Abstract:Structured Abstract Objectives External root resorption (ERR) is a serious complication of orthodontic treatment. Aim of this study was to evaluate the effects of local osteoprotegerin (OPG) gene transfection on ERR during retention. Material and Methods Eighteen 6-week-old male Wistar rats were divided into three groups. All the rats were subjected to 2 weeks of orthodontic tooth movement followed by a 2-week retention period. During retention, the three groups of rats received local OPG gene transfection (OPG transfection group, n = 6), mock vector transfection (mock group, n = 6), or no injections (control group, n = 6). ERR of all three groups was evaluated with in vivo micro-CT analysis at three different time points: baseline, the last day of orthodontic tooth movement, and the last day of retention. Results In the OPG transfection group, there was no significant difference between ERR at the baseline and ERR on the last day of retention. By the last day of retention, the repair ratio of ERR in the OPG transfection group was statistically higher in relation to the repair ratio of the other groups (p < 0.001). Conclusion The results indicated that local OPG gene transfection significantly enhanced the repair of ERR during retention. Local OPG gene transfection might therefore be a useful tool for ERR repair during retention.
Role of OPG/RANKL/RANK system during alveolar bone resorption and remodeling

Abstract:Objective To investigate the potential role of a system containing osteoprotegerin(OPG),receptor activator of NF-κB ligand(RANKL),and receptor activator of nuclear factor-kappa B(RANK),also as,OPG/RANKL/RANK system in the resorption and remodeling mechanisms of alveolar bone.Methods Mouse RAW264.7 cell line was induced to osteoclasts in vitro,and mouse MC3T3-E1 cell line was treated with bone resorption supernatant.The control cells was cultured with regular medium.The expression levels of OPG,RANKL and RANK in the cells was detected by immunofluorescence assay.Thirty 8-week-old male SD rats were used to establish the alveolar bone resorption model with the injection of E-LPS.Immunohistochemical S-P method was used to investigate the expression of OPG,RANKL and RANK in the animal model.Results The expression of OPG in MC3T3-E1 cells was up-regulated after 7 days' treatment with bone resorption supernatant than in control cells(29.636±5.652 vs 15.568±1.229,P<0.01),while,the RANKL expression was down-regulated(6.806±1.738 vs 18.082±2.732,P<0.01).The ratio of OPG to RANKL was up-regulated too(P<0.05).The OPG level in periodontal tissues of rats was decreased compared with the tissue without resorption.But the expression of RANKL was increased.Conclusion OPG/RANKL/RANK system is involved in the bone resorption and remodeling.Osteoclast bone absorbent supernatants might affect the ratio of OPG to RANKL by regulating of the differentiation and activity of osteoblast-like cells,and thus regulate the dynamic balance of bone formation and resorption.

Abstract:Periodontitis thereby the alveolar bone loss induced by inflammation, is a wide-spread phenomenon around the world. It is an ongoing challenge faced by clinicians worldwide. This study aimed to identify the effects of lipopolysaccharide (LPS) on osteoclasts (OCs) differentiation in vitro and to investigate its molecular mechanism. For bone marrow derived macrophages (considered as Pro-OCs), LPS impaired their differentiation into OCs in a dose-dependent manner. In contrast, it promoted Pre-OCs (referred to receptor activator of nuclear factor-κB ligand (RANKL) pretreated Pro-OCs) and differentiated to OCs with increased maximum diameter, quantity, the covering area and the fusion index in vitro. It also facilitated OCs proliferation, bone resorption and OCs related genes expression. Furthermore, it was revealed that LPS enhanced OCs genesis from Pre-OCs via activating autophagy pathway consequently elevated the accumulation of TRAP, Cts K and NFATC1, specific genes of OCs. The members of Wnt signaling were expressed as at lower states during the LPS induced OCs formation, but they could be rescued in the presence of autophagy inhibitor. The most promising observation was the direct interaction of LC3B and Dvl2, indicating that the crosstalk between above pathways existed in OCs. Taken together, we consider that LPS activates autophagy which negatively regulates Wnt signaling via autophagic degradation of Dvl2 is significant for osteoclastogenesis from Pre-OCs in vitro. Our study sheds light on the fact that autophagy inhibitors will become a new, potentially applicable therapeutic option in the treatment of periodontal bone loss.
Impact of pharmacologic inhibition of tooth movement on periodontal and tooth root tissues during orthodontic force application

Abstract:Objective The goal of this study was to investigate potential negative sequelae of orthodontic force application +/- delivery of an osteoclast inhibitor, recombinant osteoprotegerin protein (OPG-Fc), on periodontal tissues. Setting and Sample Population Sprague Dawley rats from a commercial supplier were investigated in a laboratory setting. Materials and Methods Rats were randomly divided into four groups (n = 7 each): one group with no orthodontic appliances and injected once prior to the experimental period with empty polymer microspheres, one group with orthodontic appliances and injected once with empty microspheres, one group with orthodontic appliances and injected once with polymer microspheres containing 1 mg/kg of OPG-Fc, and one group with orthodontic appliances and injected with non-encapsulated 5 mg/kg of OPG-Fc every 3 days during the experimental period. The animals were euthanized after 28 days of tooth movement for histomorphometric analyses. Results Root resorption, PDL area and widths were similar in animals without appliances and animals with appliances plus high-dose OPG-Fc. PDL blood vessels were compressed and decreased in number in all animals that received orthodontic appliances, regardless of OPG-Fc. Hyalinization was significantly increased only in animals with orthodontic appliances plus multiple injections of 5 mg/kg non-encapsulated OPG-Fc when compared to animals without appliances. Conclusions Results of this study indicate that while pharmacological modulation of tooth movement through osteoclast inhibition is feasible when delivered in a locally controlled low-dose manner, high-dose levels that completely prevent tooth movement through bone may decrease local blood flow and increase the incidence of hyalinization.
Small nucleolar RNA host gene 5 plays a role in orthodontic tooth movement by inhibiting osteoclast differentiation

Abstract:Background and Objectives The alveolar bone remodelling promoted by reasonable mechanical force triggers orthodontic tooth movement (OTM). The generation of osteoclasts is essential in this process. However, the mechanism of mechanical force mediating osteoclast differentiation remains elusive. Small nucleolar RNA host gene 5 (SNHG5), which was reported to mediate the osteogenic differentiation of bone marrow mesenchymal stem cells in our previous study, was downregulated in human periodontal ligament cells (hPDLCs) under mechanical force. At the same time, the RANKL/OPG ratio increased. Based on this, we probed into the role of SNHG5 in osteoclast formation during OTM and the relevant mechanism. Materials and Methods SNHG5 and the RANKL/OPG ratio under different compressive forces were detected by western blotting (WB) and qRT‐PCR. Impact of overexpression or knockdown of SNHG5 on osteoclast differentiation was detected by qRT‐PCR, WB and transwell experiments. The combination of SNHG5 and C/EBPβ was verified by RNA immunoprecipitation and RNA pull‐down assays. The expression of SNHG5 and osteoclast markers in gingiva were analysed by qRT‐PCR and the paraffin sections of periodontal tissues were used for histological analysis. Results Compressive force downregulated SNHG5 and upregulated the RANKL/OPG ratio in hPDLCs. Overexpression of SNHG5 inhibited RANKL's expression and osteoclast differentiation. SNHG5 combined with C/EBPβ, a regulator of osteoclast. The expression of SNHG5 in periodontal tissue decreased during OTM. Conclusion SNHG5 inhibited osteoclast differentiation during OTM, achieved by affecting RANKL secretion, which may provide a new idea to interfere with bone resorption during orthodontic treatment.

Abstract:Background:Because orthodontic tooth movement is dependent upon osteoclast-mediated resorption of alveolar bone adjacent to the pressure side of tooth roots, biologic mediators that regulate osteoclasts can be utilized to control tooth movement.Objectives:To develop a novel method to locally enhance orthodontic anchorage.Methods:We encapsulated osteoprotegerin (OPG) in polymer microspheres and tested the effectiveness of microsphere encapsulated versus non-encapsulated OPG for enhancing orthodontic anchorage in a rodent model of tooth movement. A single injection of 1 mg/kg non-encapsulated or microsphere encapsulated OPG was delivered into the palatal mucosa mesial to the first maxillary molar 1 day prior to tooth movement. A positive control group received injections of 5 mg/kg non-encapsulated OPG every 3 days during tooth movement. After 28 days of tooth movement, hemi-maxillae and femurs were dissected. Molar mesial and incisor distal tooth movement was measured using stone casts that were scanned and magnified. Local alveolar, distant femur bone, and tooth root volumes were analyzed by micro computed tomography. Serum OPG levels were measured by ELISA. Osteoclast numbers were quantified by histomorphometry.Results:The single injection of microsphere encapsulated OPG significantly enhanced orthodontic anchorage, while the single injection of non-encapsulated OPG did not. Injection of encapsulated OPG inhibited molar mesial movement but did not inhibit incisor tooth movement, and did not alter alveolar or femur bone volume fraction, density, or mineral content. Multiple injections of 5 mg/kg non-encapsulated OPG enhanced orthodontic anchorage, but also inhibited incisor retraction and altered alveolar and femur bone quality parameters. Increased OPG levels were found only in animals receiving multiple injections of non-encapsulated 5 mg/kg OPG. Osteoclast numbers were higher upon tooth movement in animals that did not receive OPG. Osteoclast numbers in OPG injected animals were variable within groups.Conclusions:Microsphere encapsulation of OPG allows for controlled drug release, and enhances site-specific orthodontic anchorage without systemic side effects. With additional refinements, this drug delivery system could be applicable to a broad array of potential biologic orthodontic therapeutics.
Expression of osteoprotegerin/receptor activator of nuclear factor kappa B ligand in the periodontal tissue incident to incisors intrusion on dog

Abstract:BACKGROUND: Osteoprotegerin (OPG) and receptor activator of nuclear factor-κB ligand (RANKL) play important role in osteoclast formation and activation. The expression of OPG/RANKL in the periodontal tissues during orthodontic tooth movement is to be studied. OBJECTIVE: To observe the expression of OPG/RANKL in the periodontal tissue incident to incisors intrusion on dogs. METHODS: Nine healthy hybrid dogs were selected and randomly divided into 5 groups. The control group contained 1 dog with no force loaded; Applied force for 1 week, 2 weeks, 4 weeks and 12 weeks (4 weeks after activation, then retention for 8 weeks) were the four experimental groups, which were contained 2 dogs respectively. The dog of the control group was killed immediately. Dogs of other groups were killed after the force was applied 1 week, 2 weeks, 4 weeks and 12 weeks. Then the gums and alveolar bone tissues were completely cut to make the specimens. An immunohistochemical staining technique was utilized to detect the expression of OPG/RANKL. The average optical density images were semi-quantitative analyzed by Image-Proplus software to reflect the expression levels of RANKL and OPG in the periodontal tissues in each group. RESULTS AND CONCLUSIONS: The expression of RANKL in the no force loaded group (the control group) in the periodontal ligament tissues showed weak positive. The expression of RANKL was the most significant after applying force for 1 week. The expression still showed positive, but became weak after applying force for 2 weeks. The expression was decreased significantly after applying force for 4 weeks. The expression was close to the control group after activation 4 weeks then retention for 8 weeks. The average optical density level was measured by image analysis and demonstrated that it reached peak in the 1 week (P < 0.05), then declined gradually. There were significant differences compared with the control group (P < 0.05). Expression of OPG in the no force loaded group (the control group) in the periodontal ligament tissues showed weak positive. The expression of OPG was increased significantly after applying force for 1 week. And in 2 weeks, the expression reached peak. Then the expression was decreased after applying force for 4 weeks. The positive expression regained to the level of the control group after activation 4 weeks then retention for 8 weeks. The average optical density value reached peak in the 2 weeks and it was significant stronger than the control group (P < 0.05). The results confirm that the expression change characters of RANKL and OPG are the same as the reconstruction of alveolar bone. RANKL and OPG participate in the reconstruction of periodontal tissues.
Osteoprotegerin expression changes in the periodontal tissue following titanium-alloy miniscrew as anchorage for the maxillary incisors intrusion

Abstract:Background: Osteoprotegerin (OPG) inhibits osteoclast (OC) formation and differentiation and increases bone density. It is a key factor for inhibiting bone resorption. However, the precise mechanism of OPG remains unclear following mini-implant anchorage for the maxillary incisors intrusion. Objective: To observe the expression of OPG in the periodontal tissue following the mini-implant anchorage for the maxillary incisors intrusion. Methods: Nine dogs were selected and randomly divided into 5 groups: I group was control group, including 1 dog with no force given; II, III, IV, V groups contained 2 dogs respectively. Mini-implants were inserted into the labial alveolar bone between the roots of maxillary the second and the third incisors on both sides; 100 g force was imposed on the first and the second incisors on both sides. At the end of 1, 2, 4, 12 weeks (activation for 4 weeks, then retention for 8 weeks after force removal), the animals were sacrificed, the first and second incisor teeth on the one side were randomly selected and these teeth with gums, alveolar bone tissue were completely cut. Then expression of OPG in periodontal ligament (PDL) was inspected by using immunohistochemistry. Results and Conclusion: Expression of OPG in the alveolar bone and periodontal ligament tissues increased at 1 week after force given, reached maximum at 2 weeks after activation, and then dropped to normal level at 4 weeks after activation. After 12 weeks (activation for 4 weeks, then retention for 8 weeks after force removal), it increased insignificantly, which did not differ from that at 4 weeks after activation and the control group. OPG participated in the reconstruction of periodontal tissues and dynamically changed along with time following mini-implant anchorage for the maxillary incisors intrusion.

Abstract:Objective: To test the null hypothesis that increased tooth displacement in ovariectomized rats is not related to differential expressions of OPG and RANKL in the periodontium. Materials and Methods: Eighty-four 12-week female rats were used; half were ovariectomized and half were not. Three months later, the maxillary first molar was moved mesially. Groups of rats were sacrificed at days 0, 1, 3, 5, 7, 10, and 14 after activation. Tooth movement was measured at each time point. OPG and RANKL expressions were examined through immunohistochemistry. Results: Ovariectomized and nonovariectomized rats showed three-phase tooth movement. In both groups, OPG expression increased at the tension area and RANKL increased at the pressure area. The OPG/RANKL ratio coincided with tooth movement, especially in the linear phase from 7 to 14 days. Conclusions: The null hypothesis is rejected. The increased rate of tooth movement in ovariectomized rats was related to differential expressions of OPG and RANKL. (Angle Orthod. 2009; 79:292-298.)
Osteoprotegerin Influences The Bone Resorption Activity Of Osteoclasts

Abstract:The aim of the present study was to determine whether osteoprotegerin (OPG) influences the bone resorption activity of osteoclasts. RAW264.7 cells were induced by macrophage colony-stimulating factor (M-CSF) + receptor activator of nuclear factor-kappa B ligand (RANKL) and 0, 10, 20, 50 and 100 ng/ml OPG were added into various groups in the presence of the two cytokines. The OPG treatment was continued for 24 h. Osteoclast differentiation and activation were estimated via TRAP staining assay, TRITC-conjugated phalloidin staining, resorption activity analysis. Furthermore, the expression levels of the osteoclastic bone resorption-related genes MMP-9, cathepsin K and carbonic anhydrase II (CA II) were examined using real-time polymerase chain reaction (PCR). The data demonstrated that high concentrations of OPG could inhibit the differentiation and activation of osteoclasts. Furthermore, real-time PCR analysis illustrated that OPG decreased the expression of MMP-9 and cathepsin K in different concentrations of OPG and it decreased the expression of CA II genes at 10 and 20 ng/ml concentrations of OPG. For the time gradient study, OPG decreased the expression of MMP-9 and CA II genes but not that of the cathepsin K gene. In summary, the resorption activity of osteoclasts was suppressed by high concentrations of OPG and, at the molecular level, OPG decreased the expression of osteoclastic bone resorption-related genes.

Abstract:Osteoprotegerin (OPG) inhibits the ability of receptor activator of nuclear factor κB (NF-κB) ligand (RANKL) to stimulate the differentiation, activity, and survival of bone-resorbing osteoclasts. Genetic studies in mice show that osteocytes are an important source of RANKL, but the cellular sources of OPG are unclear. We use conditional deletion of <em>Tnfrsf11b</em>, which encodes OPG, from different cell populations to identify functionally relevant sources of OPG in mice. Deletion from B lymphocytes and osteocytes, two cell types commonly thought to supply OPG, has little or no impact on bone mass. By contrast, deletion of <em>Tnfrsf11b</em> from osteoblasts increases bone resorption and reduces bone mass to an extent similar to germline deletion, demonstrating that osteoblasts are an essential source of OPG. These results suggest that, in addition to producing new bone matrix, osteoblasts also play an active role in terminating the resorption phase of the bone remodeling cycle by suppressing RANKL activity.

Abstract:In this review, most of the known and postulated mechanisms of osteopontin (OPN) and its role in bone remodeling and orthodontic tooth movement are discussed based on available literature. OPN, a multifunctional protein, is considered crucial for bone remodeling, biomineralization, and periodontal remodeling during mechanical tension and stress (orthodontic tooth movement). It contributes to bone remodeling by promoting osteoclastogenesis and osteoclast activity through CD44- and αvβ3-mediated cell signaling. Further, it has a definitive role in bone remodeling by the formation of podosomes, osteoclast survival, and osteoclast motility. OPN has been shown to have a regulatory effect on hydroxyapatite crystal (HAP) growth and potently inhibits the mineralization of osteoblast cultures in a phosphate-dependent manner. Bone remodeling is vital for orthodontic tooth movement. Significant compressive and tensional forces on the periodontium induce the signaling pathways mediated by various osteogenic genes including OPN, bone sialoprotein, Osterix, and osteocalcin. The signaling pathways involved in the regulation of OPN and its effect on the periodontal tissues during orthodontic tooth movement are further discussed in this review. A limited number of studies have suggested the use of OPN as a biomarker to assess orthodontic treatment. Furthermore, the association of single nucleotide polymorphisms (SNPs) in OPN coding gene Spp1 with orthodontically induced root resorption remains largely unexplored. Accordingly, future research directions for OPN are outlined in this review.

Integrated bioinformatic analysis of protein landscape in gingival crevicular fluid unveils sequential bioprocess in orthodontic tooth movement

Abstract:Background: The biological mechanisms driving orthodontic tooth movement (OTM) remain incompletely understood. Gingival crevicular fluid (GCF) is an important indicator of the periodontal bioprocess, providing valuable cues for probing the molecular mechanisms of OTM. Methods: A rigorous review of the clinical studies over the past decade was conducted after registering the protocol with PROSPERO and adhering to inclusion criteria comprising human subjects, specified force magnitudes and force application modes. The thorough screening investigated differentially expressed proteins (DEPs) in GCF associated with OTM. Protein-protein interaction (PPI) analysis was carried out using the STRING database, followed by further refinement through Cytoscape to isolate top hub proteins. Results: A comprehensive summarization of the OTM-related GCF studies was conducted, followed by an in-depth exploration of biomarkers within the GCF. We identified 13 DEPs, including ALP, IL-1β, IL-6, Leptin, MMP-1, MMP-3, MMP-8, MMP-9, PGE2, TGF-β1, TNF-α, OPG, RANKL. Bioinformatic analysis spotlighted the top 10 hub proteins and their interactions involved in OTM. Based on these findings, we have proposed a hypothetic diagram for the time-course bioprocess in OTM, which involves three phases containing sequential cellular and molecular components and their interplay network. Conclusions: This work has further improved our understanding to the bioprocess of OTM, suggesting biomarkers as potential modulating targets to enhance OTM, mitigate adverse effects and support real-time monitoring and personalized orthodontic cycles.

Abstract:OBJECTIVE:To investigate the level of osteoprotegerin (OPG) and receptor activator of nuclear factor kappa B ligand (RANKL) in gingival crevicular fluid (GCF) during orthodontic retention and corresponding expression of OPG and RANKL in periodontal tissues. METHODS:Fifteen male Wistar rats (age, 6 weeks) were divided into three groups with 5 rats for each. GCF samples were collected at the baseline, 14 days after orthodontic force application, and 14 days after orthodontic force removal. enzyme linked immunosorbent assay (ELISA) was used to quantify the concentration of OPG and sRANKL (soluble receptor activator of nuclear factor kappa B ligand) in GCF. Immunohistochemical staining was performed to identify the OPG and RANKL expression in periodontal tissues. RESULTS:The concentration of sRANKL in GCF increased statistically significant from baseline to T2 (P<0.05) while decreased significantly from T2 to T3 (P<0.05). The concentration of OPG had minimum fluctuation from baseline to T3 without any statistical significance (P>0.05). The sRANKL/OPG ratio in GCF and that in periodontal tissues during the three time points were similar which showed a prominent increase in T2 (P<0.05 and P<0.01, respectively) and sharp decrease in T3 (P<0.05). CONCLUSION:sRANKL/OPG ratio may be one of the predictors reflecting the remodeling of periodontal tissues in orthodontic retention.
PTH/PTHrP in controlled release hydrogel enhances orthodontic tooth movement by regulating periodontal bone remodaling

Abstract:Objective: This study aimed to evaluate the effects of local application of parathyroid hormone (PTH) or parathyroid hormone-related protein (PTHrP) on osteogenesis and osteoclastogenesis during orthodontic tooth movement (OTM). Background: Periodontal bone remodeling is the crucial biological process in the OTM that involves both bone resorption and formation, with the former more important as the initiator. PTH or PTHrP both play dual roles in bone remodeling regulation, and the balance may shift to the bone resorption side when they are given continuously, suggesting them as potential candidate medicine for OTM acceleration. Methods: A total of 40 rats underwent orthodontic mesialization of the maxillary first molars and received no micro-perforation (MOP), or MOP followed by injection of temperature-sensitive hydrogel containing PTH, PTHrP, or normal saline. The rats were sacrificed after 2-week OTM, except for the relapse groups, which had one more week of observation after removal of the force appliances. The amount of tooth movement, rate of relapse after OTM, and effects on the bone remodeling were assessed through micro-computed tomography (μCT) analysis, alkaline phosphatase (ALP) assay, alizarin red staining, tartrate-resistant acid phosphatase (TRAP) staining, immunohistochemistry (IHC) analysis, Western blot (WB), and quantitative real-time polymerase chain reaction (qRT-PCR). The effects of PTHrP on the osteogenic differentiation of human periodontal ligament cells (hPDLCs) were explored in vitro. Results: The cumulative release of PTH or PTHrP from PECE hydrogels was beyond 75% at 14 days in a sustained manner. After the intervention in vivo, the distance of OTM in the PTH (0.78 ± 0.06 mm) or PTHrP (0.81 ± 0.04 mm) group was significantly larger than that of the MOP only (0.51 ± 0.04 mm) or the no MOP (0.46 ± 0.05 mm) group. Moreover, PTH injection significantly reduced the rate of relapse after OTM (25.7 ± 4.3%) compared to the control (69.6 ± 6.1%). μCT analysis showed decreased BV/TV, BS/BV, and Tb.N, while increased Tb.Sp of alveolar bone in the PTH or PTHrP group. There were also more TRAP-positive osteoclasts in the PTH or PTHrP group with a significantly enhanced ratio of receptor activator of nuclear factor-κB ligand (RANKL)/osteoprotegerin (OPG). The protein expressions of PTH/PTHrP type 1 receptor (PTHR1), alkaline phosphatase (ALP), osteocalcin (OCN), runt-related transcription factor 2 (RUNX2), and β-catenin were significantly increased in the PTH or PTHrP group, as well as the gene expressions of Pth1r, Bglap, and Alpl. There was no significant difference between the effects of PTH and PTHrP. Nevertheless, inhibition of PTHrP on the osteogenic differentiation of hPDLCs was detected in vitro with decreased expression of OCN, RUNX2, COL-1, and ALP. Conclusion: Local injection of either PTH or PTHrP carried by controlled release PECE hydrogel similarly enhances OTM in rats through regulating periodontal bone remodeling, which deserves further study for potential clinical application.

Abstract:Orthodontic tooth movement (OTM) is achieved by the simultaneous activation of bone resorption by osteoclasts and bone formation by osteoblasts. When orthodontic forces are applied, osteoclast-mediated bone resorption occurs in the alveolar bone on the compression side, creating space for tooth movement. Therefore, controlling osteoclastogenesis is the fundamental tenet of orthodontic treatment. Orthodontic forces are sensed by osteoblast lineage cells such as periodontal ligament (PDL) cells and osteocytes. Of several cytokines produced by these cells, the most important cytokine promoting osteoclastogenesis is the receptor activator of nuclear factor-κB ligand (RANKL), which is mainly supplied by osteoblasts. Additionally, osteocytes embedded within the bone matrix, T lymphocytes in inflammatory conditions, and PDL cells produce RANKL. Besides RANKL, inflammatory cytokines, such as interleukin-1, tumor necrosis factor-α, and prostaglandin E2 promote osteoclastogenesis under OTM. On the downside, excessive osteoclastogenesis activation triggers orthodontically-induced external root resorption (ERR) through pro-osteoclastic inflammatory cytokines. Therefore, understanding the mechanisms of osteoclastogenesis during OTM is essential in reducing the adverse effects of orthodontic treatment. Here, we review the current concepts of the mechanisms underlying osteoclastogenesis in OTM and orthodontically induced ERR.
The effects of luteolin on orthodontic tooth movement and relapse

Abstract:Introduction Luteolin is a natural flavonoid compound that widely exists in human food. Studies have demonstrated luteolin has powerful anti-inflammatory properties and can affect bone remodeling in an inflammatory environment. This study aimed to investigate the effect of luteolin on orthodontic tooth movement (OTM) and relapse after OTM. Methods Male Sprague Dawley rats were randomly divided into 3 groups (n = 8): OTM, 50 mg/kg/d luteolin, and 100 mg/kg/d luteolin. Then, 50 g of orthodontic force was applied to all animals. A saline solution or corresponding concentration of luteolin was given orally. For the OTM experiment, after 14 days of force application, rats were killed, the maxilla was dissected, and then microcomputed tomography, histologic staining, and western blotting were performed. For the relapse experiment, the spring was removed, and a silicone impression was made to record the relapse status. Results Compared with the OTM alone group, systemic administration of luteolin inhibited OTM and tooth relapse ( P <0.05). Increased bone volume, reduced osteoclast activity, and a decrease in osteoclastogenesis-related protein expression were observed in luteolin-treated groups. These effects may be attributed to the inhibition of the nuclear factor-kappa B pathway. Conclusions Luteolin can significantly inhibit OTM and relapse after OTM. Thus, luteolin is a prospective candidate for enhancing tooth anchorage and preventing relapse in orthodontic treatment.

Impact of occlusal contact pattern on dental stability and oromandibular system after orthodontic tooth movement in rats

Abstract:How to ensure dental stability in new positions and reduce the likelihood of relapse is a major clinical concern in the orthodontic field. Occlusal contacts between arches may affect the transmission of masticatory forces, thereby influencing the biological response of the periodontal and the oromandibular system. Occlusion factors that may influence the stability after orthodontic tooth movement (OTM) remain largely unknown. Hence, this research was conducted in order to investigate the influence of different occlusal contact patterns on tooth stability and oromandibular system including the masseter muscle and the temporomandibular joint following OTM. By modifying the occlusal surfaces, in vivo animal study models with distinct occlusal patterns corresponding to clinical circumstances were established. The relapse distance of teeth and the level of inflammatory factors in the gingival cervical fluid were analyzed. We also closely observed the histological remodeling of periodontal tissue, masseter tissue, and joint tissue after one week of relapse. Moreover, genes expression in the alveolar bone was analyzed to illustrate the potential biological mechanisms of relapse under the influence of different occlusal contact patterns following OTM. Different occlusal contact patterns after OTM in rats were established. The intercuspation contact between cusp and fossa group exhibited the lowest level of relapse movement, inflammatory factors and osteoclast activity ( P < 0.05). On the other hand, groups with interferences or inadequate contacts exhibited more relapse movement, and tend to promote inflammation of periodontal tissue and activate bone resorption ( P < 0.05). Adequate occlusal contacts without interference may enhance tooth stability and reduce the likelihood of relapse. After active orthodontic treatment, necessary occlusal adjustment should be made to achieve the desired intercuspation contact relationship and ensure adequate contact between the arches. The elimination of occlusal interferences is crucial to achieving optimal stability and promoting overall healthy condition of the oromandibular system.

Abstract:The enhanced osteoclastogenesis contributes to alveolar bone resorption in periodontitis, which increases the risk of tooth loss. To reduce bone destruction, the inhibition of osteoclast development is proposed as a feasible treatment. CD40L-CD40-TRAF6 signal transduction plays a crucial role in inflammation, but how it regulates osteoclast activity in periodontitis has not been elucidated. In this study, we showed the potential role of CD40L-CD40-TRAF6 signaling in periodontitis. CD40L obviously promoted osteoclast formation and bone resorption capacity in vitro . Mechanistically, we found that osteoclastogenesis was enhanced by the overexpression of NFATc1 and NF-κB activation. Importantly, osteoclast activity was effectively suppressed by TRAF-STOP, a small molecular inhibitor of TRAF6. Furthermore, local injection of TRAF-STOP-loaded injectable PLGA-PEG-PLGA hydrogel could alleviate ligation-induced periodontitis in vivo . Taken together, TRAF-STOP shows promising clinical efficacy in periodontitis through alleviating osteoclastogenesis.
