Abstract:Iatrogenic bile duct injuries (BDIs) and subsequent benign biliary stricture is a medical catastrophe which is associated with significant perioperative morbidity and mortality,reduced long-term survival rate and poor quality of life.For most major BDIs (Strasberg classification E1-E4),the recommended method of repair is hepaticojejunostomy (HJ).We conducted a retrospective review aiming to examine the surgical technique of high HJ at our institution.This review highlights 4 aspects in the operation which include the hepatoduodenal ligament exposure,hepatic artery and its branches protection,exposing the intrahepatic bile duct above the stricture plane,and HJ techniques.Overall,the optimal long-term result of surgical management depends on the availability of experienced hepatobiliary surgeons and a considerable large HJ anastomosis above the stricture.

Abstract:For the characteristics including minimal invasion,low incidence of complication and better postoperative prognosis,endoscopic therapy is widely accepted to be the first line therapy for most biliary strictures.At present,repeated progressive dilation combined with multiple plastic stents placement is the main therapy for benign biliary strictures.The long-term effective rate is relatively high in surgery related strictures,but in the non-surgery related strictures,its long-term effective rate is limited and the optimal therapy should be chosen cautiously according to patients' condition.In malignent biliary stricutes,dilation combined with metallic stents placement is recommended to treat both of preoperative long-term drainage in patients with resectable tumor and palliative treatment in patients with unresectable tumor,while nosobiliary drainage is recommended in preoperative short-term drainage. The choice of unilateral or bilateral drainage depends on patients' conditons in malignent hilar biliary strictures.The merits and demerits of stents should be mastered before they are placed in stricture sections.Recently,a lot of new endoscopic technologies and methods are applied in clinical practice,but their curative effect should be tested.Therefore,accumulative evidence-based data are needed to make them more rational and more standard.

Abstract:Biliary stricture is one of the most common complications after liver transplantation,and an important cause of morbidity and mortality in transplant recipients.Bile duct stricture after liver transplantation is divided into anastomotic stricture and non-anastomotic stricture according to the stricture site.The endoscopic approach is currently the preferred initial treatment method for patients undergoing duct-to-duct biliary reconstruction.Previously,endoscopic management used to rely on balloon dilatation and single or multiple stents placement.Recently,a great progress has been achieved by the application of endoscopic devices and novel biliary stents,as well as the advances in endoscopic technologies including the rendezvous technique and magnetic compression anastomosis.This paper reviews the research progress of the occurrence,classification,cause,risk factor,clinical manifestation,diagnosis and treatment of bile duct stricture after liver transplantation.

Abstract:Introduction: Postoperative biliary stricture (POBS) is one of the common complications of biliary surgery. Previous literature on risk factors of POBS was scarce, and the classification of POBS in benign and malignant biliary diseases was incomplete. Aim: To analyze clinicopathological factors of POBS in usual biliary diseases, and to facilitate preoperative diagnosis of biliary stricture. Material and methods: A retrospective analysis was made on the clinical data of 2228 patients who underwent biliary surgery in our hospital from July 2010 to June 2022. With the inclusion and exclusion criteria, the clinicopathological factors for POBS were classified, and data analysis was conducted. Results: Benign diseases with age >= 60 years (p = 0.034), diabetes (p = 0.001), common bile duct diameter < 0.8 cm (p = 0.034), Mirizzi syndrome (p = 0.001), seniority of surgeons < 25 years (p = 0.001), and operation time for the first 6 years (p = 0.015) are more likely to evolve into POBS; malignant diseases with conjugated bilirubin >= 6.8 mu mol/l (p = 0.042), alkaline phosphatase >= 125 U/l (p = 0.042), gamma-glutamyl transferase >= 50 U/l (p = 0.047), diabetes (p = 0.038), and seniority of surgeons < 25 years (p = 0.008) are prone to POBS. Different surgical approaches affect the incidence of POBS (chi 2 = 9.717, p = 0.034). The choice of surgical site is important for the incidence of POBS in malignant diseases (chi 2 = 7.935, p = 0.041). Conclusions: Surgeons need to identify risk factors, conduct patient visits and assessments preoperatively, standardize the operation in order to avoid structural damage, and reduce the occurrence of POBS.

Abstract:Malignant obstructive jaundice is biliary obstruction disorders which are caused by various malignant tumors. Usually the disease is at its advanced stage and is inoperable when the diagnosis is confirmed. At present, percutaneous transhepatic biliary drainage(PTCD) and endoscopic or interventional implantation of plastic or self-expanding metal stent(SEMS) are the main managements in clinical practice.Due to the improved survival time, biliary stent restenosis has become a quite common clinical problem.Photodynamic therapy(PDT) and biliary tract radiofrequency ablation(RFA) have provided new therapeutic means for clinical use. Especially, with its development in technology and equipment, RFA has played more and more important role in treating biliary stent restenosis.

Abstract:Objective To investigate the efficacy of endoscopy for the treatment of benign biliary stricture after biliary surgery.Methods The clinical data of 127 patients with benign biliary stricture after biliary surgery at the First Hospital of Lanzhou University from January 2007 to December 2011 were retrospectively analyzed.According to the Bismuth classification,there were 60 patients with type Ⅰ,35 with type Ⅱ,21 with type Ⅲ and 11 with type Ⅳ.The efficacies of endoscopy for the treatment of biliary stricture with different Bismuth subtypes were analyzed.Results The location and severity of biliary stricture were confirmed by endoscopic retrograde cholangiopancreatography (ERCP) + cholangiography.Sixteen patients ( including 7 with type Ⅲ and 9 with type Ⅳ) were transferred to surgical treatment due to severe biliary stricture.A total of 111 patients underwent endoscopic treatment successfully,with the success rate of 87.4％ (111/127).The success rates of endoscopy for the treatment of patients with Bismuth Ⅰ,Ⅱ,Ⅲ and Ⅳ biliary strictures were 95％ (57/60),86％ (30/35),9/14and 1/2,respectively.Twenty-nine patients were implanted with retrievable metallic biliary stent,and 82 were implantated with plastic biliary stent.Of the 111 patients,only 6 patients were complicated by acute pancreatitis,and they were cured by conservative treatment.The alleviative rates of yellow skin and icteric sclera,tenderness and distending pain of right upper quadrant,fever were 73％ (81/111 ),83％ (74/89),90％ (73/81 ) and 89％(68/76) at 1 week after treatment,and they were 88％ (98/111),91％ (81/89),94％ (76/81) and 92％(70/76) at 8 weeks after treatment.The efficacy of endoscopy was good in 97 patients and poor in 14 patients,and the 14 patients were converted to open surgery.The symptoms including yellow skin and icteric sclera,tenderness and distending pain of right upper quadrant,fever were completely alleviated at postoperative month 6.Conclusion Endoscopic treatment for benign biliary stricture is safe and effective.

Abstract:Bile duct strictures can prove to be an extremely difficult diagnostic problem. The majority of Benign bile duct strictures (80%) are unfortunately iatrogenic, the injury occurring after a routine laparoscopic (incidence 0.4-0.6%) or open (incidence 0.2-0.3%) cholecystectomy. The existence of a number of anatomical variations add to the incidence of biliary injury either in isolation or with associated vascular injury. There are also a number of other causes of both iatrogenic and non-iatrogenic causes of benign bile duct strictures. Improved imaging modalities have aided the recognition of the anatomical extent of the stricture and in planning the treatment. Classification of bile duct injury was described by Bismuth in 1982, further modifications were described by Strasberg in 1995. Associated vascular injuries were classified both by Stewart-Way and Bektas et al in the Hannover Classification in 2007. Treatment of biliary stricture depends on the time of diagnosis and type of final injury. While surgery provides the mainstay of treatment, certain select conditions may lend themselves to endoscopic therapy and interventional therapy.

Abstract:OBJECTIVE: To understand the common types of biliary tract prosthesis, and to evaluate clinical effects of biliary tract prosthesis in treatment of various reasons-induced bile duct obstruction. METHODS: We retrieved Chinese Journal Full-text Database (1999-01/2009-06) with the Key words of "biliary tract prosthesis, obstruction, mental" in Chinese. Biochemical indicator improvement of biliary tract prosthesis in treatment of bile duct obstruction and jaundice decrease rate were used as assessment index. Clinical studies addressing biliary tract prosthesis implantation for treating bile duct obstruction were included, and animal studies were excluded. RESULTS: A total of 188 literatures were primarily obtained by computer. According to inclusion and exclusion criteria, clinical studies concerning biliary tract prosthesis in treatment of bile duct obstruction were analyzed. Biliary tract prosthesis comprised plastic internal prosthesis and mental stent. Short-term outcomes were significant in the clinic, but long-term outcomes were affected by stent displacement, restenosis and so on. Presently designed novel stent-graft could block tumor growth by the tectorial membrane, prevent the formation of biliary silt and calculus to delay stent restenosis. Studies on degradable stent had obtained some progresses, especially on benign biliary stricture, which avoided the trouble of taking out again. However, the tumor could break through degradable tectorial membrane and grew into the biliary duct, finally resulting in restenosis or occlusion. Using percutaneous transhepatic cholangiography, biliary tract mental stent implantation had obtained high success rate, was safe and effective, could effectively keep drainage smooth in the biliary tract, reduce jaundice, improve quality of life of patients, and prolong patient's life. CONCLUSION: Compared with plastic stent, mental biliary tract prosthesis has advantages in keeping long-term opened rate of the bile duct, decreasing displacement, improving quality of life, and reducing total expense. Various types and implantation methods should be used in clinic in accordance with the local and general conditions of the patient, which can elevate safety and effectiveness, and reduce the occurrence of complications.

Abstract:Objective　To evaluate the methods for the diagnosis and treatment of biliary malignant tumor(BMT) so as to achieve improvement. Methods　Various methods of diagnosis and treatment of BMT were comprehensively analysed and evaluated. Results and Conclusion　The present laboratory examinations do not show obvious help for the early diagnosis of BMT. Both bile cytology and neoplasm histology may define the character of tumor but the positive rate is low. Although imaging examinations have difficulty in finding early tumor, it may help to define the position, size, infiltration and metastasis of the neoplasm. To diagnose the tumor in its early stage, the monitor and management of some diseases related to BMT should be enhanced, and tumor markers with high susceptibility and specificity should be explored. Operation is the major choice treatment. For advanced neoplasm patients, radical operation or extented radical operation can bring great help for some cases. Palliative surgery may be performed on the patients unable to accept radical operation, and biliary drainage may be performed on the unresectable tumor. The application of special surgery apparatus, such as Peng's Multifunctional Operative Dissector(PMOD), may increase the resection rate of tumor and lymph nodules. Radiotherapy and chemotherapy need further exploration. Gene therapy may likely bring about new hope for the treatment of BMT in the future.

Abstract:With the application and development of new ideas and new technologies,great progress has been made in the diagnosis and treatment of benign biliary diseases.Surgical treatment of benign biliary tract disease mostly have a consensus,but there is still controversy,such as the timing of surgery of cholecystolithiasis complicated with acute cholecystitis,surgery of congenital biliary dilatation,cholangioenterostomy of recurrent extrahepatic bile duct stones,the value judgment in the treatment of bile duct injury,the choice of endoscopic therapy and surgery of benign tumors of ampulla.In view of the current dispute,biliary surgeon should establish the multidisciplinary team diagnosis and treatment model,then reach consensus and conduct multi center and large sample clinical research,finally propose the guidelines that can provide clinicians with reasonable,standardized therapeutic schemes and provide patients with the best,personalized treatment.

Abstract:Biliary endoprosthesis, including plastic stents and metallic stents, have been widely used in the management of various benign or malignant bile duct stenosis. Although the short-term effect is most satisfactory, the long-term result is unsatisfactory because of the displacement and restenosis of the stent. Metallic stent is superior to plastic stent in keeping the stent open for long time and the technique has been improved gradually. This paper aims to make a review of the related literatures and to summarize the recent progress in the applied research.

Abstract:Objective To explore the mechanism of benign biliary stricture.Methods A model of trauma of bile duct was established in dogs and then repaired.The expression and distribution of MΦ,TGF-β_1 and α-SMA in tissues at different time phases of the healing process were studied after the operation with immunohistochemical SP staining.Results High expression of MΦ,TGF-β_1 and α- SMA were observed in the healing process and lasted for a long time.MΦ staining was found in the in- trinsic layer under mucosa,TGF-β_1 staining in the granulation tissue,fibroblasts and endothelial cells of blood vessels while α-SMA staining in the myofibroblasts.Conclusions The high expression of MΦ,TGF-β_1 and α-SMA is closely related to active proliferation of fibroblasts,extracellular matrix overdeposition and scar contracture of bile duct.

Abstract:Biliary stricture is defined as the reduction and narrowing of the bile duct lumen, which can be caused by many factors such as cancer and inflammation. Biliary stent placement can effectively alleviate benign and malignant biliary strictures. However, the commonly used plastic or metallic biliary stents are far from ideal and do not satisfy all clinical requirements , although several types of biodegradable biliary stents have been developed and used clinically. In this review, we summarized current development status of biodegradable stents with the emphasis on the stent materials. We also presented the future development trends based on the published literature.

Abstract:The primary goal of treatment for benign biliary strictures (BBSs) is to resolve bile duct obstruction, to achieve long-term ductal patency, and to maintain liver function. The endoscopic approach has become the first-line option for most cases of BBS. Biliary drainage can be maintained through a combination of endoscopic stricture dilation and biliary stenting. The effectiveness, clinical success, and outcomes of endoscopic intervention for BBSs largely depend on specific etiologies, endoscopic techniques used, and the selection of appropriate accessories and stents, which varies in the literature.

Abstract:The mechanics of bile flow in the biliary system plays an important role in studying bile stasis and gallstone formation. Bile duct stricture is an abnormal phenomenon that refers to the bile duct getting smaller or narrower. The main objective of this study is to study the influence of stricture on bile flow dynamics using numerical methods. We employed a numerical Computational Fluid Dynamics model of the bile flow within a strictured hepatic duct. We studied and compared the influence of stricture severity, stricture length, eccentricity, and bile flow property on the bile flow dynamics. The bile flow velocity, pressure distribution, pressure drop, and wall shear stress are provided in detail. The stricture alters the normal bile flow pattern and increases flow resistance. At the location upstream and downstream of the stricture, bile flow slows down. In the area of the stricture throat, bile flow is accelerated, and recirculation forms behind the stricture. The maximum pressure drop of the biliary system increases with the stricture length. The eccentricity makes the flow deflect away from the duct's centerline. The behavior of the deflected flow is significantly altered downstream of the stricture. Such bile flow behavior as deceleration and recirculation may lead to cholestasis. Stricture alters bile flow in the biliary tract, causing changes in biliary hydrodynamic indexes, which could potentially serve as an omen for gallstone formation and other related diseases. The consideration of the bile duct stricture could lead to better patient stratification.

Abstract:Biliary obstruction is common in pancreatobiliary malignancies and has a negative impact on the patient's quality of life, postoperative complications, and survival rates. Particularly in the last decade, there has been enormous progress regarding the diagnostic and therapeutic options in patients with malignant biliary obstruction. Endoscopy has given a new insight in this direction and novel techniques have been developed for the better characterization and treatment of malignant strictures. We herein summarize the available data on the different endoscopic techniques, and clarify their role in the diagnosis and treatment of malignant biliary obstructive disease. Finally, we propose an algorithm that can facilitate management decisions in these patients.
Role of Transforming Growth Factor-Beta Signaling Pathway in Pathogenesis of Benign Biliary Stricture.

Abstract:AIM: To characterize the expression of members of the transforming growth factor-beta (TGF-beta)/Smad/ connective tissue growth factor (CTGF) signaling pathway in the tissue of benign biliary stricture, and to investigate the effect of TGF-beta signaling pathway in the pathogenesis of benign biliary stricture.METHODS: Paraffin embedded materials from 23 cases of benign biliary stricture were analyzed for members of the TGF-beta/Smad/CTGF signaling pathway. TGF-beta(1), TpR I, TPR II, Smad4, Smad7 and CTGF protein were detected by immunohistochemical strepto-advidinbiotin complex method, and CTGF mRNA was evaluated by hybridization in situ, while 6 cases of normal bile duct served as controls. The percentages of positive cells were counted. The correlation between TGF-beta(1), Smad4 and CTGF was analyzed.RESULTS: The positive expression ratios of TGF-beta(1), TpR I, TpR II, Smad4, CTGF and CTGF mRNA in 23 cases with benign biliary stricture were 91.3%, 82.6%, 87.0%, 78.3%, 82.6% and 65.2%, respectively, significantly higher than that in 6 cases of normal bile duct respectively (vs 33.3%, 16.7%, 50.0%, 33.3%, 50.0%, 16.7%, respectively, P < 0.05). The positive expression ratio of Smad7 in cases with benign biliary stricture was 70.0%, higher than that in normal bile duct, but this difference is not statistically significant 70.0% vs 50%, P > 0.05). There was a positive correlation between positive expression of TGF-beta(1), Smad4 and CTGF in cases with benign biliary stricture.CONCLUSION: The high expression of TGF-beta/Smad/CTGF signaling pathway plays an important role in the pathogenesis of benign biliary stricture.
