Clinical Study of 47 Patients with Acute Disseminated Encephalomyelitis

Abstract:Objective To evaluate the diagnosis and treatment of acute disseminated encephalomyelitis (ADEM).Methods Clinical features,experimental results,and cranial MRI were analyzed in 47 adult patients with ADEM and the curative effect between the methylprednisolone group and the methylprednisolone plus intravenous immunoglobulin group were compared.Results Of the 47 patients in this study,29(61.7%)experienced a pre- ceding infection.4 cases took repellent(8.51%);in 14 cases(29.79%)no cause could be found.The symptoms and signs typically reached a climax over several days.The most common clinical signs were cognitive impairment in 26 cases(55.32%).In MRI an asymmetrical round focus in the bilateral cerebral white matter could commonly be seen,and the deep grey matter nucleus was also involved.As for the curative effect,there was a marked difference between the methylprednisolone group and the methylprednisolone plus immunoglobulin group.Conclusion ADEM is an acute demyelinating disorder of the central nervous system and brain MRI could mainly be used as a means for early diagnosis.High dose steroids plus intravenous immunglobulin should routinely be employed.The curative effect of steroids plus immunoglobulin is superior to that of mere steroids.
