Abstract:Objective To investigate peri-operative treatment for HIV infected cases with surgical complications.Methods Thirty nine HIV-infected cases(37 males,2 females,aged 27-74 years) were enrolled.All the cases were detected for immune function prior to operation,standardized and careful surgical procedures were provided to reduce possible operational damage,coagulants(VIII factors) were infused continuously for haemophilia patients during the operation,antibiotics were conventionally applied to prevent infection and complications were promptly treated.Results Complete healing of surgical cuts was seen in 14 cases who underwent grade 1 incision;of the 10 cases who underwent grade 2 incision only 2 got infected in their surgical cuts;of the 15 cases who underwent grade 3 incision,3 had prolonged healing over 30 days.Except one case who died of severe pyemic,all were discharged with complete recovery or symptomatic improvement.Conclusions Appropriate peri-operative treatment may achieve better efficacy for patients with HIV infection.

Abstract:Objective To investigate the perioperative management plans for HIV/AIDS patients with surgical complications. Methods Totally 48 HIV-infected patients including 45 males and 3 females aged 20-74 years old were enrolled into this study. All the patients had pre-operative examination for the baseline immune function and went through standardized surgical procedures to minimize the tissue damage during the operation,followed by routine antibiotic treatment to prevent infection and prompt treatment of the complications. For the patients with hemophilia,a continuous infusion of coagulation factors Ⅷ during the operation was also applied. Results Twenty patients underwent clean (classⅠ) surgical procedures and all the surgical wounds healed in period 1. Thirteen patients underwent clean-contaminated (classⅡ) surgical procedures and 3 had surgical wound infection. Fifteen patients underwent contaminated (classⅢ) surgical procedures and surgical wound healing took more than 30 days in 3 patients. One pa-tient died from severe sepsis and the other patients were discharged from hospital with full recovery or significant improvements. Conclusion Appropriate perioperative management for HIV-infected patients may contribute to the achievement of better effect.
Analysis of Perioperative Antibacterial Usage in Surgical Oncology Patients

Abstract:OBJECTIVE To investigate the current application of perioperative antibacterial usage in surgical oncology patients so as to provide the information for rational clinical use.METHODS The questionnaire was designed,and the current medical records in surgical oncology were collected.Twenty-five pediatric medical records were selected and the data was analyzed in respect to the use of antibacterials and the rationality of use.RESULTS Among them,7 cases,17 cases and 1 case were type Ⅰ,Ⅱ and Ⅲ incision.The selection of antibacterial and the course of treatment in type Ⅰ incision were suitable according to guideline.But in type Ⅱ and Ⅲ incision,there are differences between current clinical practice and application of guidelines in China.There were defective in medical records.CONCLUSION It is very important to strengthen standardized administration of perioperative antibacterial usage in type Ⅱ and Ⅲ incision.It is also important to pay attention to the standard of medical record.

Abstract:Objective Probe the value and the operation risk for HIV/AIDS patients with surgical compli-cations. Methods Investigated 27 HIV-infected patients(male 26, female 1, age 27-74 years) were investi-gated. All patients had pre-operative detection of immune function, standardized surgical procedures, careful surgical process to reduce the operation damage, continuous infusion of coagulation factors for haemophilia during operation, conventional application of antibiotics to prevent infection and prompt treatment of compli-cations. Results No incision infected in 8 cases of class one incision, 1 incision infected in 6 cases of class two incision, 3 cases were not healed in 30 days after operation in 13 cases of class three incision. One case died of severe sepsis 12 days after operation, the others discharged with recovery or improvement. Abstract:The prevalence of human immunodeficiency virus (HIV) and acquired immune deficiency syndrome (AIDS) has emerged as a major public health concern in China. When patients with HIV infection undergo surgical treatment, there are two main challenges. Firstly, medical staff face a high risk of HIV infection due to occupational exposure. Secondly, the patient's immune function is impaired, increasing the risk of opportunistic infections and postoperative complications. The surgical treatment of such patients is unique, and the risk of occupational exposure during the operation primarily depends upon the viral load of HIV/AIDS patients. Therefore, perioperative antiretroviral therapy is of paramount importance in order to standardize the perioperative antiretroviral therapy (ART) for HIV/AIDS patients. The Surgery Group of the Chinese Association of STD and AIDS Prevention and Control, in collaboration with the Treatment Association, and Surgery Group of the Chinese Medical Association of Tropical Diseases and Parasitology, has developed an expert consensus on perioperative antiretroviral therapy for HIV/AIDS patients. This consensus encompasses various aspects, including surgical risk assessment, selection of perioperative antiretroviral therapy regimens, prevention of opportunistic infections, and the crucial focus on rapid preoperative viral load reduction and immune function reconstruction for HIV/AIDS patients.

Abstract:Background Surgical site infection (SSI) are the third most frequently reported nosocomial infection, and the most common on surgical wards. HIV-infected patients may increase the possibility of developing SSI after surgery. There are few reported date on incidence and the preventive measures of SSI in HIV-infected patients. This study was to determine the incidence and the associated risk factors for SSI in HIV-infected patients. And we also explored the preventive measures. Methods A retrospective study of SSI was conducted in 242 HIV-infected patients including 17 patients who combined with hemophilia from October 2008 to September 2011 in Shanghai Public Health Clinical Center. SSI were classified according to Centers for Disease Control and Prevention (CDC) criteria and identified by bedside surveillance and post-discharge follow-up. Data were analyzed using SPSS 16.0 statistical software (SPSS Inc., Chicago, IL). Results The SSI incidence rate was 47.5% (115 of 242); 38.4% incisional SSIs, 5.4% deep incisional SSIs and 3.7% organ/space SSIs. The SSI incidence rate was 37.9% in HIV-infected patients undergoing abdominal operation. Patients undergoing abdominal surgery with lower preoperative CD4 counts were more likely to develop SSIs. The incidence increased from 2.6% in clean wounds to 100% in dirty wounds. In the HIV-infected patients combined with hemophilia, the mean preoperative albumin and postoperative hemoglobin were found significantly lower than those in no-SSIs group (P<0.05). Conclusions SSI is frequent in HIV-infected patients. And suitable perioperative management may decrease the SSIs incidence rate of HIV-infected patients.

Abstract:BackgroundWe have updated the guideline for preventing and managing perioperative infection in China, given the global issues with antimicrobial resistance and the need to optimize antimicrobial usage and improve hospital infection control levels. MethodsWe conducted a comprehensive evaluation of the evidence for prevention and management of perioperative infection, based on the concepts of the Grading of Recommendations Assessment, Development and Evaluation (GRADE) system. The strength of recommendations was graded and voted using the Delphi method and the nominal group technique. Revisions were made to the guidelines in response to feedback from the experts. ResultsThere were 17 questions prepared, for which 37 recommendations were made. According to the GRADE system, we evaluated the body of evidence for each clinical question. Based on the meta-analysis results, recommendations were graded using the Delphi method to generate useful information. ConclusionsThis guideline provides evidence to perioperative antimicrobial prophylaxis that increased the rational use of prophylactic antimicrobial use, with substantial improvement in the risk-benefit trade-off.
Correlation between anesthesia and postoperative pulmonary infections

Abstract:OBJECTIVE To observe the impact of anesthesia-related factors on postoperative pulmonary infections so as to reduce the incidence of the postoperative infections .METHODS The clinical data of 685 patients who under-went general anesthesia in the surgery department from Jun 2013 to Jun 2014 were reviewed ,the medical records of the patients ,including the genders ,age ,types of department ,anesthesia approach ,and anesthesia time ,were statistically analyzed ;the related factors for the postoperative pulmonary infections were observed .RESULTS The postoperative pulmonary infections occurred in 76 of 685 patients ,with the infection rate of 11 .09% .The inci-dence of the postoperative pulmonary infections was significantly associated with the age ,types of department ,an-esthesia approach ,anesthesia time ,operation duration ,and underlying disease except for genders (P< 0 .05) .A total of 55 strains of pathogens have been isolated from the 76 patients with postoperative pulmonary infections , including 38 (69 .09% ) strains of gram-negative bacteria ,12 (21 .82% ) strains of gram-positive bacteria ,and 5 (9 .09% ) strains of fungi .CONCLUSION The incidence of the anesthesia-induced postoperative infections can be significantly reduced through the appropriate anesthesia approach ,use of disposable filters ,laryngoscope ,oxygen inhalation mask ,and breathing circuit as well as use of antibiotics during perioperative period .
The Risk Factors Analysis of Postoperative Infectious Complications for the Patients with HIV/AIDS

Abstract:Objective To investigate the risk factors of postoperative infectious complications and treatment measure for the patients with HIV/AIDS.Methods The clinical data of 97 HIV-infected patients admitted from Jan 2009 to Jun 2010 in Shanghai Public Health Clinical Center were analyzed retrospectively.Results All patients had peri-operative detection of immune function.The patients were divided into 3 groups according to pre-operative CD4 count:There were 33 cases of infectious complications and two cases died of sepsis in 51 cases of group A(CD4 counts200 cell /μL);There were 12 cases of infectious complications and there was no death in 31 cases of group B(200 cell/μL ≤CD4350 cell /μL);There were 5 cases of infectious complications in 15 cases of group C(CD4≥350 cell /μL).The infectious complications in group A was significantly higher than those in group B and group C(P0.05).Conclusion The incidence of postoperative infectious complications is significantly higher in the patients of HIV/AIDS with CD4 counts200 cell /μL.Intensive peri-operative treatment is very important.
Clinical observation of influence of different anesthesia on postoperative pulmonary infections in elderly patients

Abstract:OBJECTIVE To compare the effects of different anesthesia methods on postoperative pulmonary infec-tions in the elderly patients undergoing orthopedic surgery so as to choose appropriate anesthesia method for the elderly patients undergoing orthopedic surgery .METHODS Totally 192 elderly patients who underwent the ortho-pedic surgery in the first quarter of 2013 were observed ,then the anesthesia was performed according to the anes-thesia programs appointed by the preoperative consultation and the family members'willingness ,80 patients were treated with endotracheal intubation systemic anesthesia ,and 112 patients were given the epidural anesthesia . Within one week after the surgery ,the follow-up was conducted everyday to observe the clinical manifestations , including the body temperature ,cough ,sputum ,fever ,and pulmonary rales ,the blood routine examination was performed on the first and seventh day after the surgery ,and the chest X-rays were reexamined on the third and seventh day after the surgery ;the statistical analysis was performed with the use of SPSS 16 .0 software .RESULTS All the surgeries were completed successfully under the endotracheal intubation systemic anesthesia and the epidural anesthesia .Within one week after the surgeries ,the pulmonary infections occurred in one case in the systemic anesthesia group with the incidence of 1 .25% and in one case in the epidural anesthesia group with the incidence of 0 .89% ,and there was no significant difference in the incidence of postoperative pulmonary infections between the systemic anesthesia group and the epidural anesthesia group .CONCLUSION It is an effective way to adopt the modern anesthesia ,improve the level of management ,intensify the aseptic idea ,and reasonably use antibiotics so as to reduce the incidence of postoperative pulmonary infections in the elderly patients ,and the two anesthesia methods that are mentioned in the study should be taken into consideration for the single postoperative pulmonary infections .

Abstract:Objective To explore the the perioperativeperiod treatment measure and operative effect for haemophilia patients with HIV/AIDS.Methods 9 cases haemophilia infected with HIV(male,age 20-54 years old) were investigated.All patients had pre-operative detection of immune function,standardized surgical procedures,careful surgical process to reduce the operation damage,infusion of coagulation factor Ⅷ 2 000 U two hours before operation,2 000-4 000 U during operation,and post-operation infusion of coagulation factor Ⅷ(with decreasing daily dosage) were applied until operative incision healed.Other coagulation factors were also applied if liver function abnormal.Applied highly active anti-retroviral therapy(HAART),antibiotics were used to prevent infection and promptly treatment of complications.Results There were no abnormal bleeding in 8 patients during operation,damage control technique was used for turbulent bleeding in 1 patient with liver function abnormal.No incision infected in 5 cases of class one incision,2 incision infected in 4 cases of class three incision,no one died of operation.The patients were followed up 6-12 month after operation and all had good resumed.Conclusion Suitable perioperativeperiod treatment and excision of key pathological changes by operation may obtain better effect for haemophilia with HIV-infected patients.

Abstract:Understanding the composition of the microbial populations in the intestines of liver transplant patients is important to preventing postoperative infection. We investigated the relationship between the risk of postoperative infection and variation in the predominant fecal microbial composition during the perioperative period. We prospectively analyzed the predominant intestinal microbiome of five asymptomatic adult carriers of hepatitis B virus (as controls without any antibiotics) at four weekly follow-up visits and 12 patients before operation and at three weekly postoperative follow-up visits within the first month. Analysis was by denaturing gradient gel electrophoresis (DGGE) and sequencing with digital processing of DGGE profiles using BioNumerics software. Our results showed that the predominant intestinal microbial diversity decreased substantially in eight patients during the perioperative period. Among these, five patients experienced infection with a postoperative hospital stay of more than 30 days. The rest of the four patients who experienced shorter postoperative hospital stays showed only slight variation in predominant intestinal bacterial composition and temporal stability similar to asymptomatic controls. Postoperative fecal DGGE profiles showed mostly bands assigned to Bacteroides and Firmicutes. We conclude that an empiric prophylaxis strategy that destructs gut microecological balance will not be effective in reducing the risk of postoperative infection. Instead, the destruction of intestinal microbiota might result in the appearance of opportunistic pathogens such as Bifidobacterium dentium which rarely appears in the intestinal DGGE profiles of normal humans. Cognizance of the variation of intestinal microbial profiles during the perioperative period is a critical aspect of caring for liver transplant recipients.

Abstract:To investigate the management and prognosis of HIV infected patients with postoperative sepsis, the clinical data of 126 HIV-infected patients aged 20 to 74 years including 114 males and 12 females were retrospectively analyzed. Patients were divided into 3 groups according to the pre-operative level of CD4+ cells: Group A (n = 73), <200 cells/μl; Group B (n = 36): 200 cells/μl ≤ CD4 + cells < 350 cells/μl; Group C (n = 17): ≥350 cells/μl. The operation was carefully performed aiming to reduce the damage to tissues. Antibiotic prophylaxis, treatments with anti-tuberculous, antifugal and anti-retroviral drugs and timely treatment of complications were carried out. In Group A, B and C, postoperative sepsis occurred in 32, 8 and 3 patients, respectively, of whom 4, 1 and 0 died of multiple organ dysfunction syndrome (MODS), respectively. Analysis showed the proportion of patients with sepsis in Group A was significantly higher than that in Group B and Group C, but no significant difference was found between Group B and C. The HIV infected patients with CD4 + cells <200 cells/μl had significantly increased risk for postoperative sepsis. Appropriate peri-operative treatment for HIV-infected patients may achieve favorable outcome. Key words: Acquired immune deficiency syndrome, operative-risk evaluation,complication, sepsis.
Multiple measures to improve perioperative antimicrobial use in typeⅠin-cision operation

Abstract:Objective To evaluate the effect of multiple intervention measures on perioperative antimicrobial use in pa-tients undergoing typeⅠincision operation in a hospital,and provide basis for rational use of antimicrobial agents. Methods 9 823 patients with type I incision operation in April and October of 2005-2012 were surveyed retrospectively, data of 2005 was as baseline,from 2006 to 2012,multiple measures,including training,examination,supervision,feed-back,and cooperation of relevant departments were conducted,antimicrobial use before and after intervention was com-pared.Results The qualified rate of perioperative antimicrobial use in patients undergoing typeⅠ incision operation in-creased from 14.20% in 2006 to 92.30% in 2012;the rate of combined use of antimicrobial agents was relatively higher (7.00%-9.00%)in 2006-2009,had a downward trend in 2010- 2012,and decreased to 3.20% in 2012.Types of an-timicrobial agents for prophylactic use in typeⅠincision operation in 2006 and 2007 were similar to that of 2005,the main used antimicrobial agents were cephalosporins,penicillin and it’s compounds,and aminoglycosides;the major antimicrobial prophylaxis in 2008-2012 were the first and second generation cephalosporins,penicillin and it’s compounds.Multivariate non-conditional logistic regression analysis showed that age (40 - 59 years old),departments (orthopedics,general surgery,and ophthalmology),and years (2011 and 2012)were the main influencing factors for the qualified use of antimicrobial agents(all P<0.05).Conclusion Implementation of multiple intervention measures can improve the qualified rate of perioperative antimicrobial use in typeⅠ incision operation,reduce types of antimicrobial use and rate of combined antimicrobial use.

Abstract:OBJECTIVE To explore the clinical characteristics of postoperative nosocomial infections in the patients undergoing ear microsurgery in Hangzhou area and observe the antibiotics therapy so as to provide guidance for clinical diagnosis and treatment .METHODS A total of 2 864 patients (3 781 ears) who underwent the ear micro‐surgery were enrolled in the study ,then the clinical data of 79 patients with the postoperative nosocomial infec‐tions were retrospectively analyzed ,the infection sites and clinical characteristics were observed ,the body fluid specimens were collected from the infections sites for the culture of pathogens ,and the drug susceptibility testing was performed .RESULTS Of the 79 patients with infections ,51 (64 .56% ) had the surgical local infections ,21 (26 .58% ) had the pulmonary infections ,and 7 (8 .86% ) had the urinary system infections .Totally 92 strains of pathogens were isolated ,including 53 (57 .61% ) strains of gram‐positive bacteria ,38 (41 .30% ) strains of gram‐negative bacteria ,and 1 (1 .09% ) strain of fungus . Staphylococcus aureus and Enterococcus f aecalis were the predominant species of the gram‐positive bacteria and were highly susceptible to vancomycin and teicoplanin .En‐terobacter cloacae and Pseudomonas aeruginosa were dominant among the gram‐negative bacteria and were highly susceptible to imipenem ,ciprofloxacin ,cefepime ,and meropenem .CONCLUSION The pathogens causing the post‐operative nosocomial infections in the patients undergoing the ear microsurgery are diversified and vary significant‐ly in the drug susceptibility .The combination use of antibiotics can achieve good therapeutic effect in the early stage during which the species of the pathogens causing the infections are unknown .

Abstract:OBJECTIVE To find out perioperative nosocomial infection conditions in our hospital, study their risk factors and provide evidence for infection prevention and treatment.METHODS A total of 322 cases of perioperative nosocomial infections in our hospital in 2000 and 2001 were retrospectively investigated on operation department to analyze the susceptible factors and prognosis. RESULTS Total perioperative nosocomial infection rate was 1.7% and dead rate was 5.6% in 322 cases of 2000 and 2001;malignant tumor was one of the highest susceptible factors (33.0%); broad-spectrum antibiotics weighed 97.8% in preoperative antibiotics used; preoperative urethral catheter used weighed 88.2%; trachea-cathetered general anesthesia weighed 81.6%; more than 4h of operative duration weighed 42.1%; the lower respiratory tract infection was the most frequent infection site and weighed 38.%; staphylococcus and fungi were the most frequent pathogens and weighed 16.3% and 13.8%, respectively; drug-resistant pathogens weighed 66.4%; the infection rate (45.5%) and the dead rate (9.1%) of 22 cases of liver transplantation were higher than those of other operations. CONCLUSIONS Preoperative antibiotics used, preoperative urethral catheter used, trachea-cathetered general anesthesia, operative duration and nosocomial duration were the main risk factors of perioperative nosocomial infections.
Analysis of the Improvement of Perioperative Antibiotic Prophylaxis in Our Hospital

Abstract:OBJECTIVE To analysis the improvement of perioperative antibiotic prophylaxis in 3 kinds of surgical incisions after we strengthened the management of antibiotics in our hospital and provide reference for clinical rational use of antibiotics.METHODS Random selected 139 medical records from January to October in 2011 as the control group,and random selected 150 medical records from January to October in 2012 as the monitoring group.Compared the surgical site infection rates,antimicrobial utilization rate,the average number of medication days,the cost of antimicrobial drugs and the rationality of the antibiotics,etc.between the two groups in order to investigate if the monitoring group significantly improved.RESULTS There was no significant difference(p=1.00) in Surgical site infection rates between the two groups.The antimicrobial utilization rate(P <0.001),the average number of medication days(P=0.001),the average cost of hospitalization(P=0.006),the average drug costs(P<0.001),the average antibacterial drug costs(P<0.001),the average antibacterial drug cost / the average cost of hospitalization(P<0.001),the reasonable rate for the choice of the drug(P=0.003),the reasonable rate for the intraoperative medication time(P=0.007) in the monitoring group were significantly better than the control group in type 1 incision operation;in type 2 incision operation the average number of medication days(P=0.05),the average antibacterial drug costs(P=0.015),the average antibacterial drug costs / the average inpatient costs(P=0.03),the reasonable rate for the preoperative medication time(P<0.001),thereasonable rate for the postoperative medication(P=0.001) in the monitoring group were significantly better than the control group;and in type 3 incision operation only the average antibacterial drug costs / the average inpatient costs(P=0.046) and the reasonable rate for the intraoperative medication time(P=0.057) in the monitoring group were significantly better than the control group.COMCLUSION The Measures to strengthen the management of antimicrobial drugs in our hospital can improve the rational use of antimicrobial agents in type 1 incision operation.However,we need to further strengthen the management of preventive antibacterial drugs in type 2 and type 3 incision operation.
Preoperative Cd4 Count or Cd4/Cd8 Ratio As A Useful Indicator for Postoperative Sepsis in Hiv-Infected Patients Undergoing Abdominal Operations

Abstract:Background. CD4 count or CD4/CD8 ratio has been found to be a valuable marker of disease progression in HIV and AIDS. Our objective was to evaluate preoperative CD4 count or CD4/CD8 ratio as a useful indicator for postoperative sepsis in HIV-infected patients undergoing abdominal operations.Methods. Retrospective analysis of 35 HIV-infected patients (four females, 31 males) undergoing abdominal operations. All patients were divided into postoperative sepsis group (A) and non-sepsis group (B). Demographic and clinical information were entered into a database and included: type of surgical procedure, age, peripheral blood cells, plasma albumin, CD4 counts, and CD4/CD8 ratios. Furthermore, we stratified and compared the incidence of postoperative sepsis according to the preoperative CD4 counts with breakpoint value of 200 cells/mu L and preoperative CD4/CD8 ratios with breakpoint value of 0.15.Results. Overall postoperative sepsis morbidity in our study was 51.43% (18/35). In sepsis group, the mean preoperative and postoperative CD4 counts, CD4/CD8 ratios, and postoperative platelet count were found significantly lower, respectively, than those in non-sepsis group (P < 0.05). The incidence of postoperative sepsis in the patients with preoperative CD4 counts <= 200 cells/mu L was markedly higher than those with CD4 counts > 200 cells/mu L (83.3% versus 17.65%; P = 0.000). Likewise, the incidence of postoperative sepsis in the patients with preoperative CD4/CD8 ratios <= 0.15 was dramatically higher than those with CD4/CD8 ratios > 0.15 (90% versus 36%; P = 0.007).Conclusions. Our preliminary study showed that HIV-infected patients with preoperative CD4 count <= 200 cells/mu L or CD4/CD8 ratio <= 0.15 had overall higher postoperative sepsis morbidity. Preoperative CD4 count or CD4/CD8 ratio may be used as a useful indicator for postoperative sepsis in HIV-infected patients undergoing abdominal operations. (C) 2012 Elsevier Inc. Abstract:Background. To explore the efficacy of postoperative antifungal treatment for preventing the recurrence of pulmonary cryptococcosis (PC) and occurrence of cryptococcal meningitis (CM), a retrospective study was conducted in 112 hospitalized PC patients with or without antifungal treatment following surgery. Methods. The treatment failure rate, PC recurrence rate, and CM incidence were compared. Additionally, the effectiveness of postoperative antifungal therapy was assessed by gathering and analyzing the published literature. Results. The failure rate (P= .054) and recurrence rate (P= .178) were similar in the 2 groups, but the incidence of CM was lower in the group that received postoperative antifungal treatment (P = .039). Conclusions. 'Ibis study did not show any difference in the PC recurrence rate or failure rate in the different treatment duration groups. Thus, a shorter antifungal treatment course of 2 months may be an optional treatment. In addition, upon review of the literature, no case of CM occurrence was reported among the 169 cases given postoperative antifungal treatment.
