Residual Lesions after Cytoredutive Surgery in Evaluation of Effectiveness of Chemotherapy in Patients with Epithelial Ovarian Cancer

Abstract:Objective: To investigate the histopathological values of tumor residual lesions in primary cytoredutive surgery and interval debulking surgery for prognosis prediction and choice of adjuvant chemotherapy in patients with epithelial ovarian cancer. Methods: 41 cases with suboptimal primary cytoredutive surgery and adjuvant chemotherapy were retrospectively analyzed. All cases were assigned into group A( lesion size remission more than 50%,n =21),group B( lesion size remission less than 50%,n = 17),and group C( lesion size remission no change or even grown,n =3) based on the changes of residual lesions in primary cytoredutive surgery and interval debulking surgery. All of them received platinum-based chemotherapy. Results: ① The clinical benefit rate evaluated by the changes in size of residual leisions for groups A,B and C was 100%,35. 3%,and 0,respectively( P<0. 05) 。The rates were also statistically different when evaluated by the pathological examination( 57. 1% vs5. 9% vs 0,P<0. 05). ②There were significant difference in the recurrence rate among group A,B and C( 71. 4% vs94. 1% vs 100%,P =0. 021). ③The median progression-free survival( PFS) for patients in groups A and D( B+C) was20 and 6 months,respectively( P<0. 05). The median overall survival( OS) was 42 and 20 months,respectively( P <0. 05). ④There were significant difference in the PFS and the OS between group A1 and D1. There were also statistically different in PFS between group D1 and D2. Conclusions: The histopathological changes of tumor residual lesions in primary cytoredutive surgery and interval debulking surgery may be of some value to evaluate the effectiveness of adjuvant chemotherapy in patients with epithelial ovarian cancer,which may contribute to the choice of adjuvant chemotherapy after interval debulking surgery.
