Abstract:Aim: To evaluate cardiac function after dilated cardiomyopathy treated by cardiac synchronization therapy used the tissue synchronization imaging technique that based on tissue Doppler imaging technique. Methods: 1 Twelve patients with dilated cardiomyopathy, who were treated with both ventricles synchronous pacing at the Zhongshan Hospital, Fudan University from September 1999 to August 2004, including 10 males and 2 females, and they all agreed to join the experiment. 2 Ten patients could have effective both ventricles pacing (effective group) and 2 patients could not have effective both ventricles pacing (ineffective group). 3 Ejection fraction of the left ventricle was detected with GE Vivid 7 ultrasound imaging system, which had tissue synchronization imaging technique. TVI images of four chamber of cardiac apex, two chamber of cardiac apex and long-axis view before and after cardiac resynchronization therapy for 1 day were collected and kept in the hard disk at the monitoring of electrocardiography. 4 Difference of measurement data was compared with t-test. Results: Twelve patients wit h dilated cardiomyopathy were all involved in the result analysis. 1 After cardiac synchronization therapy, shrinkage of unsynchronization of ventricle in 10 patients with dilated cardiomyopathy improved significantly (P < 0.01), while after cardiac synchronization therapy, shrinkage of unsynchronization of ventricle in 2 patients with dilated cardiomyopathy improved insignificantly. 2 The tissue synchronization imaging technique showed the delayed parts in the patients of effective group were as follows: Post-interval and inferior wall with 2 cases; posterior wall, lateral wall and inferior wall with 3 cases; pre-interval, post-interval and inferior wall with 2 cases; posterior wall and lateral wall with 2 cases; posterior wall with 1 cases. Besides, part implanted wire of left ventricle was at posterior vein of left ventricle, exactly at the delayed part of the ten above-mentioned patients. Delayed parts showed by tissue synchronization imaging technique in the patients of ineffective group were as followed: forearm, pre-interval and pre-lateral wall with 1 cases; inferior wall with 1 case. But the part implanted wire was at posterior vein of left ventricle, did not reach its delayed parts. Conclusion: 1 The effect of cardiac synchronization therapy is related with the coincidence of delayed parts showed by tissue synchronization imaging and implanted place of wire into left ventricle. 2 The tissue synchronization imaging technique can objectively display delayed segment of shrinkage activity of left ventricle, which provide objective evidence for choosing suitable patients performing cardiac synchronization therapy.
