Correlation between postoperative cognitive dysfunction and peri-operative inflammation in elderly patients undergoing total hip-replacement surgery

Abstract:Objective To observe the incidence of postoperative cognitive dysfunction (POCD) in elderly patients undergoing total hip-replacement and to explore the relationship between POCD and peri-operative inflammation. Methods Thirty-seven elderly patients (all aged more than 60) undergoing selective hip-replacement surgery were enrolled in this study. Neuropsychological testing was performed with mini-mental state examination (MMSE) on the preoperative day and on the 7th postoperative day. The patients were divided into two groups, POCD group and non-POCD group, according to whether POCD occurred in 24 h after the operation. Interleukin (IL)-1β, interleukin-6, tumor necrosis factor-α (TNF-α), and C-reactive protein (CRP) were measured before operation, and 1 and 6 h after operation. Results The incidence of POCD on the 1st postoperative day was 45.9% (17/37). In patients with POCD, the postoperative levels of IL-1β and IL-6 were significantly increased than the preoperative levels (P<0.05, respectively). In patients without POCD, only the postoperative level of IL-6 was significantly increased than the preoperative level (P<0.05), and that of IL-1β showed no significant difference (P>0.05). Compared with the patients without POCD, the level of IL-1β at 1 h after operation and the level of IL-6 at 6 h after operation were significantly increased in the patients with POCD (P<0.05, respectively). The levels of TNF-α and CRP were not significantly different between patients with and without POCD (P<0.05, respectively), though the level of the latter was significantly increased at 6 h after operation than preoperative (P<0.05). Conclusions Elderly patients have a relatively higher incidence of POCD after hip-replacement. Early postoperative inflammation response may be associated with the development of POCD.
