Abstract:Ingested foreign bodies occasionally migrate to the paraglottic space. The external transcervical approach is almost always required to extract completely embedded foreign bodies. We report a case of an ingested fishbone embedded in the paraglottic space, which was successfully removed through transcervical exploration of the paraglottic space via the posterolateral approach. The posterolateral approach is safe and effective for the removal of foreign bodies completely embedded in the paraglottic space.

Abstract:Fishbones are the most commonly ingested foreign bodies that cause gastrointestinal tract penetration. However, fishbones embedded in the gastrointestinal tract that lead to foreign body granulomas that mimic submucosal tumors are rare. Herein, we describe a 56-year-old woman who presented with a 20-day-history of upper abdominal pain. Endoscopy revealed an elevated lesion in the gastric antrum. An abdominal computed tomography scan showed a mass in the gastric antrum and a linear calcified lesion in the mass. An endoscopic ultrasonography examination revealed a 3.9 cm x 2.2 cm, irregular, hypoechoic mass with indistinct margins in the muscularis propria layer. The patient was initially diagnosed as having a submucosal tumor, and subsequent surgical resection showed that the lesion was a foreign body granuloma caused by an embedded fishbone. Our case indicated that the differential diagnosis of a foreign body granuloma should be considered in cases of elevated lesions in the gastrointestinal tract. Delayed Endoscopic Removal of Sharp Foreign Body in the Esophagus Increased Clinical Complications

Abstract:Foreign bodies impaction in the esophagus is a common clinical emergency. The aim of this study was to investigate the clinical features of foreign body ingestion, and to analyze the risk factors of complications during the endoscopic procedure. From 18 general hospitals in Zhejiang Province in China, 595 patients who underwent gastroscopic removal of ingested foreign bodies were prospectively recruited. Patient characteristics, clinical features, foreign body features, clinical outcomes, and complications were documented. The most common types of foreign body in the esophagus were sharp objects (75.9%), including fish bones (34.0%), chicken bones (22.1%), and fruit nuclei (17.1%). The remaining types were non-sharp objects (24.1%), including food bolus (14.6%). Most objects were lodged in the proximal esophagus (75.9%). Foreign body-related complications occurred in 63 patients (10.5%), including hemorrhage (5.0%), perforation and infection (5.5%). The complication rate was increased by 4.04- and 8.48-fold when endoscopic retrieval was performed after impacted for over 24 and 48hours, respectively, after impaction, as compared with within 12hours. Logistic regression analysis revealed that the patients with sharp objects developed more complications than those with non-sharp ones (odds ratio, 2.85; 95% confidence interval, 1.08-7.50; P=.034). However, complications were unrelated with the location in the esophagus or length of foreign bodies (P>.05). Sharp objects were the most frequently ingested foreign bodies in the esophagus in China. The prevalence of complications was increased in the patients with long foreign body retention time (>24 hours) and sharp objects. Sharp foreign bodies in the esophagus are recommended to be removed within 24hours.

Abstract:Objective: To explore clinical features, diagnosis, localization, and therapeutic strategy of migratory pharyngeal and cervical esophageal foreign bodies. Methods: A total 23 cases of pharyngeal and cervical esophageal migratory foreign bodies were admitted between January 2015 and December 2021. There were 14 females and 9 males with the age ranged from 35 to 82 (55.0±12.7)years. In all the cases, esophageal CT was taken to confirm the esophageal foreign body. Multiplanar reconstruction (MPR) was performed to locate the foreign body from the horizontal, coronal and sagittal dimensions as well as the corrected reconstructed MPR. According to the location of the foreign body, appropriate surgical method was selected.The symptoms, complications, types of foreign body, positioning, surgical methods, and relevant information were recorded.Data were analyzed using the descriptive method and SPSS 25.0 software. Results: The clinical symptoms of 23 migrating esophageal foreign bodies included pharyngodynia (20/23), foreign body sensation (6/23), hoarsenss (1/23), difficulty in turning neck(1/23), difficulty in opening mouth (1/23), fever (7/23), poor appetite (1/23), and abdominal pain (1/23). The foreign bodies included 19 fish bones, 2 wires, 1 embroidery needle and 1 chicken bone. There were 9 cases (39.1%) of foreign bodies located in extraluminal cervical esophagus, 2 cases (8.7%) of foreign bodies located in the muscular layer of the cervical esophagus and 12 cases (52.2%) of foreign bodies located in pharynx. Twenty-one cases of foreign bodies were removed by cervical lateral incision, in which 11 were removed by cervical lateral incision directly, 10 by the second lateral cervical incision after the foreign bodies were accurately located by MPR and/or corrected MPR, 1 foreign body was removed by incision of the pharyngeal mucosa under suspension laryngoscope, 1 foreign body was removed by tracheoscopy. Compared with patients with intraluminal foreign bodies (n=308) treated in the same period, intake of fishbone [19 (19/23) vs. 133 (82.6% (43.2%, 133/308), OR=7.31] and first visit was more than 24 hours [20(87.0%, 20/23) vs. 77(25.0%, 77/308),OR=17.2] were the significant risk factors of migratory esophageal foreign bodies. Conclusions: MPR and the corrected MPR can accurately locate the migrating pharyngeal and cervical esophageal foreign bodies, by providing more intuitive imaging evidence for doctors, which provide imaging basis for formulation of surgical programs. Foreign bodies in pharyngeal and cervical esophagus need to be treated as soon as possible, otherwise they are easy to migrate, leading lead to serious complications.
The Role Of Endovascular Stent Graft In The Successful Management Of Esophageal Foreign Body With Dangerous Vascular Complications

Abstract:The esophageal foreign body with dangerous vascular complications is a rare but usually fatal disease, including aortoesophageal fistula (AEF), arterio-esophageal fistula, penetrating aortoesophageal injury, artery dissection etc. Although surgical intervention is mandatory to these entities, there is little consensus about the optimal management of these conditions due to rare cases reported. With advent of the endovascular surgical strategy, the notion of treatment has been changed, and the successful cases reported had increased. Herein, we report three cases with dangerous vascular complications. First case is a 31-year-old man accidentally swallowing a pill with an oblong plastic packaging one day before admission and clinical manifestation of retrosternal pain and worsening dysphagia, with the diagnose of potential AEF. The second case is a 58-year-old woman swallowing a duck bone accidentally five days before admission and clinical manifestation of mild odynophagia, with the diagnose of aortic intramural hematoma. The third case is a 46-year-old-man swallowing a fish bone accidentally one day before admission and clinical manifestation of mild odynophagia and abnormal foreign body sensation, with the diagnose of one penetrating aortoesophageal injury. They were all successfully managed by combination of an emergent endovascular stent graft and endoscopic technique, and uneventful with follow-up of 14 months, 19 months and 23 months respectively.

Abstract:Foreign body ingestion is a common problem encountered at gastroenterology clinics and emergency rooms which can cause serious complications. Usually, foreign bodies are directly visible with flexible endoscopes and can be readily removed. However, when foreign bodies migrate into the deeper tissue of the esophagus, surgery is typically required. There is currently no consensus regarding the best treatment. In this report, we present two cases in which fish bones embedded in the submucosal and muscularis propria of the esophagus were successfully removed via endoscopic submucosal dissection (ESD). Both patients were discharged without any complications.

Abstract:Abstract Background Foreign body ingestion is a common emergence in gastroenterology. Foreign bodies are most likely to be embedded in the esophagus. The sharp ones may penetrate the esophageal wall and lead to serious complications. Case presentation A 72-year-old Chinese female was admitted to our hospital with a 4-day history of retrosternal pain and a growing cough after eating fish. Chest computed tomography scan indicated that a high-density foreign body (a fish bone) penetrated through the esophageal wall and inserted into the left main bronchus. First, we used a rigid esophagoscope to explore the esophagus under general anesthesia. However, the foreign body was invisible in the side of the esophagus. Then, the fiberoptic bronchoscopy was performed. We divided the fish bone, which traversed the left main bronchus, into two segments under holmium laser and removed the foreign body successfully. The operation time was short and there were no complications. The patient was discharged 1 week postoperatively and was symptom free even under a liquid diet. Conclusions There are several challenges in the management of this rare condition. We applied the technique of interventional bronchoscopy to the management of esophageal foreign body flexibly in an emergency. A surgery was avoided, which was more invasive and costly.

Multidisciplinary Team in Severe Infectious Complications Caused by Pharyngeal Foreign Body

Abstract:Background: Pharyngeal foreign body is not only a common emergency in people’s daily life, but also a common simple disease in otorhinolaryngology. However, the disease is easy to be ignored due to its common occurrence, which may lead to a series of serious complications including sepsis and local abscess. Case Presentation: In this case, a patient with sepsis, piriform fistula, deep neck abscess, and mediastinal abscess caused by a fishbone was reported. After reviewing the international literature on severe infectious complications caused by foreign bodies in pharynx, the authors emphasize the important role of multidisciplinary team in dealing with complex complications. Result: The whole process of patient from onset to recovery was summarized in a timeline. During hospitalization, the value of leukocyte counts and C-reactive protein in routine blood test was record dynamically. The electronic laryngoscopy, neck computed tomography and chest computed tomography were used to judge treatment efficacy. After about 2 months of hospitalization, the patient was recovered without sequelae. No adverse reactions were found during the follow-up. Conclusion: The incorrect method of removing foreign body after mis-swallowing may develop serious complications. Therefore, It is important to strengthen medical preaching for general population of correct treatments after foreign body ingestion. In addition, the case reminds clinicians to pay more attention to patients’ medical history and details of all medical examinations, which may provide significant clues for making the correct diagnosis and treatment quickly.

Abstract:A 66-year-old woman came to the emergency room with symptoms of foreign-body sensation and pharyngeal pain after the accidental ingestion of a foreign body during breakfast, but she had no idea of what she had swallowed. Computed tomography of the neck, performed to confirm the presence of a foreign body and clarify its relationship with surrounding structures, showed a metallic foreign body in the right pharyngeal cavity ([Fig. 1 a]), and synthesized three-dimensional images of the structures of the neck showed a foreign body located at the level of the fifth cervical vertebra ([Fig. 1 b]). Esophagogastroscopy along the right piriform sinus revealed a needle-like foreign body with one end embedded in the wall ([Fig. 2]).

Abstract:A 37-year-old man was referred to our clinic after swallowing a fishbone that had become stuck in his throat. The fishbone was of Pelteobagrus fulvidraco, a common species in West China. The patient had unsuccessfully attempted to clear away the fishbone by swallowing additional food. Esophagoscopy performed in the local hospital was unable to identify any foreign body. A cervical enhanced computed tomography scan revealed that the entire fishbone had passed through the esophagus to the paravertebral space, with swelling of the surrounding tissues (Fig. 1, Fig. 2). The fishbone was dangerously close to the left vertebral artery. An exploratory surgery by an anterior approach was performed and the fishbone was found in the musculi longus cervicis at the C3~C4 level. The fishbone was removed intact. No esophagus fistula or infection symptom was detected after surgery or at 1 month follow-up. Fig. 2Cervical-enhanced CT reconstruction showed the fishbone was located on the left before the C3~C4 intervertebral space. It was dangerously close to the left vertebral artery. Abstract:Foreign body ingestion is common to children and adults. Once the foreign body embedded in the soft tissues of the pharynx, it will cause diagnostic problems. A 52-year-old woman presented with mild pharyngalgia, pharyngeal paresthesia, and halitosis for 3 months. Laryngoscopy revealed a large non-ulcerated, red subepithelial mass arising from the left hypopharynx. Magnetic resonance imaging (MRI) scan was obtained, with and without contrast, and revealed a mass in the left hypopharynx with inhomogeneous enhancement. (18)FDG PET/CT was performed and revealed increased uptake in the left hypopharynx. A pharyngeal foreign body, a plant stem, was found on the left side of the hypopharynx by a rigid laryngoscope, which was then removed successfully under general anesthesia. The symptoms of the patients were completely relieved and no complications were found at 23 months of follow-up postoperative. A completely embedded pharyngeal foreign body should be considered in cases of pharyngalia, pharyngeal paresthesia and halitosis, although, the images are similar to malignant tumor.

Abstract:Objective:To assess the effectiveness of diagnostic and endoscopy theraputic measures for foreign body in median esophagus. Methods:Using different apparatuses to remove foreign bodies according to the different locations under the vision of endoscopy.Results:In 53 cases, foreign bodies were successfully removed with only one case of mediastinal emphyseme. The other 5 cases (4 fishbones, 1 false teeth) all had a history more than 48 h with signs of perforation. Two of them died after surgery.Conclusion:Removing foreign bodies in median esophagus by endoscopy is effective and safe.

Abstract:Foreign body ingestion is a common occurrence. However, thyroid penetrating injury caused by fishbone migration is rare.In this study, we present a rare misdiagnosed case of a fishbone fixed in the thyroid with a negative diagnosis on laryngoscopy examination. A 52-year-old woman experienced severe and persistent left anterior cervical pain when eating fish. Laryngoscopic imaging at an external hospital, revealed a cyst with a maximum length of 2 cm on the epiglottis, and epiglottic cyst resection under a laryngoscope was performed. The pain was not relieved, and the patient was referred to our hospital. Computed tomography and ultrasound examinations showed a sloping foreign body with a length of approximately 2.5 cm from top to bottom in the left lobe of the thyroid gland. The possibility of a penetrating fishbone injury was considered, and the foreign body was surgically removed. Postoperative drainage, fasting, and antibiotics were prescribed for 3 days, and then the patient was discharged. Follow-ups for 40 days suggested that she had recovered well without complications.The presence of a migratory fishbone should be considered when a patient has a history of fishbone ingestion but laryngoscopy or esophagoscopy assessment of foreign body ingestion is negative.

Abstract:Airway foreign bodies are a common clinical emergency, predominantly occurring in children, with adults less frequently affected. Airway foreign bodies can cause a variety of symptoms, with adults more commonly presenting with cough, a factor that often leads to misdiagnosis or missed diagnosis. Due to anatomical features, most foreign bodies are more likely to be aspirated into the right bronchus, especially the right lower and middle bronchi, with the right upper lung less commonly involved. Here, we report a case where a small, sharp foreign body (a chicken bone) became lodged at the opening of the right upper lobe bronchus in a healthy middle-aged male. Initially, the foreign body was mistaken for an old tuberculous calcification due to recurrent coughing and shortness of breath over two years, misdiagnosed as chronic obstructive pulmonary disease. Eventually, the foreign body was successfully removed. Therefore, the possibility of foreign body inhalation should not be overlooked in adult patients presenting with recurrent cough symptoms.

Abstract:RATIONALE:Accidental ingestion of a foreign body is common in daily life. But the hepatic migration of perforated foreign body is rather rare.PATIENT CONCERNS:A 37-year-old man presented with a history of vague epigastric discomfort for about 2 months.DIAGNOSIS:A diagnosis of the foreign body induced hepatic inflammatory mass was made based on abdominal computed tomographic scan and upper gastrointestinal endoscopy.INTERVENTIONS:The patient underwent laparoscopic laparotomy. During the operation, inflammatory signs were seen in the lesser omentum and segment 3 of liver. B- Ultrasound guided excision of the mass (in segment 3) was performed. Dissecting the specimen revealed a fish bone measuring 1.7 cm in length.OUTCOMES:The patient recovered uneventfully and was discharged on day 5 after surgery.LESSONS:This study shows the usefulness of endoscopy for final diagnosis and treatment in foreign body ingestion. Early diagnosis and decisive treatment in time are lifesaving for patients with this potentially lethal condition.
Endoscopic Management of Foreign Bodies in the Upper Gastrointestinal Tract in South China: A Retrospective Study of 561 Cases

Abstract:Objectives Ingestion of a foreign body (FB) is a common occurrence; however, few studies have reported its characteristics and endoscopic management in South China. The aim of this study is to investigate the clinical features of FB ingestion in South China and report its management through endoscopy. Methods Data were collected from all consecutive patients with suspected FB ingestion accepting endoscopic management in our endoscopy center during the past 10 years. The demographic data, clinical presentations, and endoscopic management were reviewed and analyzed retrospectively. Results A total of 561 patients with suspected FB ingestion underwent endoscopic management. Among them, 447 FBs were found in 439 (78.3%) patients. Fish bone ingestion was most common (60.4%). Most of the FBs were located in the esophagus (84.5%). The majority of patients (96.1%) were successfully treated with flexible endoscopy with the addition of suitable devices without any serious procedure-related complications. There was a significant difference in the types of FBs between the former and latter 5 years ( P < 0.05), and fish bone ingestion became more prevalent with time. Compared with the conventional methods, a new technique with a reworked transparent cap was successful in dealing with small sharp FBs impacted at the entrance of esophagus. The new technique had a shorter operation time (11.7 versus 17.9 min, P = 0.02) and an increased success rate (by 8%). Conclusions In South China, fish bone is the most common FB found in the upper gastrointestinal tract. Endoscopic management is a safe and effective approach in managing FBs. A new technique with reworked transparent cap is useful and promising, but further study is needed for confirmation.
