Diagnostic value of Confocal Laser Endomicroscopy in Chronic Atrophic Gastritis and Intestinal Metaplasia

Abstract:Objective To investigate the diagnostic value of confocal laser endomicroscopy( CLE) in chronic atrophic gastritis and intestinal metaplasia. Methods 89 Patients with gastric mucosal lesion diagnosed by gastrointestinal endoscopy were enrolled in the study. The mucosal condition was observed under white light imaging( WLI) and confocal imaging. All the lesions will be taken biopsy for histologic diagnosis. Results There were 95 lesions diagnosed as atrophic gastritis under WLI endoscopy,40 lesions under CLE,and 37 lesions under histology. The diagnose accordance rate was 38. 95% between WLI and histology,and 94. 6% between CLE and histology. There were 54 lesions diagnosed as intestinal metaplasia under WLI endoscopy,170 lesions under CLE,and 161 lesions under histology. The diagnose accordance rate was 33. 54% between WLI and histology,and 84. 47% between CLE and histology. The diagnostic rate of atrophic gastritis and intestinal metaplasia under CLE were higher than WLI. The CLE had good consistency with histology( P = 0. 453,kappa value = 0. 895,and P = 0. 298,kappa value = 0. 577). Conclusion CLE was valuable in diagnosis of atrophic gastritis and intestinal metaplasia.
