Abstract:Objective: To achieve an accurate assessment of orthodontic and restorative treatments, tooth segmentation of dental panoramic X-ray images is a critical preliminary step, however, dental panoramic X-ray images suffer from poorly defined interdental boundaries and low root-to-alveolar bone contrast, which pose significant challenges to tooth segmentation. In this article, we propose a multi-feature coordinate position learning-based tooth image segmentation method for tooth segmentation. Methods: For better analysis, the input image is randomly flipped horizontally and vertically to enhance the data. Our method extracts multi-scale tooth features from the designed residual omni-dimensional dynamic convolution and the designed two-stream coordinate attention module can further complement the tooth boundary features, and finally the two features are fused to enhance the local details of the features and global contextual information, which achieves the enrichment and optimization of the feature information. Results: The publicly available adult dental datasets Archive and Dataset and Code were used in the study. The experimental results were 87.96% and 92.04% for IoU, 97.79% and 97.32% for ACC, and 86.42% and 95.64% for Dice. Conclusion: The experimental results show that the proposed network can be used to assist doctors in quickly viewing tooth positions, and we also validate the effectiveness of the proposed two modules in fusing features.
Analysis of Dental Plaque by Using Cellular Neural Network-Based Image Segmentation

Abstract:OBJECTIVE To establish and evaluate a new method for measurement of dental plaque by using cellular neural network-based image segmentation. METHODS A total of 195 subjects were selected from community population. After dental plaque staining, oral digital picture of anterior teeth area was taken by an Olympus digital camera (C-7070 Wide Zoom). At the same time, the Turesky dental plaque indices of anterior teeth were evaluated. The image analysis was conducted by cellular neural network-based image segmentation. RESULTS The image cutting errors between two operators were very small. The Kappa value is 0.935. Pearson's correlation coefficient is 0.988 (P < 0.001). There was high correlative consistency between traditional dental plaque index and plaque percentage obtained by using image analysis. Pearson's correlation coefficient was 0.853 (P < 0.001). CONCLUSIONS Cellular neural network-based image segmentation is a new method feasible for evaluating dental plaque.

Abstract:Abstract Digital dentistry plays a pivotal role in dental healthcare. A critical step in many digital dental systems is to accurately delineate individual teeth and the gingiva in the three-dimension (3D) intraoral scanned (IOS) mesh data. However, previous state-of-the-art methods are either time-consuming or error-prone, hence hinder their clinical applicability. In this paper, we present an accurate, efficient, and fully-automated deep learning model, trained on a dataset of 4,000 IOS data annotated by experienced human experts. On a hold-out dataset of 200 scans, our model achieves a per-face accuracy, average-area accuracy and area under the receiver operating characteristic curve (AUC) of 96.94%, 98.26%, and 0.9991, respectively, significantly outperforming the state-of-the-art baseline. In addition, our model only takes about 24 seconds to generate segmentation outputs, as compared to over 5 minutes by the baseline and 15 minutes by human experts. A clinical performance test of 500 patients with malocclusion or/and abnormal teeth shows that 96.9% of the segmentations are satisfactory for clinical applications, 2.9% automatically trigger alarms for human improvement, and only 0.2% of them need rework. Our research demonstrates the potential for deep learning to improve the efficacy and efficiency of dental treatment and digital dentistry.
Robust Automated Teeth Identification from Dental Radiographs Using Deep Learning.

Abstract:OBJECTIVES:This study developed and validated a deep learning-based method to automatically segment and number teeth in panoramic radiographs across primary, mixed, and permanent dentitions.METHODS:A total of 6,046 panoramic radiographs were collected and annotated. The dataset encompassed primary, mixed and permanent dentitions and dental abnormalities such as tooth number anomalies, dental diseases, dental prostheses, and orthodontic appliances. A deep learning-based algorithm consisting of a U-Net-based region of interest extraction model, a Hybrid Task Cascade-based teeth segmentation and numbering model, and a post-processing procedure was trained on 4,232 images, validated on 605 images, and tested on 1,209 images. Precision, recall and Intersection-over-Union (IoU) were used to evaluate its performance.RESULTS:The deep learning-based teeth identification algorithm achieved good performance on panoramic radiographs, with precision and recall for teeth segmentation and numbering exceeding 97%, and the IoU between predictions and ground truths reaching 92%. It generalized well across all three dentition stages and complex real-world cases.CONCLUSIONS:By utilizing a two-stage training framework with a large-scale heterogeneous dataset, the automatic teeth identification algorithm achieved a performance level comparable to that of dental experts.CLINICAL SIGNIFICANCE:Deep learning can be leveraged to aid clinical interpretation of panoramic radiographs across primary, mixed, and permanent dentitions, even in the presence of real-world complexities. This robust teeth identification algorithm could contribute to the future development of more advanced, diagnosis- or treatment-oriented dental automation systems.

Abstract:The segmentation of dental models is a crucial step in computer-aided diagnosis and treatment systems for oral healthcare. To address the issues of poor universality and under-segmentation in tooth segmentation techniques, an intelligent tooth segmentation method combining multiple seed region growth and boundary extension is proposed. This method utilized the distribution characteristics of negative curvature meshes in teeth to obtain new seed points and effectively adapted to the structural differences between the top and sides of teeth through differential region growth. Additionally, the boundaries of the initial segmentation were extended based on geometric features, which was effectively compensated for under-segmentation issues in region growth. Ablation experiments and comparative experiments with current state-of-the-art algorithms demonstrated that the proposed method achieved better segmentation of crowded dental models and exhibited strong algorithm universality, thus possessing the capability to meet the practical segmentation needs in oral healthcare.

Abstract:Automatic tooth instance segmentation on 3D dental models is crucial for digitizing dental treatments and enabling computer-assisted treatment planning. However, It is challenging since the tight arrangement of dental structures and the consequential impact of dental ailments on their morphological characteristics. To address these challenges, we propose a novel method called THISNet. Unlike existing methods, THISNet focuses on highlighting tooth regions rather than relying on bounding box detection, leading to improved accuracy in tooth segmentation and labeling. By incorporating the highlighted tooth regions with a tooth object affinity module, our method effectively integrates global contextual information, considering the relationships between neighboring teeth and their surrounding structures. THISNet adopts an end-to-end learning approach, reducing complexity and enhancing segmentation efficiency compared to multi-stage training methods. Experimental results demonstrate the superiority of THISNet over existing approaches, highlighting its potential in various dental clinical applications.

Abstract:This paper presents a two-step method to automatically and accurately segment the dental crown components from CT images. Firstly, a multi-scale attention based U-Net model is proposed for pulp segmentation, which is embedded with global and local attention modules. The constructed attention modules can automatically aggregate pixel-wise contextual information and focus on catching the real dental pulp region. Secondly, two efficient level set models are proposed: one is the shape constraint-based level set model for enamel and dentin segmentation, the other is the region mutual exclusion-based level set model for neighboring teeth segmentation. The proposed shape constraint term can better handle topology changes of teeth and the region mutual exclusion term can more effectively avoid intersecting segmentation. Besides, a starting slice initialization method is introduced to achieve automatic segmentation, and an accurate contour propagation strategy is developed for slice-by-slice segmentation. We set up a series of comparative experiments for evaluation. Experimental results verify that the proposed method obtains promising performance for each crown component segmentation, and outperforms state-of-the-art tooth segmentation methods in terms of accuracy. This suggests that the proposed method can be used to accurately segment the crown components for precise tooth preparation treatment. Note to Practitioners—The motivation of this work is to reduce the burden on dentists during tooth preparation treatment, which requires accurate segmentation of crown components (i.e., enamel, dentin, and pulp) from dental CT images. Existing methods only focused on the segmentation of teeth or alveolar bone. Therefore, we present a novel automatic segmentation model for the dental crown components with high accuracy. A key strength of this study is the combination of a data-driven method (deep learning) and model-driven methods (level-set), which can provide good accuracy under limited training samples. This ability is highly desirable for practitioners by saving labor-intensive, costly labeling efforts. Furthermore, our proposed method will provide tools to help reduce subjectivity and human errors, as well as streamline and expedite the clinical workflow. This will significantly facilitate tooth preparation automation.

Deep learning approaches for tooth segmentation employ convolutional neural networks (CNNs) or Transformers to derive tooth feature maps from extensive training datasets. Tooth segmentation serves as a critical prerequisite for clinical dental analysis and surgical procedures, enabling dentists to comprehensively assess oral conditions and subsequently diagnose pathologies. Over the past decade, deep learning has experienced significant advancements, with researchers introducing efficient models such as U-Net, Mask R-CNN, and Segmentation Transformer (SETR). Building upon these frameworks, scholars have proposed numerous enhancement and optimization modules to attain superior tooth segmentation performance. This paper discusses the deep learning methods of tooth segmentation on dental panoramic radiographs (DPRs), cone-beam computed tomography (CBCT) images, intro oral scan (IOS) models, and others. Finally, we outline performance-enhancing techniques and suggest potential avenues for ongoing research. Numerous challenges remain, including data annotation and model generalization limitations. This paper offers insights for future tooth segmentation studies, potentially facilitating broader clinical adoption.

Abstract:As the most economical and routine auxiliary examination in the diagnosis of root canal treatment, oral X-ray has been widely used by stomatologists. It is still challenging to segment the tooth root with a blurry boundary for the traditional image segmentation method. To this end, we propose a model for high-resolution segmentation based on polynomial curve fitting with landmark detection (HS-PCL). It is based on detecting multiple landmarks evenly distributed on the edge of the tooth root to fit a smooth polynomial curve as the segmentation of the tooth root, thereby solving the problem of fuzzy edge. In our model, a maximum number of the shortest distances algorithm (MNSDA) is proposed to automatically reduce the negative influence of the wrong landmarks which are detected incorrectly and deviate from the tooth root on the fitting result. Our numerical experiments demonstrate that the proposed approach not only reduces Hausdorff95 (HD95) by 33.9% and Average Surface Distance (ASD) by 42.1% compared with the state-of-the-art method, but it also achieves excellent results on the minute quantity of datasets, which greatly improves the feasibility of automatic root canal therapy evaluation by medical image computing.

Abstract:The accurate detection of dental plaque at an early stage will definitely prevent periodontal diseases and dental caries. However, it remains difficult for the current dental examination to accurately recognize dental plaque without using medical dyeing reagent due to the low contrast between dental plaque and healthy teeth. To combat this problem, this paper proposes a novel network enhanced by a self-attention module for intelligent dental plaque segmentation. The key motivation is to directly utilize oral endoscope images (bypassing the need for dyeing reagent) and get accurate pixel-level dental plaque segmentation results. The algorithm needs to conduct self-attention at the super-pixel level and fuse the super-pixels' local-to-global features. Our newly-designed network architecture will afford the simultaneous fusion of multiple-scale complementary information guided by the powerful deep learning paradigm. The critical fused information includes the statistical distribution of the plaques color, the heat kernel signature (HKS) based local-to-global structure relationship, and the circle-LBP based local texture pattern in the nearby regions centering around the plaque area. To further refine the fuzed multiple-scale features, we devise an attention module based on CNN, which could focalize the regions of interest in plaque more easily, especially for many challenging cases. Extensive experiments and comprehensive evaluations confirm that, for a small-scale training dataset, our method could outperform the state-of-the-art methods. Meanwhile, the user studies verify the claim that our method is more accurate than conventional dental practice conducted by experienced dentists.

Abstract:Deep learning has become an active research topic in the field of medical image analysis. In particular, for the automatic segmentation of stomatological images, great advances have been made in segmentation performance. In this paper, we systematically reviewed the recent literature on segmentation methods for stomatological images based on deep learning, and their clinical applications. We categorized them into different tasks and analyze their advantages and disadvantages. The main categories that we explored were the data sources, backbone network, and task formulation. We categorized data sources into panoramic radiography, dental X-rays, cone-beam computed tomography, multi-slice spiral computed tomography, and methods based on intraoral scan images. For the backbone network, we distinguished methods based on convolutional neural networks from those based on transformers. We divided task formulations into semantic segmentation tasks and instance segmentation tasks. Toward the end of the paper, we discussed the challenges and provide several directions for further research on the automatic segmentation of stomatological images.

Abstract:The issue of missing teeth caused by dental microcracks has become a central topic in dental clinical medicine. Near-infrared technology combined with advanced image segmentation algorithms has become one of the leading technical routes for dental clinical medicine diagnosis. In this study, the NIR image segmentation method of dental microcracks was studied in depth to further utilize the advantages of near-infrared imaging technology in the detection technology of dental microcracks. The main contributions and research contents of this paper include (1) we designed a near-infrared imaging system for dental imaging, which provides solid data support for the study of advanced segmentation algorithms; (2) we designed a self-equilibrium segmentation algorithm for occult fracture of teeth based on U-Net++. The core module of this algorithm includes a multi-resolution residual encoder and a self-equilibrium composite loss function. The multi-scale residual encoder significantly improves the feature extraction capability of U-Net++. The self-equilibrium combined loss function ensures the consistency of the model’s attention to positive and negative samples, which enhances the robustness of U-Net++. (3) We have demonstrated through extensive experiments that our algorithm performs better than other dental occultation segmentation methods. This study is valuable for improving the diagnostic accuracy of dental occult fractures and promptly detecting the potential danger of the dental.

Abstract:Semantic segmentation on medical Computed Tomography (CT) images is of great significance to research and clinical diagnosis. However, methods based on neural network have competitive advantages for segmentation of dental CT images. In this paper, a 3D multi-feature fusion method for tooth segmentation is proposed. In order to obtain the body space of the data, first of all, the dental CT training set is compressed in NII format, and the body space data is processed; then the proposed 3D convolution network is used to train the data, extract the feature vectors, and obtain the probability distribution; to handle the situation that 3D neural network always leads to fuzzy boundary and unclear topology, the new CRF algorithm is used to refine the probability distribution which removes the redundant information generated by the neural network model, and makes the segmentation results more accurate. Compared with diverse contemporary segmentation algorithms, the effectiveness and superiority of our proposed method are verified, proving the conclusion that the supervision mechanism, neural network model components, and optimization proposed methods can improve the accuracy of tooth segmentation is reliable and valid.

Abstract:In this paper, we present a novel approach for 3D dental model segmentation via deep Convolutional Neural Networks (CNNs). Traditional geometry-based methods tend to receive undesirable results due to the complex appearance of human teeth (e.g., missing/rotten teeth, feature-less regions, crowding teeth, extra medical attachments, etc.). Furthermore, labeling of individual tooth is hardly enabled in traditional tooth segmentation methods. To address these issues, we propose to learn a generic and robust segmentation model by exploiting deep Neural Networks, namely NNs. The segmentation task is achieved by labeling each mesh face. We extract a set of geometry features as face feature representations. In the training step, the network is fed with those features, and produces a probability vector, of which each element indicates the probability a face belonging to the corresponding model part. To this end, we extensively experiment with various network structures, and eventually arrive at a 2-level hierarchical CNNs structure for tooth segmentation: one for teeth-gingiva labeling and the other for inter-teeth labeling. Further, we propose a novel boundary-aware tooth simplification method to significantly improve efficiency in the stage of feature extraction. After CNNs prediction, we do graph-based label optimization and further refine the boundary with an improved version of fuzzy clustering. The accuracy of our mesh labeling method exceeds that of the state-of-art geometry-based methods, reaching 99.06 percent measured by area which is directly applicable in orthodontic CAD systems. It is also robust to any possible foreign matters on model surface, e.g., air bubbles, dental accessories, and many more.

FDNet: Feature Decoupled Segmentation Network for Tooth CBCT Image

Abstract:Precise Tooth Cone Beam Computed Tomography (CBCT) image segmentation is crucial for orthodontic treatment planning. In this paper, we propose FDNet, a Feature Decoupled Segmentation Network, to excel in the face of the variable dental conditions encountered in CBCT scans, such as complex artifacts and indistinct tooth boundaries. The Low-Frequency Wavelet Transform (LF-Wavelet) is employed to enrich the semantic content by emphasizing the global structural integrity of the teeth, while the SAM encoder is leveraged to refine the boundary delineation, thus improving the contrast between adjacent dental structures. By integrating these dual aspects, FDNet adeptly addresses the semantic gap, providing a detailed and accurate segmentation. The framework's effectiveness is validated through rigorous benchmarks, achieving the top Dice and IoU scores of 85.28% and 75.23%, respectively. This innovative decoupling of semantic and boundary features capitalizes on the unique strengths of each element to elevate the quality of segmentation performance.

Abstract:Dental caries is a bacterial infectious disease that destroys the structure of teeth. It is one of the main diseases that endanger human health [R. H. Selwitz, A. I. Ismail, and N. B. Pitts, Lancet 369 (9555), 51–59 (2007)]. At present, dentists use both visual exams and radiographs for the detection of caries. Affected by the patient's dental health and the degree of caries demineralization, it is sometimes difficult to accurately identify some dental caries in x-ray images with the naked eye. Therefore, dentists need an intelligent and accurate dental caries recognition system to assist diagnosis, reduce the influence of doctors' subjective factors, and improve the efficiency of dental caries diagnosis. Therefore, this paper combines the U-Net model verified in the field of biomedical image segmentation with the convolution block attention module, designs an Attention U-Net model for caries image segmentation, and discusses the feasibility of deep learning technology in caries image recognition so as to prepare for the next clinical verification. After testing, the Dice similarity coefficient, mean pixel accuracy, mean intersection over union, and frequency-weighted intersection over the union of teeth segmentation with Attention U-Net are 95.30%, 94.46%, 93.10%, and 93.54%, respectively. The Dice similarity coefficient, mean pixel accuracy, mean intersection over union, and frequency-weighted intersection over the union of dental caries segmentation with Attention U-Net are 85.36%, 91.84%, 82.22%, and 97.08%, respectively. As a proof of concept study, this study was an initial evaluation of technology to assist dentists in the detection of caries. There is still more work needed before this can be used clinically.

Refined Individual Tooth Segmentation from Cone Beam CT Images

Abstract:In the field of dental imaging, precise segmentation of individual teeth from 3D Cone Beam Computed Tomography (CBCT) images remains a challenging task. This paper proposes a novel coarse-to-fine segmentation framework specifically optimized for individual tooth segmentation in CBCT imagery. At its core, in the coarse stage, the framework employs an embedding grouping network that ensures accurate localization and establishes a strong tooth-specific prior. To address the intricacies of tooth morphology, in the fine stage, our architecture designs a Tripartite Fusion Module and a Decouple Body and Edge Module. These modules work in concert to refine the segmentation process, thereby improving the granularity of individual tooth delineation. We have tested our framework on a real-world dataset, and the results demonstrate improvement over existing methods.

Abstract:The panoramic and lateral radiographs are commonly used in orthodontic dentistry to acquire patient-specific tooth morphology for diagnosing and treatment planning. Considering the variational dentition configurations and image blurs caused by device-specific artefacts and structure overlapping, the robust tooth segmentation and 3D reconstruction from radiographs remain a challenging issue. We propose a deformable exemplar-based conditional random fields (CRF) model for tooth segmentation and 3D shape estimation from the panoramic and lateral radiographs. The shared tooth foreground in the lateral and the panoramic radiographs are utilized for consistent labeling. The 3D deformable exemplars are introduced to provide a regularization of tooth contours to improve the tooth parsing in noisy and ambiguous radiographs. We introduce an alternating optimization scheme to solve the discrete superpixel labels and the continuous deformation of the 3D exemplars simultaneously. Extensive experiments on clinically obtained radiographs demonstrate that the proposed approach is effective and efficient for both tooth segmentation and 3D shape estimation from the panoramic and lateral radiographs.
