Abstract:Objective: To investigate the distribution ofH. pylori antigens in the gastric mucosa in patients withH. pylori infection, and the relationship between the distribution and gastric cancer. Methods: Of 112 patients confirmed by pathological study to have chronic superficial gastritis, precancerous changes (chronic atrophic gastritis, intestinal metaplasia or atypical hyperplasia) and gastric cancer, 28 wereH. pylori negative and 84 wereH. pylori positive.H. pylori antigens in the gastric mucosa were detected by immunohistochemistry. Results: TheH. pylori positive group, comprised 12 of 22 (50.0%) in the chronic superficial gastritis group, 22 of 25 (88.0%) in the precancerous changes group and 13 of 35 (37.1%) in the gastric cancer group. The positive rates ofH. pylori antigens in the cytoplasm progressively increased, respectively at 0.0% (0/12), 63.6% (14/22) and 84.6% (11/13) for the same groups (χ2=19.76,P=0.000);H. pylori antigens were located in the mucus layer and above the neck of the mucosal gland in 9 of 12 (75.0%) cases with chronic superficial gastritis, at the neck of the mucosal gland and the isthmus in 12 of 22 (54.5%) cases with precancerous changes, below the isthmus in 9 of 13 (69.2%) cases with gastric cancer (χ2=25.30,P=0.000). In theH. pylori negative group, noH. pylori antigen was observed. Conclusion: With the progression of chronic superficial gastritis→precancerous changes→gastric cancer,H. pylori antigens progressively migrated from the outer part to the inner part of the cell, and from the superficial to the deep gastric mucosa.
Relative Risk of Gastric Cancer Between Those with and Without Helicobacter Pylori Infection History in Japan

Abstract:Background and Aim: The causal relationship between Helicobacter pylori (H. pylori) infection and gastric cancer has been established. Although the magnitude of the carcinogenic effect of H. pylori is the next concern, it has not been sufficiently evaluated in Japan. Spontaneous disappearance of H. pylori infection may have provoked underestimation of the carcinogenic effect of the infection. To reduce the influence, a comparison should be carried out between subjects with and without the infection history. Cutoff values of H. pylori antibody lower than the manufacturer's recommendation are known to be more appropriate to diagnose history of H. pylori infection. The aim was to evaluate the carcinogenic effect of H. pylori. Methods: A case-control study consisting of 275 gastric cancer patients and 275 age- and sex-matched controls was performed. Serum H. pylori antibody was measured using the "JHM-Cap" kit with a domestic antigen (cut value of the manufacturer's recommendation was 2.3 EV: ELISA value). Using a conditional logistic model, the odds ratios (ORs) for five cutoff values adjusted for smoking and drinking doses were calculated. Results: For cutoff values of 1.25, 1.5, 1.75, 2.0, and 2.3 EV, the ORs (95% confidence intervals) were 67.7 (9.1, 502), 37.2 (8.8, 157), 21.3 (9.0, 60.2), 25.5 (9.0, 72.7), and 25.9 (9.2, 73.2), respectively. Conclusions: These results suggest that the risk ratio of gastric cancer between subjects with and without history of H. pylori infection in Japan may exceed 20.
H. pylori Infection and the Anatomic Distribution of Chronic Atrophic Gastritis in a Population at High Risk of Gastric Cancer

Abstract:Objective To determine the relationship between H. pylori infection and the anatomic distribution of chronic atrophic gastritis, a cross-sectional study was carried out in a population at high risk of gastric cancer. Methods From 1989 to 1990, 3 400 residents aged 35~64 in Linqu County, Shandong Province in China received gastroscopic examinations with biopsies taken from 7 standard sites in the stomach and the status of H. pylori infection of each participant was determined by ELSIA. The relationship between H. pylori infection status and the extension of CAG in gastric mucosa was analyzed. Results A total of 1 428 subjects were diagnosed of CAG and 801(56.1%) of those with CAG were serum H. pylori antibody positive. The proportion of CAG with more extensive distribution among H. pylori infection positive was significantly higher than the subjects with H. pylori infection negative (52.2% vs 36.2%, OR 3.2, 95% CI: 2.4～4.4, sex and age adjusted). Conclusion H. pylori infection was more likely to play an important role in on the progression of CAG from local to extensive distribution in the gastric mucosa. This population-based study provides further evidence that H. pylori infection is an important risk factor for the progression of CAG.
Helicobacter Pylori Infection is Associated with Gallstones: Epidemiological Survey in China.

Abstract:AIM: To elucidate the prevalence and risk factors for gallstones, primarily focusing on Helicobacter pylori (H. pylori) infection.METHODS: A total of 10016 Chinese subjects, who had undergone physical examination, fasting C-13 urea breath test and abdominal ultrasonography, had sufficient blood test data, and had finished a questionnaire, were included in this cross-sectional study. Participants (n = 1122) who had previous eradication of H. pylori were studied separately.RESULTS: Gallstones were discovered in 9.10% of men and 8.58% of women, with no significant sex difference. Multivariate analyses displayed that age, aspartate aminotransferase, total cholesterol, H. pylori infection, hepatitis C virus (HCV) infection, and fatty liver had a significant association with gallstones (P < 0.05). Successive multiple logistic regression analysis including index of odds ratio (OR) and standardized coefficient (beta) indicated that older age (OR/beta = 1.056/0.055), H. pylori infection (OR/beta = 1.454/0.109), HCV infection (OR/beta = 1.871/0.123), and fatty liver (OR/beta = 1.947/0.189) had a significant positive association with gallstones. After age stratification, H. pylori infection and fatty liver still had a significant positive association with gallstones in any age-specific groups, whereas HCV infection had a significant positive association in patients aged > 40 years. The prevalence of gallstones among H. pylori -positive, H. pylori -eradicated, and H. pylori -negative subjects was 9.47%, 9.02%, and 8.46%, respectively. The matched analysis showed that gallstones among H. pylori eradicated subjects was significantly lower compared with H. pylori-positive subjects (P < 0.05).CONCLUSION: H. pylori infection and fatty liver have a significant positive association with gallstones. H. pylori eradication may lead to prevention of gallstones.
Helicobacter Pylori Antibodies in Relation to Precancerous Gastric Lesions in a High-Risk Chinese Population.

Abstract:Helicobacter pylori infection is a major cause of gastritis and may be a key risk factor for stomach cancer, but its role in the process of gastric carcinogenesis is not well understood. Herein, we examine H. pylori prevalence in relation to demographic and lifestyle factors and to severity of precancerous lesions in an area of China with one of the highest rates of stomach cancer in the world. H. pylori serum IgG antibody positivity was assayed among 2646 adults, ages 35-64, participating in a population-based gastroscopic screening survey in the high-risk area. The prevalence of positivity was evaluated according to gastric histology, environmental and lifestyle variables determined by interviews during the screening, and level of serum pepsinogens. The odds of advanced precancerous lesions (intestinal metaplasia and dysplasia) of the stomach among those with antibody positivity were estimated by logistic regression. Seventy-two % of the population was H. pylori antibody-positive, with nonsignificant variation by sex, age, income, education, family size, and cigarette smoking habits. H. pylori positivity was higher among those who ate sour pancakes, a fermented indigenous staple that is a risk factor for gastric dysplasia and stomach cancer in this population. The prevalence of H. pylori varied most notably, however, with gastric pathology. The percent of H. pylori positivity increased from 55 to 60 to 87% among those with superficial (nonatrophic) gastritis, mild chronic atrophic gastritis, and severe chronic atrophic gastritis, respectively, before falling to 78% among those with intestinal metaplasia or dysplasia. H. pylori antibody positivity also was strongly correlated with serum pepsinogen concentrations, particularly pepsinogen II, but knowledge of H. pylori status did not markedly improve serological identification of advanced precancerous lesions above that provided by pepsinogen ratios alone. The findings suggest that H. pylori infection contributes to the process of gastric carcinogenesis, particularly during the early stages, in this high-risk area.
Helicobacter Pylori Infection and Gastric Cancer: Evidence from a Retrospective Cohort Study and Nested Case-Control Study in China.

Abstract:AIM:To explore the association between Helicobacter pylori (Hp) infection and risk of gastric cancer in China.METHODS:Utilizing gastroendoscopic biospsy tissue banks accumulated from 1980 to 1988 in Shandong, Zhejiang, and Jiangsu, where stomach cancer incidence was high, during stomach cancer screening conducted by Health Science Center of Peking University, School of Medicine of Zhejiang University, and Zhongshan Hospital of Fudan University. Warthin Starry silver staining method was applied to determine H. pylori infection status of biopsies collected during gastroendoscopic examination. In the retrospective study, the subjects were divided into two cohorts, the exposure cohort was positive H. pylori infection, and the non-exposure cohort was negative. Death from stomach cancer was determined as the outcome of the study. Logistic regression and Cox regression were applied to analyze the association between Helicobacter pylori infection and gastric cancer risk. In the nested case-control study, there were 28 deaths from gastric cancer in the fields of Muping, Shandong province, and Zhoushan, Zhejiang provinces. 4 controls were matched to each case on the basis of age (+/-5 years old), sex, residential place at the same time entered into the study. Conditional logistic regression analysis was used to analyze the data.RESULTS:There were a total of 2 719 subjects (male 1 399, female 1 320) with gastroendoscopic biopsies stored available treated as a cohort. H. pylori positive cohort included 1 671 subjects (61.5 %) and H. pylori negative cohort 1 048 subjects(38.5 %). These subjects were followed up for 1-19 years, averaged 10.88 years. The outcome of death from stomach cancer in the exposure cohort was 33, and in the non-exposure cohort 11. After adjustment for age and sex, RR=1.9850 (P=0.0491), 95 % CI was 1.0026, and 3.9301. The results of conditional logistic regression showed an OR of 4.467 and 95 % CI of 1.161, and 17.190 for the nested case control study.CONCLUSION:The results from the retrospective cohort study and the nested-case control study on the association of H. pylori infection and gastric cancer in China suggested that Helicobacter pylori infection might increase the risk of stomach cancer.

Abstract:AIM:To clarify the association between Helicobacter pylori (H. pylori) infection and the risk of esophageal carcinoma through a meta-analysis of published data.METHODS:Studies which reported the association between H. pylori infection and esophageal cancer published up to June 2013 were included. The odds ratios (ORs) and corresponding 95%CIs of H. pylori infection on esophageal cancer with respect to health control groups were evaluated. Data were extracted independently by two investigators and discrepancies were resolved by discussion with a third investigator. The statistical software, STATA (version 12.0), was applied to investigate heterogeneity among individual studies and to summarize the studies. A meta-analysis was performed using a fixed-effect or random-effect method, depending on the absence or presence of significant heterogeneity.RESULTS:No significant association between H. pylori infection and esophageal squamous cell carcinoma (ESCC) risk was found in the pooled overall population (OR = 0.97, 95%CI: 0.76-1.24). However, significant associations between H. pylori infection and ESCC risk were found in Eastern subjects (OR = 0.66, 95%CI: 0.43-0.89). Similarly, cytotoxin-associated gene-A (CagA) positive strains of infection may decrease the risk of ESCC in Eastern subjects (OR = 0.77, 95%CI: 0.65-0.92), however, these associations were not statistically significant in Western subjects (OR = 1.26, 95%CI: 0.97-1.63). For esophageal adenocarcinoma (EAC) the summary OR for H. pylori infection and CagA positive strains of infection were 0.59 (95%CI: 0.51-0.68) and 0.56 (95%CI: 0.45-0.70), respectively.CONCLUSION:H. pylori infection is associated with a decreased risk of ESCC in Eastern populations and a decreased risk of EAC in the overall population.
Relationship of Helicobacter Pylori Infection to the Expression of Proliferating Cell Nuclear Antigen and Apoptosis in Gastric Carcinoma

Abstract:To determine whether there exist any relationship between Helicobacter pylori (Hp)infection and the expression on proliferating cell nuclear antigen (PCNA) and apoptosis in gastric carcinoma (GC),145 samples of gastric mucosal lesions identified by pathological examinations were tested by using the immunohistochemical testings, the infections with Hp were examined by Warthin-starry method and the TUNEL method was used to detect the occurrence of apoptosis. It was found that the expression percentages of PCNA in chronic superficial gastritis (CSG), chronic atrophic gastritis plus intestinal mataplasia (CAG+IM), dysplasia (DYS),early GC and advanced GC were 25.53%, 51.28%, 61.54%, 66.67% and 68.42% respectively, with a higher positive rates in CAG+IM, DYS and GC. Meanwhile, the apoptotic indexes (AI) in the corresponding groups were 4.55±2.33 %, 6.43±5.60%, 6.45±5.12%, 6.55±4.80%, 8.84±5.63% with a higher rate in advanced GC. As demonstrated in the rank correlation test, the expression of PCNA increased with apoptotic indexes, showing a close relationship. In addition, the expression of PCNA was correlated significantly with histologic types, serosal invasion and lymph node metastasis, that is, its expression was rather higher in Borrmann type IV GC than in early GC or Borrmann type I and II, it also was higher in cases with GC infected with Hp than those cases not infected with Hp, and the survival rates of patients with Hp infection was significantly shorter than those of patients without Hp infection. It is concluded that Hp infection and PCNA expression may be associated with apoptosis and the malignant transformation of gastric carcinoma.

Abstract:To investigate the role of cyclooxygenase (COX)-2/prostaglandin E-2 (PGE(2)) in the process of Helicobacter pylori-induced gastric carcinogenesis, a prospective study based on an intervention trial was conducted in Linqu County, China. A total of 1401 subjects with histopathologic diagnosis were investigated at baseline, among those, 919 completed subsequent interventions (anti-H. pylori and/or celecoxib treatment). Expressions of COX-2 and Ki-67 were assessed by immunohistochemistry, and PGE 2 levels were measured by enzyme immunoassay before and after interventions, respectively. We found a grade-response relationship between COX-2 expression level and risk of advanced gastric lesions at baseline. Stratified analysis indicated an additive interaction between COX-2 expression and H. pylori infection on the elevated risk of advanced gastric lesions. The odds ratios (ORs) for both factors combined were 9.31 [95% confidence interval (CI): 4.13-20.95] for chronic atrophic gastritis, 16.26 (95% CI: 7.29-36.24) for intestinal metaplasia and 21.13 (95% CI: 7.87-56.75) for dysplasia, respectively. After interventions, COX-2 expression and Ki-67 labeling index (LI) were decreased in anti-H. pylori group (OR: 1.65, 95% CI: 1.36-1.99 for COX-2; OR: 1.78, 95% CI: 1.49-2.12 for Ki-67) or anti-H. pylori followed by celecoxib group (OR: 1.41, 95% CI: 1.17-1.70 for COX-2; OR: 1.63, 95% CI: 1.37-1.94 for Ki-67). PGE 2 levels were decreased in all treatment groups. Furthermore, the regression of gastric lesions was associated with the decrease of COX-2 expression or Ki-67 LI after interventions. Our findings indicate that H. pylori-induced COX-2/PGE(2) pathways play an important role on the progression of precancerous gastric lesions in a Chinese population.

Abstract:AIM:To evaluate the relationship between gastric lesions and cell proliferation and apoptosis activity. METHODS:A total of 1523 subjects received an endoscopic examination and five biopsies were obtained from standard sites of the stomach.Ex- pression of Ki-67 was assessed by immunohis- tochemistry,and apoptotic cells were detected with the terminal deoxynucleotide transferase mediated dUTP nick end-labeling(TUNEL) method. RESULTS:Among the subjects screened by en- doscopic examination,93.8% had chronic atro- phic gastritis,intestinal metaplasia or dysplasia. The prevalence rates of gastric lesions varied in different sites of the stomach.In the greater curvature of the body,the proportion of normal mucosa or superficial gastritis was 84.56%,but dysplasia was only 0.07%.In the angulus,the proportion of normal mucosa or superficial gas- tritis was reduced to 19.19%,while the propor- tion of dysplasia reached 5.26%.Furthermore, gastric lesions were positively associated with cell proliferation activity,showing a tendency to increase Ki-67 labeling index according to the se- verity of gastric lesions.However,no significant association was seen between gastric lesion and cell apoptosis(P=0.159). CONCLUSION:Advanced gastric lesions,such as intestinal metaplasia and dysplasia,tend to occur along the lesser curvature of stomach, where gastric cancer occurs.Imbalance in cell proliferation and apoptosis can be observed in advanced gastric lesions.

Abstract:OBJECTIVE:To study the effect of helicobacter pylori on gastric mucosal cell proliferation in gastritis.METHODS:Fifty-six gastritis patients with or without Helicobacter pylori infection (Hp+ 27; Hp- 29) were selected. The expression of proliferation cell nuclear antigen (PCNA), epidermal growth factor receptor (EGFR), transforming growth factor beta receptor type I and type II(TGFbetaRI, TGFbetaRII) in gastric mucosa were examined by immunohistochemical method.RESULTS:The PCNA and EGFR were significantly higher in Hp positive chronic gastritis patients than in Hp negative ones(P<0.05); The TGFbetaRI(P=0.16) and TGFbetaRII(P=0.97) were lower.CONCLUSION:Hp infection promotes over proliferation of gastric mucosal cells.
Characteristics of gastric cancer in peptic ulcer patients with Helicobacter pylori infection

Abstract:Aim: To evaluate the incidence and clinical characteristics of gastric cancer (GC) in peptic ulcer patients with Helicobacter pylori (H. pylori) infection. Methods: Between January 2003 and December 2013, the medical records of patients diagnosed with GC were retrospectively reviewed. Those with previous gastric ulcer (GU) and H. pylori infection were assigned to the HpGU-GC group (n = 86) and those with previous duodenal ulcer (DU) disease and H. pylori infection were assigned to the HpDU-GC group (n = 35). The incidence rates of GC in the HpGU-GC and HpDU-GC groups were analyzed. Data on demographics (age, gender, peptic ulcer complications and cancer treatment), GC clinical characteristics [location, pathological diagnosis, differentiation, T stage, Lauren's classification, atrophy of surrounding mucosa and intestinal metaplasia (IM)], outcome of eradication therapy for H. pylori infection, esophagogastroduodenoscopy number and the duration until GC onset were reviewed. Univariate and multivariate analyses were performed to identify factors influencing GC development. The relative risk of GC was evaluated using a Cox proportional hazards model. Results: The incidence rates of GC were 3.60% (86/2387) in the HpGU-GC group and 1.66% (35/2098) in the HpDU-GC group. The annual incidence was 0.41% in the HpGU-GC group and 0.11% in the HpDU-GC group. The rates of moderate-to-severe atrophy of the surrounding mucosa and IM were higher in the HpGU-GC group than in the HpDU-GC group (86% vs 34.3%, respectively, and 61.6% vs 14.3%, respectively, P < 0.05). In the univariate analysis, atrophy of surrounding mucosa, IM and eradication therapy for H. pylori infection were significantly associated with the development of GC (P < 0.05). There was no significant difference in the prognosis of GC patients between the HpGU-GC and HpDU-GC groups (P = 0.347). The relative risk of GC development in the HpGU-GC group compared to that of the HpDU-GC group, after correction for age and gender, was 1.71 (95%CI: 1.09-2.70; P = 0.02). Conclusion: GU patients with H. pylori infection had higher GC incidence rates and relative risks. Atrophy of surrounding mucosa, IM and eradication therapy were associated with GC.

Abstract:Helicobacter pylori infection is associated with extragastric diseases. The thyroid may be one of the targets of chronic inflammation. Here, we sought to investigate whether H. pylori infections were associated with the presence of thyroid nodules. A total of 988 euthyroid subjects from China were included in this cross-sectional study. Four hundred thirty-five (44.0%) subjects were diagnosed as having thyroid nodules, and 486 (49.2%) were diagnosed with H. pylori infections. The thyroid nodules group had a higher proportion of H. pylori infections than the control group (P = 0.002). Free thyroxine (FT4) levels were lower and the prevalence of thyroid nodules was higher in patients with H. pylori infection compared to those without infection, even after adjustment for age, gender, and body mass index (BMI; all P < 0.05). The prevalence of H. pylori infection showed a decreasing trend as serum FT4 level increased (P-trend = 0.020). Stepwise logistic regression analysis showed that H. pylori infection was significantly associated with the risk of thyroid nodules (odds ratio: 1.390, 95% confidence interval: 1.059-1.824, P = 0.018). Our results suggested that H. pylori infections were positively associated with the presence of thyroid nodules in the euthyroid population, whose thyroid functions were in the reference range.
Promoter Methylation of P16 Associated with Helicobacter Pylori Infection in Precancerous Gastric Lesions: A Population‐based Study

Abstract:To investigate the relationship between p16 methylation and Helicobacter pylori infection in precancerous gastric lesions, a population‐based study was conducted in Linqu County, a high‐risk area of gastric cancer in China. Methylation status of p16 was evaluated by methylation‐specific polymerase chain reaction in 920 subjects with precancerous gastric lesions. H. pylori status was determined by 13C‐urea breath test and the density of H. pylori in biopsy specimens used for detecting methylation status was assessed by the modified Giemsa stain. The frequency of p16 methylation was significantly higher in subjects with H. pylori positive than those with H. pylori negative in each category of gastric lesion (p < 0.001, respectively). Compared with H. pylori negative, the odds ratios (ORs) of p16 methylation were markedly elevated in subjects with H. pylori positive for superficial gastritis (OR, 9.45; 95% confidence interval [CI]: 2.94–30.41), chronic atrophic gastritis (OR, 15.92; 95%CI: 7.60–33.36), intestinal metaplasia (OR, 4.46; 95%CI: 2.44–8.13), indefinite dysplasia (OR, 3.67; 95%CI: 1.90–7.10), and dysplasia (OR, 2.48; 95%CI: 1.02–5.99). Moreover, the frequencies of p16 methylation increased steadily with the severity of H. pylori density in gastric mucosa. Compared with H. pylori negative, the OR of p16 methylation was 1.02–16.13 times higher in subjects with mild H. pylori infection, and 2.69–38.73 times higher in those with moderate/severe infection, respectively. Our findings indicate that p16 methylation was significantly associated with H. pylori infection in precancerous gastric lesions, suggesting that H. pylori infection could potently induce methylation of p16 CpG island. © 2008 Wiley‐Liss, Inc.
Apoptosis and proliferation in gastric epithelium due to Helicobacter pylori: an immunohistochemical and ultrastructural study

Abstract:The effect of H. pylori infection on gastric epithelial cell apoptosis and proliferation is contradictory. Using immunohistochemistry and electron microscopy, this study sought to demonstrate gastric epithelial changes (ie, apoptosis and proliferation) due to chronic H. pylori infection. Methods: Eighteen female 6- to 8-week old Swiss Albino mice were inoculated intragastrically with 3 doses of 10(9) CFU/mL H. pylori prepared in a Brucella Broth in 5 days. Nine others served as a control group. At the end of 28 weeks, tissue specimens from the gastric antrum were excised and examined immunohistochemically (epithelial growth factor for regeneration and Caspase-3 for apoptosis) and electron microscopically. Immunohistochemical assessment was performed using the indirect peroxidase-antiperoxidase method. Results: In the H. pylori-infected group, EGF staining in gastric epithelium was found to be decreased significantly compared to that in control group (P < 0.001). Caspase-3 reactivity was commonly observed in surface epithelial cells and glandular epithelial cells in H. pylori-infected group and totally it was statistically significant compared to Caspase-3 staining in control group (P < 0.001). Electron micrograph images demonstrated numerous apoptotic cells with condensed chromatin. Conclusion: Chronic H. pylori infection of 28 weeks' duration increases apoptosis in gastric epithelium; however, increased apoptosis does not induce proliferation.

Abstract:Helicobacter pylori‐specific proteins are involved in gastric carcinogenesis. To investigate the seroprevalence of six H. pylori‐specific antibodies in patients with different gastric histology, and the impact of seropositivities on the evolution of precancerous gastric lesions, a follow‐up study was conducted in Linqu County, China. The seropositivities for CagA, VacA, GroEL, UreA, HcpC and gGT were assessed by recomLine analysis in 573 H. pylori‐positive subjects and correlated with evolution of precancerous gastric lesions. We found that the score of H. pylori recomLine test was significantly increased in subjects with chronic atrophic gastritis (CAG, p < 0.0001) or intestinal metaplasia (IM, p = 0.0125), and CagA was an independent predictor of advanced gastric lesions, adjusted odds ratios (ORs) were 2.54 (95% CI = 1.42–4.55) for IM and 2.38 (95% CI = 1.05–5.37) for dysplasia (DYS). Moreover, seropositivities for CagA and GroEL were identified as independent predictors for progression of gastric lesions in a longitudinal study, and ORs were 2.89 (95% CI = 1.27–6.59) and 2.20 (95% CI = 1.33–3.64), respectively. Furthermore, the risk of progression was more pronounced in subjects with more than three positive antigens (pfor trend = 0.0003). This population‐based study revealed that seropositivities for CagA and GroEL might be potential markers to identify patients infected with high‐risk H. pylori strains, which are related to the development of GC in a Chinese high‐risk population, and recomLine test might serve as a tool for risk stratification.
[Helicobacter Pylori and the Progression of Gastric Cancer: a 10-Year Cohort Study].

Abstract:OBJECTIVE:To search the role of Helicobacter pylori (H. pylori) in the progression of gastric carcinogenesis.METHODS:A cohort study was conducted by cluster randomized sampling in Linqu County, a rural area in Shandong Province in northeast China, where the mortality of gastric cancer (GC) is among the highest in the world. In 1989-1990, a gastroscopic examination and serum test was launched among subjects aged 35-64 from 14 randomized villages. Histological diagnosis of gastric mucosa and enzyme-linked immunosorbent assay was used to detect gastric lesions and anti-H. pylori of all participants. A repeated gastroscopic screening was offered to all cohort members in 1994 and 1999 respectively, and all the GC cases, regardless of natural ones or ones by gastroscopy, were registered in the follow-up periods.RESULTS:Fifty-eight cases of GC were identified in 2469 subjects during the 10-year follow-up, 44 were observed in 1603 H. pylori-positive persons (2.74%) and 14 in 866 H. pylori-negative persons (1.62%). The incidence rate of GC in H. pylori-positive is significantly higher than in H. pylori-negative (OR = 1.871, 95% CI: 1.012 - 3.459). There was no significant difference in the progression of gastric carcinogenesis between H. pylori-positive and H. pylori-negative (P > 0.05) among subjects with normal or superficial gastritis (SG) at baseline. In contrast, the significant difference was found in baseline chronic gastritis (CAG) group (P < 0.01), and the GC incidence rate in H. pylori-positive (7/615) is higher than in H. pylori-negative (0/470, P = 0.019). The difference of gastric lesions between H. pylori-positive and H. pylori-negative in subjects with intestinal metaplasia (IM) and dysplasia (DYS) at baseline was statistically significant after a 10-year follow-up (P < 0.05, P < 0.01, respectively), but the GC incidence rate in H. pylori-positive is similar to the rate in H. pylori-negative at the two groups (17/573 and 7/224 in IM group, P = 0.907; 19/368 and 7/122 in DYS group, P = 0.806).CONCLUSIONS:H. pylori is a key risk factor that prompts the transition from chronic atrophic gastritis to advanced precancerous lesions and influences the whole progression of precancerous gastric lesions. H. pylori, perhaps can not cause GC directly, increases the risk of GC in company with other carcinogens. H. pylori seemingly has no important effect on the development of GC when gastric mucosa is normal or superficial gastritis, but this conclusion is required to be further confirmed.

Abstract:OBJECTIVE:To explore the relation between H. pylori infection and the development of gastric mucosa lesions, and to evaluate the effect of H. pylori infection on cell proliferation, p53 mutation, and MG-7 antigen expression in patients with gastric precancerous lesions. METHODS:One hundred and nine gastric biopsy specimens were divided into five groups of different gastric mucosa lesions according to pathologic findings and the results of mucosa histochemical staining: type I intestinal metaplasia (IM), type IIIM, type III IM, dysplasia (Dys) and gastric cancer (GC). H. pylori and its cagA status were assessed by microdissection/PCR method. Expression of P53 protein and MG-7 antigen were examined by immunohistochemical staining. AgNOR staining was performed for each specimen. RESULTS:(1) Expression of P53 increased in more severe gastric mucosa lesion groups. The P53 expression rate in GC group was significantly higher than other groups (P < 0.05). There was no difference in expression of P53 between groups of different H. pylori status as while as different cagA status. (2) The expression of MG-7 in GC group was significantly higher than that in other groups (P < 0.05). There were no difference of expression of MG-7 between subgroups of different H. pylori infection status or cagA status in all groups (P > 0.05). (3) The counts of argyrophil protein of the nucleolar organizer regions (AgNORs) were significantly increased in more severe gastric mucosa lesion groups (P < 0.05), and were significantly higher in H. pylori positive and cagA positive specimens than in other specimens (P < 0.05). CONCLUSIONS:Infection with H. pylori, particularly cagA-positive strains, is associated with the development of more severe gastric mucosa lesions; it seems to have effects on cell proliferation in patients with gastric precancerous lesions. Expression of P53 and MG-7 are earlier events in gastric cancer carcinogenesis.

Abstract:BACKGROUND The prevalence of Helicobacter pylori (H. pylori) infection varies by geographic locations. Studies indicate that the infection rate of H. pylori was previously high in China but that rates had been declining worldwide over recent decades. THE AIMS OF OUR STUDY WERE (1) to determine the current prevalence of H. pylori infection among children and adults residing in areas with high (Muping County, Shandong) and low (Yanqing County, Beijing) incidences of gastric cancer in China, and (2) to compare the prevalence for 2006 with the prevalence for the early 1990s. METHODS Using Warthin-Starry silver staining of gastric mucosal biopsy specimens and H. pylori stool antigen tests (HpSA), we tested a total of 2065 asymptomatic children aged 8 - 15 years and adults aged 40 - 79 years in the above two regions from May to July 2006. We evaluated 520 children and 526 adults from Muping, and 516 children and 503 adults from Yanqing. Subjects were selected randomly and H. pylori status was determined by HpSA in children and either HpSA or histology of gastric biopsies in adults. Data obtained in the early 1990s in the same two areas of China were also collected and studied. RESULTS For children, the prevalence of H. pylori infection was significantly higher in Muping (37.69%) than it was in Yanqing (25.58%, P < 0.001). In both regions, the prevalence of H. pylori increased with age but was not related to gender. A significant difference was observed between 8 - 9-years old and 10 - 11-years old (P < 0.05), but not between other adjoining age groups (P > 0.05). From 1991 to 2006 H. pylori prevalence among 8 - 10-year-old children decreased in Muping (60.00% vs 32.07%, P < 0.001), but not Yanqing (24.06% vs 19.10%, P > 0.05). In the adult group, H. pylori prevalence was 50.95% in Muping, which was significantly higher than the 41.35% positive rate in Yanqing (P < 0.01). But there were no statistically significant differences between different age groups of 40 - 49, 50 - 59, and 60 - 79 years, or between males and females. A significant decrease in H. pylori prevalence in both regions was observed when the results of 2006 were compared with the data obtained in 1990 in Muping (50.95% vs 73.78%, P < 0.001) and in 1992 in Yanqing (41.35% vs 55.35%, P < 0.01). CONCLUSIONS After fifteen years, the prevalence of H. pylori infection among both children and adults remained significantly higher in areas with a high incidence of gastric cancer in China compared with that in areas with a low incidence of gastric cancer. H. pylori infection rates have decreased in the general Chinese population during recent years.
Expression of P53 and iNOS in patients with Helicobacter pylori-associated gastric diseases

Abstract:The expression of P53 and iNOS in Helicobacter pylori(Hp) infections and the possibility for Hp infection to induce the development of gastric cancer were investigated bydetecting the expressions of P53 and iNOS in patients with chronic superficial gastritis(CSG),chronic atrophic gastritis(CAG),intestinal metaplasia(IM),dysplasia(DYS) and gastric cancer(GC). In this study,Hp was examined with Gram staining,rapid urease test and serum CagA antibodies,and the expression of P53 and iNOS was detected by immunohistochemical assays.It was found that the positive rates of Hp in cases of CSG,CAG,IM DYS and GC were 45.9%,68.4%,71.4% 75.0% and 54.8% respectively.There was significant difference in the expression rates of P53 and iNOS between CSG and others.The expression rates of P53 and iNOS in Hp infection positive groups were comparatively higher than those in the Hp infection negative groups and there was correlation between Hp infection and the expression of P53 and iNOS.These results suggest that the Helicobacter pylori infection might induce mutation of P53 gene and abnormal expression of iNOS,thus leading to the development of gastric cancer,possibly by inducing proliferation acceleration and cell apoptosis anbormality.
