Correlation of Pulmonary Artery Systolic Pressure With Left Ventricular Ejection Fraction and Other Cardiac Function Parameters

Abstract:Objective:To evaluate the correlation of pulmonary artery systolic pressure(PASP) with left ventricular ejection fraction(LVEF),NYHA classification,N-terminal pro-B-type natriuretic peptide(NT-proBNP),and to explore the clinical value of PASP on cardiac function.Methods:Our retrospective study recruited 695 consecutive patients treated in our hospital from January to August 2009.The patients were divided into two groups,LVEF<0.5 group,n=70,and LVEF≥0.5 group,n=625.The echocardiography,plasma NT-proBNP level,NYHA classification and other basic clinical information were studied and compared in both groups.PASP was derived from the tricuspid regurgitation velocity.Results:Multi regression analysis indicated that PASP had no significant correlation with LVEF in LVEF<0.5 group,while it had positive correlation with LVEF in LVEF≥0.5 group.PASP was positively correlated to NYHA classification and lgNT-proBNP in LVEF<0.5 group and LVEF≥0.5 group respectively(r=0.584,P<0.001 and r=0.405,P<0.001);(r=0.642,P<0.001 and r=0.420,P<0.001).NYHA classification and NT-proBNP were increased and LVEF was decreased in patients with secondary pulmonary hypertension than those without it(P<0.001 respectively) in both groups.Conclusion:PASP had positive correlation with NYHA classification and plasma NT-proBNP.When LVEF≥0.5,it had correlation with PASP,which presented that LVEF was not as higher as better.Keeping LVEF at a proper range might be good for controlling the pulmonary pressure.Echocardiography was necessary to estimate PASP through tricuspid regurgitation.
