Local Warming and Lidocaine Pretreatment Reduces Injection Pain of Rocuronium

Abstract:Objective To compare the effect of local warming at injection site and lidocaine pretreatment on the prevention of pain from injection of rocuronium.Methods Between March and August 2011,150 adult patients undergoing elective laparoscopic cholecystectomy were randomly divided into three groups: local warming group(group W), lidocaine group(group L) and the control group(group C).All patients were catheterized in dorsum of non-dominant hand. Patients in group W were subjected to warming using an air-warming device at 40 °C for 1 minute at the site of venous access. In group L,tourniquet compression with a pressure to stop the transfusion was applied to the non-dominant hand and 2 mL of lidocaine was injected in 2 seconds,the occlusion was released after 1 minute.Then 1 mL(10 mg) of rocuronium was injected intravenously within 2 seconds in all groups.The patients then were observed and asked immediately if they had any discomfort and to quantify their discomfort on a 4-point scale.Results Age,sex,weight and height were comparable between the three groups.The incidence of pain in group C(82%) was significantly higher than that in group L(34%) and group W(62%)(P < 0.05).The incidence of pain in group W was significantly higher than that in group L(P < 0.05). The incidence of severe pain in group C(30%) was significantly higher than that in group L(0%) and group W(6%)(P < 0.05).The incidence of moderate and severe pain in group W(18%) was significantly higher than that in group L(2%)(P < 0.05).Conclusions Local warming at injection site and lidocaine pretreatment both effectively reduce the incidence of pain caused by intravenous rocuronium and decrease the degree of pain.Lidocaine pretreatment is more effective.
