Abstract:Bronchial asthma (asthma) is a common chronic airway inflammatory disease with heterogeneity.Although various treatments are available, including inhaled corticosteroids, many asthmatics are still not well controlled.According to studies, many inflammatory mediators are involved in the pathogenesis of asthma which result in a variety of monoclonal antibodies and small-molecule chemotherapeutic drugs targeting the inflammatory mediators being developed for the treatment of asthma.Thus, treatment of asthma enters the era of targeted therapy.This review discusses the progress of targeted therapy in asthma and possible new targets so as to provide more individualized therapy for patients.

Abstract:The prevalence of allergic rhinitis and asthma increased over the last few decades.Current therapeutics is mainly confined to symptom controlling by pharmacological treatment, with only a small proportion of patients under the treatment of allergen-specific immunotherapy.Recently, based on the development of molecular biology, immunology, genetics and the related sciences, basic and clinical studies have been conducted successfully in new therapeutics of allergic airway diseases, including genetic recombinant vaccine, anti-IgE antibody, anti-cytokine, interference in gene expression of transcription factors, antisense therapy and RNAi, in order to explore novel methods and approaches for blocking the initiation of disease.This review summarized the research advances in new therapeutics for allergic rhinitis and asthma.

Abstract:To identify a novel and effective therapy for allergic rhinitis and asthma (ARA), the present study focused on treatment with tumor growth factor (TGF)-β1 neutralizing antibody. In the present study, four medications were administered to mice with ovalbumin-induced allergic inflammation. Allergic symptoms in the lungs and nasal mucosa were evaluated by detecting the secretion of cytokines from helper T cells (Th) in the peripheral blood, nasal lavage fluid and bronchoalveolar lavage fluid using ELISA. Defects in regulatory T (Treg) cells in peripheral blood mononuclear cells were also detected using flow cytometry. Furthermore, the expression of TGF-β1 and activation of Smad2/3 pathways were assessed using immunohistochemical staining, reverse transcription-quantitative polymerase chain reaction, and western blotting. It was observed that TGF-β1 neutralizing antibody inhibited symptoms of inflammation in the upper and lower airways. TGF-β1 neutralizing antibody also restored the Th1/Th2 balance and ameliorated Treg cell defects induced by ARA. Furthermore, the therapeutic effects of TGF-β1 neutralizing antibody were related to its inhibitory effects on TGF-β1 expression and Smad2/3 signaling in nasal and lung tissues. Therefore, TGF-β1 neutralizing antibody may be an effective medicine for the treatment of ARA.

Abstract:Asthma is a chronic airway inflammatory disease with complex etiology and pathogenesis.Due to the deep research about its etiology,pathogenesis and mechanisms of the drugs,the therapeutic regimen and guidelines of asthma are updating continuously.In these years,some randomized controlled trials of asthma-treating drugs have been published,and new guidelines have become available.This article discussed the category,side effects and status in asthma-controlling strategy of asthma-treating drugs,and introduced some new types of these drugs.The purpose of this article is to favor the standardization of pharmacologic treatments for asthma patients,and help to achieve the goal of controlling asthma more effectively.

Abstract:Mechanistic studies have improved our understanding of molecular and cellular components involved in asthma and our ability to treat severe patients. An mAb directed against IgE (omalizumab) has become an established add-on therapy for patients with uncontrolled allergic asthma and mAbs specific for IL-5 (reslizumab, mepolizumab), IL-5R (benralizumab), and IL-4R (dupilumab) have been approved as add-on treatments for uncontrolled eosinophilic (type 2) asthma. While these medications have proven highly effective, some patients with severe allergic and/or eosinophilic asthma, as well as most patients with severe non-type-2 disease, have poorly controlled disease. Agents that have recently been evaluated in clinical trials include an antibody directed against thymic stromal lymphopoietin, small molecule antagonists to the chemoattractant receptor-homologous molecule expressed on TH2 cells (CRTH2) and the receptor for stem cell factor on mast cells (KIT), and a DNA enzyme directed at GATA3. Antibodies to IL-33 and its receptor, ST2, are being evaluated in ongoing clinical studies. In addition, a number of antagonists directed against other potential targets are under consideration for future trials, including IL-25, IL-6, TNF-like ligand 1A, CD6, and activated cell adhesion molecule (ALCAM). Clinical data from ongoing and future trials will be important in determining whether these new medications will offer benefits in place of or in addition to existing therapies for asthma.

Abstract:OBJECTIVE: To review the current approach to the management of bronchial asthma with particular reference to drug therapy.DATA SOURCES: All significant English language journals were searched for current studies, particularly controlled clinical trials, in the management of asthma.STUDY SELECTION: Sixty articles were reviewed. They were selected on the basis of the known reputation of the author or the unit from which the study emanated, the place that the study has assumed in academic respiratory circles, and whether the study contributed something new to our understanding of drug therapy in asthma. Review articles from eminent scientific bodies were also analysed.CONCLUSIONS: Asthma is an inflammatory disease and therapy must be directed towards this as well as to bronchoconstriction. Inhaled corticosteroids are the mainstay of treatment and inhaled beta-agonists are best used on an as needed basis. Regular objective assessments of patients' progress are essential, as are written, individualised action plans. The adjunctive use of other drugs is discussed and new therapies which may prove useful in the future are reviewed.

Abstract:Asthma is a chronic airway inflammatory disease with complex mechanisms, and these patients often encounter difficulties in their treatment course due to the heterogeneity of the disease. Currently, clinical treatments for asthma are mainly based on glucocorticoid-based combination drug therapy; however, glucocorticoid resistance and multiple side effects, as well as the occurrence of poor drug delivery, require the development of more promising treatments. Nanotechnology is an emerging technology that has been extensively researched in the medical field. Several studies have shown that drug delivery systems could significantly improve the targeting, reduce toxicity and improve the bioavailability of drugs. The use of multiple nanoparticle delivery strategies could improve the therapeutic efficacy of drugs compared to traditional delivery methods. Herein, the authors presented the mechanisms of asthma development and current therapeutic methods. Furthermore, the design and synthesis of different types of nanomaterials and micromaterials for asthma therapy are reviewed, including polymetric nanomaterials, solid lipid nanomaterials, cell membranes-based nanomaterials, and metal nanomaterials. Finally, the challenges and future perspectives of these nanomaterials are discussed to provide guidance for further research directions and hopefully promote the clinical application of nanotherapeutics in asthma treatment.

Abstract:The landscape of asthma has considerably changed after 40 years of inhaled corticosteroid development and nearly 20 years since the first monoclonal antibodies (mAbs) were approved. New members of pharmacological families and more effective drug-delivery devices have been designed but the proportion of uncontrolled patients, unfortunately, remains stable. The most promising treatments now rely on targeted therapies that encourage the improvement of the characterisation of our patients. These clinical (phenotype) or new biological (endotype) tools lead to palpable personalised medicine. This review examines not only the future of mAbs and other new ways of treating asthma but also describes futuristic views based on the paradigm shifts that are ready to occur.

Abstract:Asthma is a disease of the airways with an underlying inflammatory component. The prevalence and healthcare burden of asthma is still rising and is predicted to continue to rise in the current century. Inhaled β2-adrenoceptor agonists and corticosteroids form the basis of the treatments available to alleviate the symptoms of asthma. There is a need for novel, safe treatments to tackle the underlying inflammation that characterizes asthma pathology. Furthermore, there is a requirement for new treatments to be developed as oral therapy in order to alleviate patient compliance issues, especially in children. A multitude of new approaches and new targets are being investigated, which may provide opportunities for novel therapeutic interventions in this debilitating disease. For simplicity, these approaches can be divided into two categories. The first comprises therapies directed against specific components or steps seen in allergic asthma. By ‘components’ we mean the key inflammatory cells (T cells [in particular Th2], B cells, eosinophils, mast cells, basophils and antigen presenting cells [APC]) and mediators (immunoglobulin E [IgE], cytokines, histamines, leukotrienes and prostanoids) believed to be involved in the chronic inflammation seen in asthma. By ‘steps’ we mean the allergic response, such as antigen processing and presentation, Th2-cell activation, B-cell isotype switching, mast cell involvement and airway remodeling. The other category of novel approaches to disease modification in asthma encompasses general anti-inflammatory therapies including phosphodiesterase 4 (PDE4) inhibitors, p38 mitogen-activated protein kinase (MAPK) inhibitors, peroxisome proliferator-activated receptor-γ (PPARγ) agonists, and lipoxins.

Abstract:Over the past two decades, the management of severe asthma has shifted from relying on inhaled corticosteroids and bronchodilators to more precise, targeted approaches. Monoclonal antibodies (mAbs) designed to address specific molecular pathways in asthma have transformed care for patients with severe asthma. Since therapy targeting immunoglobulin E (IgE) became the first biologic developed for allergic asthma in 2003, monoclonal antibodies targeting interleukin (IL)-5, IL-5 receptor, IL-4/13 receptor, and thymic stromal lymphopoietin (TSLP) have been approved for treating difficult-to-treat asthma, improving symptoms, reducing exacerbations, and reducing oral corticosteroid dosing.Despite these advances, many patients continue to experience asthma exacerbations and symptoms, and fail to achieve remission. To address this, pharmaceutical companies and researchers are exploring novel therapies targeting different aspects of asthma pathophysiology, including cytokines, enzymes, and cellular pathways. Innovative treatments like inhaled biologics, ultra-long-acting biologics, and combination biologics are in development. New molecular targets, such as Bruton tyrosine kinase (BTK), Ox40 ligand, and Janus kinase (JAK), offer promise for addressing unmet needs in asthma care.While many therapies have failed to get approval for use due to lack of efficacy, trial design, or toxicity, these experiments still provide insights into asthma's underlying mechanisms. The future of asthma management looks promising, with emerging therapies aiming to improve patient outcomes. The challenge will lie in identifying the right therapy for each patient and developing personalized treatment strategies.

Abstract:Introduction . The relevance of studying the possibility of using targeted therapy in the treatment of polypous rhinosinusitis in patients with comorbid bronchial asthma is due to the observed growth of the disease and the disclosure of new pathophysiological mechanisms of their development. Aim . Based on the generalization of research results and the analysis of our own clinical observations, to improve the effectiveness of treatment and the quality of life of patients with polypous rhinosinusitis with a history of severe bronchial asthma, using genetically engineered biological therapy. Materials and methods . The literature of the eLibrary databases is analyzed.RU RSCI, Medline, Scopus, Web of Science for the period 2011–2023. The inclusion of monoclonal antibodies in the treatment regimen for diseases that are difficult to treat with medication is based on the results of clinical studies and meta-analysis data. A study of the quality of life was conducted, according to the SF-36 Health Status Survey, 47 patients receiving targeted therapy with monoclonal antibodies in medical and preventive institutions of the megalopolis. Results . The assessment of the quality of life of patients with polypous rhinosinusitis and comorbid bronchial asthma confirmed the effectiveness of targeted therapy with monoclonal antibodies. According to the SP-36 questionnaire, before treatment, the low-est scores were on the Role-Physical Functioning scales – 51.5 points and General Health – 49.1 points, respectively. At the 2 nd and 16 th weeks of treatment, gradually increasing positive dynamics was noted on all scales and by the end of the 52 nd week of monoclonal antibody use, values as close to normal as possible were recorded in the Social Functioning, Role Emotional, Mental Health scales and amounted to 97.7; 98,3 and 98.7 points. The effectiveness of the treatment was confirmed by two clinical cases. Conclusions . Knowledge of the immunological and pathogenetic mechanisms of the disease allows us to open up significant prospects for diagnosis and treatment. Recommendations and drug selection should be consistently followed within the framework of the clinical recommendations of the relevant diseases. Strict consideration of the development of short-term and long-term risks of the use of genetically engineered biological therapy is necessary.

Abstract:The recent advances in the knowledge of the basic mechanisms underlying asthmatic inflammation have significantly contributed to the delineation of new therapeutic perspectives for asthma. There are currently three main approaches to the development of novel antiasthma treatments: 1) improvement in existing classes of drugs 2) identification of new compounds able to interfere with the complex network of proinflammatory mediators, cytokines, chemokines, and adhesion molecules involved in the pathogenesis of asthma 3) utilization of new forms of immunotherapy aimed at blocking the unbalanced Th2 response which characterizes the pathophysiology of asthma. Such a remarkable expansion in available therapeutic options will probably allow us, over the next decade, to treat asthma by more selectively targeting the pathogenetic events responsible for this widespread airway disease.

Abstract:It has been considered by modern medicine that bronchial asthma is a chronic airway allergic inflammation (AAI) which is the major factor that induces reversible airway ventilating disturbance and bronchial hyper-responsiveness (BHR) in asthmatic patients, as well as a correlation between the delayed-phase onset of asthma and AAI is more significant than immediate-phase onset of asthma in its pathogenesis. Anti-inflammatory therapy as a fundamental principle of treatment for bronchial asthma has been brought forward during the interval phase so that a large number of anti-AAI drugs to significantly enhance the curative effect such as inhalant corticoid, disodium cromoglycate, and leukotriene receptors antagonist, etc., has been rapidly developed. Additionally, traditional Chinese medicine (TCM) and integration of TCM and western medicine (WM) therapies, including acupuncture, are gradually taken into account by the medical circle as a characteristic in our country for treatment of bronchial asthma. It is worthy of being pointed out that the curative effect of acupuncture and moxibustion in treating asthma is evident, and that its therapeutic mechanism has to a certain extent, been found along with the recent deep-going acupunctural research.
