Abstract:Stroke is a leading cause of death and disability world-widely. The incidence rate of stroke has been increasing due to the aging population and lifestyle changes. At present, the only drug approved by the US Food and Drug Administration (FDA) for the treatment of ischemic stroke is tissue plasminogen activator (t-PA), but its clinical application is greatly limited because of its narrow time window and bleeding risk. Natural products have a long history of being used in traditional medicine with good safety, making them an important resource for the development of new drugs. Indeed, some natural products can target a variety of pathophysiological processes related to stroke, including oxidative stress, inflammation and neuronal apoptosis. Therefore, the development of high-efficiency, low-toxicity, safe and cheap active substances from natural products is of great significance for improving the treatment alternatives of patients with stroke. This article reviews the neuroprotective effects of 33 natural compounds by searching recent related literature. Among them, puerarin, pinocembrin, quercetin, epigallocatechin-3-gallate (EGCG), and resveratrol have great potential in the clinical treatment of ischemic stroke. This review will provide a powerful reference for screening natural compounds with potential clinical application value in ischemic stroke or synthesizing new neuroprotective agents with natural compounds as lead compounds.

Blood pressure management in acute ischemic stroke: the state-of-the-art and the prospects

Abstract:Blood pressure (BP) management in acute ischemic stroke is one of the important intervention affecting clinical prognosis. Many factors in patients with ischemic stroke can lead to increased BP in the acute stage, which is relatively common, while few patients may manifest hypotension. BP is a key factor affecting the clinical outcomes according to the previous studies. Too high or too low BP is related to the poor prognosis, but the specific management strategy of BP in the acute stage has not been determined yet. Increased BP during the acute phase of recanalization treatment is associated with an increased risk of symptomatic intracranial hemorrhage. The management of BP needs to be more rigorous, but the need for intensive BP control should be analyzed in light of the individual patient's features.

Abstract:Purpose of review: The present manuscript examines the significance of blood pressure elevation in patients with acute ischemic stroke, the physiologic principles worthy of consideration during its treatment, and the recent empirical evidence that should guide management protocols. It also provides a sound and practical approach to treatment along the time continuum, with particular relevance to reperfusion strategies. Recent findings: The existing evidence shows that both insufficient and excessive blood pressures are detrimental to the outcome of patients with acute ischemic stroke. This "U-shaped" relation, however, relates to measurements at the time of presentation, and clinical studies lack detail and specificity relative to differential measurements along the time continuum, particularly prior to and following reperfusion. Extrapolating from recent series, it is possible to construct treatment protocols balanced for effectiveness and safety. The management of blood pressure after acute ischemic stroke is an important, complex, and challenging aspect of care, requiring a thorough understanding of cerebrovascular physiology. Along the time continuum, the therapeutic priorities start with the preservation of penumbral tissue prior to reperfusion and then follow with the limitation of the damaging effects of excessive blood pressure readings after reperfusion, optimizing the chances of improved outcomes.
Survey of Treatment of Hypertension among Patients with Ischemic Stroke

Abstract:Background Hypertension is the most important risk factor of stroke in Chinese people.Antihypertensive treatment can reduce recurrence of stroke considerably.Objective To investigate the blood pressure control and antihypertensive medication use in patients with established ischemic stroke and determine the factors influencing blood pressure control.Methods Patients with ischemic stroke were screened to consider participation in a clinical trial and information was collected about their disease history and medication use.Also with E-questionnaire,their blood pressure measurement,as well as body mass index and waist circumference,was recorded.Results Between June 2007 and October 2009,6417 patients with ischemic stroke from 14 cities in China were interviewed.Their mean age was 64 years.A history of hypertension was present in 4733(74%)patients,20% of whom took no antihypertensive drug,and only 995 patients(21%)reached the goal of blood pressure control according to current guidelines.The incidence of comorbid diabetes was associated with poorer blood pressure control(P0.01).A history of myocardial infarction or previous coronary intervention was associated with better blood pressure control.So was the taking of anti-platelet agents or statins(P0.05).Elevated blood pressure occurred in 1684(60%)patients without the history of hypertension.Conclusion A substantial proportion of patients with ischemic stroke have not reached the target blood pressure recommended by current guidelines.The prognosis of these patients should be improved by effective blood pressure control.

Positive Influence of Stroke Health Manager on Risk Factors Control and Medication Adherence After Ischemic Stroke

Abstract:Background: From 2017, the Stroke Health Manager Training Project was carried out by the Chinese Government to strengthen health management and follow-up intervention after ischemic stroke. The aim of this study was to investigate whether after the intervention of the stroke health manager, the control of blood pressure, low-density lipoprotein cholesterol (LDL-C), glucose level, and the use of secondary prevention medications improved 3 months after discharge from our center following ischemic stroke.Methods: The study used a history-controlled approach. Patients who received stroke health manager intervention from May 1, 2018, to March 31, 2019, were considered as the intervention group; those from May 1, 2017, to April 30, 2018, were enrolled as the control group. Stroke health manager intervention included health education, discharge advice, online WeChat public group follow-up, and clinical consultation.Results: In total, 642 patients with ischemic stroke were enrolled in this study (277 in the control group, 365 in the intervention group). At 3 months, the blood pressure, LDL-C and glucose control in the intervention group were better than in the control group (all P < 0.05). At the same time, the overall persistence for secondary prevention medications at 3 months after discharge increased from 201/277 (72.56%) to 303/365 (83.01%, P = 0.001). The persistence for patients taking antiplatelet, hypoglycemic and statins were significantly higher in the intervention group (P < 0.05).Conclusions: Stroke health manager intervention improved the control of blood pressure, LDL-C, glucose levels and the persistence for secondary prevention medications 3 months after discharge.

Identification of Serum Biomarkers of Ischemic Stroke in a Hypertensive Population Based on Metabolomics and Lipidomics

Abstract:Hypertensive individuals are at a high risk of stroke, and thus, prevention of stroke in hypertensive patients is essential. Metabolomics and lipidomics can be used to identify diagnostic biomarkers and conduct early assessments of stroke risk in hypertensive populations. In this study, serum samples were collected from 30 hypertensive ischemic stroke (IS), 30 matched hypertensive and 30 matched healthy participants. Metabolomics and lipidomics analyses were conducted via liquid chromatography-tandem mass spectrometry, and the data were analyzed using multivariate and univariate statistical methods. A random forest algorithm and binary logistic regression were used to screen the biomarkers and establish diagnostic model. We detected 21 differential metabolites and 38 differential lipids between the hypertensive IS and healthy group. Moreover, we found 18 differential metabolites and 31 differential lipids between the hypertensive IS and hypertension group. In particular, the following seven metabolites or lipids distinguished the hypertensive IS from the healthy group: 4-hydroxyphenylpyruvic acid, cafestol, phosphatidylethanolamine (PE) (18:0p/18:2), PE (16:0e/20:4), (O-acyI)-1-hydroxy fatty acid (36:3), PE (16:0p/20:3) and PE (18:1p/18:2) (rep). The following seven biomarkers distinguished the hypertensive IS from the hypertension group: diglyceride (DG) (20:1/18:2), PE (18:0p/18:2), PE (16:0e/22:5), phosphatidylcholine (40:7), dimethylphosphatidylethanolamine (50:3), DG (18:1/18:2), and 4-hydroxyphenylpyruvic acid. The aforementioned panels had good diagnostic and predictive ability for hypertensive IS. Our study determines the metabolomic and lipidomic profiles of hypertensive IS patients and thereby identifies potential biomarkers of the presence of IS in hypertensive populations.
Study on the intervention effect of SMG health management in patients with ischemic stroke

Abstract:Introduction: The aim of the study was to explore the clinical effect of brain and heart health managers combined with the "SMG" health management mode on nursing intervention in ischemic stroke patients. Material and methods: A total of 187 ischemic stroke patients divided into 96 patients in the observation group and 91 patients in the control group with the random number table method. The control group conducted the routine care intervention, and the observation group used the brain and heart health managers combined with the "SMG" health management model for the nursing intervention. The control of stroke risk factors was explored by comparing blood pressure, blood glucose and blood lipid and other indicators between the two groups before and after treatment. Results: Compared with that before the intervention, the Stroke Self-Efficacy Questionnaire (SSEQ) score of both the observation group and the control group were significantly higher ( p < 0.05), and the Hamilton Anxiety Scale (HAMA), Hamilton Depression Scale (HAMD), National Institutes of Health Stroke Scale (NIHSS) and modified Rankin scale (mRS) scores were all decreased ( p < 0.05). The proportion of patients with treatment adherence did not differ significantly before and after the intervention in the control group ( p > 0.05), and it increased significantly in the observation group after the intervention ( p < 0.05). The observation group had higher SSEQ score and lower HAMA, HAMD, NIHSS, and mRS scores after the intervention compared with the control group, with statistically significant differences. Conclusions: The combination of brain and heart health managers and "SMG" is more conducive to improving the selfefficacy of ischemic stroke patients, alleviating patient anxiety and depression, improving patient treatment compliance, controlling stroke risk factors, and promoting neurological function recovery.
Blood pressure control and antihypertensive medication use after discharge and prognosis of ischemic stroke

Abstract:Objective: To investigate the effect of consistently blood pressure (BP) control status after discharge on adverse clinical outcomes among ischemic stroke (IS) patients. Methods: Three thousand, four hundred and six acute IS patients were included and followed up at 3 months, 12 months, and 24 months after stroke. Study outcomes were defined as death, vascular events and composite of death or vascular events. Cox proportional hazard models were used to estimate hazard ratios (HR) and 95% confident interval (CI) of death and the composite outcome of death or vascular events associated with BP control and antihypertensive medication use. Results: The multivariable adjusted HRs were 0.22 [95% confidence interval (CI): 0.09–0.57] for death and 0.60 (95% CI: 0.39–0.97) for the composite outcome of death or vascular events among participants with consistently controlled BP compared with those with consistently uncontrolled BP. The participants with both consistently controlled BP and regular use of antihypertensive medication had the lowest risks of death [hazard ratio (HR): 0.18, 95% CI: 0.04–0.75] and composite outcome of death or vascular events (HR: 0.54, 95% CI: 0.29–0.98) in comparison with those with both uncontrolled BP and irregular use of antihypertensive medication. Discussion: Continuous BP control and regular use of antihypertensive medications after discharge can decrease the risks of death and composite outcome of death or vascular events among IS patients, suggesting the importance of continuous BP control and regular use of antihypertensive medications after discharge for improving prognosis of IS.

Prehospital blood pressure lowering in patients with ischemic stroke: A systematic review and meta-analysis of randomized controlled trials

Abstract:Background: Whether prehospital blood pressure control improves outcomes among patients with acute ischemic stroke is uncertain. This systematic review and meta-analysis aimed to evaluate the effect of prehospital blood pressure reduction treatment in patients with ischemic stroke. Methods: We searched PubMed, Embase, and the Cochrane Central Register of Controlled Trials for randomized controlled trials that compared prehospital blood pressure reduction treatment with usual treatment or no treatment in patients with stroke. The primary outcome was functional outcome, assessed with the distribution of modified Rankin Scale (mRS) at 90 days, while secondary outcome was mortality at 90 days. Results: Of 428 studies reviewed, three were appropriate for analysis, totaling 3878 patients. In patients with ischemic stroke, prehospital blood pressure reduction treatment was associated with higher mRS scores at 90 days (common odds ratio (OR) for worse mRS, OR: 1.27, 95% confidence interval (CI): 1.08-1.49) and increased risk of mortality at 90 days (OR: 1.28, 95% CI: 1.02-1.61) compared with the usual treatment. Conclusion: In patients with ischemic stroke, prehospital blood pressure reduction treatment was associated with a higher likelihood of poor functional outcome and an elevated risk of mortality.
Role Of Blood Pressure Management In Stroke Prevention: A Systematic Review And Network Meta-Analysis Of 93 Randomized Controlled Trials

Abstract:Background and Purpose The present study aimed to compare the efficacy and tolerability of different blood pressure (BP)-lowering strategies.Methods Randomized controlled trials that compared various antihypertensive treatments and stroke outcomes were included. Eligible trials were categorized into three scenarios: single or combination antihypertensive agents against placebos; single or combination agents against other agents; and different BP-lowering targets. The primary efficacy outcome was the risk reduction pertaining to strokes. The tolerability outcome was the withdrawal of drugs, owing to drug-related side effects (PROSPERO registration number CRD42018118454 [20/12/2018]).Results The present study included 93 trials (average follow-up duration, 3.3 years). In the pairwise analysis, angiotensin-converting enzyme inhibitors (ACEis) and beta-blockers (BBs) were inferior to calcium channel blockers (CCBs) (odds ratio [OR], 1.123; 95% confidence interval [CI], 1.008 to 1.252) (OR, 1.261; 95% CI, 1.116 to 1.425) for stroke prevention, BB was inferior to angiotensin II receptor blockers (ARB) (OR, 1.361; 95% CI, 1.142 to 1.622), and diuretics were superior to ACEi (OR, 0.871; 95% CI, 0.771 to 0.984). The combination of ACEi+CCB was superior to ACEi+diuretic (OR, 0.892; 95% CI, 0.823 to 0.966). The network meta-analysis confirmed that diuretics were superior to BB (OR, 1.34; 95% CI, 1.11 to 1.58), ACEi+diuretic (OR, 1.47; 95% CI, 1.02 to 2.08), BB+C-CB (OR, 2.05; 95% CI, 1.05 to 3.79), and renin inhibitors (OR, 1.87; 95% CI, 1.25 to 2.75) for stroke prevention. Regarding the tolerability profile, the pairwise analysis revealed that ACEi was inferior to CCB and less tolerable, compared to the other treatments.Conclusions Monotherapy using diuretics, CCB, or ARB, and their combinations could be employed as first-line treatments for stroke prevention in terms of efficacy and tolerability.
[Management of Blood Pressure in Acute Ischemic Stroke: a Meta-Analysis].

Abstract:Objective: We are aiming at conducting a meta-analysis of the literature to elucidate the efficacy and safety of anti-hypertension therapy in acute stage of ischemic stroke (AIS). Methods: Researches on the efficacy and safety of anti-hypertension therapy published on PubMed before 1st September 2017 were independently retrieved by two authors.Clinical benign prognosis and death measured as modified Rankin Scale (mRS) ≤2 and mRS=6 were taken as primary indicators of efficacy and safety, respectively.A meta-analysis of included randomized controlled trials (RCTs) was performed by RevMan 5.3 software, with funnel plot evaluating the publication bias.A systemic literature review was also made to illustrate the management of blood pressure in AIS. Results: Altogether 13 papers were included in the meta-analysis, among which 12 researches (including 7 596 versus 7 503 cases in the treatment and control group, respectively) based meta-analysis showed that anti-hypertensive therapy initiated within 72 h in AIS did not increase the rate of clinical benign prognosis (OR=1.01, 95%CI 0.94-1.08, P=0.79), and 11 studies (including 7 503 versus 7 369 cases in the treatment and control group, respectively) based analysis suggested there was no significant difference in mortality between the two groups (OR=1.03, 95%CI 0.92-1.15, P=0.59). No significant publication bias for the included studies was showed in the funnel plots. Conclusions: Despite a relative safe profile of anti-hypertensive therapy for AIS, the overall therapeutic efficacy was not improved.The efficacy of anti-hypertensive therapy may be associated with the status of recanalization of blood vessels, further researches are needed to validate the efficacy of anti-hypertension therapy in certain subgroups of IS.

Abstract:Abstract: Objective: To investigate the management of perioperative target blood pressure in the treatment of acute ischemic stroke with intravenous thrombolytic bridging. Methods: Retrospective analysis of the blood pressure management and nursing experience of 36 patients with acute ischemic stroke who received endovascular treatment with bridge mode from November 2017 to January 2019 in our hospital. Through correct monitoring of basic blood pressure, rapid and stable blood pressure reduction before the bridge treatment, close cooperation during the operation, and close observation and treatment of postoperative blood pressure fluctuations, the patientâ€TMs blood pressure can be controlled within the target range. Results: The blood vessels of 36 patients were partially or completely recanalized after treatment. Clinical outcomes: Two cases died. After 90 days, 29 patients with good clinical outcomes were followed up, and 5 patients with poor clinical prognosis. Conclusion: Effective blood pressure management is a necessary measure in the perioperative period of bridging therapy for patients with acute ischemic stroke, which can reduce the postoperative complications of patients treated with bridging therapy, obtain good therapeutic effect and improve the prognosis of patients.

Abstract:Objective: To analyze the risk factors of hypertensive patients combined with stroke.Methods: 1 628 cases of patients with hypertension for 15 years in shougang community from the Effect on Cardiovascular and Cerebrovascular Events by early Hypertension Intervention study were selected.Medical check was given to this cohort again from April to July 2009,and hypertensive patients combined with stroke were divided into group Ⅰ(n=361),hypertensive patients without stroke were divided into group Ⅱ(n=1 267).Questionnaire survey,physical examination and laboratory examination were used to select relevant data.Logistic regression was adopted to proceed data analysis.Results: Age,BIM,smoking rate,dyslipidemia rate and diabetes morbidity rate of group Ⅰwere higher than those of groupⅡ,the differences were statistically significant(P0.05).Level of FBG,CRP,TC,TG,HDL-C,HDL-C were compared between groupⅠand groupⅡ,the differences were statistically significant(P0.05).Logistic regression analysis showed that age and lipid disorders were closely related with stroke(OR=1.06,2.13 resectively).Conclusion: Early management of patients with hypertension,healthy lifestyle guidance can improve the patients life quality and reduce the incidence of stroke,in order to provide guidance for stroke prevention.

Abstract:Stroke affects people's quality of life because of its high morbidity and mortality. In China, stroke is easily affected by hypertension, and the morbidity of hypertension is rising quickly year by year. However, most of adults who have hypertension do not know about their disease. In the 20th century, researchers focused on the relationship between blood pressure and stroke, and found that hypertension could lead to stroke. The incidence and recurrence of stroke could be decreased through antihypertensive treatment. In the 21th century, researchers gradually transfer their attention to the daily monitoring of blood pressure. The ambulatory blood pressure monitoring reveals that blood pressures during different periods of one day have different correlations with stroke occurrence. Home blood pressure monitoring (HBPM) provides longtime blood pressure changes. In the future, after the overall monitoring and treatment of hypertension, we believe that we can decrease the overflow of stroke prevalence. DOI: 10.3969/j.issn.1672-6731.2015.01.003
Blood pressure lowering and stroke prevention: a systematic review and network meta-analysis protocol.

Abstract:Background: Hypertension is a leading cause of stroke and the significance of blood pressure lowering treatment strategies for prevention of stroke has been well established. Despite the established and widespread use of BP lowering drugs, which one is better for stroke prevention is still debated. This study aimed to determine the most effective and safest blood pressure lowering treatments for stroke prevention among various single and combined therapies. Methods: A systematic search will be performed in PubMed and the Cochrane Library to identify RCTs and meta analyses of different pharmacological interventions for stroke prevention from January 01, 1966 to December 01, 2018. Primary efficacy outcome will be reduction of stroke incidence and safety outcome will be drug-related side effects withdraw. Study quality will be critically appraised based on the seven-point tool for assessing risk of bias by Cochrane collaboration. Pairwise meta-analyses and Bayesian network meta-analyses will be performed for all related outcome measures. We will conduct subgroup analyses and sensitivity analyses to assess the robustness of our findings. Discussion: This network meta-analysis will summarize the direct and indirect evidence aiming to provide a ranking of various blood pressure lowering strategies for prevention of stroke. The results of this meta-analysis may help the physicians in determining the best treatments for their patients in stroke prevention.
Survey of Stroke Risk Factors in Beijing Hypertensive Patients

Abstract:Objective To investigate relative risk factors for hypertension in cerebral stroke patients.Methods The transect epidemiological survey was carried out in 5 005 hypertensive patients in Beijing.The risk factors in these patients and target organ damage conditions including blood pressure,blood glucose,blood lipids,family history,carotid lesion,left ventricular hypertrophy,etc.,were recorded.Results The up-to-standard rates of blood pressure and low density lipoprotein cholesterol in the stroke patient group and the non-stroke patient group were significantly different(40.6% vs 51.5%,P=0.007;44.7% vs 59.9%,P=0.001).In patients with diabetes mellitus,the up-to-standard rates of glycohemoglobin in the two groups were not significantly different(42.4% vs 48.8%,P=0.124).Carotid artery intima-media thickness in the two groups was significantly different [(1.18±0.36) mm vs(1.10±0.31) mm,P=0.036].Left ventricular hypertrophy in the two groups was significantly different(21.3% vs 25.8%,P=0.001).There was significant relation between blood pressure,glycohemoglobin,atrial fribrillation,family history and stroke.Conclusions Treatment of relative risk factors was unsatisfactory.Strictly controlling risk factors was benefit for primary prevention and secondary prevention of cerebral stroke.
