh ET(190 cases) and FET cycles(97 cases).Results: The pregnancy rate,implantation rate and abortion rate were 47.4%(90/190),30.2%(103/341) and 10.0%(9/90) in fresh ET cycles,comparing with 60.8%(59/97),47.0%(86/183) and 10.2%(6/59) respectively,in FET cycles.The pregnancy rates and implantation rates were significantly different between fresh ET and FET cycles(P<0.05),the abortion rates were not significantly different(P>0.05).Conclusion: The fresh embryos were cryopreserved and then transferred in FET cycles in patients with some symptom and sign of ovarian hyperstimulation syndrome(OHSS),and it did not reduce implantation rates and pregnancy rates.Modifying the transfer strategy will reduce the risk of OHSS later and gain a better clinical outcome.

Abstract:It has been reported that up to 1 in 6 couples may experience fertility issues. [ 1 , 2 ] The use of assisted reproductive technology by couples seeking a healthy live birth is becoming increasingly common. Treatment cycles involving frozen (thawed) embryo transfer (FET) have been demonstrated to enhance cumulative pregnancy rates, reduce costs, and are relatively straightforward to perform. [ 3 , 4 ] Assisted reproduction has extensively utilized FET. It is primarily employed in cases where women do not conceive following fresh embryo transfer cycles, where the endometrium is unsuitable for fresh embryo transfer, or when there is a high risk of ovarian hyperstimulation syndrome. Typically, FET is conducted using various cycle regimens: spontaneous ovulatory cycles (natural cycle, NC); hormone therapy FET cycles, where the endometrium is artificially prepared using estrogen and progesterone; and cycles where ovulation is induced by medication (ovulation induction FET cycles).

Abstract:OBJECTIVE:To analyze the factors influencing the outcome in a frozen thawed embryo transfer (FTET) program.METHODS:Sixty FTET cycles performed in 53 cases from September 1997 to May 2000 were analyzed retrospectively. The related parameters were compared between the conceived and non-conceived cycles.RESULTS:One hundred and seventy eight out of 252 thawed embryos were survival. Embryo transfers were undergone in 57 cycles (53 patients), resulting in 17 pregnancies (30% per transfer or 28% per thawed cycle). There were no significant difference between conceived and non-conceived cycles in terms of the controlled ovarian hyperstimulation protocols, numbers of developed follicle and oocyte retrieved, fertilization rate, numbers of frozen embryo and the luteinizing hormone, estradiol, progesterone levels on human chorionic gonadotropin triggering day. The percentage of good quality embryo before freezing and after thawing and survival thawed embryos were higher in conceived cycles than those in non-conceived cycles (83.8% Vs 60.7%, 76.8% Vs 50.0%, 82.4% Vs 66.3%, respectively P < 0.05).CONCLUSIONS:Good quality of frozen-thawed embryos and the trilaminar sonographic pattern of endometrium may be reliable predictors of success in pregnancy.

Abstract:OBJECTIVE:This study aimed to compare the clinical outcomes for transfer of Day 3 (D3) double cleavage-stage embryos and Day 5/6 (D5/6) single blastocysts in the frozen embryo transfer (FET) cycle to formulate a more appropriate embryo transplantation strategy.METHODS:We retrospectively analyzed 609 FET cycles from 518 women from April 2017 to March 2021. All FETs were assigned to the D3-DET group (transfer of a Day 3 double cleavage-stage embryo), D5-SBT group (transfer of a Day 5 single blastocyst), or D6-SBT group (transfer of a Day 6 single blastocyst). Clinical outcomes were comparatively analyzed.RESULTS:There were no significant differences in the biochemical pregnancy rate, clinical pregnancy rate, or ongoing pregnancy rate between the D3-DET and D5-SBT groups, but these rates in the two groups were all significantly higher compared with those in the D6-SBT group. The implantation rate in the D5-SBT group was significantly higher than that in the D3-DET group. The twin pregnancy rate in the D5-SBT and D6-SBT groups was significantly lower than that in the D3-DET group.CONCLUSION:This study suggests that D5-SBT is the preferred option for transplantation. D6-SBT reduces the pregnancy rate, making it a more cautious choice for transfer of such embryos.

Abstract:Objective To analyse factors influencing the outcome of frozen-thawed embryo transfer (FET)Method A retrospective analysis was performed in our center on 129 thawing cycles from March 2001 to April 2003. The related parameters were compared between conceived and non-conceived cycles.Results There were totally 129 clinical pregnancies in these transfers (pregnancy rate: 27.1％). Frozen-thawed embryos were transferred to natural cycles and CC cycling and hormone replacement treatment had equal success. Groups of IVF and ICSI did not differ significantly in pregnancy rates (P＞0. 05). The pregnancy rates for one, two, three and four pre-embryos transfer were 0, 20.0％,44.1％ and 75.0％,respectively (P＜0. 05). There were statistical differences between pregnancy group or non- pregnancy group in the endometrial thickness, CES, CES/No. Of embryo. A higher pregnancy rate was observed in embryo transfers which had at least one 4-cell grade I embryo (d 2)(P＜0.01). Conclusions The most important factors influencing the implantation rate and pregnancy rate of frozen-thawed embryo transfer are age, endometrium thickness, and the number, morphology and growth rate of transferred frozen embryos of women participants.

Abstract:BACKGROUND:Considerable debate exists which factors influence the outcomes of the vitrified frozen-thawed embryo transfer because lack of clinical applications.OBJECTIVE:To explore the factors influencing the outcomes of the vitrified frozen-thawed embryo transfer in assisted reproductive technology.METHODS:A retrospective statistical analysis was performed in Reproductive Medicine Center,Renmin Hospital of Wuhan University of 142 patients,154 thawing cycles.The patients were grouped according to the age of patients,embryonic development,fertilization methods,endometrial preparation programs,the endometrial thickness,the process of transplantation and the survival cell ratio of embryo recovery,the implantation rate and clinical pregnancy rate were compared between various groups.RESULTS AND CONCLUSION:Among the groups of the different age,fertilization methods,endometrial preparation programs,endometrial thickness and the process of embryo transfer embryo implantation rate and clinical pregnancy rate were no significant difference(P > 0.05);between the two groups of the second day fertilized embryos(D2)and the third day(D3)fertilized embryos,the clinical pregnancy rate was not significant(P > 0.05),but the embryo implantation rate of D3 group was significantly higher than D2 group.The survival cell ratio of embryo recovery has a significant effect on implantation rate and clinical pregnancy rate(P < 0.05).In the frozen-thawed embryo transfer cycles,embryo quality plays a major role in pregnancy rate,and preparation for appropriate endometrial thickness can improve the clinical pregnancy rate.
Prognostic Factors of Frozen Thawed Embryo Transfer: Analysis on 2433 Cycles

Abstract:Objective To investigate the factors influencing the prognosis of frozen thawed embryo transfer(FET)cycles.Methods Totally 2433 FET cycles were analyzed,955 of them were pregnant,while 626 were live birth,and 1478 non pregnant.They were assigned to natural cycle,ovulation induction cycle or hormone therapy(HT)cycle.Results Total clinical pregnancy rate was 35.2%(856/2433)and live birth rate was 25.7%(626/2433)in our study.There were 6203 embryos transferred to form 1026 gestational sac.The implantation rate was 16.5%(1026/6203).The pregnancy rate was 40.0%(758/1897),37.8%(70/185),and 36.2%(127/351)in natural cycle,ovulation induction cycle,and HT cycle,respectively(χ2=1.94,P=0.38),no significant differences were found in these cycles.Multiple pregnancy rates were 3.1%(1/32),14.7%(34/231)and 38.3%(227/593)when one,two,or three embryos were transferred,while the implantation rate and live birth rate were similar.Through Logistic regression analysis,we found that the numbers of high quality embryos transferred and patients' age played important roles in their clinical results,especially the number of high quality embryos transferred(OR=1.41,95% CI:1.11-1.79).It appeared a downtrend of pregnancy and live birth rate with the age.Conclusions The clinical outcomes of FET are not related to natural or ovulation induction,or HT cycles.The quality of embryos,especially the numbers of high quality embryos transferred,shows close correlation with FET outcomes.The pregnancy rate must be elevated and multiple conceive be reduced through transferring high quality embryos.

Abstract:Objective To observe the influential factors on the pregnancy outcome of frozen embryo transfer(FET) and to provide basis for improving clinical pregnancy rate.Methods Total of 124 FET cycles from January 2009 to January 2011 were analyzed retrospectively,which were divided into pregnant group and non-pregnant group according to pregnancy outcome.The single-factor analysis of relevant factors and logistic multivariate regression analysis were used.Results Factors related the FET pregnancy outcome were endometrial thickness,morphology,serum E2 and P levels,E2/P ratio at embryo transfer(ET) day and good quality of embryos and intact blastomere embryos after thaw;Logistic regression analysis showed that the pregnancy rate was correlated with the number of intact blastomeres embryos(OR=2.02),endometrial thickness(OR=1.55),morphology(OR=1.36),E2/P ratio(OR=0.71)(all P<0.05).Conclusion The number of intact blastomeres embryos has the closest relationship with the pregnancy outcome of FET,the other influential factors are endometrial thickness,morphology and serum E2/P ratio respectively.
