Unmasking Elevated Tryptase In An Infant: Is It Systemic Mastocytosis?

Abstract:Introduction Hereditary alpha-tryptasemia (HaT) is a highly penetrant autosomal dominant genetic trait defined by extra TPSAB1 gene copies on chromosome 16 and elevated baseline tryptase level (BST). Our case illustrates diagnostic and therapeutic challenges. Case Description A 7-month-old full term female presented with elevated tryptase. She has episodic flushing, hives, vomiting, non-bloody diarrhea with poor weight gain despite diet modification. Exams revealed no cutaneous mastocytosis. Her medications included cetirizine, famotidine, and esomeprazole. Previous workup revealed elevated BSTs (17.3 ug/L and 16.5 ug/L) with negative food IgE. Epi pen was prescribed. Family history includes atopy in both parents. Evaluation for systemic mastocytosis showed normal CBC, CMP, CRP, vitamin D, Iron studies, KIT D816V mutation analysis, peripheral flow cytometry, and abdominal ultrasound. TPSAB1 genetic analysis was consistent with hereditary alpha-tryptasemia. Given persistent symptoms, montelukast, hydroxyzine, and cromolyn were initiated sequentially. Discussion While up to two-thirds of HaT patients are asymptomatic, some patients may develop symptoms consistent with mast cell degranulation due to alpha-/beta-tryptase heterotetramers formation. The genotype-phenotype correlation has not been clearly delineated, making differential diagnosis challenging. There appears to be increased frequency/severity of immediate hypersensitivity reactions and associations with systemic mastocytosis, Hymenoptera venom allergy, and idiopathic anaphylaxis. Elevated BST (>11.4 ng/mL) in the absence of HaT can also be caused by renal impairment or clonal myeloid neoplasms. Interestingly, her previously healthy older brother subsequently developed similar episodes with elevated tryptase of 15.9 ug/L with normal level in mother. The heterogenous clinical manifestations of HaT emphasize the importance of multidisciplinary approach in these cases.
