The CT Findings of Pulmonary Complications in 32 Hematopoietic Stem Cell Transplantation Recipients

Abstract:Aim:To investigate the CT findings of pulmonary complications in hematopoietic stem cell transplantation(HSCT) recipients.Methods: The CT findings of pulmonary complications in 32 HSCT recipients were studied.Results: Twenty-two episodes of pulmonary complications which were final diagnosed or clinical diagnosed were documented in 21 patients.Among them,pulmonary complications were due to fungal pneumonia(n=13),cytomegalovirus pneumonia(n=3),bacterial pneumonia(n=3),bronchiolitis obliterans(BO,n=1),leukemic pulmonary infiltration(n=1) and lung sarcoidosis(n=1).Regarding the fungal pneumonia,the most frequent features were nodules,masses,or nodules and masses,and there were 18 cavitations in the course of fungal pneumonia,some cavitaties had septatums pattern or were surrounded with halo sign among them.Areas of ground-glass opacification were found in all the patients with cytomegalovirus pneumonia,Air-space consolidation and reticular changes were also identified.The most common CT findings of bacterial pneumonia were Air-space consolidation and nodule.In BO,CT revealed bronchial dilatation and bronchial wall thickening.The CT finding of leukemic pulmonary infiltration was single nodule surrounding with halo sign.Lung sarcoidosis showed mediastinal and hilar adenopathy,patchy of Air-space consolidation co-existed with miliary nodules.In these 32 patients,we identified 3 cases(9.4%) presenting with a "crazy-paving" pattern,the 3 patients died from respiratory failure within 40 days after the CT examination.Conclusion:In the 32 HSCT recipients,the CT findings of pulmonary complications are various,the cavitation with septatums pattern or halo sign strongly suggests fungal pneumonia,the "crazy-paving" pattern probably suggests poor prognosis.
