Surgical Treatment of Metatarsal Fractures

Abstract:Objective To explore the operative treatment of metatarsal fractures., Methods Seventy-nine patients with metatarsal fractures were treated from January 2003 to January 2006. They were 64 males and 15 females, with an average age of 45.5 (11 to 65) years. They included 17 cases of the first metatarsal bone fractures, 6 the second metatarsal bone fractures, 5 the third metatarsal bone fractures, 4 the fourth metatarsal bone fractures, 27 the fifth metatarsal bone fractures, and 20 multi-metatarsa] fractures (4 of whom were Lisfrane joint fracture-dislocation). There were altogether 10 open fractures. The surgical treatments included close and open reduction, internal fixation by plate, screw, intramedullary screw and K-wire. Results All the 64 patients were followed up for 14 months on average (ranging from 8 to 24 months). All the cases healed well, and the mean time of bone union was 11 weeks (ranging from 8 to 16 weeks). No infection was found in all the cases. The mean time of complete weight loading walking was 13 weeks (ranging from 9 to 18 weeks) . According to the midfoot and forefoot scale by American Orthopaedic Foot and Ankle Society (AOFAS), the mean score was 84.4 (ranging from 72 to 100). Conclusion Anatomic reduction and stable internal fixation is the best solution for dislocated metatarsal fractures, as well as is the foundation of reconstruction of foot form and foot arch function.
