Effects of SERCA2a Over-Expression on Post-Mi Survival and Time Course of Ventricular Remodeling in Transgenic Rats

Abstract:Objective:Heart failure (HF) following myocardial infarction (MI) is characterized by decreased systolic Ca 2+ release from the sarcoplasmic reticulum (SR) and increased diastolic cytoplasmic Ca 2+ concentration in the cardiomyocyte. The myocyte SR Ca 2+ -ATPase (SERCA) plays a major role in Ca 2+ reuptake into the SR during diastole. A decreased expression of SERCA after MI has been found in human and animal failing hearts. Our study is aimed at demonstrating the effects of SERCA2a over-expression on post-MI survival and time course of ventricular remodeling in transgenic rats. Methods: Age matched transgenic rats (TG) with 37% cardiac overexpression of SERCA2a and wild-type control rats (WT) were studied. Myocardial infarction (MI) was induced by left anterior descending coronary artery ligation. The post-MI arrhythmia and left ventricular function had been analyzed by ECG monitoring and echocardiography. Results: During the acute postinfarction period (24 h ),TG rats had higher mortality than WT rats (73% vs. 35%,P0.001). This was associated with the increase in ventricular tachycardia incidence in TG rats,which could be prevented by lidocaine treatment. One month post-MI,myocardial diastolic function was preserved and post-MI left ventricular (LV) remodeling was reduced in TG rats as compared with WT rats. However no difference was found in systolic function in both groups. Conclusion:This study demonstrates that the preexisting over-expression of SERCA2a in transgenic rats improves LV diastolic function and reduces LV remodeling. However,the SERCA2a over-expression is associated with proarrhythmic effects and increased mortality in the acute phase after MI.
