Percutaneous transhepatic bile drainage and percutaneous transhepatic cholangial stenting in treatment of malignant obstructive jaundice

Abstract:Objective: To evaluate the short-term and long-term clinical efficacy of percutaneous transhepatic bile drainage (PTBD) and PTBD+percutaneous transhepatic cholangial stent (PTCS) in the treatment of malignant obstructive jaundice. Methods: Sixty patients of malignant obstructive jaundice underwent interventional therapy, of which 30 underwent PTBD+PTCS (group A), and the other 30 underwent only PTBD (group B). The total bilirubin (TBIL), direct bilirubin (DBIL), complications, patency period and the survival time after intervention were analyzed. Results: One week after treatment, TBIL decreased by (174.81±49.13)μmol/L, (95.23±42.33)μmol/L in group A and B, respectively (P=0.02), while DBIL decreased by (96.01±36.51)μmol/L and (58.16±31.21)μmol/L, respectively (P=0.03). One month after treatment, TBIL decreased by (284.25±112.53)μmol/L and (276.42±125.47)μmol/L in group A and B, respectively (P=0.09), while DBIL decreased by (152.06±41.32)μmol/L and (147.06±43.83)μmol/L, respectively (P=0.13). The overall incidence rate of complications was 36.67% (11/30) in group A, significantly lower than that in group B (27/30, 90.00%), and the difference was statistically different (χ 2=18.37, P<0.05). The median patency period and the median survival time of two groups were 162.5 vs 78.5 days and 203.5 vs 126.0 days (both P<0.01), respectively. Conclusion: Compared with simple PTBD, PTBD with PTCS is more effective for the treatment of malignant obstructive jaundice, being able to improve the quality of life and prolong survival time of these patients.
