Effects of penehyclidine on balance between cerebral oxygen delivery and consumption during cardiopulmonary bypass

Abstract:Objective To investigate the effects of penehyclidine hydrochloride on the balance between cerebral oxygen delivery and consumption during cardiopulmonary bypass(CPB).Methods Twenty patients scheduled for selective cardiac valve replacement were randomly distributed to two groups receiving an injection of 3mg of penehyclidine hydrochloride (group Ⅰ,n=10) or 5ml 0.9%NS in the priming solution (group Ⅱ,n=10) respectively before CPB. The change of cerebral oxygen extraction rate (CERO_2), arterial-jugular bulb venous lactate difference (DLa-jv) and arterial-jugular bulb venous O_2 content difference(Ca-jvO_2) were calculated according to blood-gas analysis. Clinical complications were collected after operation.Results The DLa-jv was increased after CPB in both groups,but the variation of DLa-jv in group Ⅰ was less than that in group Ⅱ (P<0.05). The cerebral CERO_2 and Ca-jvO_2 were decreased during CPB in both groups.The variation of CERO_2 in group Ⅰ was less than that in group Ⅱ,but the variation of Ca-jvO_2 in group Ⅰ was more than that in group Ⅱ(P<0.05).There was no statistical significance in clinical complications between two groups. Conclusion To some degree, penehyclidine hydrochloride may be better than atropine in maintaining the balance between cerebral oxygen delivery and consumption during CPB.
