Abstract:Natural products have been applied to human healthcare for thousands of years. Drug discovery in ancient times was largely by chance and based on clinical practices. As understanding of therapeutic benefits deepens and demands for natural products increase, previously serendipitous discoveries evolve into active searches for new medicines. Many drugs presently prescribed by physicians are either directly isolated from plants or are artificially modified versions of natural products. Scientists are looking for lead compounds with specific structures and pharmacological effects often from natural sources. Experiences and successes of Chinese scientists in this specialized area have resulted in a number of widely used drugs. The tremendous progress made in life sciences has not only revealed many pathological processes of diseases, but also led to the establishment of various molecular and cellular bioassays in conjunction with high-throughput technologies. This is advantageous and permits certain natural compounds that are difficult to isolate and purify, and compounds that are difficult to synthesize, to be assayed. The transition from traditional to empirical and to molecular screening will certainly increase the probability of discovering new leads and drug candidates from natural products.

Abstract:With the rapid development of life science and bioinformatics,clinical biochemistry is in a period of vigorous development under the new situation of medical reform.The work of mutual recognition of test results was carried out smoothly,new guidelines for clinical applications were introduced in succession,testing technology was gradually improved.Automation,molecularization and "omics" study has become three trends in the development of clinical biochemistry,leading clinical biochemistry's development in an all-round way.In this article we reviewed the main events of clinical biochemistry in 2012,and look forward to witnessing that laboratory medicine will play a more important role in the diagnosis and treatment of diseases in 2013.

Abstract:New drug development is a hot point in life sciences. For a developing country like China,it is always economically and socially important to deliver a series of new drugs or repurposed drugs in order to provide people with safe,effective and affordable therapies. Because of low throughput pharmacology /toxicology assays and clinical trials,traditional drug development pipelines are subject to long cycles,high cost and failure risk. As an interdisciplinary subject,bioinformatics shows specific advantage in management and processing of large-scale biological and medical data,at relatively low cost and high throughput. In drug development,bioinformatics is more often used to discover drug efficacy,mechanisms of action and side effects and to guide research and development. Based on the experience of drug development and drug repositioning in international pharmaceutical enterprises, the authors proposed the application of bioinformatics to new drug development and illustrated the importance and necessity of bioinformatics platforms in China.

Abstract:Glucagon-like peptide-1 (GLP-1) receptor agonists are new anti-diabetic agents. They show the beneficial metabolic effects such as lowering blood glucose, reducing body weight, lowering blood pressure and improving lipid profiles. Moreover, they also have the protective effects on several organs or tissues including cardiovascular system, kidney and brain. Recently, Chinese researchers have gained abundant data in clinical and basic studies on GLP-1 receptor agonists for treating diabetes. Here we summarize the progress about this study field in China.

Abstract:The research and development (R&D) of new drugs indicates scientific progress and economic development. However, little is known regarding ongoing or recent clinical trials in China. We analyzed data from clinical trials published before December 31, 2019, and found that the annual registration numbers are increasing annually in the country. Based on clinical indications, most tested drugs target cancers, nervous system, infections, and the cardiovascular system. Furthermore, clinical trials are mostly concentrated in Beijing, Shanghai, and Jiangsu, and conducted by large pharmaceutical companies, with multiple trials for several generic drugs. Going forward, it will be necessary to promote R&D in China of clinically relevant innovative drugs, drug delivery systems, and novel traditional Chinese medicine (TCM) and biological products, as well as to have a balanced distribution of clinical trials to sustainably meet public health needs.

Advances in clinical trials of diabetes drugs research in China, 2019 to 2021

Abstract:Objective:To explore the latest development of clinical trials of diabetes drugs in China from 2019 to 2021.Methods:Information based on the platforms for drug clinical trial registration and information and accepted variety catalog registration information of Center for Drug Evaluation ( http://www.chinadrugtrials.org.cn/). Extracting the information of clinical trials of diabetes drugs during 2019 to 2021, including basic information, management information, the scope and stage of the trial, type of the sponsor and drugs.Chi-square was used to compare the counting data between domestic and foreign pharmaceutical enterprises. Results:Five hundred and eighty nine clinical trials of diabetes drugs registered in China during 2019 to 2021, accounted for 7.36% (589/8 002) of all varieties. From the scope of trials, domestic trials accounted for 96.26%(567/589), international multi-center trials taked up 3.74% (22/589). From the type of sponsors, domestic pharmaceutical companies initiated 542 (92.02%) and among which, global pharmaceutical companies initiated 47 (7.98%). Analyze by the phase of the trials, the bioequivalence test was 50.76% (299/589), the phase Ⅰ clinical trial was 29.54% (174/589), and the phase 2 and 3 trials were 19.00% (112/589). From the perspective of the type of the drugs, innovative drugs were the most common type of drugs (34.13%, 201/589), generic drugs accounted for 26.66% (157/589) and imported medicine occupied 6.11% (36/589). According to registration classification were divided into chemical drugs (77.42%, 456/589), biological products (21.22%, 125/589) and traditional chinese medicine/natural medicine (1.36%, 8/589). The main focus in terms of the type of drug being tested was dipeptidyl peptidase Ⅳ inhibitor (105/589, 17.83%), glucagon-like peptide-1 receptor agonist (65/589, 11.04%), sodium-glucose cotransporter 2 inhibitor (64/589, 10.87%), insulin and insulin analogues (53/589, 9.00%), compound preparations (49/589, 8.32%) and metformin (48/589, 8.15%).Conclusions:There are a large number of diabetes clinical drug trials in my country from 2019 to 2021, and the innovation of new drug research and development has been greatly improved. Some multinational companies are introducing early research on new drugs into China actively.

Abstract:In China, research in drug design began in the late 1970s and has flourished in the early 2000s through several generations of effort, which has placed the research level of China in this field at the global forefront. With the application of computer-aided drug design approaches, many small-molecule drug candidates have entered the clinical trial stage, and some have been approved for therapeutic use by the National Drug Administration of China. This paper reviews the development of drug design in China and summarizes the main achievements in this field over the past 40 years in terms of establishment, growth, maturation, and thriving stages.

Abstract:Clinical trials on innovative drugs in China have witnessed a stage of rapid development in recent years. The introduction of a number of government policies has stimulated enthusiasm for research and development in the pharmaceutical industry. We analyzed the data of the early-phase clinical trials registered in the Chinese Center for Drug Evaluation (CDE) from September 6, 2013, to December 31, 2022. All related registration information disclosed on the CDE website, including posted time, drug classification, dosing formula, indications, trial design, and trial status, were summarized and analyzed. A total of 5336 early-phase clinical trials were identified. The quantity and growth rate of early-phase clinical trials have increased substantially each year. Chemical drugs accounted for the largest proportion of early-phase clinical trials, although it dropped from 85.7% in 2013 to 59.5% in 2022. Moreover, the number of oncology drugs has increased yearly, accounting for 49.2% of all early-phase clinical trials in 2022. We can conclude that during 2013–2022, the development of early-phase clinical trials has progressed due to government support and advanced development techniques. To enhance the efficiency and success rate of early-phase clinical trials in the future, it is necessary to acquire advanced technical support and improve standardized supervision systems.

Abstract:MOTIVATION:Traditional Chinese medicine (TCM) is gaining increasing attention with the emergence of integrative medicine and personalized medicine, characterized by pattern differentiation on individual variance and treatments based on natural herbal synergism. Investigating the effectiveness and safety of the potential mechanisms of TCM and the combination principles of drug therapies will bridge the cultural gap with Western medicine and improve the development of integrative medicine. Dealing with rapidly growing amounts of biomedical data and their heterogeneous nature are two important tasks among modern biomedical communities. Bioinformatics, as an emerging interdisciplinary field of computer science and biology, has become a useful tool for easing the data deluge pressure by automating the computation processes with informatics methods. Using these methods to retrieve, store and analyze the biomedical data can effectively reveal the associated knowledge hidden in the data, and thus promote the discovery of integrated information. Recently, these techniques of bioinformatics have been used for facilitating the interactional effects of both Western medicine and TCM. The analysis of TCM data using computational technologies provides biological evidence for the basic understanding of TCM mechanisms, safety and efficacy of TCM treatments. At the same time, the carrier and targets associated with TCM remedies can inspire the rethinking of modern drug development. This review summarizes the significant achievements of applying bioinformatics techniques to many aspects of the research in TCM, such as analysis of TCM-related '-omics' data and techniques for analyzing biological processes and pharmaceutical mechanisms of TCM, which have shown certain potential of bringing new thoughts to both sides.

Abstract:53 New drugs including 38 chemical entities and 15 biologics were approved by the U.S. Food and Drug Administration during 2020. Among the marketed drugs, 34 new small molecule drugs and 4 new diagnostic agents with privileged structures and novel clinical applications represent as promising leads for the development of new drugs with the similar indications and improved therapeutic efficacy. This review is mainly focused on the clinical applications and synthetic methods of 34 small molecule drugs newly approved by the FDA in 2020.

[Application of bioinformatics in study of Chinese medicine resources]

Abstract:In the late 20th century, the rapid development of life sciences resulted masses of biological data, in which laws of life sciences urgently needed to be excavated. Together with the progress of computer science and technology, bioinformatics was born. Chinese medicinal resources, especially the plant resources, are the basis for the development of the Chinese pharmaceutical industry. As a new discipline and important life science research tools, bioinformatics applied rapidly in the study of Chinese medicine resources and involved in different aspects of genomics in recent years. Bioinformatics provides a new research approach to reveal the biological principles, methods and techniques and powerful tools of excavation, at the same time it is speeding researches in the field of Chinese medicine resources and the development of the industry. This article summarizes a comprehensive collection of bioinformatics application data in the study of Chinese medicine resources, prospects the application in Chinese medicine resources, and provides a basis bioinformatics information for the study of Chinese medicine resources.
Trends of Phase I Clinical Trials of New Drugs in Mainland China Over the Past 10 Years (2011–2020)

Abstract:Background: In recent years, the number of clinical trials initiated in China has increased rapidly. The aim of this study was to overview the changing landscape of phase I clinical trials in mainland China from 2011 to 2020. Methods: We analyzed phase I clinical trials registered on 3 websites including the Chinese Clinical Trial Registry, ClinicalTrials.gov , and the China National Medical Products Administration Center for Drug Evaluation platform. Findings: A total of 2,842 phase I clinical trials were posted from January 1, 2011, to December 31, 2020. The overall number of clinical trials for innovative drugs was 1,497, accounting for half of all the phase I clinical trials (53%). Among these 1,486 innovative drug clinical trials, 924 were newly tested drugs with an average annual growth rate of 59%. Biological drug research increased significantly from 22.6% during 2011–2015 to 33.3% during 2016–2020. These principal investigators (PIs) of these clinical trials were mainly from Beijing ( n = 871), followed by Shanghai ( n = 496) and Jiangsu ( n = 281). As for the therapeutic area of phase I clinical trials, cancer took up the most percentage of all the clinical trials (35%), followed by infectious disease (9%), nervous system disease (9%), etc. Most phase I clinical trials are conducted on healthy volunteers ( n = 1,642, 57.8%), some cancer drugs are conducted in patients with cancer ( n = 846, 29.8%), and only a few clinical trials were conducted in the elderly ( n = 7). Among these clinical trials of the newly tested innovative drugs, the first in human (FIH) clinical trials accounted for 82% (744), and the First in Chinese (FIC) clinical trials only took up 18% (167). Only a small number of drugs could be made the transition to phase II ( n = 207, 22%). In addition, despite the number of newly tested drugs during 2011–2015 ( n = 163) was much less than that in 2016–2020 ( n = 761), the percentage of drugs that could enter into phase II clinical trials in 2011–2015 (34%) was higher than that in 2016–2020 (20%). Conclusion: In the past 10 years, the development of phase I clinical trials has achieved great progress in mainland China due to the novel design and drug innovation policy. Nevertheless, future efforts are needed to make for improving the phase transition success rate of innovative drugs.

Abstract:Since the 50s in the last century,cancer gradually became one of the priorities in public health,and new anticancer drugs have been studied in this field.Due to lack of proper guidelines in the early days,the development of new anticancer drugs was slow and difficult.Only after the mid of 80s,clinical trials were started according to GCP principles designed by the Bureau of Drugs under Ministry of Health.Several effective new anticancer drugs were tested and available in the hospital for cancer patients.Since the establishment of State Food and Drug Administration (SFDA) ten years ago,number of clinical trials reached more than 100 every year in recent 5 years,and some of them were global multi-centers trials.We have developed several new drugs such as endostatin (Endostar) and Rg3 as anti-angiogenesis agents,and the active components from Chinese medicinal herbs are also marketed such as Kanglaite and -elemene.New targeted drugs,such as Eketinib and Nimotuzumab,are currently under clinical trial.There are 12 new drugs or approaches developed in China,which have improved the clinical practice and bring benefit to cancer patients in the last 5 decades.Several basic principles of traditional Chinese medicine are integrated into the individualization of clinical oncology practice such as disease treatments and different diseases same treatment.It is concluded that the development of new anticancer drugs should be one of our priorities and innovation in clinical oncology can save life.

Abstract:As the second largest pharmaceutical market with a great potential for future growth, China has drawn much attention from the global pharmaceutical community. With an increasing government investment in biomedical research, the domestic biopharmaceutical (biotechnological) companies in China are turning their attention to the development of innovative medicines and targeting the global market. To introduce innovative products to Chinese patients sooner, to improve the efficiency of its review and approval processes, and to harmonize its regulatory science with international standards, the China Food and Drug Administration (CFDA) has initiated a series of major changes to its policies and regulations. This paper presents a snapshot of China’s pharmaceutical market, and research and development status, and introduces technical guidelines pertaining to clinical trials and new drug applications. The recent wave of ground-breaking reforms in CFDA’s regulatory science is discussed. Examples of clinical trials and new drug applications are provided throughout the discussion.

Abstract:Antibacterial resistance is a global health threat that requires further concrete action on the part of all countries. In this context, one of the biggest concerns is whether enough new antibacterial drugs are being discovered and developed. Although several high-quality reviews on clinical antibacterial drug pipelines from a global perspective were published recently, none provides comprehensive information on original antibacterial drugs at clinical stages in China. In this review, we summarize the latest progress of novel antibacterial drugs approved for marketing and under clinical evaluation in China since 2019. Information was obtained by consulting official websites, searching commercial databases, retrieving literature, asking personnel from institutions or companies, and other means, and a considerable part of the data covered here has not been included in other reviews. As of June 30, 2023, a total of 20 antibacterial projects from 17 Chinese pharmaceutical companies or developers were identified and updated. Among them, two new antibacterial drugs that belong to traditional antibiotic classes were approved by the National Medical Products Administration (NMPA) in China in 2019 and 2021, respectively, and 18 antibacterial agents are in clinical development, with one under regulatory evaluation, five in phase 3, six in phase 2, and six in phase 1. Most of the clinical candidates are new analogs or mono-components of traditional antibacterial pharmacophore types, including two dual-acting hybrid antibiotics and a recombinant antibacterial protein. Overall, despite there being 17 antibacterial clinical candidates, our analysis indicates that there are still relatively few clinically differentiated antibacterial agents in stages of clinical development in China. Hopefully, Chinese pharmaceutical companies and institutions will develop more innovative and clinically differentiated candidates with good market potential in the future research and development (R&D) of original antibacterial drugs.

Abstract:With the rapid innovation of nanoscience and technology, nanomaterials have also been deeply applied in the medical and health industry and become one of the innovative methods to treat many diseases. In recent years, bioactive nanomaterials have attracted extensive attention and have made some progress in the treatment of some major chronic diseases, such as nervous system diseases and various malignant tumors. Bioactive nanomaterials depend on their physical and chemical properties (crystal structure, surface charge, surface functional groups, morphology, and size, etc.) and direct produce biological activity and play to the role of the treatment of diseases, compared with the traditional nanometer pharmaceutical preparations, biological active nano materials don't exert effects through drug release, way more directly, also is expected to be more effective for the treatment of diseases. However, further studies are needed in the evaluation of biological effects, fate in vivo , structure-activity relationship and clinical transformation of bionanomaterials. Based on the latest research reports, this paper reviews the application of bioactive nanomaterials in the diagnosis and treatment of major chronic diseases and analyzes the technical challenges and key scientific issues faced by bioactive nanomaterials in the diagnosis and treatment of diseases, to provide suggestions for the future development of this field.

These marketed new drugs represent privileged structures and novel action of mechanism, and thus can be served as leads to discover new drugs with the similar biological targets and improved therapeutic efficacy. This review aims to provide an overview regarding the synthetic approaches of 33 new chemical entities approved by the FDA in 2019 and their clinical applications.
