Abstract:目的探讨胞曼不动杆菌致下呼吸道感染暴发的危险因子,为临床干预及控制提供依据.方法按照卫生部医院感染诊断标准对14位胞曼不动杆菌感染患者进行回顾性病史分析.结果 14例病例中70岁以上患者占79%,57%的患者具有一种以上潜在性疾病,平均住院日延长了7倍,78%的患者感染前有长期二联以上抗生素使用史,100%患者在感染前接受侵入性治疗.在医护人员手、呼吸机积水器、口腔护理用水、气管切开面罩、床头柜上检测出本次感染暴发的细菌.结论接触传播是此次感染暴发的传播途径.加强医护人员手部卫生和无菌操作,重视环境、设备表面消毒,建立预警机制可降低或控制感染暴发的发生。

Clinical analysis of 114 cases of bronchiolitis in infants

Abstract:Background: Bronchiolitis is a common lower respiratory tract infection in infants and young children. Severe cases may be accompanied by obvious dyspnea and oxygen saturation decline. Aim: To summarize the clinical features, standard diagnosis, and treatment of bronchiolitis. Methods: This is a retrospective analysis of 114 pediatric patients (74 males, 40 females) who were first diagnosed as having bronchioles at the Department of Pediatrics of Tongling Maternal and Child Health Hospital from January 2019 to December 2019. The clinical features, imaging features, treatment, and other clinical data were recorded and analyzed. Results: The age of onset of the disease was mainly from 1 mo to 6 mo (75.4%), and the time to hospital visit was mostly from the 2nd day to the 4th day of the course of the disease (75.4%). Lung imaging examination showed increase in lung texture, fuzzy (93.8%). The main treatment was atomization therapy: Budesonide combined with terbutaline (45.6%) and budesonide combined with salbutamol (38.5%). The average hospitalization time was 7.1 ± 2.4 d, and the overall cure rate was 94.7%. In patients without bacterial infection, the use of antibiotics significantly prolonged the length of hospital stay (7.8 ± 2.5 d vs 5.7 ± 1.8 d) and improved the cure rate (98.3% vs 87.9%, P < 0.05). Conclusion: Infants with bronchiolitis are mainly male and tend to have a good prognosis. However, the unneeded use of antibiotics may prolong the length of hospital stay significantly, which imposes the burden both on the patients and hospital system.

Abstract:Abstract Background Pseudomembranous necrotizing laryngotracheobronchitis refers to an acute diffuse necrotizing inflammation in the mucosa of the larynx, trachea, and bronchus. It often occurs in infants and children having viral infections secondary to bacterial infections. Mycoplasma pneumoniae (M. pneumoniae) is a common pathogen that causes pneumonia in children. In recent years, serious complications due to M. pneumoniae infection, including necrotizing pneumonia, pulmonary embolism, and pleural effusion, have been increasingly reported. Case presentation An 11-year-old girl was admitted to our unit with cough, fever, and hoarseness persistent for a week. The results of the M. pneumoniae serological test, PCR examination with bronchial aspirate and bronchoalveolar lavage fluid (BALF), next-generation sequencing (mNGS) for BALF, all suggested the presence of M. pneumoniae infection. High-resolution CT scanning of the chest showed inflammation of the middle and lower lobes of the right lung. By bronchoscopy, the necrosis of the vocal cords, trachea, and bronchial mucosa was observed; each bronchial lumen contained a large amount of white viscous sputum. Pathological findings for bronchial mucosa suggested inflammatory necrosis. After administration of azithromycin and glucocorticoids, the symptoms of the patients were ameliorated. After 2 weeks post-discharge, the X-ray scan of her chest indicated the pneumonia resolution in the right lung. Conclusions In patients with pneumonia due to M. pneumoniae infection, which causes obvious hoarseness, bronchoscopy is necessary even if the lung lesions are not massively consolidated. When necrotizing lesions of the larynx, trachea, and bronchi are detected by bronchoscopy, the necrotic tissues in the corresponding parts should be conducted tissue biopsy for pathological examination. Apart from macrolide antibiotics, the administration of small doses of glucocorticoids is necessary.
Effect and security of bronchoalveolar lavage on infected atelectasis in children

Abstract:【Objective】 To explore the effect and security of bronchoalveolar lavage(BAL) on infected atelectasis in children. 【Methods】 The inpatients who diagnosed with infected atelectasis were retrospectively analyzed in our hospital,the control group(n=40) received only conventional therapy and antibiotic was based on Sputum culture and drug susceptibility,while the treatment group(n=40) received additional bronchoalveolar lavage via fibrobronchoscopy,both the treatment course were 4 weeks,then after 4 weeks the symptoms and signs,X ray or CT changes in the lung were observed,at the same time,the complications were also observed. 【Results】 The effect in the treatment group was better than in the control group,there was significantly difference between the two groups(P<0.01),The symptoms and signs recovered sooner in the treatment group than in the control group(P<0.01),X ray or CT changes in the lung recovered sooner in the treatment than in the control group(P<0.05)after 4 weeks.The incidences of complications in the treatment group were,hemorrhage 2.5%,fever 5%,laryngeal edema 2.5%,bronchospasm 2.5%,cyanosis 2.5%. 【Conclusions】 Bronchoalveolar lavage on infected atelectasis early can improve the transepithelial liquid to be transported,promote to absorbed the inflammation as early as possible and to reexpand the lung.The effect and security of BAL is definite and it deserves promotion.

Post-infectious Bronchiolitis Obliterans in Children: Clinical Features and Bronchoalveolar Lavage Therapy

Abstract:Objective To investigate the clinical features and bronchoalveolar lavage (BAL)therapy of postinfectious bronchiolitis obliterans (BO) in children. Method Ten children, who had post-infectious BO from February 2009 to February 2010, received BAL therapy, and were retrospectively analyzed. The data included pathology,chnical feature,chest HRCT scan, BALF cellular, levels of blood T cell subtypes and outcome of BAL therapy. Results Adenoviruses or mycoplasma pneumoniae were the most common etiologic agents (4/10, respectively). All patients presented persistent or recurrent dyspneic respirations and wheezing since the initial lung infection. The findings of HRCT included mosaic pattern of perfusion (6/10), accompanied by gas retention,bronchiectasis, atelectasis and bronchial wall thickening. The percentage of neutrophils in BALF was significantly increased in all cases (10/10). There were predominance of CD8+ T cell subtype (9/10) and lower ratio of CD4 +/CD8+ ( 10/10)in blood. Reduced symptoms and shortened hospital stay of BO in 9 of all 10 cases were observed after BAL therapy. Conclusions Severe adenovirus or mycoplasma pneunoniae bronchiolitis and/or pneumonia has higher risk for developing BO in children. Increased percentage of neutrophils in BALF and predominance of CD8 +T cell subtype may play an important role in the mechanism of BO. BAL therepy can reduce the respiratory symptoms of BO in children.

Abstract:哮喘的发病与环境及遗传因素有关,但其发病率在发展中国家的显著上升显然不能单以遗传因素来解释,环境因素发挥重要的影响作用[1].呼吸道病毒感染、过敏原暴露、环境污染以及被动吸烟等均是哮喘发生的危险因素.婴幼儿时期反复呼吸道感染,尤其是毛细支气管炎(毛支炎)对婴幼儿哮喘发病及迁延至儿童时期均有很大影响.毛支炎是婴幼儿时期,尤其6个月以下婴儿常见的下呼吸道感染性疾病,也是儿童时期发生哮喘的危险因素.因此,加强毛支炎的防治对减少哮喘的发病至关重要。
Application of Flexible Bronchoscopy in the Aetiological Diagnosis of Childhood Refractory Wheezing

Abstract:Objective: To explore the value of flexible bronchoscopy in aetiological diagnosis of paediatric refractory wheezing. Methods: A total of 246 patients with refractory wheezing performed flexible bronchoscopy were analysed. They were 146 males and 100 females with a ratio of male to female 1.5:1. Their age ranged from 1 month to 9 years with a median age of 17 months. Results: Flexible bronchoscopy was performed 269 times. By bronchoscopy, mucus plug was found in 52 patients (21.1%), airway malacia in 76 (30.9%), structural deformity in 32 (13.0%), foreign bodies in 22 (8.9%), tuberculosis in 3 and tumor in one. The aetiology of refractory wheezing varied with different ages. Airway malacia (61.2%) was the main reason in infants, then mucus plug (15.3%). Among toddlers, the most common reason was mucus plug (24.5%), followed by airway malacia (15.1%) and foreign body (14.2%). Bronchoalveolar lavage demonstrated pathogens in 17(17.3%) of 98 samples. The most common complication was hypoxia during the procedures. They all were transient and alleviated by oxygen supplement or temporary cessation of the procedure. Conclusion: Flexible bronchoscopy is effective in the aetiological diagnosis of childhood refractory wheezing. Mucus plug, airway malacia, structural deformities and foreign body are the main reasons for childhood refractory wheezing.

Abstract:Bronchiolitis obliterans (bronchiolitis obliterans BO) is a chronic airflow limitation caused by bronchial inflammatory injury.The clinical manifestations are persistent cough,wheezing and dyspnea after airway injury.The conventional bronchodilator therapy is invalid for BO.At present,the treatment for BO is still in lack of uniform standard.The early stage is the key period of clinical treatment.Treatment for children BO includes general supportive treatment,use of corticosteroids,bronchodilators,macrolides,intravenous immunoglobulin and other antibiotics,leukotriene receptor antagonists and immunosuppressants,as well as pulmonary physical therapy and lung transplantation.

Abstract:This study aimed to establish a registry database of different treatment modalities for lower respiratory tract infections (LRTIs) in Chinese children, thereby filling gaps in knowledge on clinical characteristics, treatment modalities, and rational drug use in relation to LRTIs in Chinese children, and providing large amounts of continuous, complete, scientific, and objective clinical data, and an information exchange platform. Multicenter data from these children's clinical visits were collected, pooled, and analyzed using medical informatics and statistical techniques to explore their potential value. The database was preliminarily established and a real‐world study cohort was constructed based on a total of 4805 patients registered in this database. Pneumonia was identified as the most common type of LRTIs (72.44%), followed by acute bronchitis (20.71%). The mean age of the enrolled children with LRTIs was 3.26 ± 2.84 years, and boys accounted for 59.21% of the samples. Among the enrolled children, pneumonia and acute bronchitis had the highest incidence in children aged 1–3 years (27.44%) and those aged 3–6 years (34.16%), respectively. In this national, multicenter, observational database of LRTIs in children, the real‐world characteristics and treatment modalities for LRTIs in Chinese children are elucidated. This database will help improve the research efficiency of clinicians and facilitate the exploration of underlying clinical patterns in real‐world medical big data.

Abstract:患儿: 女, 3岁4个月. 因发热、咳嗽2 d, 左侧胸痛伴呼吸困难1 d入院. 既往健康. 入院前2 d受凉后咳嗽伴中度发热, 咯少量白色泡沫痰, 在当地医院儿科诊断为上呼吸道感染, 给予抗感染和对症等治疗. 1 d前剧烈咳嗽后出现左侧持续胸痛伴呼吸困难, 摄胸片示左侧胸腔内巨大液气平, 纵隔明显右移, 以左侧脓气胸转入外科, 麻醉下行左侧胸腔穿刺, 抽出少许灰白色混浊液后, 行左侧胸腔置管闭式引流. 引流1 d未引出液、气, 并且术后患儿症状缓解不明显, 再次行胸部X线摄片检查, 左胸病变无明显改善, 转入我院小儿外科进一步诊治. 入院时患儿呈急性面容, 神萎, T 38.8 ℃, P 150次/min, R 45次/min, BP 13/8 kPa. 左侧胸廓饱满、肋间隙增宽, 叩诊呈鼓音, 呼吸音减弱. 血常规: WBC: 21.1×109/L, 中性分叶核细胞53%, 带状核细胞38%, HGB: 134 g/L。
Clinical analysis of 35 cases of severe mycoplasma pneumonia

Abstract:目的探讨重症支原体肺炎的诊断和治疗.方法总结2007年1月至2009年3月我院收治的35例重症支原体肺炎患儿临床特征以及诊治过程.结果在35例重症支原体肺炎患儿中,所有患儿均有发热、咳嗽,气促2例,喘息6例,胸痛5例,肺部有阳性体征17例.所有患儿均有肺部异常X线平片和CT改变、红细胞沉降率增高以及C反应蛋白增高.结论肺炎支原体可引起重症肺炎,对于临床上有上述表现的重症肺炎,应考虑到重症支原体肺炎的可能性.根据发病机制,对重症支原体肺炎患儿除应用大环内酯类抗生素外,可并用肾上腺皮质激素和大剂量丙种球蛋白以及纤维支气管镜冲洗等治疗。
Rupture of Main Bronchus after Blunt Chest Trauma in Children

Abstract:Objective To evaluate the clinical features of rupture of main bronchus after blunt chest trauma in children and the importance of early diagnosis and surgical treatment.Methods 11 consecutive patients from ages 3 to 7 years(5.1±1.4),with the main bronchial disruptions due to blunt chest trauma were reported(four right,seven left).8 boys and 3 girls were included.4 patients underwent immediate surgical correction (end-to-end anastomosis)within 24 hours after the accident.7 patients had a delayed diagnosis for 2-12 months after injury.Among these 7 patients,end￣to￣end anastomosis was used in 3 cases,four patients required total pneumonectomy.Results There were no deaths in this series.7 patients who received end-to-end anastomosis experienced an uneventful recovery.Conclusion Recognition of the clinical features of main bronchial rupture of main bronchus after blunt chest trauma in children,early diagnosis and bronchial repair could significantly decrease the incidence of complication and mortality.
Clinical epidemiological analysis of acute lower respiratory tract infections in 1 726 hospitalized infants in single center

Abstract:Objective To investigate the epidemiology and clinical manifestation of acute lower respiratory tract infections( ALRIs) in hospitalized infants,and to analyze the clinical characteristics of severe ALRIs. Methods Information about ALRIs for infants was collected who were admitted to Children 's Hospital of Fudan University from March 1st,2011 to February 29th,2012. Patient information included demographic characteristics,clinical presentation,accessory examination,etiology,treatment,prognosis,and disease burden. Results( 1) A total of 1726 children with ALRIs were included in this study,comprising 25. 3% of hospitalized infant patients,with peak proportion in autumn and winter. There were more preterm,low-birth-weight and under-six-month infants in severe ALRIs group.( 2) The main clinical symptom characteristics of ALRIs infants were cough( 70. 3%) and fever( 12. 2%). Severe ALRIs infants commonly had tachypnea,cranosis and apnea.( 3) The blood gas analysis showed elevated PaCO 2 and reduced PaO 2 and SaO 2 in the severe ALRIs infants. Among all ALRIs infants,91. 3% patients had confirmed pneumonia by X-ray.( 4) The etiologic agents were identified in 1249( 72. 4%) ALRIs infants. Respiratory syncytial virus( RSV) was detected in 913 cases( 52. 9%),which was the most predominant pathogen. In severe ALRIs group,more RSV, bacteria and co-infection were detected.( 5) 69. 5% severe ALRIs patients had underlying illness,with congenital heart disease being the commonest,followed by hyperbilirubinemia,bronchopulmonary dysplasia,airway abnormalities and undernutrition.( 6) The median length of hospital stay and cost of ALRIs infants in hospital were 9 days and $ 6 864 RMB respectively,where both of these figures were higher in severe cases than in mild to moderate cases. Conclusion Severe ALRIs was the common disease in hospitalized infants aged below 6 months. RSV was the most important pathogen. Severe ALRI infants had high incidence of underlying illnesses and disease burden.

Abstract:BACKGROUND:Foreign body aspiration (FBA) into the tracheobronchial tree is a common problem in children necessitating prompt recognition and management. This study aimed to report our experience in airway foreign body removal by flexible bronchoscopy in children.METHODS:A total of 1027 patients with FBA were reviewed retrospectively. They were 626 boys and 401 girls aged from 5 months to 14 years with a median age of 17 months. The clinical manifestations, radiological findings, bronchoscopic findings and complications of the procedure were analyzed.RESULTS:Among the patients, only 53.4% had a definite history of FBA. The most frequent symptom was paroxysmal cough (84.3%), followed by stridor or wheezing, fever and dyspnea. Chest X-ray showed emphysema in 68.8% of the patients, atelectasis in 13.3% and bronchopneumonia in 56.3%. A bronchoscope was inserted intranasally in most children, but through mouth and endotracheal tube in 17 and 3 children, respectively. Foreign bodies were removed successfully by flexible bronchoscopy with disposable grasping forceps or biopsy forceps in 938 (91.3%) of the patients. The other 89 patients turned to rigid bronchoscopy. During the procedures, 132 (12.9%) of the patients showed transient hypoxia, which was alleviated by oxygen supplement and/or temporary cessation of the procedure. A small amount of bleeding was found in 17 patients and bradycardia in 3. Air leak and laryngeal edema were noted in 2 patients and relieved within 24 hours.CONCLUSIONS:Flexible bronchoscopy is useful and safe in retrieving airway foreign bodies in children. With skilled personnel and perfect equipments, flexible bronchoscopy could be considered as the first choice for the removal of airway foreign body.

Abstract:Although children bronchiolitis is often encountered in the clinical practice of pediatricians,the diagnosis and treatment of bronchiolitis in children is quite difficult because of its varied clinical manifestation and non-specific imaging. In this paper,the anatomy,definition,etiological classification,diagnosis,differential diagnosis,and therapeutic principles of bronchiolitis in children will be discussed. Then the diagnosis and therapy of bronchiolitis based on different etiology will be explored.

Abstract:Objective To study diagnose,classification and airway management of tracheal bronchus in children.Methods Nine children with wheezing were examined by chest CT scanning(three-dimensional airway reconstruction) and 5 children underwent fiberoptic bronchoscopy.Results According to chest CT scanning of three-dimensional airway reconstruction,seven of the nine children were found to suffer from tracheal bronchus.Fiberobronchoscopic examination confirmed that all the five children hadt racheal bronchus including 2 children with negative result of chest CT scanning of three-dimensional airway reconstruction.At the same time,we detected other airway abnormalities including trachea or bronchus stenosis,epiglottis malacoma and glottis paralysis.Conclusion Tracheal bronchus was usually found by a chance.Bronchoscopic examination is the gold standard for diagnosis.If a child is associated with congenital heart diseases,intensive care and surgical repair in time are usually necessary for achieving higher survival rate.A child with tracheal bronchus usually needs tracheal intubation or one-lung ventilation,thus airway management should be focused on.
